Abstract

本出願は、本明細書においてその全内容が引用により組み込まれている2004年7月15日出願の米国特許仮出願第60/588,406号の恩典を請求するものである。

本発明の様々な実施形態によれば、神経刺激が患者に印加されるか又は送出される方法を制御、調節、修正、及び/又は変調することにより、電力消費を改善又は低減することができ、及び/又は神経刺激効力を増大、保存、又はほぼ維持することができる。上に示すように、本明細書に説明する特定のシステム及び/又は方法では、1つ又はそれよりも多くの閾値下及び/又は閾値上振幅、レベル、又は強度で1回又はそれよりも多くの回数、神経刺激を印加又は送出することができる。閾値下刺激振幅は、患者が特定の種類の応答を示す最低又はほぼ最低試験刺激信号振幅の特定の画分又はパーセントに対応することができる。この応答は、運動、EEG信号のような内部で測定可能又は観察可能な信号に関する効果、患者が報告する感覚、及び/又は別の種類の応答のような外部で測定可能又は観察可能な応答に対応することができる。
様々な実施形態では、神経刺激負荷サイクルを制御、調節、又は修正することにより、電力消費を低減し、及び/又は神経刺激効力を維持又は増大させることができる。負荷サイクルは、考慮中の時間領域の間又はそれに対して装置が「オン」であるか、電力を消費するか、電源を消耗する時間のパーセントであると定めることができる。上に示すような時間ベースの時間領域、秒ベースの時間領域、及びサブ秒ベースの時間領域を含む様々な種類の時間領域を定めることができる。従って、一実施形態では、負荷サイクルは、所定の種類の時間領域に対して以下のように定めることができる。

様々な実施形態では、電力消費を低減することができ、及び/又は神経刺激効力には、場合によっては考慮中の1つ又はそれよりも多くの時間領域に対して1つ又はそれよりも多くの種類の刺激周波数特性を調節又は修正することによって影響を及ぼすことができる。実施形態の詳細に応じて、刺激周波数特性を修正すると、負荷サイクルが修正されることになるか又はそれに対応することができる。その結果、上述の特定の考察は、以下に説明する1つ又はそれよりも多くの実施形態に同一に、類似して、又は同様に適用することができる。
様々な実施形態では、電力消費及び/又は神経刺激効力は、考慮中の1つ又はそれよりも多くの時間領域に対して1つ又はそれよりも多くの刺激振幅特性(例えば、ピーク電流及び/又はピーク電圧レベル)を修正することによって影響を及ぼすことができる。様々な実施形態では、刺激振幅特性は、時間ベースの時間領域、秒ベースの時間領域、サブ秒ベースの時間領域、及び/又は別の種類の時間領域に対して変動することができる。
患者治療から見ると、振幅変動に伴う影響は、特定のパルス相又はパルスのサブ間隔の間に送出される電荷の量を考慮すると、負荷サイクル変動及び/又はパルス繰返し周波数の変動に伴う影響に同一、本質的に同一、類似する、同様、又はほぼ同様とすることができる。
神経刺激器は、1つ又はそれよりも多くの神経母集団に制御可能及び/又は治療的な方式でエネルギを付与することができる装置と見なすことができる。このようなエネルギは、神経膜電位に作用する、影響を及ぼす、又はそれを変化させることができる電気及び/又は磁気刺激信号を含むことができる。上述のように、刺激信号は、1組又は一連のパルス又はパルス列を含むことができる。ある一定の実施形態では、神経刺激が神経組織及び/又はそれに伴う膜電位に影響を及ぼす範囲又は平均範囲は、神経刺激強度として定めることができる。

神経刺激効力は、1つ又はそれよりも多くの刺激パラメータ値に依存する場合がある。例えば、神経刺激効力は、パルス繰返し周波数依存性とすることができる。更に、患者の神経性機能障害の性質に応じて、神経刺激効力は、パルス繰返し周波数に依存する方式で長時間中には低下又は減衰する可能性がある。すなわち、第1のパルス繰返し周波数又はパルス繰返し周波数範囲は、症状的な恩典の急速又はほぼ急速な開始に関連する場合があるが、恩典の持続時間又は半減期は、短期間か又は比較的短いであろう。第2のパルス繰返し周波数又はパルス繰返し周波数範囲は、症状的な恩典のゆっくりした又は遅延した開始に関連する場合があるが、恩典の持続時間又は半減期は長いであろう。第1及び第2のパルス繰返し周波数に対応する神経刺激効力は、本質的に同一とすることも異なるものとすることもできる。
本発明のある一定の実施形態に従って与えられる、印加される、又は送出される神経刺激は、1つ又はそれよりも多くの累積する、永続性、及び/又は半永続性神経機能効果を助ける及び/又は発生させることができ、及び/又は患者の脳内(例えば、1つ又はそれよりも多くの皮質領域内)に神経可塑性変化を促進し、及び/又は達成することができる。実施形態の詳細、及び/又は患者の神経性機能障害の性質、病状、及び/又は治療歴に応じて、1つ又はそれよりも多くのこのような効果及び/又は変化は、神経刺激がなくても永久的、本質的に永久的、永続的、ほぼ永続的、持続的、及び/又は幾分持続的とすることができる。追加的又は代替的に、1つ又はそれよりも多くのこのような効果及び/又は変化は、神経刺激を中断又は中止した後、限定範囲及び/又は限定時間間隔に存在し、場合によっては、治療プログラムの課程の間及び/又はその後に、存在する範囲及び/又は間隔が増大するようにする。神経刺激を中断又は停止した後に、限定又は増大する期間持続する神経機能効果により、その後に電力を低減するか又は周波数の小さい神経刺激を行うことで良好又は適切な症状的恩典をもたらすことができる可能性を増大させることができる。更に、ある一定の状況では、特に、持続的神経機能効果が発現する時に症状的恩典を小さい強度及び/又は小さい周波数の刺激で達成することができるために、持続的又はほぼ持続的な神経機能効果は、刺激信号に対する望ましくない神経適応又は神経適合を妨害するのを助けることができる。

Claims (44)
- 患者の神経性病状を治療する方法であって、
第1の組のパラメータを有する第1の波形で第1の時間領域にわたって電磁気刺激を患者の神経系に印加する段階と、
少なくとも1つのパラメータが前記第1の組の対応するパラメータと異なる第2の組のパラメータを有する第2の波形で第2の時間領域にわたって電磁気刺激を患者の神経系に印加する段階と、
を含むことを特徴とする方法。 - 前記第1及び第2の時間領域の少なくとも一方にわたって電磁気刺激を前記印加する段階は、閾値下レベルで電磁気刺激を印加する段階を含むことを特徴とする請求項1に記載の方法。
- 前記第1及び第2の時間領域の少なくとも一方にわたって電磁気刺激を前記印加する段階は、閾値下、閾値、及び閾値上レベルの少なくとも2つで電磁気刺激を印加する段階を含むことを特徴とする請求項1に記載の方法。
- 前記第1及び第2の時間領域の間で電磁気刺激の前記印加を中断する段階と、
前記第1の時間領域中の電気刺激による電力消費に比較して前記第2の時間領域中の電気刺激による電力消費を低下させるように該第2の組の前記少なくとも1つのパラメータを選択する段階と、
を更に含むことを特徴とする請求項1に記載の方法。 - 電磁気刺激の前記印加を中断する段階及び前記少なくとも1つのパラメータを選択する段階の少なくとも一方は、該電磁気刺激のための電力を供給する電源が有意に枯渇する前に行われることを特徴とする請求項4に記載の方法。
- 第2の時間領域にわたって電磁気刺激を印加する段階は、前記第1の時間領域に続く第2の時間領域にわたって電磁気刺激を印加する段階を含むことを特徴とする請求項1に記載の方法。
- 前記第1の時間領域にわたって電磁気刺激を印加する段階は、第1の位置で電磁気刺激を印加する段階を含み、
前記第2の時間領域にわたって電磁気刺激を印加する段階は、前記第1の位置と異なる第2の位置で電磁気刺激を印加する段階を含む、
ことを特徴とする請求項1に記載の方法。 - 第2の時間領域にわたって電磁気刺激を印加する段階は、前記第1の時間領域の開始後に開始されて該第1の時間領域の完了前に完了する第2の時間領域にわたって電磁気刺激を印加する段階を含むことを特徴とする請求項1に記載の方法。
- 電磁気刺激を印加する段階は、前記第1の時間領域内に入れ子になった複数の第2の時間領域にわたって電磁気刺激を印加する段階を含むことを特徴とする請求項1に記載の方法。
- 前記第1の時間領域は、時間ベースであり、前記第2の時間領域は、時間より短い期間に基づいていることを特徴とする請求項9に記載の方法。
- 前記時間領域の少なくとも一方の間に電磁気刺激を印加する段階は、非周期的に変動する方法で該電磁気刺激を印加する段階を含むことを特徴とする請求項1に記載の方法。
- 電磁気刺激を印加する段階は、カオス的に変動する方法で電磁気刺激を印加する段階を含むことを特徴とする請求項11に記載の方法。
- 電磁気刺激を印加する段階は、反復関数に少なくともほぼ対応する方法で電磁気刺激信号を印加する段階を含むことを特徴とする請求項11に記載の方法。
- 電磁気刺激を印加する段階は、無限級数の部分和に少なくともほぼ対応する方法で電磁気刺激信号を印加する段階を含むことを特徴とする請求項11に記載の方法。
- 電磁気刺激を印加する段階は、準安定アトラクタに少なくともほぼ対応する方法で電磁気刺激信号を印加する段階を含むことを特徴とする請求項11に記載の方法。
- 前記少なくとも1つのパラメータを選択する段階は、前記第1の時間領域中の刺激からもたらされる恩典を再開するか、少なくともほぼ維持するか、又は再開して少なくともほぼ維持するように該少なくとも1つのパラメータを選択する段階を含むことを特徴とする請求項1に記載の方法。
- 患者の神経性病状を治療するための器具であって、
患者の神経系の神経組織と信号通信して位置決めされるように構成された少なくとも1つの刺激器を有する刺激装置と、
信号発生装置と、
前記刺激装置と前記信号発生装置の間に作動的に連結された信号通信リンクと、
第1の組のパラメータを有する第1の波形で第1の時間領域にわたって患者の神経系への電磁気刺激の印加を指示し、かつ
少なくとも1つのパラメータが前記第1の組の対応するパラメータと異なる第2の組のパラメータを有する第2の波形で第2の時間領域にわたって患者の神経系への電磁気刺激の印加を指示する、
命令を前記信号発生装置に与えるように構成され、該信号発生装置と作動的に連結されたコントローラと、
を含むことを特徴とする器具。 - 前記信号発生装置は、閾値下レベルで電磁気刺激の印加を指示するように構成されていることを特徴とする請求項17に記載の器具。
- 前記コントローラは、前記第1及び第2の時間領域にわたる電磁気刺激を指示する記憶された命令を含むことを特徴とする請求項17に記載の器具。
- 前記刺激装置は、電極装置を含み、
前記少なくとも1つの刺激器は、少なくとも1つの電極を含む、
ことを特徴とする請求項17に記載の器具。 - 前記信号発生装置は、患者に植え込まれるように構成された植え込み可能パルス発生装置を含むことを特徴とする請求項17に記載の器具。
- 前記信号発生装置は、患者に植え込まれるように構成された植え込み可能ハウジングを含み、
前記ハウジングは、第1のパルス発生装置及び第2のパルス発生装置を担持している、
ことを特徴とする請求項17に記載の器具。 - 前記第1のパルス発生装置は、前記第1の時間領域にわたって電磁気刺激を送出するように構成され、前記第2のパルス発生装置は、前記第2の時間領域にわたって電磁気刺激を送出するように構成されていることを特徴とする請求項22に記載の器具。
- 前記第1のパルス発生装置は、患者の身体の第1の位置に電磁気刺激を送出するように構成され、前記第2のパルス発生装置は、患者の身体の第2の位置に電磁気刺激を送出するように構成されていることを特徴とする請求項22に記載の器具。
- 前記コントローラは、前記第1の時間領域に続く第2の時間領域にわたる電磁気刺激の前記印加を指示する命令を与えるように構成されていることを特徴とする請求項17に記載の器具。
- 前記コントローラは、前記第1の時間領域内に入れ子になった複数の第2の時間領域にわたる電磁気刺激の前記印加を指示する命令を与えるように構成されていることを特徴とする請求項17に記載の器具。
- 前記コントローラは、
前記第1及び第2の時間領域の間で電磁気刺激の前記印加を中断し、かつ
前記第1の時間領域中の電気刺激による電力消費に比較して前記第2の時間領域中の電気刺激による電力消費を低減するように該第2の組の前記少なくとも1つのパラメータを選択する、
命令を与えるように構成されている、
ことを特徴とする請求項17に記載の器具。 - 前記信号発生装置に連結された電源を更に含み、
前記中断命令及び前記選択命令の少なくとも一方は、前記電源が有意に枯渇する前に与えられる、
ことを特徴とする請求項27に記載の器具。 - 患者の神経性病状を治療するための器具であって、
患者の神経系の神経組織と信号通信して位置決めされるように構成された少なくとも1つの植え込み可能刺激器を有し、該患者に植え込まれるように構成された植え込み可能刺激装置と、
植え込み可能信号発生装置と、
前記刺激装置と前記信号発生装置の間に作動的に連結された信号通信リンクと、
非周期的に変動する方法での電磁気刺激の患者への印加を指示する命令を前記信号発生装置に与えるように構成にされ、該信号発生装置と作動的に連結されたコントローラと、
を含むことを特徴とする器具。 - 前記信号発生装置は、閾値下レベルで電磁気刺激の印加を指示するように構成されていることを特徴とする請求項29に記載の器具。
- 前記信号発生装置は、閾値下、閾値、及び閾値上レベルの少なくとも2つで電磁気刺激の印加を指示するように構成されていることを特徴とする請求項29に記載の器具。
- 前記コントローラは、カオス的に変動する方法での電磁気刺激の印加を指示する命令を与えるように構成されていることを特徴とする請求項29に記載の器具。
- 前記コントローラは、反復関数に対応する方法での電磁気刺激の印加を指示する命令を与えるように構成されていることを特徴とする請求項29に記載の器具。

前記刺激装置と前記信号発生装置の間に作動的に連結された信号通信リンクと、
自然に発生する脳波の変動と少なくともほぼ同様に変動する方法での電磁気刺激の患者への印加を指示する命令を前記信号発生装置に与えるように構成にされ、該信号発生装置と作動的に連結されたコントローラと、
を含むことを特徴とする器具。 - 前記信号発生装置は、閾値下レベルで電磁気刺激の印加を指示するように構成されていることを特徴とする請求項36に記載の器具。
- 前記コントローラは、バースト周波数と該バースト周波数よりも大きなイントラバースト周波数とを有する電磁気刺激の印加を指示する命令を与えるように構成されていることを特徴とする請求項36に記載の器具。
- 前記コントローラは、バースト周波数と該バースト周波数よりも大きなイントラバースト周波数とを有する電磁気刺激の印加を指示する命令を与えるように構成されており、
