Also provided is a method for applying injection fluids to a subsurface formation from a horizontal well. first, one or more steam ducts are introduced into the horizontal well, said steam ducts having an outer sleeve and a sliding inner sleeve with one or more nozzle assemblies disposed in the outer sleeve. then the sliding inner sleeve is moved on the outer sleeve to at least partially cover one or more nozzle assemblies on the outer sleeve. Finally, the injection fluid is injected into the formation through the steam duct nozzles. An additional method is provided which comprises introducing into the well heel site a first and second pair of flow control handles and polished orifice receptacles, connected to an intermediate compartment and cemented in place. the first pair of flow control handles and polished orifice receptacle are connected to an injection / production liner and one or more steam ducts are then connected to the second pair of flow control handles and polished orifice receptacle.
