Abstract

The screw (1) has a thread (1b) extending along its whole length. It has apertures in both ends, one in its tip to receive a guide rod (2), and one in its head for a drive tool (3). The aperture (1a2) in the head of the screw is inscribed within a circle and has two pairs of opposite flat surfaces. The flat surfaces in each pair lie at an obtuse angle to one another and at some 15 degrees to the vertical. The screw's thread has rounded outer and inner surfaces. At its conical tip, the bases of the thread grooves lie at an angle of 45 degrees to the axis of the screw. At the head end the groove, the bases form an angle of 20 degrees.

Description

VIS D'INTERFERENCE RESORBABLE, NOTAMMENT POUR LIGAMENTOPLASTIES.  RESORBABLE INTERFERENCE SCREWS, ESPECIALLY FOR LIGAMENTOPLASTIES.

The invention relates to the technical sector of orthopedics.

It is perfectly known to a person skilled in the art to use interference screws to perform ligamentoplasties, in particular for the restoration of the posterior cruciate ligaments of the knee. In this regard, the injured ligament is replaced by a graft generally removed from a part of the patellar tendon with bone rods at each end. The graft is introduced into two bony tunnels established at the femoral level and at the tibial level. The tibial bone tunnel opens very appreciably at the level of the ligament insertions. The interference screws are engaged in the bone tunnels to block the graft in combination with each bone rod. The installation of the interference screws is generally carried out by means of a screwdriver in combination or not with a guide and centering pin.

These interference screws may or may not have an operating head. However, most of them do not have an operating head but an internal bore, the cross section of which is determined to allow the engagement of an attached member capable of driving the screw concerned.

Interference screws have also been proposed which are made of bioresorbable material as is apparent, for example, from the teaching of patent EP-AO 615,732. Essentially, this patent describes a headless screw with a round thread and manufactured at from a stereocopolymer of lactic acid of high chemical purity. This screw has a through axial bore, a large part of which is formed by a hollow prism to be driven by an actuator of complementary section. The front end of the screw is made in the form of a cone trunk on which the thread extends.

From this state of the art, the problem which the invention proposes to solve is to improve the mechanical strength of the screw taking into account the nature of the material constituting it in order to make it bioresorbable. Indeed, in the case of an absorbable screw, by definition, the mechanical resistance is low, of the order of 50 megapascals, in opposition to a non-absorbable screw, made of titanium for example, whose mechanical resistance is order of 800 megapascals. It therefore appeared important to conserve the maximum amount of material, in particular at the level of the internal drive part which the screw has.

In other words, the problem which the invention proposes to solve is to produce a bio-absorbable screw, the drive bore of which has a cross section farthest from a circle, in order to have a high tightening torque. It appeared advantageous to have a flat section, while having the objective of being able to introduce a centering pin.

To solve such a problem, it has been designed and developed a resorbable interference screw, in particular for ligamentoplasties having a coaxial bore opening out with an end span of reduced length and of circular cross section for the engagement of a centering pin and a drive surface capable of receiving an operating member, the thread extending over the entire length of the screw, characterized in that the cross section of the drive surface is very substantially flat; in particular, this span is inscribed in a circle and has two opposite, symmetrical flat reaches delimiting between them an obtuse angle and in that each flat reach makes an angle of about 15 with the vertical.

Another problem which the invention proposes to solve, taking into account the use of a grub screw, is to allow the practitioner to physically feel the end of the tightening by creating a wedging effect while having the objective of facilitating the screw engagement in the previously established tunnel.

To solve such a problem, the bottom of the thread is successively and continuously established from a frustoconical engagement end range extended by a cylindrical bearing itself extended by another frustoconical end bearing of progressive cross section, the external diameter of the thread being constant and cylindrical over the length of the intermediate span and the end span, opposite to the engagement span.

- les portées d'extrémités tronconiques sont de longueur réduite par rapport à la portée intermédiaire cylindrique;
The invention is also remarkable for the characteristics below
- the tapered ends of the ends are of reduced length compared to the cylindrical intermediate span;
- the engagement end reach delimits, at the bottom of the net, an angle of about 400
- the end range opposite the engagement range defines, at the bottom of the net, an angle of about 200
the length of the cylindrical intermediate span and of the frustoconical end span opposite to the frustoconical engagement span, is very substantially equal to the length of the internal drive span
- the bottom of the net and its top are largely rounded.

The screw designated as a whole by (1) comprises a coaxial bore opening (la). This screw (1) has externally over its entire length a thread (lb). The whole of the screw does not have a head and is made of an absorbable material, for example the screw can be made, in a known manner, from a stereocopolymer of lactic acid of chemical purity. Reference is made to the teaching of patent EP 0 615 732 which provides a person skilled in the art with information on the characteristics of this bioresorbable material.

The bore (la) has an end span (lal) of reduced length and of circular cross section. In known manner, this internal bearing (lal) is intended to cooperate with sliding capacity with a centering pin (2). This scope (lal) is extended by a scope (la2) whose cross section is determined to allow engagement and coupling of an active part (3a) of an operating member (3).

According to the basic characteristics of the invention, this drive surface (la2) has a cross section inscribed in a circle and has two flat surfaces (la3), (la4) opposite and symmetrical. The ranges (la3) and (la4) define between them, an obtuse angle. In particular, each flat surface (la3) and (la4) delimits, with the vertical XX ′, an angle (a) of approximately 15 ". It therefore appears that this cross section is almost flat and allows a maximum of material to be kept at level of the entire bore (la2).

The operating member (3), in the form of a screwdriver, therefore has an active part (3a) in the form of a rod whose end delimits a bearing (3al) of cross section complementary to the cross section (la4) , (la3) of the part (la2) of the bore (la). The entire rod is also drilled coaxially to allow engagement of the centering pin (2) capable of cooperating with the bore (lal) of the screw. The mounting of the interference screw (1) in combination with the centering pin (2) and by means of the screwdriver (3) is not described in detail since it is perfectly known to a person skilled in the art. Note, however, that the active part (3a) of the screwdriver (3) has a very small cross-section to pass through the tibial tunnel in which the graft has previously been placed. It should also be noted that the tunnel receiving the graft can, before the installation of the interference screw, be subjected to the action of a shaping device (4) which acts as a tap.

Advantageously, this shaper (4) can be of the type illustrated in FIG. 6. The head (4a) is attached relative to the body (4b). For example, the head (4a) has a polygonal surface cooperating with a complementary imprint formed at the end of the rod (4b). Given these provisions, it is particularly advantageous to proceed under arthroscopy, conforming the entire femoral part, after having passed through the tibial tunnel. The knee being in the flexion position, the rod (4b) is engaged through the tibial tunnel and, after overflowing, the operator couples the head (4a) to execute the femoral part. This results in perfect alignment for the installation of the screw (1).

Obviously, we do not exclude making a one-piece type conformator.

According to another important characteristic of the invention, the bottom (lbl) of the net (lb) is established successively and in continuity from three successive spans (a, b, c).

The end bearing (a) is of generally frustoconical shape and constitutes the engagement bearing. This range (a) delimits at the bottom of the net an angle (ss) of about 40 ".

The intermediate bearing (b) is cylindrical over its entire length and is extended, opposite to its connection with the bearing (a), by the bearing (c) of frustoconical shape. This span (c) is of progressively increasing cross section and delimits at the bottom of the net an angle of about 20. The frustoconical end bearing surfaces (a) and (c) are of reduced length compared to the cylindrical intermediate bearing (b).

The surfaces (b) and (c) have a length (L) substantially equal to the length of the internal drive surface (la2).

The external diameter (D) of the net (lb) is constant and cylindrical over the entire length L so that at the level of the span (c), the bottom of the net (lbl) must create a wedging effect at the end tightening the screw.

In known manner, the bottom of the thread (lbl) and its top (lb2) are largely rounded.

Obviously, without departing from the scope of the invention, the screw (1) can be executed according to different diameters and lengths determined according to the pathological cases to be treated.

The advantages are apparent from the description, in particular it is emphasized and recalled
- the increase in mechanical resistance resulting from the specific cross section at the level of the internal driving range of the screw which avoids excessive removal of material
- the possibility of feeling the end of the tightening under the effect of wedging resulting from the bottom of the net established from the frustoconical end bearing, the external diameter of the net remaining constant.
