Изобретение относится к медицине, а именно к хирургии и травматологии, и предназначено для лечения больных с загрязненными, в частности, укушенными ранами.The invention relates to medicine, namely to surgery and traumatology, and is intended for the treatment of patients with contaminated, in particular, bitten wounds.
В связи с увеличением числа травм от укусов млекопитающими животными в Москве [Яшина Т.Н., Плохой А.А., 2002], Кемерово [Старых B.C., Поткина Т.Н., Головина Л.Н., Агафонов А.Н., 2000, 2003] и в других городах, актуальность проблемы лечения таких пострадавших возрастает. Установлено, что укушенная рана отличается от раны, нанесенной холодным или огнестрельным оружием, более высокой загрязненностью патогенной микрофлорой, содержащейся в слюне укусившего, и часто становится инфицированной [Колонтай Ю.Ю., Панченко М.К., Андрусон М.В., Открытые повреждения кисти. Известно, что через эпителий десневых сосочков в межзубные промежутки поступают спущенные клетки эпителия, лейкоциты вместе с микроорганизмами и с продуктами их жизнедеятельности, которые могут быть причиной инфицирования укушенной раны. Даже в полости рта чистящего зубы здорового человека в 1 мм3 зубного налета содержится 100-200 миллионов колоний сапрофитов и патогенных микроорганизмов, грамположительные и грамотрицательные кокки, спирохеты, вибрионы, актиномицеты, простейшие, грибы, причем в различной их комбинации [Н.Ф. В результате значительного бактериального загрязнения укушенной раны наблюдается высокая частота инфекционных осложнений. Общепринятый способ лечения при укушенной ране, нанесенной млекопитающим животным, включает обильное промывание раны водой с мылом, обработку раны 70% спиртом, а при обширной ране - наложение наводящих швов [Инструкция МЗ РФ «О порядке работы лечебно-профилактических учреждений и центров государственного санитарного эпидемиологического надзора по профилактике заболевания людей бешенством», утвержденная Приказом МЗ РФ №297. 7 октября 1997 года]. Однако, во время лечения промыванием раны, например спринцовкой, жидкость вытекает из раны, загрязняя окружающие предметы, что, при подозрении на бешенство животного создает повышенную опасность для участников лечения, снижает гигиеничность процедуры и требует дополнительной уборки, повышая трудозатраты персонала. The need to reduce environmental pollution in the treatment of bacterially contaminated bitten wounds, prompted us to develop a special device.

Технический результат изобретения достигается тем, что устройство для лечения загрязненной раны включает закрытую емкость с жидкостью, соединенную трубкой с элементом нагнетания воздуха, и снабженную шлангом подачи жидкости в рану, и трубку оттока жидкости из раны. Емкость выполнена многокамерной, и каждая камера соединена собственной трубкой подвода к коллектору, снабженному переключающей ручкой, от которого шланг подачи жидкости в рану проведен в полость жесткой чашки с нижним краем из упругоэластичного материала. На трубке оттока жидкости наличие регулятора уровня оттока обеспечивает управление скоростью оттока и наполнением чашки жидкостью. Жесткая чашка с нижним краем из упругоэластичного материала обеспечивает возможность герметичного укрывания ею раны для промывания последней под управляемым давлением без вытекания жидкости из бактериально загрязненной раны на окружающие предметы. Наличие у чашки трубки отвода промывной жидкости обеспечивает отток жидкости из раны в любом заданном направлении. Выполнение на верхней поверхности чашки жесткой ручки с возможностью герметизирующего прижатия упругоэластичного края чашки к коже вокруг раны, обеспечивает возможность более удобного и надежного удержания и надавливания сверху на чашку для повышения герметичности между упругоэластичным краем чашки и кожей вокруг раны. Размещение в чашке трубки оттока с возможностью расположения ее конца в глубине дна раны улучшает эффективность оттока жидкости из раны.
