Description
本出願は、2009年10月6日出願の米国特許出願第61/249207号からの優先権の利益を主張するものであり、その出願の開示は全て参照することにより本明細書の一部である。
好酸球性食道炎(EoE)は、小児および成人どちらでも発症する疾患であり、食物アレルギーによって引き起こされると考えられており、食道扁平上皮内部の好酸球の存在によって特徴付けられる。現在、EoEは、一般的な疾患であり、発生率が急速に増加していることが知られている。EoE患者は、吐き気や嘔吐から嚥下障害や食片圧入までの症状を示すことがある。一般に、治療しないでおくと、好酸球性炎症が、基底膜の線維化、狭穿、穿孔を含めた重大な永久的な損傷をもたらすおそれがあると考えられている。そのため、現在、EoE患者は、症状と食道好酸球の両方が寛解するまで治療を続けられる。
好酸球性食道炎(EoEまたはEE)は、主に食物アレルギーによって引き起こされる食道の炎症性疾患である。EoEを患う患者の約50〜70%が、別の併発のアトピー疾患を患っている。EoEは、小児でも成人でも発症し、白色人種(>90%)および男性(2〜4:1の比率)でより一般的である。EoEの顕著な特徴は、食道扁平上皮の内部への好酸球性侵潤であり、これは、減酸治療後にも消失せず、胃腸管中にはどこにも存在しない。EoEを患う患者は、例えば胸痛、吐き気、嘔吐、嚥下障害、食片圧入、および成長障害など様々な好ましくない症状を示すことがある。この疾患の自然史は十分には理解されていないが、専門家は、長期にわたる好酸球性炎症は永久的な食道の損傷をもたらすことがあり、基底膜の線維化、食道狭窄、狭穿、および穿孔を引き起こすことがあると危惧している。
近年、EoEの診断は、生検と共に上部内視鏡検査を必要とすることがある。上部内視鏡検査では、EoE患者は、脈管質の減少(diminished vascularity)、長手索溝(longitudinal furrowing)、脆砕性、微小膿瘍(白点)、滲出物、食道輪(食道の気管化)、食道口径の狭窄、狭穿、および食片圧入の所見を有することがある。これらの所見は他の疾患でも見られることがあり、内視鏡検査では最大30%の症例で食道が正常とされることがあるので、EoEに関する内視鏡検査単独での精度はそれほど高くない。EoE診断のための唯一の客観的方法は、内視鏡生検である。現在の治療の標準は、食道の遠位部分(胃食道接合部(GEJ)から約5cm)と、近位部分(GEJから約15cm)で得られるピンチ生検である。生検は、異常な内視鏡的所見を有する部位から取られ、また疾患の視覚的な証拠を示さない位置からも取られる。得られる生検の数も重要である。なぜなら、最近の研究で示されたように、EoEの組織病理学的診断は、1個の生検では患者の55%においてしか正確になされなかったが、5個の生検の後には100%まで上がったからである。生検標本の任意の1つにおいて高出力領域(HPF=400倍)につき15個を超える好酸球が見つかると、EoEの診断となる。好酸球脱顆粒、微小膿瘍、好酸球の表面層化、基底層過形成、マスト細胞および他の白血球数の増加など、他の組織病理学的所見もEoEで見られるが、現在、診断には必要とされていない。
薬理学的療法と食事療法はどちらも症状を緩和することができ、食道を組織学的に正常に戻す。唯一の確立されている薬理学的療法は、コルチコステロイドである。急性症状を速やかに和らげるために全身性ステロイド剤が推奨され、管理のために経口または嚥下局所投与経路が推奨される。経口および局所コルチコステロイドの主な合併症は食道カンジダ症であり、これは、最大20%の患者で観察されている。コルチコステロイドの主な限界は、服用を止めたときにEoEが一般に再発することであり、その結果、コルチコステロイドが唯一の治療ラインである場合、疾患の期間中はずっと継続して服用しなければならない。ステロイド投与を調節しなければならない、または中断しなければならない時を決定するための生検を用いた追跡内視鏡検査の間隔は、現在議論中である。EoE患者での寛解の持続に関するコルチコステロイドの長期的な効果もよく分かっていない。好酸球走化性を制御するサイトカインに対する抗体であるanti−IL−5やIL−13など、他のより新しい薬剤は現在研究中である。
生活の質に対するEoEの影響、およびこの疾患が未知の長期的な危険を伴う食道線維化まで進行する可能性の懸念により、専門家は、症状および好酸球浸潤が消失するまでEoEを治療することを推奨している。これらの患者の管理は、食道好酸球を評価するために、食物再導入中および/またはコルチコステロイド投与プロセス中に何度も繰り返し内視鏡生検を必要とする。このプロセスは、患者やその家族にとって不便であり負担となる。また、このプロセスは、内視鏡に関連する小さいが現実的な合併症の危険にも関連付けられる。さらに、現在、EoE監視のための内視鏡生検には、1回の処置あたり平均約1000ドルかかり、健康管理システムに対するかなりの出費となることがある。これらの考慮事項から、食道好酸球数を取得するための、より低侵襲性でより費用対効果の高い方法を見出すことが望まれている。
鎮静剤および麻酔性鎮痛薬のIV投与による鎮静は、上部内視鏡検査のコストを高める唯一の最重要な寄与因子であり、処置コスト全体の30〜50%を占めると推定される。鎮静に関係する合併症に関わる死亡率および罹患率により、患者は、内視鏡処置中に継続的な心肺監視および看護補助を受けなければならない。処置後の回復期間も出費の一因となる。なぜなら、大きな特殊な物理空間でのさらなる看護、監視、および患者ケアを必要とするからである。退院後、患者は家まで送ってもらう必要があることもたびたびあり、成人は、少なくとも1日仕事ができなくなる。最後に、鎮静処置に対する患者の許容度は、非鎮静処置に比べて低い。鎮静の出費以外で、変えることができる他の上部内視鏡検査コストは、医師の時間および生検処置を含む。食道好酸球の評価は、その処置を看護婦または技師が行うことができれば安価になることは疑いない。生検は、標本あたり約150ドルのコストがかかる可能性がある。これらの因子を考慮すると、上部内視鏡検査の総コストの最大で半分は鎮静によるものとなり得る。
内視鏡生検の高いコストと不便さといった制限を克服するために、食道好酸球の代用物となり得る血液バイオマーカーが活発に研究されている。この疾患の病態生理学の研究から、EoEが、IL−5およびIL−13で媒介され、エオタキシン−3(Eotaxin−3)に依存して、TH2炎症プロセスから生じていることが示されている。これらのサイトカインに加えて、末梢血中好酸球、血清IgE、CD23、および好酸球由来神経毒も、生検好酸球レベルに関する有望なバイオマーカー代用物として提案されている。これらのバイオマーカーとEoEのいくらかの相関は分かっているが、この分野の研究は初期段階であり、これらの試験のコスト、ロバストなカットオフ値、ならびに年齢、症状、および性別の影響など、重要なパラメータはまだ分かっていない。特に他の共存するアトピー疾患を患っていると考えられる患者における食道好酸球に対するこれらのバイオマーカーの特異性が、療法決定を導くのに十分なものであるかどうか判断することが好ましいことがある。
例えば、食道への経鼻および経口到達は、成人および小児患者集団どちらに関しても、鎮静なしで外来環境で達成することができる。非鎮静患者にカテーテルを通すとき、一般に、経口処置で生じる咽頭反射のほうが激しいので、経口手法よりも経鼻到達のほうが許容度が高い。経鼻胃管(NG管)挿入など標準的な経鼻処置は、年間で数百万人の患者に行われており、主な合併症は、あったとしても年間数例である。NG管挿入では、最初に、直径10〜18F(3〜6mm)の可撓性管を、鼻孔から剣状突起までの寸法に合わせる。次いで、麻酔ジェルで潤滑し、患者の鼻孔の一方に挿入する。鼻孔は、局所リドカインスプレーで事前に麻酔しておく。管を、後方に向けて下へ進める。管が後咽頭を通るとき、咽頭反射を抑えるためにストローで水を飲むまたはすするように患者に要求する。管は、咽頭を通るとき、食道を通って胃まで容易に進められる。管が胃の中にあることの確認は、管を通して空気を注入し、聴診器で聴診する、または吸入された流体のpHを測定することによって達成することができる。次いで、管を食道内に引き戻すことができる。処置後、患者は通常、普段の活動をすぐに再開することができる。
反射型共焦点顕微鏡法(RCM)は、EoE患者での食道好酸球を撮像するのに適していることがある。なぜなら、この顕微鏡は非侵襲性の光学撮像法であり、約1μmの分解能で細胞構造の視覚化を可能にし、また外因性造影剤の投与を必要としないからである。RCMは、組織からの多重散乱光を拒否または無視し、強集束ビームから反射される光の共焦点選択を採用することによって、構造情報を含む1回後方散乱光子を検出する。最も一般的には、RCMは、集束されたビームを組織表面に平行な平面内で高速走査することによって実施され、組織の横方向画像または正面画像が得られる。RCMで使用される対物レンズの大きい開口数(NA>0.3)が、非常に高い空間分解能を生み出す。RCMの撮像侵入深さ(数百ミリメートル)は、典型的には管腔表面付近に現れるEoEの検出によく適している。
今日まで提案されているすべての内臓共焦点顕微鏡撮像診断は、内視鏡によって案内しながら離散的な位置でのみ顕微鏡画像を提供する。これは、いわゆる「点サンプリング」法である。点サンプリングは、RCMにおいて避けられないものであり得る。なぜなら、RCMは、非常に限られた視野(約200〜500μmの範囲内)を有するからであり、この視野は、EoE診断に使用されるピンチ生検よりも小さい。本明細書で提案する経鼻手法によって、内視鏡による案内なしで診断プローブを挿入することができ、データを自動的に獲得することができる。したがって、この「案内なし」の方法が、増加した好酸球を含んでいる、全体的な疾患状態を代表する領域からの情報を確実に捕捉できるようにするために、食道の比較的大きい領域を撮像すべきである。
この動作形態は、パラダイムシフトを必要とすることがあり、これは「包括的体積顕微鏡法(CVM)」と呼ぶことができ、すなわち、大きな粘膜領域の上皮の顕微鏡画像を3次元で得ることができる機能である。現在、この分野での他の研究者が、このコンセプトの価値を認識している。興味深いモザイク手法が開発されており、複数の共焦点画像を一体に継ぎ合わせて数mm2をカバーするが、この技術は、好酸球に関して食道の十分な長さ(10+cm)を走査するのに必要とされる包括的な撮像にはまだ遠い。
これまで、食道好酸球のRCM装置および方法の例示的実施形態は述べられていなかった。そこで、本開示の例示的実施形態によれば、図1に示されるように、そのような細胞100をRCMによって高いコントラストで視覚化することができる。例えば、この機能に関する物理的な基礎は、おそらく、大きなサイズ、高い屈折率、および好酸球の細胞質内部の粒子の存在度に主に関係付けられる。細胞からの光散乱のモデル化によって、高い屈折率勾配を有する大きい細胞小器官がより強い光学的後方散乱、したがってより強いRCM信号を示すことを示すことができる。好酸球の細胞質中に存在する粒子の3つの主要なタイプのうち、主要な塩基性タンパク質、好酸球ペルオキシダーゼ、好酸球カチオンタンパク質、好酸球由来神経毒を含めた「特異性」晶質粒子が最も多いことがあり得る。これらの晶質粒子は、直径約0.5〜1.3μmであることがあり、他の顆粒球で見られる直径よりもかなり大きい。これらの粒子の屈折率は直接測定されていないが、中に含まれる晶質タンパク質の密度およびタイプにより、例えば約1.6に近付くほど高いことがあり得る。細胞質の屈折率は1.35〜1.37であるので、これらの粒子によって誘発される屈折率勾配は非常に大きい。対照的に、扁平上皮細胞はそのような粒子を有さず、比較的均質な細胞質を有する。その結果、好酸球の細胞質は、周囲の食道上皮よりも強く光を後方散乱することができ、高いRCM画像コントラストをもたらす。粒子に加えて、核も、それらのサイズおよびクロマチンの屈折率(1.39〜1.45)により、RCM信号を発生することができる。したがって、好酸球はさらに、それらの2葉の形状など核形態学的な手がかりを利用することによってRCM画像内で区別することができる。
RCM装置および方法は、それを皮膚に適用する場合については述べられているが、内臓撮像のための小さい共焦点顕微鏡プローブは、走査顕微鏡の小型化に関連する技術的な課題により開発されていなかった。1つの難点は、小さい直径の可撓性プローブの遠位端で、集束されたビームを高速でラスタ走査するためのメカニズムの開発であり得る。この問題に対処するために、遠位マイクロ電気機械システム(MEMS)ビーム走査デバイスの使用やシングルモードファイバ束の近位走査を含めた様々な手法が試みられている。別の課題は、光学的な区分化に使用される高いNAの対物レンズの小型化であり得る。
本明細書では、食道生検研究からの結果、OFDI臨床食道撮像デバイスの説明、および経鼻的SECMプローブの実装に必要とされる重要な構成要素を組み込むベンチトップSECMプローブプロトタイプを含めて、好酸球計数用の経鼻SECMデバイスの実現可能性を説明する。
図2に、単一光ファイバ共焦点顕微鏡構成を撮像に利用するSECMシステム/装置の例示的実施形態を示す。特に、図2に示す例示的なシステム/装置は、広帯域幅または波長掃引光源200を含み、この光源200からの光がカプラまたはサーキュレータ210に入力されて、光スペクトルの空間情報の1次元を符号化する。例示的なシステム/装置のプローブの遠位端で、シングルモードまたは二重クラッドファイバ230のコアからの出力が透過回折格子240を照明する。対物レンズ250が、標本270、組織、または別の解剖学的構造、またはそれらのうちの複数の内部における異なる空間位置260に対して1つまたは複数の電磁放射(例えば光)の各回折波長を合焦することができる。組織で反射した後、電磁放射は、レンズ250を通って戻って進み、格子240によって再結合され、ファイバ230によって収集される。ファイバの開口が空間フィルタリング構成/構造を提供して、焦点がずれている電磁放射を拒否またはフィルタ除去する。
本開示によるSECM技法および/または構造を利用するシステムおよび方法の例示的実施形態を使用して、形成異常および初期段階の腺癌を含めたバレット食道を診断することができる。診断に関するSECMの精度を決定するために、組織病理学のための切除した上部GI生検試料をSECM撮像することが可能であり、これもまた好酸球の視覚化を容易にする。
食道の大きな領域を撮像するための例示的な光コヒーレントトモグラフィまたは例示的な光学周波数領域撮像プローブを提供することができる。図8A〜図8Dは、大きな食道粘膜領域を撮像するために利用することができる、本開示によるいくつかの例示的な設計および構成を示す。例示的なカテーテルの例示的な一形態は、光ファイバおよび遠位光学系を組み込むことができる内部ケーブル(図8Aおよび図8B)を含むことができる。このケーブルは、外側透明シース内部で回転および並進(移動)することができ、食道のヘリカルスキャンを行う。図8Aに示されるように、吹送されていない食道の管腔への挿入時、食道がプローブの周りにしぼむことがあり、したがって、例えば対象の食道表面積全体の約10%を撮像できるようになる(図9C)。本開示によるシステム、装置、および方法の別の例示的実施形態では、センタリングバルーンを透明シース内部または透明シース上に提供することができる(図8Cおよび図8D)。バルーンは、食道の拡張を促すことができ、全周を撮像することができるように光学系を中心合わせすることができる(図8Cおよび図10B)。また、本明細書で上述したカテーテル構成の例示的実施形態を、経鼻で実装することができるように、および大きい食道領域の細胞レベル解像度の画像を得るためのSECM撮像を行うために構成することができるように変形することもできる。

本開示によるプローブの例示的実施形態は、遠位食道の包括的SECM撮像を容易にするために提供することができる。そのような例示的なプローブ(例えば図11Aおよび図11B)では、ファイバ結合型の2.0mWスーパルミネセントダイオードを提供することができ、これは、800nmに中心を合わされ、45nmの帯域幅を有し、50/50シングルモード光ファイバビームスプリッタを照明する。スプリッタの1つのポートを通して伝送された光を、合焦装置を通して格子−レンズ対(例えば、レンズ−350230−B非球状、f=4.5mm、開口径=5.0mm、NA=0.55)にコリメートして伝送することができ、それにより、円筒形の試料の内面に、合焦されたスペクトル符号化されたスポットの500μm長手方向直線アレイを生成する。例示的な格子−レンズ対を、カスタム加工されたハウジングによってモータ(例えば1516SR、直径15mm)のシャフトに取り付けることができる。モータが回転したとき、試料の内周にわたって、スペクトル符号化されたラインを走査した。同時に、モータ、ハウジング、およびレンズ−格子対を、ステージによって円筒体の長手方向軸(z)に沿って並進(移動)させて、試料の内面全体のヘリカルスキャンを生み出すことができる。試料で反射された光を、光学システムを通してシングルモードファイバに戻し、ファイバによって、カスタム構成された分光計および直線型CCD(2048ピクセル、30kHzラインレート)に送った。1.0μm円周方向サンプリングを実現するために、例えば、モータ1回転につき約60,000点(0.5Hzまたは30rpm)をデジタル化することができる。モータの長手方向速度は、約0.25mm/sとすることができる。約2.0cm(直径)×2.5cm(長さ)の領域の1回の完全なスキャンにかかる時間は100秒とすることができる。
好酸球の定性的識別および好酸球の自動的な定量化のための例示的な実施形態は、信号強度(図5A〜図7F)および従来の組織学的特徴(例えば細胞のサイズ、細胞質の性質、および核形態)に基づく識別を含むことができる。定量的な形態学的基準は、散乱強度、細胞サイズ、ならびに自動化された組織病理学および細胞病理学検査のために開発された核測定基準(nuclear metrics)に基づくことがある。図13に示されるように、これらのパラメータに基づいて例示的な分割を行うことができ、従来のブロブ計数法(blob counting methods)を使用して細胞を計数することができる。計数法のロバスト性を最適化するために、複数の深さ平面を使用することができる。SECM画像内で他の炎症性細胞(例えばマスト細胞、リンパ球、好中球)を識別する場合には、これらの細胞タイプを識別するために、やはり信号強度および核形態に基づく基準を使用することができる。炎症性細胞に加えて、基底細胞過形成、微小膿瘍、脱顆粒、基底膜線維化を含めたEoEの他の組織病理学的特徴の特性も評価および定量化することができる。これらの特徴は、EoE診断のためのスコアリングシステムを形成するために収集することができる。例えば図13に示されるように、本開示の1つの例示的な実施形態によれば、組織を処理するために例示的なコンピュータを使用することができる。その後、パラフィンブロック作製を開始することができる。さらに、画像の区分化を行うことができる。これは、生成された画像を区分化することによって行うことができ、3次元形式で提供することができる。カバーガラスを染色することができ、染色されたスライドを保存することができる。
一般に、病理学者は、断面向きで生検を観察することができる。図14A〜図14Cに示されるように、生検の横軸に沿ってSECMおよび組織学画像および/またはデータを取得することができ、この横方向断面は、断面平面に対して垂直でよい。試料内への深さに応じて複数の横方向断面を得ることができるので、断面SECMおよび組織学画像を再構成することができる。本開示の1つの例示的な実施形態では、この例示的な断面領域は、組織学的な高倍率領域(例えばHPF、40倍)と同じ横方向寸法とすることができる。本開示の例示的実施形態の例示的実装では、生検から得られたSECM好酸球数を組織病理学的な好酸球数と比較して、2つの方法の間の高い相関性(図14D参照)、および約15を超えるeos/HPFの好酸球数カットオフ値に基づくEoEの診断の高い精度を見出すことができる。
本開示の例示的実施形態による経鼻デバイスの概略図が図15に示されている。例示的なデバイスは、SECM撮像プローブ1530を含むことができ、このプローブ1530は、透明な感圧経鼻胃(PS−NG)管1510の主管腔1500の内部に提供することができる。例示的なSECMプローブ1530は、内部トルク伝達ケーブル1520の内部にある光ファイバと、遠位撮像光学系1520とを含むことができる。PS−NG管1510は、標準的な10F(3.3mmOD)NG管と寸法的および機械的に同一または同様でよい。この例示的な外径を使用することができるのは、成人および小児集団において不快感なく使用するのに十分に小さいからである。例示的なプローブの複雑さを最小限にするために、1つの例示的な実施形態によれば、例示的なデバイスは、センタリングバルーンを除くことができ、吹送されていない食道1550内でヘリカルスキャン1540を行うことができる(図8A、図9C)。PS−NG管1510の例示的な直径に基づき、この例示的な構成は、対象の食道表面積全体の約10%の撮像を容易にする(例えば図9C)。
油圧センサ、ソリッドステートセンサ、圧電センサ、および光センサを含めた、PS−NG管の壁に組み込むことができる様々なタイプの圧力センサがあり得る。光圧力センサを使用することが好ましいことがある。なぜなら、光圧力センサは、非常に小型にすることができ、正確であり、ロバストであり、PS−NG管を通した流体および電流の伝達を必要としないからである。1つの例示的な光圧力センサは、図16に示されるように光ファイバ1600の先端に配置されたファブリ−ペロー隔膜フィルタである。隔膜1625からの第1の反射1610と第2の反射1620の干渉を、白色光または低コヒーレンス干渉法を使用して検出することができる。スペクトル分解された干渉縞の周波数は、キャビティの幅Ls1630の決定を容易にすることができ、次いでこれを使用して圧力1640を決定することができる。
本開示によるSECMプローブの例示的実施形態の詳細な概略図が図17に示される。例えば、内部巻線ケーブル1700が、撮像光を送受信する二重クラッドファイバ(DCF)1705を取り囲むことができる。スペックルノイズを減少させるために、DCF1705のコアを介して試料を照明し、試料からDCF1705の内側クラッドを介して伝送されて戻る光を受信することによって撮像を達成することができる。内部巻線ケーブル1700は機械的なハウジング1710に取り付けることができ、ハウジング1710は遠位光学系を含むことができる。機械的なハウジング1710は、PS−NG管1720の内腔から光学系を隔離することができる薄い透明シース1715によって取り囲むことができる。近位端での巻線ケーブル1700の回転および並進(移動)により、約10cmの長さにわたってヘリカル撮像を容易に行うことができる。例示的なSECMプローブの近位端では、巻線ケーブル1700は、先行技術文献で説明されている回転接合部を使用してコンソールとインターフェースすることができる。
図18は、本開示によるSECMシステムコンソールの例示的実施形態の例示的なブロック図を示す。例えば、そのような例示的なシステムコンソールは、SECM撮像に必要な光源と検出器を含むことができる。1つの例示的実施形態では、高速波長掃引レーザ1800(中心波長=1300nm;帯域幅=160nm;繰り返し率=254kHzなどの仕様を有する)を撮像に利用することができる。レーザ1800からの照明光を回転接合部1810に送ることができ、回転接合部1810は、この光をSECM DCF1820のコアに結合させることができる。組織から伝送されて戻る光は、DCF1820の内側クラッドから回転接合部1810に向けて送ることができる。回転接合部1810からの光は、マルチモードファイバを通して、高速InGaAs光検出器1830に送ることができる。収集された光検出器1830からのデータをデジタル化し、高いサンプリングレートでコンピュータに転送し、データ記録システム1840(例えばSignatec DR350(Newport Beach,CA))にリアルタイムで保存することができる。圧力測定に関しては、感知モジュール1850が照明光を圧力センサに提供して、圧力センサからの戻りの光を、PS−NG管1870の壁に位置する個々の感圧光ファイバ1860を介して検出することができる(図15、図18)。別の例示的実施形態によれば、例示的なシステムは、広帯域光源と、スペクトル符号化された光を検出するための分光計とを備えることができる。
現在のレーザ技術によって、より広い帯域幅にわたってより高速の波長調整を容易に行うことができる。例示的なスクリーニング処置の合計時間を短縮することができる。従来のSECMシステムと比較して、本開示によるシステムの例示的実施形態は、例えば50倍を超える速さで撮像するように構成することができる(例えば254kHz対5kHzAのラインレート)。撮像速度を上げるために、新規のデジタルデータ獲得ハードウェアを組み込む高速動作のための例示的なレーザ構成を提供することができる。伝送ライン幅を狭めずに動作の自由スペクトル領域を大きくするために、例示的な波長掃引レーザの走査スペクトルフィルタの例示的な特性が検討されている。このようにフィルタを動作させると、レーザ掃引速度を倍増させることができ、放出のデューティサイクルを半分に短縮することができる。すなわち、レーザからの出力は、1回の高速の波長スキャンと、それに続く、放出のない同じ時間間隔とでよい。デューティサイクルを完了させ、繰り返し率の倍増を実現するために、直接のレーザ出力を、放出時間と等しい相対遅延を有する2つの経路に分割することができる。次いで、2つの経路からの光を、Huber他によって提案されているバッファリング法と同様にして再結合させることができる。この多重化操作での損失を補償するために、バッファ半導体光増幅器を組み込んで、平均出力パワーを例えば約80mWまで回復することができる。
表1は、例示的なSECMプローブおよびシステムのための例示的な仕様および目標(objective performance target;OPT)を示す。本開示のいくつかの例示的実施形態の目的に見合うOPTは、食道の内視鏡撮像に使用することができる共焦点顕微鏡の構成に基づくことができる。経鼻プローブの外径は約3.3mm(10F)、剛性部の長さは約10mmとすることができ、これは、小さな子供および成人に対する好都合な経鼻到達を容易にするはずである。約10cmの長手方向引き戻しスキャン長さは、標準的な内視鏡生検処置で利用される近位生検と遠位生検の間の典型的な距離に合致する。この例示的な設計における例えば0.46の対物レンズNAおよび例えば16のDCF内側クラッドモード数に関して、約1.4μmの横方向分解能および約13.5μmの軸方向分解能を実現することができ、これらの値は、本発明者らの予備研究において好酸球を視覚化することができることが実証されたベンチトップSECMシステムの値と同様であり得る(図5〜図7参照)。レーザ源の調整帯域幅および格子−レンズ構成に鑑みて、波長スキャン当たり10個の光学区域にわたって分散された例えば総計236点の解像可能な点を得ることができる。検出器の帯域幅は、全体の獲得速度に上限を課す。例えば60MHzの検出器の帯域幅を仮定すると、レーザ調整速度は254kHzとすることができ、ナイキスト周波数で食道を空間サンプリングすることが有益である場合には、データセットは約21Gを備え得る。したがって、254kHz×236×2点=120Ms/s=240MB/sの16ビットA/Dサンプリングレートを仮定すると、10個の1000mm2の光学区域を提供する1回の完全なヘリカルスキャンを行うのに、例えば約2.9分かかり得る。必要であれば、この総計の画像獲得時間は、例示的な設計の何らかの例示的な変更によって減少させることができる。
本明細書では最小の検出可能な反射率(SNR>1)として定義する感度は、画像品質および侵入深さに影響を及ぼす重要なシステムパラメータである。例えば、最大300μmの深さにある皮膚で入射光の10-4〜10-7が反射されることが示されている。皮膚は、食道の非角化上皮粘膜よりも光を大きく減衰するので、2つのシステム間での波長および対物レンズNAの差を考慮すると、組織内部130μmの位置で、SECMプローブの対物レンズが照明光の約3×10-3〜3×10-6を集光することができると推定される。したがって、例示的な光源が例えば80mWを放出することができ、例示的な最大10dBの2回通過挿入損失(例えばプローブから6dB、光ファイバおよび回転接合部から4dB)を仮定すると、反射される出力は、検出器上で約25μW(最大)〜25nW(最小)の範囲になると推定される。検出器でのノイズは、ショットノイズ、相対強度ノイズ(RIN)、および熱的/電気的ノイズからなることがある。検出される試料出力がより高いとき、本発明による掃引光源の狭いライン幅(約0.6nm)により、RINノイズがショットノイズよりも優勢である。図20は、本開示によるSECMシステム/装置の例示的実施形態に関するノイズ分析のプロットを示す。図20から分かるように、本発明による最大撮像深さで検出することを予想することができる、伝送されて戻る出力の全範囲にわたって、SNR>1である。より高い感度が必要であると判断された場合、検出器を冷却して、熱的なノイズを減少させ、最大約10dBの感度改良を実現することができる。
(PS−NG管)
例えば、外側に1cmごとに印を付けられた二重内腔10F(OD=3.3mm)経鼻管をカスタム押出成形することができる。可撓性と操作性の適切な組合せを決定するために、様々な壁厚、およびポリウレタン、ポリ塩化ビニル(PVC)、タイゴン、およびシリコーンを含めた様々な材料を提供することができる。挿入されたSECMプローブと共に、PS−NG管を、可撓性および追従性に関して試験することができ、市販のNG管と比較し、経鼻または経口挿入に適切な機械的特性を有することを保証することができる。さらに、本発明者によるSECMプローブをPS−NG内部で引き戻すことができる能力をさらに試験することもできる。
小さな直径の圧力センサは、被覆された、バッファされた、および裸のファイバを備えることができ、それらを可撓性および耐性に関して試験することができる。追従性および押しやすさ(pushability)を、内腔PS−NG管内に挿入されたセンサによって試験することができる。気密封止ファントム内部の様々な既知の圧力を使用して、管の外部および管の内部にあるセンサを用いて圧力測定値を試験および較正することができる。隔膜を保護する様々な材料を、安定性、滅菌性、および正確性かつ高信頼性を保って外圧を変換することができる能力に関して試験することができる。
SECMプローブを小型化するために、例示的な光学構成要素を提供することができる。プローブ直径および長さを最小にするために、例えばコリメートレンズ(例えばシングレット;BK7;OD=1.2mm;f=5mm)を作製することができる。PS−NG管を通る食道への非対称光路によって誘発される非点収差を補償するために、補償プレート(例えばLASF35;OD=1.2mm;f=233mm)を製造および使用することができる。SECM撮像に使用することができるスペクトル領域に関する回折限界光学性能を有するように、カスタム対物レンズ(ダブレット;LASF18およびLASF35;OD=1.2mm;f=1.1mm;NA=0.46)を作製することができる。1220nm〜1380nmの間のスペクトルで最大の1次回折効率を有するように格子(溝密度=1240lpmm)を設計および作製することができる。光学モデリングプログラム/ソフトウェアで例示的なプローブ光学系(図21A)を提供することができ、シミュレーション結果は、プローブ光学系が、視野にわたってほぼ回折限界性能を有することができることを実証する(図21B)。小型の光学構成要素は、直径0.5mmで以前に開発されている。焦点距離での伝送および分解能をSECM帯域幅全体に関して試験することができる。横方向分解能は、基準エッジ標準の画像から抽出された線広がり関数(LSF)の半値全幅(FWHM)として測定することができる。軸方向分解能は、電動式の並進(移動)ステージによって走査されるミラーを撮像することによって測定することができる。ロンキールーリングを撮像することによって決定されるスペクトル視野は、横方向分解能がその所望のOPTを保つ視野長さとすることができる(表1参照)。
プローブアセンブリのための例示的な機械的ハウジングおよび透明シースを提供することができる。図22Aおよび図22Bは、本開示によるプローブアセンブリの例示的実施形態およびその分解図をそれぞれ示す。例示的な機械的ハウジングは、作製を容易にするために複数の部品から構成することができる。各部品の例示的な寸法は、約1mm〜7mmで異なることがあり、最小の要素のサイズは125μmでよい。これらの寸法を有する部品の機械加工は、厳密なコンピュータ数値制御(CNC)機械加工システム/装置で行うことができる。例示的な光学構成要素および機械加工部品を、3軸ステージを使用して厳密に位置合わせして、光学グレードのエポキシを使用して一体に組み立てることができる。その1つの例示的実施形態が図27に示されている。
例えば軸方向および横方向分解能OPTを満足するのに必要なコア内側クラッドモード比を含むことができる、様々な例示的なDCFを提供することができる。
例示的な多層巻線駆動シャフトを使用して、例示的なカテーテル設計内部の遠位光学系をヘリカルスキャンすることができる。運動変換精度および再現性のために、カテーテルを通してカスタム巻線ケーブルを提供することができる。例えば、多層構成を有する巻線ケーブルを提供して、SECM画像内の並進(移動)および回転歪みを減少するか、または最小限にすることができる。
SECMプローブとPS−NG管との間の潤滑剤として、生理食塩水を使用することができる。また、光学的性能をさらに改良するために、PS−NG管の屈折率により良く合致する比較的高い屈折率を有する様々な潤滑剤を試験することができる。
光学回転接合部(図19)を使用して、シングルモード光を伝送し、内側クラッドを通して伝送されるマルチモード光を検出することができる。回転接合部は、機械的なモデリングプログラムで設計して、光学モデリングソフトウェアでシミュレートすることができる。最大のスループットならびに製造の容易さおよび公差に関して、例示的な構成を最適化することができる。1回通過および2回通過スループットおよび回転一様性に関して、選択された例示的な構成を提供することができる。さらに、標準的な電動式引き戻しトレイの内部に適合するように回転接合部を提供することができる。
撮像レーザの例示的な実施形態は、出力、スペクトル、瞬時ライン幅、および繰り返し率に関して構成することができる。例示的な光学系は、適切なスループットおよび効率を得られるように提供することができる。カートに組み込むために、小さなブレッドボード上で光学レイアウトを組み立てることができる。
回転接合部を制御し、圧力センサからデータを読み取って表示するために、ソフトウェアを提供することができる。例えば、新たなボードを制御し、デジタル化されたデータを処理して画像にするために、Signatec PDA16データ獲得ソフトウェアをプログラムすることができる。ソフトウェアは、圧力センサの読取値のリアルタイム表示のために提供することができ、グラフィカルユーザインターフェースに一体化することができる。多重解像度SECM画像ナビゲーションおよび自動化された好酸球計数を、既存のコードおよびユーザインターフェースに組み込むことができる。
滅菌して再使用することができるように、本開示による経鼻プローブ構成の例示的な実施形態を使用することができ、これは、例示的なデバイス/装置、したがって例示的な監視処置のコストを下げる。例えば、外面を滑らかに欠陥なしで保つことができ、CIDEXまたはETO滅菌処置後に安定な材料を利用する。
経鼻処置が禁忌である特定の患者では、PS−NG管を経口で配置することができる。安全性の理由から、NG管が気道に入らないことが好ましいことがある。管の位置を検証するための例示的な手法は、吸入流体から酸性pHを求めることを含むことができる。流体吸入のためにPS−NG管の壁に別の内腔を設けることができる。あるいは、遠位端でPS−NG管を開き、SECM内腔を使用して流体吸入を行うことができる。この場合、SECMプローブとPS−NG管の間で追加のシースを使用する、またはSECMプローブを滅菌可能にすることができる。1つの例示的な実施形態によれば、食道の寸法および形状によっては、撮像領域の一部に関して、PS−NG管と食道壁の接触を保つことができないことがある。代替手法は、バルーンセンタリングSECMカテーテルを提供し、撮像のためにカテーテルをPS−NG管に通して導入するというものであり得る(例えば図8Cおよび図8D参照)。例えば、バルーンセンタリング食道カテーテルは、例示的なOFDI臨床研究で実証されている(図8Dおよび図10B参照)。図23Aおよび図23Bは、経鼻バルーンセンタリングSECMカテーテル設計の例示的実施形態の概略図、および本開示によるその動作モードを示す。これらの図面に示されるように、小さな直径(例えば2.5mm)のバルーンセンタリングSECMカテーテル2300を、PS−NG管2310(OD=3.3mm)を通して導入することができる。PS−NG管2310は、LES2320の近位で5cmの位置(2340)に位置決めされるとき(図23A参照)、約10cm(2350)だけ引き戻して、バルーン2330を露出させることができる(図23B)。バルーン2330を膨張させることができ(図23B)、10cmの長さにわたってSECM撮像を行うことができる。撮像後、バルーン2330を収縮させることができ、バルーンカテーテルをPS−NG管2300内に引き戻し、例示的なデバイス全体を患者から取り外すことができる。
角度研磨された円筒体に、エポキシベースの格子を刻むことができる。
一連の円周方向スキャン中に大きな長手方向移動が発生することがある場合、スキャンの位置が整わないことがある。SECMデータセットを再構成する前に、隣接するスキャンに対する相関技法を使用することによって、この発生を補正することができる。
SECMプローブの測定深さは、例えば約130μmとすることができる。EoEの好酸球浸潤は典型的には管腔表面の近くに現れることがあるので、この深さにわたって得られる画像が、正しい診断を行うのに十分な好酸球の分布に関する情報を提供すると考えられる。より大きな撮像侵入深さが好ましいと判断される場合、対物レンズの色収差を増加するために例示的な設計を変更することができ、これは測定深さを増加する。代替手法は、フレネル対物レンズを利用するものでよい。また、撮像侵入深さを増加させるこれらの構成は、撮像することができる異なる深さ位置の数を増加させることもでき、これにより、場合によっては獲得時間が長くなる。
正確な好酸球計数を行うために断面撮像が使用され、DCFを介するマルチモード検出が、断面再構成画像上で好酸球を視覚化するのに十分に高い軸方向分解能を提供しないことがある場合、より少数のモードを有する別のDCF構成を使用することができる。例えば、モードの数を約16から10へ減少させることで、スペックルコントラストは約6%しか増加せず、その一方で軸方向分解能を27%改良することができる。
例示的な構成は、約10個の異なる光学区域で、食道の例えば1000mm2の面積を約2.9分で走査することができ、これは、成人患者が十分に耐えられるものである。この撮像時間が長すぎると判断される場合、光学区域の数を減少させる、データを臨床サンプリングする、または引き戻し長さを減少させることによって撮像時間を短縮することができる。例えば、総撮像時間を減少させるための1つの手法は、例えば近位食道および遠位食道において2つの1cmの長手方向区間を撮像するものでよく、これは、獲得時間を約30秒に短縮することができる変更である。
例示的な非鎮静経鼻SECMは、食道好酸球を計数するための内視鏡生検と同程度の精度にすることができる。これは、EoEに関する評価を受ける例えば300人の患者において、非鎮静経鼻SECMを上部内視鏡生検と比較することによって試験することができる。さらに、例示的なSECM処置の患者許容度に関する情報を得ることができる。
鎮静および上部内視鏡検査の前に、本明細書で説明する例示的な経鼻SECMプローブおよびシステムを使用して、多数またはすべての患者を撮像することができる。本開示による代替実施形態は、本明細書で説明する経口プローブを利用することができる。食道内圧による案内を使用する例示的な挿入法は、本明細書で上述したものと同様または同一でよい。プローブがGEJから約5cmの位置に配置されると、ヘリカルSECM撮像が開始される。例示的なプローブ内部の例示的な光学系構成は、遠位食道から近位食道へ例えば約10cmの長さにわたって、約0.5mm/秒の速度でヘリカルスキャンで引き戻すことができ、これにより例えば約2.9分の引き戻し時間となる。例示的なSECMデータをリアルタイムで処理して、引き戻しが完了した直後に閲覧して、データが診断品質であることを確認することができる。経鼻プローブの例示的な光学系構成を位置決めし直すことができ、必要な場合または望まれる場合には例示的な撮像処置を繰り返すことができる。例示的な撮像処置の後、次いで、例示的な経鼻プローブを患者から取り外すことができる。本明細書で説明する方法に従って、患者、撮像領域、あるいは1つまたは複数の高出力領域に関して好酸球を識別して計数することができる。次いで、定性的なユーザ適用基準、半自動、または自動画像処理構成を使用して、SECMまたはRCMデータセットに対して、基底層過形成、脱顆粒、膿瘍、基底膜線維化を含めたEoEの他の特徴を表すことができる。その後、この情報を使用して、EoEの診断を行う、または患者の診断ステータスに関する追加情報を提供することができる。

- 前記管腔器官が、食道または肺気道の少なくとも一方である、請求項5に記載の装置。
- 前記少なくとも1つの第1の構成または前記少なくとも1つの第2の構成の少なくとも一方がカテーテル内に提供される、請求項1に記載の装置。
- 前記カテーテルが、
(i)経口または経鼻で挿入されて前記組織に達するように構造化される、
(ii)(a)5mm未満の断面直径を有するまたは(b)体内に挿入される実質的に可撓性の部分を有する、
(iii)バルーン構成、巻線ケーブル、または体内の前記組織の圧力を測定するように構成されたさらなる構成を含む、
(iv)経鼻胃管内に進められるように構成化される、の少なくともいずれか1つである、請求項7に記載の装置。 - さらに、体内の組織の圧力を測定するように構成されたバルーン構成を含み、該バルーン構成が自動合焦構成を含み、該自動合焦構成が、前記少なくとも1つの部分に自動合焦するように構成される、請求項8に記載の装置。
- 前記少なくとも1つの特定の細胞が複数の特定の細胞であり、かつ、前記少なくとも第3の構成が、(i)前記特定の細胞の数を決定するようにまたは(ii)前記特定の細胞の数を自動的に計数するようにさらに構成される、請求項1に記載の装置。
- 前記第1の構成または前記第2の構成の少なくとも一方が、
(i)前記第1の電磁放射または前記第2の電磁放射の少なくとも一方をそれぞれスペクトル分散するように構成されたさらなる構成を含む、または、
(ii)複数の導波領域を有する少なくとも1つの光ファイバ構成を含む、請求項1に記載の装置。 - 前記少なくとも1つの光ファイバ構成が二重クラッドファイバコアを含む請求項11に記載の装置。
- 前記少なくとも1つの第1の構成が、広帯域の光または周波数が時間と共に変化する光の少なくとも一方を伝送するように構成される、請求項1に記載の装置。
- 前記少なくとも1つの第3の構成が、
(a)基底層過形成、膿瘍、好酸球脱顆粒、または基底膜線維化の少なくとも1つを他の細胞から区別するようにさらに構成される、または
(b)前記少なくとも1つの特定の細胞の核の形状に基づいて前記少なくとも1つの特定の細胞を区別するようにさらに構成される、請求項1に記載の装置。 - 前記少なくとも1つの第3の構成が、さらに、前記第2の電磁放射に基づいて、前記少なくとも1つの部分内で、好酸球、マスト細胞、好塩基球、単核球、または好中球の少なくとも1つである少なくとも1つの特定の細胞を他の細胞から区別するように構成されている、請求項1に記載の装置。
- 前記第1の構成又は前記第2の構成のうちの少なくとも1つは、(a)前記第1の電磁放射の各波長が該少なくとも1つの部分内で別個の横断方向及び軸方向の位置を照明するように、ある角度にて前記少なくとも1つの部分を照明する、又は(b)前記角度にて提供された前記少なくとも1つの部分からの戻り放射を検出する、の少なくとも1つを実行するように構成されている、請求項1に記載の装置。
