この発明は、病院薬局等で用いられる薬袋印刷装置に関し、詳しくは、薬袋に納めるべき薬剤に関する処方情報を薬袋へ印刷するに際して配合に関係する関連表記も行う技術に関する。

【従来の技術】
薬袋印刷装置には、薬袋に対して印刷を行うプリンタに加えて、その印刷情報を処方情報から作成する情報処理装置が設けられており、この情報処理装置には、処方箋やホストコンピュータ等から処方情報を入力するためのキーボードや読取装置あるいは通信装置等の入力手段に加えて、ハードディスク等の記憶手段も付設されている。その記憶手段には、種々の薬剤に関する既知情報が予め記憶されており、例えば薬品マスタファイルやその派生ファイルには薬品コードや薬品名などが保持されている。

そして、処方情報が入力されると、それに含まれる患者名や,薬品名,用量などの一連の情報が、印刷に適合した形式の印刷情報に変換されて、薬袋に印刷される。その際、薬品マスタファイル等を参照することで、処方指示が薬品コードでされていればそれを薬品名に直したり、処方指示が力価でされていればそれを用量に直したり、といった変換処理も行われる。さらに、薬品マスタファイル等に各薬剤の説明文も含まれている場合には、その説明文も付加して薬袋に印刷される。

また、自動調剤機器のうちには、不適切な薬剤の処方をより確実に防止するべく、配合禁忌に関する既知情報を保持していて、調剤動作に先だち、処方箋や調剤データにて指示された薬剤について配合しても良いか否かを判定したり表示したりするものも有る。

【発明が解決しようとする課題】
そこで、調剤時だけでなく施用段階でも不適切な薬剤の使用を確実に防止すべく、薬袋印刷装置にて薬袋に処方情報等を印刷する際に、配合の可否を付記するようにすることも考えられる。
しかしながら、一般の配合禁忌は薬剤の対ごとに確認され確定するものであるのに対し、一部の薬剤には、配合の可否が用量等によって変わったり、配合が可能であっても配合の順序に制約が伴っていたりするものも有る。例えば、酸性状態で有効な注射薬とアルカリ性状態で有効な注射薬とを配合した場合、混合後の酸アルカリの程度を示すペーハー値(pH値、指標値)が何れの有効範囲にも納まっていれば良いが、そうでなければ薬効が損なわれてしまう。かかる薬剤は種類も配合条件も多岐に亘るので総ての対ごとに配合結果を確定しておくのは困難である。

また、そのような注射薬は生理食塩水等の輸液で薄めて施用されることが多く、その場合、配合時に基本の作業手順の通り各注射薬を別々に輸液へ混入すれば良いが、緊急を要する場合など施用者等が配合作業の促進に気を取られて、うっかり確認せずに、薄める前に注射薬同士を混合してしまう虞がある。
このため、薬袋に対する印刷を行うに際して配合の可否を付記するだけでなく、上述のような場合でも役に立つ情報を表記することが望まれる。
そこで、配合時の酸アルカリの変動に起因する薬剤の配合変化を確認するのに要する施用者等の負担を軽減すべく、薬袋の印刷に更なる工夫を凝らすことが技術的な課題となる。

この発明の目的は、配合変化を容易に確認しうる情報を薬袋に印刷する薬袋印刷装置を提供することである。

請求項1の薬袋印刷装置は、薬袋に対して印刷を行うプリンタと、処方情報を入力する 入力手段と薬剤に関する既知情報を保持する記憶手段とを有し前記入力手段にて入力した処方情報を前記既知情報に基づき印刷情報に変換して前記プリンタに送出する情報処理装置とを備えた薬袋印刷装置において、前記既知情報には、複数の薬剤について配合の可否を示す情報に加えて薬剤自体の酸アルカリ度を示す自己pH値と酸アルカリの有効範囲を示す指標値が含まれており、前記情報処理装置は、前記処方情報にて指示された複数の薬剤について夫々配合が可であるか不可であるかの判定を行う配合不可確認手段と、この配合不可確認手段で配合可と判定された複数の薬剤のうち2種類の薬剤の組み合わせすべてについて、夫々、前記処方情報で指示された用量と前記自己pH値とに基づいて配合時のpH値を算出すると共にこのpH値を該当する薬剤の酸アルカリの有効範囲と比較することにより、pH値の変動の適否を判定する酸アルカリ変動確認手段とを備えたことを特徴としている。

この薬袋印刷装置においては、印刷情報が情報処理装置によって作成されプリンタによって薬袋に印刷されるが、その印刷情報を情報処理装置が薬剤の既知情報に基づいて処方情報から変換作成する際に、情報処理装置の酸アルカリ変動確認手段によって、既知情報に予め含まれている薬剤毎の薬剤自体の酸アルカリ度と酸アルカリの有効範囲が参照され、それに基づいて、処方情報にて指示され配合可と判定された複数の薬剤のうち2種類の薬剤の組み合わせについて、配合時のpH値がこの配合に供する薬剤の酸アルカリの有効範囲から外れる可能性が有るか否かの判定がなされる。

これにより、その判定結果を印刷情報に反映させることが可能となり、そうすることで、薬袋には、配合の可否とは異質な配合変化の確認に資する情報が印刷されることとなる。具体的には、施用者等が薬袋から注射薬等を取り出す際に薬袋を見れば自然に、配合時の酸アルカリの変動に起因する薬剤の配合変化を確認するよう、施用者等の注意が喚起される。また、配合変化の判定を処方対応の印刷の度に動的に行うようにしたことにより、配合変化の確認のために既知情報として備えて置くべき情報が個々の薬剤ごとの指標値等で足りることとなり、その結果、他の多くの薬剤との関係を予め確定して多量の情報を準備しておく必要が無く、新薬の追加等に際してのデータ更新も容易になる。
したがって、この発明によれば、配合変化を容易に確認しうる情報を薬袋に印刷しうる薬袋印刷装置を実現することができる。
請求項2の薬袋印刷装置は、薬袋に対して印刷を行うプリンタと、処方情報を入力する入力手段と薬剤に関する既知情報を保持する記憶手段とを有し前記入力手段にて入力した処方情報を前記既知情報に基づき印刷情報に変換して前記プリンタに送出する情報処理装置とを備えた薬袋印刷装置において、前記既知情報には、複数の薬剤について、配合の可否を示す情報に加えて薬剤自体の酸アルカリ度を示す自己pH値と酸アルカリの有効範囲を示す指標値と、配合時に他の薬剤による酸アルカリの影響の受け易さを示す緩衝能情報が含まれており、前記情報処理装置は、前記処方情報にて指示された複数の薬剤について夫々配合が可であるか不可であるかの判定を行う配合不可確認手段と、この配合不可確認手段で配合可と判定された複数の薬剤のうち2種類の薬剤の組み合わせのすべてについて、夫々、一方の薬剤の自己pH値が他方の薬剤の酸アルカリの有効範囲に入っているか否かの判断を行い、否定判断された場合には、前記緩衝能情報に基づいて酸アルカリの変動について判断することにより、pH値の変動の適否を判定する酸アルカリ変動確認手段とを備えたことを特徴としている。
この薬袋印刷装置においては、情報処理装置の酸アルカリ変動確認手段によって、請求項1のような用量を用いずに、配合不可確認手段で配合可と判定された複数の薬剤のうち2種類の薬剤の組み合わせのすべてについて、夫々、一方の薬剤の自己pH値が他方の薬剤の酸アルカリの有効範囲に入っているか否かの判断を行い、否定判断された場合に、緩衝能情報に基づいて酸アルカリの変動について判断することができる。

請求項3の薬袋印刷装置は、請求項1又は2に記載の発明において、前記酸アルカリ変 動確認手段の判定結果に応じて、酸アルカリの変動に関して注意を促す情報を選択的に前記印刷情報に含ませることを特徴としている。
この薬袋印刷装置においては、配合時に酸アルカリの変動に起因する薬剤の配合変化を確認することが必要な薬剤を収容する薬袋には、注意を促す注意書きが明記される。これにより、施用者等は、薬袋から注射薬等を取り出す際、常態ではもちろん緊急時であっても自然に、酸アルカリの変動に起因する薬剤の配合変化を確認するようになる。したがって、この発明によれば、薬剤の配合変化を容易に確認することができる。

【発明の実施の形態】
このような解決手段で達成された本発明の薬袋印刷装置および薬袋について、これを実施するための具体的な形態を、以下の第1〜第4実施例により説明する。
図1〜図4に示した第1実施例は、上述した第1〜第4の解決手段を総て具現化したものであり、図5に示した第2実施例、図6に示した第3実施例、及び図7に示した第4実施例は、その変形例である。
なお、それらの図示に際しては、簡明化等のため、筐体パネルや,ベース,フレーム,ボルト等の締結具,ヒンジ等の連結具などは図示を割愛し、発明の説明に必要なものや関連するものを中心に図示した。

【第1実施例】
本発明の薬袋印刷装置の第1実施例について、その具体的な構成を、図面を引用して説明する。図1(a)は、全体構造を示すブロック図であり、同図(b)〜(d)は、それぞれ、配合不可ファイル,pH変動ファイル,配合注意ファイルのデータ構造を示している。

この薬袋印刷装置には、プリンタ50(印刷装置)と、その印刷情報を調製するマイクロプロセッサ30(情報処理装置の本体)とが設けられている。
プリンタ50は、薬袋20に対して印刷を行えるものであれば、転写方式やインクジェット方式あるいはその他いずれの印字方式のものであっても良く、単体でも良く(特開2000−116753号公報等参照)、複数台が並設されていても良い(特許第2905460号公報等参照)。薬袋20は、薬品収容可能で印刷も可能なものであれば、透明でも不透明でも良く、マチが付いていても付いていなくても良い(特開2000−116753号公報等参照)。

マイクロプロセッサ30は、プリンタ50のコントローラを兼ねていても良いが、ここでは独立して設けられ、パラレル方式のプリンタケーブルやUSBケーブル等にてプリンタ50と接続されていて、印刷情報をプリンタ50に送信して薬袋20に印刷させることができるようになっている。
また、マイクロプロセッサ30には、プログラム保持用メモリや、各種の薬剤に関する既知情報を幾つかのファイル41〜44に分けて保持している内蔵の又は外付けのハードディスク40(記憶手段)に加えて、処方箋10やその相当物に記載された処方情報を入力する入力手段として、図示は割愛したが、手入力用のキーボードや、機械読取用のOCR、バッチ処理用のフロッピー(登録商標)ディスク、オンライン処理用の通信ユニット、その他の入力装置の何れかが付設されている。

ハードディスク40の薬品マスタファイル41には、既知情報のうち一般的な情報が保持される。例えば、各薬剤毎にレコードが割り当てられ、各レコードには、薬品コードや,薬品名,その薬剤の一般的な説明事項などのデータが記録されるようになっている。

配合不可ファイル42には、各種の薬剤について配合の可否を示すために、既知情報のうち配合の禁止される薬剤同士の情報が保持される。例えば、配合不可となっている一対の薬剤毎にレコードが割り当てられ、各レコードには、その対の一方の薬品コードと他方の薬品コードとが記録されていて、配合不可の組み合わせが明示される。配合可能な薬剤同士に関するファイルまで積極的に作成しても良いが、ここでは、配合不可ファイル42に無い組み合わせは配合可とされるようになっている。

pH変動ファイル43には、配合禁止ではないが酸アルカリの変動に起因した配合変動の可能性が有る薬剤に関してその確認に必要な既知情報が保持される。具体的には、そのような薬剤毎にレコードが割り当てられ、各レコードには、該当する薬品コードに加えて、例えば、薄めるのに適した輸液であるか否かを示す輸液フラグや、薬剤自体の酸アルカリ度をペーハー値で示す自己pH値(試料pH)、配合時に他の薬剤による酸アルカリ変動の影響の受け易さを数値等で示す緩衝能、薬効を維持する酸アルカリの有効範囲を一対のペーハー値で示す指標値のうちの一方である下限pH(酸性pH、酸側変化点pH,酸側最終pH)、その対の他方である上限pH(アルカリpH、アルカリ側変化点pH,アルカリ側最終pH,塩基側最終pH)等のデータが記録されるようになっている。

配合注意ファイル44には、酸アルカリの変動に限らず他の配合変動の可能性が有る薬剤に関してその注意を促すのに必要な既知情報が保持される。例えば、配合時に注意を要する一対の薬剤毎にレコードが割り当てられ、各レコードには、その対の一方の薬品コードと他方の薬品コードとその組み合わせに対する注意文の文字列データ等が記録されるようになっている。

さらに、マイクロプロセッサ30には、入力手段にて入力した処方情報を既知情報に基づき印刷情報に変換してプリンタに送出するといった一連の情報処理をプログラム処理にて遂行するために、薬剤に関する既知情報のうち一般情報を処理する一般情報処理ルーチン31と、処方情報にて指示された薬剤について配合が可であるか不可であるかの判定を行う配合不可確認ルーチン32(配合不可確認手段)と、処方情報にて指示された薬剤について配合時に各々の酸アルカリの有効範囲から外れる可能性が有るか否かの判定を行うpH変動確認ルーチン33(酸アルカリ変動確認手段)と、酸アルカリの変動に関して注意を促す情報をpH変動確認ルーチン33の判定結果に応じて選択的に印刷情報に含ませる配合注意確認ルーチン34と、それらのルーチンにて調製された印刷情報をプリンタ50に送出して薬袋20に印刷させる印刷ルーチン35とがインストールされている。

また、マイクロプロセッサ30は、例えばpH変動確認ルーチン33等の処理によって、その判定結果に応じて選択的に薬剤の区分情報を印刷情報に含ませる処理や、処方情報を印刷情報に変換する際にやはりその判定結果に応じて薬剤毎の印刷情報の並びを調整する処理も行う。そして、これらの各ルーチン31〜35による一連の処理が、一の処方箋10の処方情報を入力する度に実行されるようになっているが、その処理の詳細は、以下の動作説明にて例示する。なお、この例では、簡単のため、全薬剤で緩衝能が同じであるとして説明する。

この第1実施例の薬袋印刷装置について、その使用態様及び動作を、図面を引用して説明する。図2は、薬剤の既知情報の具体例であり、(a)が配合不可ファイルのデータ例、(b)がpH変動ファイルのデータ例、(c)が配合注意ファイルのデータ例である。また、図3(a)は、処方情報の一例、図3(b)は、それに対応した印刷例である。さらに、図4(a)は、処方情報の他の例であり、図4(b)は、それに対応した印刷例である。なお、それらの図示に際し、薬剤A等の薬品名と薬品コードは、区別せずに、同じ文字「A」等で示した。

ここでは(図2参照)、5種類の薬剤A〜Eを処方する場合について詳述するが、それらに関する既知情報として、例えば薬剤Bと薬剤Cとの配合が不可であるとするとそれに対応して、配合不可ファイル42には薬剤B,Cの薬品コードを組にしたレコードが含められている。また、pH変動ファイル43では、薬剤Aのレコードにおいて、薬剤Aの特性に基づき、例えば輸液フラグが偽値「−」、自己pH値が「6」、緩衝能が「・」、下限pH値が「3」、上限pH値が「7.3」に設定される。他のレコードの各アイテムすなわち輸液フラグと自己pHと緩衝能と下限pHと上限pHについても同様に設定される。

例えば、そのアイテム順に列挙すると、薬剤Bのレコードには「−」,「9」,「・」,「6」,「10」が設定され、薬剤Cのレコードには「−」,「7.1」,「・」,「5.2」,「9.3」が設定され、薬剤Dのレコードには「○」(眞値),「7」,「・」,「2」,「12」が設定され、薬剤Eのレコードには「−」,「7」,「・」,「5」,「10」が設定される。さらに、例えば薬剤Aと薬剤Eとを配合したとき約1時間後に配合変化が発生しやすいという特質が判っていればそれに対応して、配合注意ファイル44には両薬剤A,Eの薬品コードと注意文「1時間後に極微黄変」とを組にしたレコードが含められる。これらの既知情報は、薬袋20の印刷に先だって予め、例えば装置の新規導入時やその後の取扱対象薬剤の追加作業時等に薬袋印刷装置の初期設定機能等を利用して、ハードディスク40に記録されている。

また、或る処方箋10(図3(a)参照)には、1番目の薬剤Aを250mg、2番目の薬剤Bを250mg、3番目の薬剤Cを250mg、4番目の薬剤Dを500ml、5番目の薬剤Eを250mgを同時期に注射するという処方指示が記載されていたとする。そして、その処方情報が薬袋印刷装置に入力されると、マイクロプロセッサ30で一般情報処理ルーチン31と配合不可確認ルーチン32とpH変動確認ルーチン33と配合注意確認ルーチン34と印刷ルーチン35とがその順に実行されて、作業用RAM等に割り付けられている印刷情報用記憶領域に印刷情報が作成され、それがプリンタ50に送られて薬袋20に印刷される(図3(b)参照)。

それらの処理について順を追って詳述すると、先ず、一般情報処理ルーチン31によって、患者名や薬袋No.等の一般情報に加えて、薬品リスト21のタイトル等の定形文が、印刷情報に含められる。また、配合不可確認ルーチン32によって、処方情報にて指示された薬剤のうち2種類の組み合わせ総てについて、その組み合わせが配合不可ファイル42に含まれているかどうかの検索が行われる。そして、含まれていなければ配合が可であると判定されるが、含まれていれば配合が不可であると判定され、不可の場合は、何れか一方の薬剤が薬剤毎の印刷情報の並びの最後に移される。すなわち、上から下に並ぶ薬品リスト21において最下行のところに印刷される。この場合、薬剤Cが最下行に来る。しかも、その行と直上の行との間に二本線等からなる仕切線23が印刷されるとともに、薬品リスト21において各薬剤毎に表示される可否マーク25の該当個所には配合不可のマーク「×」が印刷される。さらに、薬品リスト21の下方等の注記文24の欄にも、薬剤B,Cを特定した注意書き例えば「BC×配合不可」が印刷される。そのようなデータが配合不可確認ルーチン32によって印刷情報に含められる。

それから、pH変動確認ルーチン33によって、処方情報にて指示された薬剤総てについて、pH変動ファイル43の検索が行われ、輸液フラグが眞「○」の薬剤があれば、薬品リスト21の最上行に移される。この場合、薬剤Dが最上行に来る。そして、その行の可否マーク25には輸液であることを示すマーク「輸」が付記される。
また、pH変動確認ルーチン33によって、処方情報にて指示された薬剤のうち配合可と判定された2種類の組み合わせ総てについて、原液同士を仮に直接混合したときの酸アルカリのpH値がpH変動ファイル43の自己pH値や用量値に基づいて算出されるとともに、そのpH値がpH変動ファイル43の下限pH値および上限pH値による有効範囲に入っているか否かが判定される。

そして、有効範囲から外れていると判定された場合は、何れか一方の薬剤が薬品リスト21において上述の仕切線23の直ぐ上に位置する行のところに移される。この場合、薬剤Aと薬剤Bとを250mgずつ原液で混合するとpH値が「7.5」となり薬剤Aの上限pH値「7.3」を超えてしまうので、薬剤Aと薬剤Bは、酸アルカリの変動に関して注意を促すことが必要な組み合わせのものと判定されるとともに、この判定結果に基づいて薬剤Bが仕切線23の直上行に来る。しかも、その行と直上の行との間に一本線等からなる仕切線22(区分情報、区分表記)が印刷されるとともに、注記文24の欄にも、薬剤A,Bを特定した注意書き例えば「●同一シリンジ内pH注意1(A)2(B)」が印刷される。なお、そのような薬剤A,Bにも薬剤Eと同様に可否マーク25の該当個所には配合可のマーク「○」が印刷される。そのようなデータが印刷情報に含められる。

さらに、配合注意確認ルーチン34によって、処方情報にて指示された薬剤のうち2種類の組み合わせ総てについてその組み合わせが配合注意ファイル44に含まれているかどうかの検索が行われ、含まれている組み合わせが有れば、該当するレコードから注意文が読み出されて、それも印刷情報に含められる。この場合、薬剤A,Eの対が該当しているので、注記文24の欄に、薬剤A,Eを特定した注意書き例えば「AE1時間後に極微黄変」が印刷されるようなデータが含められる。

そのようにして調製された印刷情報は、印刷ルーチン35によってプリンタ50に送出され、プリンタ50によって薬袋20に印刷される。そして、出来上がった薬袋20の印刷面には、薬剤の順番が調製された処方情報(薬品リスト21)に加えて、配合による酸アルカリの変動に関する注意書き(注記文24)や、配合可能な薬剤同士を配合時の酸アルカリの変動に基づいて区分する区分表記(仕切線22)も、印刷されている。

それから、その薬袋20には各薬剤A〜Eが収容され、それが最終監査等を経て該当患者の治療に供され、施用時には薬剤A〜Eが薬袋20から取り出される。そして、注射や点滴を出来るだけ纏めて行うことで施用の迅速を図るために、可能な薬剤は配合されるが、その際、配合しようとしている薬剤を確認するために施用者等は薬剤容器のラベルと共に薬袋20の薬品リスト21も読む。すると、自然に、施用者等は薬品リスト21における薬剤の順番に配合作業を進めるので、この場合、薄め液となる薬剤Dを最初に取り上げ、次に薬剤Aを取り上げて薬剤Dと混合し、それから薬剤Eを取り上げてそれも混合することとなる。さらに、薬剤Bも取り上げてそれも混合するが、そのときには、自然に目に入ってくる仕切線22によって注意を促されるので、さらには赤色等で目立っている注記文24の注意書きによっても注意を促されるので、少なくとも薬剤Bを原液のまま薬剤Aの原液に混合してしまうという不所望な事態は例え緊急時であっても確実に回避される。同様に、薬剤Cについても、仕切線23や注記文24によって注意を促されるので、他の薬剤と配合するという不都合な事態が回避される。
