Det är teoretiskt möjligt att laga mitralisklaffap- paraten om en väsentligen normal främre segelklaff finns tillgänglig. De grundläggande fyra teknikerna vid lagning innefattar användandet av en ringformad plastring, kva- dratisk segmental resektion av insjuknad bakre segel- klaff, avkortning av förlängda sensträngar samt omflytt- ning av den bakre segelklaffen till den främre segelklaf- fen.
Ringformade plastringar krävs för att uppnå en håll- bar minskning av ringdilationen. Samtliga vanligt före- kommande ringar sys längs med den bakre mitralissegel- klaffen nära mitralisringen i det vänstra förmaket.
Duran-ringen omger klaffapparaten fullständigt, medan de andra är öppna mot den främre segelklaffen. Ringen kan antingen vara styv, som den ursprungliga Carpentier- ringen, eller vara flexibel men icke elastisk som Duran-ringen eller Cosgrove-Edwards-ringen.
Effektiv behandling av bristande tillslutningsför- måga hos mitralisklaffapparaten kräver för närvarande öp- pen hjärtkirurgi genom användning av total kardiopul- monal bypass, tvärklämning av aorta samt hjärtslags- blockering.
Detta är särskilt riskfyllt för vissa patientgrup- per. Särskilt äldre patienter, patienter med dålig väns- tersidig hjärtkammarfunktion, njursjukdom, allvarlig aor- taförkalkning, tidigare hjärtkirurgiska ingrepp eller andra åtföljande sjukdomar skulle högst troligt mä bäst av mindre invasiva ingrepp, även om lagningen inte är fullständig. Den nuvarande trenden mot mindre invasiva kranskärlsoperationer, utan kardiopulmonal bypass, såväl som PTCA kommer också att fordra en utveckling mot mindre invasiva metoder för lagning av den ofta åtföljande lO 15 20 25 30 35 514 718 3 bristande tillslutningsförmàgan hos mitralisklaffappara- ten.
Ett ändamål med föreliggande uppfinning är därför att tillhandahålla en anordning för behandling av bris- tande tillslutningsförmåga hos mitralisklaffapparaten, utan behov av kardiopulmonal bypass och öppnande av brös- tet och hjärtat.
Detta och andra ändamål uppnås med en anordning en- ligt det bifogade patentkravet 1.
Enligt föreliggande uppfinning innefattar en anord- ning för behandling av bristande tillslutningsförmàga hos mitralisringen en långsträckt kropp, som har sådana di- mensioner att den är införbar i stora hjårtvenen, och som har två tillstånd, nämligen ett första tillstånd, i vil- ket den långstràckta kroppen har en form som är anpass- ningsbar till stora hjärtvenens form, och ett andra till- stånd, til vilket den làngsträckta kroppen år överfö- ringsbar från det första tillståndet för att anta en minskad krökningsradie, varigenom den stora hjårtvenens krökningsradie minskas liksom mitralisklaffringens om- krets, när den långsträckta kroppen är placerad i den stora hjärtvenen.
Företrådesvis är organ anordnade för överföringen av den làngsträckta kroppen till det andra tillståndet genom böjning och/eller förkortning av kroppen från en större krökningsradie till en mindre krökningsradie. Överföringsorganen kan innefatta organ för böjning och/eller förkortning av den làngsträckta kroppen genom en företrädesvis asymmetrisk sammandragning av denna.
Den långsträckta kroppen kan vidare innefatta ett minnesmaterial som åstadkommer överföringen till det andra tillståndet.
I en föredragen utföringsform kan den långsträckta kroppen innefatta en stent. I en alternativ utföringsform kan anordningen enligt uppfinningen innefatta flera stentsektioner, och böjnings- och/eller förkortnings- lO 15 20 25 30 35 514 718 4 organen kan innefatta tràdar för förkortning av avståndet mellan stentsektionerna.
Ett sätt för reducering av mitralisklaffringens om- krets innefattar àtgärderna att föra in en làngsträckt kropp i stora hjärtvenen i närheten av mitralisklaffappa- ratens bakre segelklaff, och att sedan åstadkomma en böj- ning och/eller förkortning av den làngsträckta kroppen, när denna är placerad i den stora hjärtvenen, för att re- ducera dennas krökningsradie och därigenom minska mitra- lisklaffringens omkrets.
Sättet utnyttjar sålunda att den stora hjärtvenen är placerad nära mitralisringen. Detta möjliggör lagning ge- nom användning av nuvarande kateterstyrda tekniker.
Kransvenerna dränerar blod fràn hjärtmuskulaturen till det högra förmaket. De mindre venerna dränerar blod direkt in till förmakskaviteten, och de större venerna följer huvudartärerna och löper in i den stora hjärtve- nen, som väsentligen omger mitralismynningen och mitra- lisringen. Den stora hjärtvenen löper in i den bakre atrioventrikulära fördjupningen, som ligger i fettväv- naden mellan den vänstra förmaksväggen och den ventriku- lära hjärtmuskulaturen, innan den mynnar ut i det högra förmaket mellan förmaksskiljeväggen och den bakre Eusta- chiska venen.
Hos en vuxen kan den stora hjärtvenens bana ligga inom 5-15 mm till den mediala infästningen av mitralis- klaffapparatens bakre segelklaff. Preliminära mätningar, som har gjorts vid obduktioner på vuxna med normal vikt, visar liknande resultat med ett avstånd pà 5,3 ¿ 0,6 mm vid den mediala infästningen och ungefär 10 mm vid den bakre segelklaffens laterala sida. Den stora hjärtvenens omkrets var 18,3 ¿ 2,9 mm vid mynningen (vilket ger en diameter på den bakre segelklaffen på 5,8 i 0,9 mm) och 9,7 i 0,6 mm längs den bakre segelklaffens laterala sida (motsvarande en diameter pà 3,1 i 0,2 mm). 10 15 20 25 30 35 514 7-18 5 Uppfinningen förstås lättare genom följande beskriv- ning av föredragna utföringsformer med hänvisning till bifogade ritningar, pà vilka: Fig 1 är en tvärsnittsvy av en hjärtdel.
Fig 2 och 3 är schematiska vyer av en första utfö- ringsform av en anordning enligt föreliggande uppfinning.
Fig 4-6 är schematiska vyer som visar ett instrument som kan användas vid placering av anordningen, som visas i fig 2 och 3, i stora hjärtvenen.
Fig 7 är en delförstoring av den första utförings- formen enligt fig 2.
Fig 8 och 9 är schematiska vyer som visar placer- ingen av anordningen enligt fig 2 och 3 i stora hjärtve- nen.
Fig 10 och ll är schematiska vyer som visar placer- ingen av en andra utföringsform av anordningen enligt fö- religgande uppfinning i den stora hjärtvenen.
Fig 12 och 13 är schematiska vyer som visar placer- ingen av en tredje utföringsform av anordningen enligt föreliggande uppfinning i den stora hjärtvenen.
Fig 1 är en tvärsnittsvy genom hjärtomràdet i det bakre atrioventrikulära fördjupning l, som är fylld med fettvàvnad. Figuren visar mitralisklaffapparatens bakre segelklaff 2 och de angränsande delarna 3, 4 av förmakets hjärtmuskulatur samt den ventrikulära hjärtmuskulaturen.
Den stora hjärtvenen 5 visas nära mitralisringen 6 och bakom den bakre segelklaffens 2 infästning 7. Eftersom den stora hjärtvenen 5 väsentligen omger mitralisringen 6, resulterar en minskning av krökningsradien pâ den böj- da stora hjärtvenen 5 även i en minskning av mitralis- ringens 6 diameter och omkrets.
Anordningen i fig 2 innefattar en làngsträckt kropp 8 som är tillverkad av minnesmetall, exempelvis Nitinol, eller annat liknande material som har ett minne av en ur- sprunglig form, vilken visas i fig 3, och som tillfälligt kan tvingas att inta en annan form, vilken visas i fig 2.
Denna làngsträckta kropp 8 innefattar en, två eller flera lO 15 20 25 30 35 514 71.8 6 minnesmetalltràdar 9 av skruvlinjeform eller någon annan form, sä att de passar samman och kan medge de rörelser som beskrivs nedan. Ett flertal hakar 10 är fästa utmed den lángsträckta kroppen 8 sà att de skjuter ut radiellt fràn denna. Dessa hakar 10 är täckta av ett täckande höl- je 11 i fig 2.
Den làngsträckta kroppen 8 tvingas att inta ett ut- sträckt eller expanderat tillstànd med hjälp av ett sta- biliserande instrument 12, vilket visas i fig 4. Detta instrument 12 har tvà armar 13 vid en distal ände 14 av en stav 15 samt ett läsorgan 16 vid en proximal ände av staven 15. Avståndet mellan stavens 15 ändar motsvarar den längsträckta kroppens 8 önskade längd när den förs in i den stora hjärtvenen 5.
Armarna 13 är fria att röra sig mellan den position som visas i fig 4 och en position i linje med staven 15, sàsom visas i fig 6. Läsorganet 16 har tvà läsknappar 17, som pressas radiellt ut frän staven 15 av tvà bladfjädrar 18. Den längsträckta kroppen 8 kan sålunda träs över det stabiliserande instrumentets 12 stav 15, varpå kroppen sträcks ut mellan armarna 13 och knapparna 17, och slut- ligen läses i sitt utsträckta tillstànd pä det stabilise- rande instrumentet 12 mellan armarna 13 och knapparna 17, sàsom visat i fig 5.
Staven 15 kan vara en metalltràd, som mellan den bortre änden 14 och läsorganet 16 är relativt styv men dock sä böjbar att den kommer att följa den stora hjärt- venens 5 form. Det stabiliserande instrumentets 11 me- tallträd är mer böjlig proximalt om läsorganet 16 för att lätt kunna följa venernas krökar.
Den ovan beskrivna làngsträckta kroppen 8 placeras i den stora hjärtvenen 5 pä följande sätt: Ett införingsrör (ej visat) av syntetiskt material kan användas för att ge åtkomst till vensystemet. När man har fàtt ätkomst till vensystemet, förs en läng styrtràd (ej visad) av metall in genom införingsröret och via ven- systemet in i den stora hjärtvenen 5. Denna styrtràd är lO 15 20 25 30 35 514 .718 7. försedd med röntgenstràleavstàndsmarkörer, så att styr- tràdens position i stora hjärtvenen 5 kan övervakas.
Den làngsträckta kroppen 8 läses på det stabili- serande instrumentet 12, såsom visas i fig 5, och förs in i det långa täckande höljet 11 av syntetiskt material.
Detta aggregat trycks därefter in genom införingsröret och vensystemet fram till den stora hjärtvenen 5, buret av styrtráden. Efter exakt positionering av den làng- sträckta kroppen 8 i den stora hjärtvenen 5, såsom visas i vilken mitralisklaffen 19 visas innefattande dras det täckande höljet 11 till- baks och frilägger den làngsträckta kroppen 8 inuti den i fig 8, en central öppning 20, stora hjärtvenen 5. Denna manöver gör det möjligt för ha- karna 10 på den làngsträckta kroppen 8 att gräva sig in i den stora hjärtvenens 5 väggar och in i hjärtat. Den làngsträckta kroppen 8 är fortfarande làst pà det stabi- liserande instrumentet 12, så att hakarna 10 är i ingrepp med den stora hjärtvenens 5 väggar i den làngsträckta kroppens 8 utsträckta eller expanderade tillstànd.
En kateter 21, som visas i fig 6, trycks framåt över styrtráden och staven 15 för att frigöra den làngsträckta kroppen 8 fràn làsorganet 16, genom att pressa bladfjäd- rarna 18 mot staven 15. Denna rörelse frikopplar knap- parna 17 och armarna 13 från ingrepp med den làngsträckta kroppen 8, som dras samman sàsom visas i fig 9, vilket resulterar i att kroppen böjs mot mitralisklaffringen 6 och flyttar sin bakre del framåt (visas medelst pilar i fig 9). omkrets och tillsluter därigenom den centrala öppningen 20.

Denna anordning befinner sig på ett av- stånd fràn den bakre segelklaffens infästning, som inte mycket överskrider det avstånd pà vilket nu använda ring- formade plastringar placeras med hjälp av öppna kirur- giska metoder, och den stora hjärtvenen är utmed hela sin bana tillräckligt stor för att rymma en sådan anordning.
Anordningen kan sättas på plats genom kateterteknik eller genom annan adekvat teknik och erbjuder ett säkrare al- 514~ 7158 9 ternativ till nuvarande öppna kirurgiska metoder. Anord- ningen kan utformas eller heparinbeläggas för undvikande av tromboser i den stora hjärtvenen och därigenom minska behovet av behandling med aspirin, tiklopedin eller anti- koagulerande medel.
Det inses att modifieringar av den ovan beskrivna anordningen kan göras av personer som är insatta i tek- nikomràdet utan att uppfinningens andemening och skydds- omfàng kringgàs.
Claims (6)
