Abstract

Instrument for cutting from a body drawn out free endings of a surgical knot made in distant spaces in the inside of organism by minimal invasive technique, endoscopic or catheter based interventions. It comprises an external element, a narrow longitudinal metal pipe (canilla) with a tubular extension at the final (distal) part; of internal element comprised of the final (distal) tubular extension coupled with metal long wire cable that passes through the inside of a long tubular part of external element, and at the run-out of its initial part it is fixed to the control part. Outlet of equal diameter is made on a lateral wall of a tubular extension and the outer and the internal element thereof. Free ending of the surgical knot is drawn through final outlet of a tubular extension of both elements hence by passing a short way through inside of extension of both two internal and external pipes it is drawn out through parallel and in the same plane are set lateral outlets of external and internal element. By drawing the wire cable of the internal element while the external element is motionless, by manipulation of the control element (manual or electromotor twisting) the tubular extension of the external element where sharp edges of the brim of two lateral outlets (a farther one is distal edge of internal and a closer one - proximal edge of external) are folded over by cutting at the same time the ending of the surgical knot that is set between them. The instrument is introduced into the body by a catheter sheath guided over the extruded end of surgical knot by so called monorail method without a direct optical control due to which along with Roentgen - or ultrasound control it may be used for cutting the endings of the knot in internal spaces filled with blood and those are cardial cavities of beating heart.
