発明の背景
1、発明の分野
本発明は皮下埋設可能な医療用装置に関し、特に患者の姿勢を検出するための長
期的に皮下埋設可能な装置に関する。
2、先行技術
姿勢探知装置は、以前より公知の技術で、しばしば皮下埋設されて人工心臓ペー
スメーカーのベーシングレートのコントロールに使用されてきた。例えば代表的
方法には、水銀スイッチを用いて重力を利用する方法がある。もちろんこの方法
は、スイッチが開いているか閉じているかを示す2進出力か2状態出力を与える
。
血管内圧力センサーはアンダーソン(Anderson)氏に対して発行された
米国特許第4,407,296号に記載されており、同特許は心臓血管系におい
て長期的に皮下埋設可能な圧力変換器を開示している。改善された電子回路を有
する圧力変換器が、モンロー(Monr。
e)氏に対する米国特許第4.432,372号に開示されている。なおいっそ
う改善された圧力変換器が、アンダーソン(Anders。
n)氏等に対する米国特許第4.485,813号に開示されている。
これらの圧力センサーは、圧力か圧力変化を測定してベーシングレートに変える
人工心臓ペースメーカー用アルゴリズムのコントロールを指向している。
ミロウスキー(Mirowski)氏等に対する米国再発行特許第30.372
号が、血管内圧力センサーを使用している皮下埋設可能な細動除去器のコントロ
ールを開示している。これらの引用例のいずれも、圧力差を測定して患者の姿勢
を検出する方法を開示しない。
発明の開示
本発明は、脈管系に長期的に皮下埋設された少くとも2つの圧力センサーを使用
する。これらのセンサーは圧力変化を患者の姿勢変化に応じて感知するように配
置される。患者の姿勢を検出するために、センサーは姿勢変化に伴う圧力差を示
す変化を感知するように配置される。
この圧力差変化は、脈管系内の異なる点に対する引力の影響から生じる。例えば
、心臓右側の圧力センサーと、仰向けに寝た患者の工大静脈の圧力センサーによ
る圧力測定値はおおよそ同じである。しかしながら患者が直立したとき、工大静
脈中のセンサーの測定圧力値は、心臓右側のセンサーより大きくなる。患者が直
立していることを見分けるのはこの圧力差による。
センサーは実際の圧力を測定するので、完全に仰向けに寝た状態から完全に直立
する状態まで連続的に変化する。それゆえに、患者が座っているときの測定圧力
差の値は、仰向けに寝たか直立したときの圧力差測定値から区別することができ
る。同じように他の姿勢変化も検知できる。
圧力差によって姿勢を検出することの利点は、センサーに全ての等しい圧力変化
生じさせる要因を相殺し、かつ除去できることである。高度と大気圧の相違が、
このような要因の代表的な例である。この方法によって、例えばせきや(しやみ
による短期的症状が排除される。
姿勢検出には、多くの使用態様がある。代表的適用例は、心臓ベーシングしてい
る心臓病患者に対するものである。直立している患者は、仰向けに寝た患者より
高い心臓ベーシングデマンドを必要とすることから、姿勢検出は直立している姿
勢の患者のためにベーシングレートを増加させることに使用することが容易に考
えられる。また、姿勢検出は皮下埋設された薬剤分配装置の薬剤供給処方計画を
可変する(例えば投与薬剤量を増加させたり、心臓用薬剤もしくは他の薬剤の供
給を初期化する)ために利用できる。姿勢の変化に応じて治療を可変することを
可能とすることから、本発明は人体に薬剤治療を与えるいかなる皮下埋設可能な
装置にとっても有用であろう。

詳細な実施例の説明
本発明は、心臓血管系内に置かれた少くとも2個の長期的に皮下埋設可能な圧力
センサーを用いる。本発明の好ましい態様では、2つの圧力センサーを使用する
が、追加のセンサーを本発明の教示に従って使用してもよい。本明細書に引用す
るアンダーソン(Anderson)氏等に対する米国特許第4,485,81
3号で開示される圧力センサーが、本出願のために好ましいセンサーである。他
の適当なセンサーとしては、それぞれ本明細書に引用するモンロー(Monro
e)氏に対する米国特許第4.432,372号と、アンダーソン(Ander
s。
n)氏に対する米国特許第4,407,296号に開示されている。
図1は、仰向けに寝た患者1oを示す概念図である。患者10は、上記のような
長期的に心臓血管系に皮下埋設した2つの圧力センサーを有する。これらのセン
サーの一つは、退京の皮下埋設技術を使用して心臓12の右側に皮下埋設されて
いる。二つ目の圧力センサーは、位置14で工大静脈で皮下埋設されている。2
つの圧力センサーは、直線距離L16だけ離れている。患者10が水平に横たわ
っているならば、心臓12内で感知される圧力と、工大静脈の位置14における
圧力センサーによって感知される圧力とはおおよそ同じである。
図2は、同じく長期的に皮下埋設された圧力センサーを有する患者10を示す概
念図である。本図の患者10は、上半身を半ば起こした状態をとっている。図示
のように、患者10の胴は水平に対して角度18で傾斜している。心臓12中の
圧力センサーは、位置14の圧力センサーより位置が高くなる。二つのセンサー
間の垂直距離は、二つのセンサーの間の距離L(図1参照)と胴の傾斜角度18
から計算される。垂直路11120は、距離りに角度18の正弦を乗じたもので
ある。水力学の原則から、心臓12内で感知された圧力が垂直路!!20に比例
して位置14の圧力センサーによって測定された圧力より低いことが容易に分か
る。
従ってこの圧力差を測定することによって角度18を算出することができる。こ
の方法において患者1oの胴の直立程度を算出することができる。
この実施例では、すっかり横たわっている患者1oとまっすぐに立っている患者
10を圧力差によって識別することができないことに留意する必要がある。その
ような場合でも姿勢を検出できるようにするには、位置14の圧力センサーを足
の中に移動させなければならない。しがしながらそのようにすることは注入手順
が複雑化するだけであり、従ってそのような位置の変更は十分な精度が必要とさ
れる場合にだけ行なわれるべきである。
図3は、心臓血管系に皮下埋設される2つの圧力センサーの図である。図示のよ
うに、心臓12は圧力センサー36を右心房内に皮下埋設しである。圧力センサ
ー36は、静脈を経由する心臓のベーシングと同様に皮下埋設可能なパルス発生
器22へ静脈系を通した電気的リード24を経て連結している。
同様に圧力センサー32が、工大静脈に配置されている(図1、図2能なパルス
発生器22に電気的に連結する。リード24.3oは位置34で鎖骨下静脈26
に入り、皮下埋設可能なパルス発生器22につながっている。心臓12の右心房
への進入は、上大静脈28を経由する。
好ましい実施例では、皮下埋設可能なパルス発生器22は、人工心臓ペースメー
カーとして作動する。必要なベーシングリードは、図面を複雑化させないために
図示を省略しである。図示の実施例は本発明の代表的な適用例を示すもので、本
発明が図示のものに限定されることを意味するものではない。図示の実施例にお
いては、圧力センサーにはそれぞれリードを接続することもできる。しかしなが
ら同一のリードに両方の圧力センサーを接続することもできる。−個の圧力変換
器を使用した圧力差の感知が、米国特許第4,407,296号に開示されてお
り、該特許にはある状況においては2個の圧力センサーを用いるより望ましい場
合があることが説明されている。
図4は、皮下埋設可能なパルス発生器22のブロック図である。圧力センサー3
6.32から信号をそれぞれ転送するリード24.3oが示されている。信号処
理回路102.104はこれらの信号の増幅、積分を行なう。2つの圧力センサ
ーによって計測された圧力における相違が、圧力差演算回路106によって検出
される。この回路は、標準的な電圧制御発振器回路を使用しているエスケープイ
ンターバルタイマー108を制御する。圧力差が大きくなるに連れて、エスケー
プインターバルタイマー108によって作り出されるインターバルは短くなる。
図4は、本発明で採用する回路の一般的タイブのものを示す。もちろん代替回路
も採用できる。例えば信号処理回路102,104はローパスフィルターとする
こともでき、積分/濾過は、単一の圧力差変換器を用いる実施例のように、圧力
差の演算後に行なわれるようにすることもできる。 感度アンプ110は、禁止
論理回路112及び刺激パルスアンプ114が通常のデマンドペースメーカーと
して作動することを禁止する。静脈を経由するベーシングリード100を使用し
て、感知と刺激が与えられる。皮下埋設可能なパルス発生器22のベーシング出
力が通常のデマンドペーサ−と異なるのは、2個の圧力センサー32.36が感
知した圧力がほとんど同じ状態から圧力差が増えるにしたがって基礎となる人ロ
ペーシングレートが減少する場合だけである。
図5は、患者姿勢の変化に伴う圧力センサー32の理想化された出力のグラフ4
0を示す図である。縦軸42に沿う投影が、感知された圧力、即ち、圧力センサ
ーの電圧出力である。横軸44に沿う投影は、いろいろな姿勢変化をとった場合
の各姿勢の時間である。図中52は、患者が仰向けに寝たときの測定レベルを示
す。患者は、図中46で起きあがり、そこからグラフは水平レベル48を保って
いる。患者は図中50において、再び仰向けに寝た水平レベル52状態となって
いる。この理想化されたデータについて注意すべきことは、−個の圧力センサー
で姿勢変化をモニターできることである。
図6は、ある短い時間における同じ時間軸に沿った圧力センサーと算出された圧
力差の出力グラフである。
上段の図は、患者10が仰向けに寝た状態における、圧力センサー36の測定値
である水平圧力レベル6oを示す。実際に患者が立ち上がると、圧力センサー3
6によって計測される圧力は、水平レベル62へ減少する。図中64.66にお
ける圧力増加は、姿勢変化と関連がない。
図中64における増加は、くしゃみか咳のような原因による一時的なものである
。図中66から始まって図中68で終わる変化は、長い持続期間を有する。この
状態は、主として静脈の血圧のような生理的理由によるものか、飛行機やエレベ
ータ−による高度の変化のような外的理由によるものであろう、どちらの状況に
おいても、飛行機やエレベータ−による高度の変化は姿勢変化としては検出され
ない。またどちらの状況においても、圧力センサー36による圧力変化としては
姿勢変化は検出されない。
中段の図は、同一時間に渡る圧力センサー32の出力を示す。仰向けに寝た状態
は図中70である。患者10が立ち上がると、図中76に至る圧力増加74とし
て検出される。外乱的圧力上昇78と姿勢変化によらず図中82に至る圧力上昇
80が圧力センサー36によって検出される。
下段の図は、2つの圧力センサーの圧力差を示す。これは演算された圧力差10
6の出力に相当する(図4参照)。図中レベル9oは、はとんど圧力差がないこ
とを示す。このレベルは、患者10が仰向けに寝た状態に相当する。本実施例で
は、これは最も長いエスケープインターバルあるいは最もレベルのベーシングレ
ートとなる。患者が立ち上がると、圧力差は図中92のように上昇して新しいレ
ベル94に至る。これは短か目のエスケープインターバルに相当する。
圧力差のカーブが外乱的変化64/78や姿勢変化によらない圧力変化66〜8
0のいずれも含まないことに留意して欲しい。これは、両方のセンサーが同じ姿
勢変化によらない圧力変化にさらされるときには、圧力差が同じように変動する
ことによる。
本発明の他の適用は、当業者にとっては本願の請求項の範囲からの逸脱すること
な(容易に想到できるものであろう。

要約書
患者の姿勢の変化を検出する装置と方法である。2個の圧力センサーを、長期的
に心臓血管系の異なる場所において皮下埋設する。例えば1つつは心臓右側に置
き、他の1つは工大静脈に配置する。圧力センサーの配置は、長期にわたる皮下
埋設が容易であるように選定する。しかしながらその選定位置は、姿勢変化に伴
う圧力差を区別して測定できる位置でなければならない。2つの位置において感
知された圧力変化の差が、電子回路による患者の姿勢変化検出を可能にする。こ
れらの姿勢変化は、心臓血管系のベーシングデマンドの増減のような生理的変化
や他の恒常的な機能変化を示すために用いられよう。この心臓血管系や他の恒常
的な機能変化に対する補償は、デマンド型心臓ペースメーカーのエスケープイン
ターバルの増減を通して、または皮下埋設可能な薬剤供給装置の薬剤供給処方計
画を変えることによって部分的に達成される。

Claims (11)
- 1.以下の要件からなる装置。 a.心臓血管系に配置された複数の圧力センサー、そしてb.上記2つの圧力セ ンサーで測定された圧力の差を演算するための上記各センサーそれぞれに連結し た手段。
- 2.上記演算手段に接続し、該演算手段が算出した上記圧力差に基づいて患者の 姿勢を検出する手段を含む請求項1の装置。
- 3.圧力センサーを2個とした請求項2の装置。
- 4.上記2個の圧力センサーのうちの一方が、上記患者の心臓内に置かれる請求 項3の装置。
- 5.上記2個の圧力センサーのうちの他方が、下大静脈内に置かれる請求項4の 装置。

- 7.上記心臓ペースメーカーがデマンドタイプであり、上記ペーシンクレートが 上記心臓ペースメーカーのエスケープインターバルを増やすことによって減少す る請求項6の装置。
- 8.以下のステップからなる患者の姿勢を検出する方法。 a.上記患者の心臓血管系に複数の圧力センサーを皮下埋設し、b.上記圧力セ ンサーごとに圧力を測定し、c.少なくとも2個の圧力センサーで測定された圧 力の差を求め、そして d.該圧力差により上記患者の姿勢を算定する。
- 9.治療措置を人体に与えるための皮下埋設可能な装置において、心臓血管系内 の2つの異なる位置間の圧力差を感知する手段であって、上記患者が直立してい る姿勢でいるときに上記位置の一方が上記位置の他方よりも上側に位置する手段 と、そして上記人体への治療措置を可変するための上記圧力差の測定に感応する 手段とを含む装置。
- 10.上記治原措置を与える装置が、所定のベーシングレートで刺激パルスを発 生させる心臓ペースメーカーであり、そして上記可変手段は、上記ペーシンクレ ートを可変する手段である請求項9の皮下埋設可能な装置。
- 11.上記圧力差の感知手段が、近隣する2つの位置に配置した第4及びび第2 の圧力センサーである請求項9の皮下埋設可能な装置。
