Abstract

La présente invention concerne un nouvel anticorps monoclonal d'isotype IgM anti-HLA-DR et son utilisation seul ou en complément d'anticorps d'isotype IgG1 ou d'IgM anti-CD20, dans le traitement des hémopathies lymphoïdes de phénotype B, et en particulier du lymphome non-hodgkinien B, de la leucémie lymphoïde chronique (LLC) et des hémopathies lymphoïdes malignes à cellules B en général.

Description

Domaine technique de l’invention
La présente invention concerne un nouvel anticorps monoclonal d’isotype IgM anti-HLA-DR et son utilisation seul ou en complément d’anticorps d’isotype IgG 1 ou d’IgM anti-CD20, dans le traitement des hémopathies lymphoïdes malignes de phénotype B, telle la leucémie lymphoïde chronique (LLC) et les lymphomes non hodgkiniens B.
La présente invention trouve une application en santé humaine, par exemple en immuno-hématologie, hémato-oncologie et oncologie, et hémopathies lymphoïdes malignes à cellules B.

Etat de la technique
Au cours des dernières décennies, l’immunothérapie est devenue l’une des avancées les plus remarquables dans le domaine du traitement du cancer en particulier des hémopathies malignes lymphoïdes, essentiellement de phénotype B. Contrairement aux approches traditionnelles telles que la chimiothérapie et la radiothérapie, qui visent principalement à éliminer directement les cellules tumorales, l’immunothérapie exploite le pouvoir du système immunitaire pour combattre la maladie.
Les succès remarquables de l’immunothérapie dans le traitement de certains types de cancer ont suscité un vif intérêt et ont ouvert de nouvelles perspectives dans le domaine de la recherche et du développement de médicaments.
Pour l’immunothérapie anti-tumorale, les anticorps monoclonaux (AcM) humanisés ou humains lgG1 divalents représentent la majorité (80 %) des anticorps approuvés pour différentes indications oncologiques et hématologiques.
Un exemple d’immunothérapie actuelle est celle qui vise la protéine CD20, ou cluster de différenciation 20, qui est une glycoprotéine transmembranaire exprimée principalement à la surface des lymphocytes B matures. Son rôle principal est d’agir comme un régulateur crucial de la prolifération et de la différenciation des lymphocytes B, ainsi que dans leur activation et leur fonction immunitaire. La protéine CD20 agit comme un canal calcique, elle présente en effet des similitudes structurelles avec certains canaux ioniques (CD20 : protéine tétraspan spécifique des cellules B). La protéine membranaire CD20 régule un canal d'entrée du calcium qui agit en synergie avec le récepteur B à l’antigène (« B-cell receptor » ou BCR).
Elle est devenue une cible thérapeutique importante dans le traitement de diverses maladies, en particulier des troubles lymphoprolifératifs de phénotype B tels que les lymphomes non hodgkiniens B et la leucémie lymphoïde chronique. Ainsi, le traitement des hémopathies malignes lymphoïdes B implique l'utilisation d'anticorps monoclonaux anti-CD20 qui permettent une élimination des lymphocytes B pathologiques et clonaux, tout en préservant les fonctions des autres cellules immunitaires, en particulier les lymphocytes T et lymphocytes NK. Par exemple, le rituximab (RTX) (AcM anti-CD20 de type I) a démontré une activité clinique significative contre la majorité des néoplasies à cellules B tels que le lymphome diffus à grandes cellules (DLBCL), le lymphome folliculaire (FL), le lymphome à cellules du manteau (MCL). Le RTX peut être utilisé seul dans les lymphomes non hodgkiniens indolents à cellules B tels que le FL en immunothérapie ou associé à la chimiothérapie dans les lymphomes non hodgkiniens agressifs à cellules B sous la forme d'une immunochimiothérapie.
Dans la leucémie lymphoïde chronique (maladie clonale des lymphocytes B CD20+ CD19+ CD5+ CD23+), l'AcM anti-CD20 de référence est l'Obinutuzumab (OBZ) (AcM anti-CD20 de type II), utilisé, selon les recommandations cliniques actuelles (recommandations du FILO-LYSA 2025), en association avec un inhibiteur de l’oncogène du lymphome 2 à cellules B, Bcl-2 (le Vénétoclax). Aujourd'hui, les approches "sans chimio" (« chemo-free ») tendent à devenir la règle en pratique clinique et dans les protocoles de recherche clinique, mais aussi la préférence des malades, par rapport à l’immunochimiothérapie. Lorsqu’un AcM anti-CD20 est utilisé en monothérapie, par exemple le RTX, une maladie résiduelle mesurable persiste après le traitement (incidence de 67%) [1-3]. La poursuite de l'approche thérapeutique « sans chimio » nécessite le renforcement de la bioactivité des AcM anti-CD20 ou de tout autres AcM anti-lymphocytaire B, (anti-CD19, anti-CD38 (le daratumumab et isatuximab), anti-CD47...), sans entrainer de toxicité. Au-delà de leur utilisation en tant qu'agent thérapeutique unique ou en tant qu’anticorps de référence dans les recommandations cliniques, l'étape actuelle consiste à envisager leur association avec une thérapie ciblant une protéine tumorale lymphocytaire B tel le Vénétoclax (thérapie ciblée) ou encore potentiellement dans le futur, avec un modulateur de chromatine ou un autre agent (alisertib, inhibiteur de TGF-P, FOXO1 ou farnésyltransférase) [4-10]. Malgré des avantages très prometteurs, l’immunothérapie présente des défis, notamment la variation de la réponse des patients, l’identification de biomarqueur(s) prédictif(s) de la réponse clinique à un agent immunothérapeutique et/ou à une thérapie ciblée et l’émergence possible de résistance biologique (par exemple, régulation négative de l’épitope). L’atteinte d’un statut de maladie résiduelle non-mesurable (« measurable residual disease » ou « minimal residual disease » ou MRD) constitue un enjeu majeur pour sécuriser l’évolution clinique favorable du malade, évolution favorable sur la survie sans maladie ou « progression-free survival ou PFS, et la survie globale ou « overall survival » ou OS. Le statut de la MRD prédit bien la PFS et la qualité de la rémission de la maladie maligne hématologique. En cas d’obtention d’une rémission complète hématologique, mais associée à une positivité de la MRD > 10'4 (soit MRD intermédiaire : 10'4-10'2 ; ou MRD élevée : > 10’2), un renforcement du traitement est actuellement considéré comme bénéfique. De même, la surveillance évolutive de la MRD est conseillée quand une MRD basse ou indétectable < 10'4 est obtenue, ce qui doit faire considérer le traitement si la MRD redevient détectable, en amont de la rechute clinique. Les mécanismes de résistance aux AcM d’isotype lgG1 peuvent exister naturellement (résistance primaire) ou apparaître après traitement (résistance acquise dans les cellules tumorales résiduelles). Le niveau d'expression de marqueurs de surface tels que la densité du CD20, l’expression élevée des protéines CD55 et CD59 induit une résistance au RTX. La résistance au RTX implique également l'hyper-expression des molécules Bcl-2 ou Bcl-xL, en tant que facteurs anti-apoptotiques.
Dans la LLC, l’OBZ est le traitement anticorps de référence utilisé, conformément aux recommandations cliniques du FILO-LYSA 2025, dans le cadre du traitement de 1 ere ligne (en 1 ere rechute, le RTX est préconisé), OBZ (ou RTX en 1 er rechute) prescrit en association avec le Vénétoclax qui est une thérapie ciblée anti-Bcl2 : traitement limité dans le temps à 1-2 ans, maladie résiduelle non-mesurable (MRD) atteignable (MDR négative : 62% pour la combinaison RTX + Vénétoclax dans l’essai clinique MURANO; 76% pour la combinaison OBZ + Vénétoclax dans l’étude CLL14. Cette combinaison d’immunothérapie et de thérapie ciblée est très onéreuse. L’utilisation d’un iBTK (traitement continu à vie, persistance d’une maladie résiduelle détectable dans 90% des cas pour l’ibrutinib en monothérapie), est également une thérapie très onéreuse car l’utilisation d’un iBTK constitue un traitement considéré actuellement de durée continue [1 -3]. Un iBTK peut également être combiné avec le Vénétoclax, mais le risque de complications cardiologiques est réel : hypertension artérielle, fibrillation atriale. En effet, cette combinaison est associée à une compétition sur le cytochrome P450, le CYP3A4, ceci pour le catabolisme hépatique des deux molécules et susceptible d’augmenter ou de modifier le niveau de leurs concentrations plasmatiques. La combinaison iBTK + Vénétoclax augmente considérablement le cout de cette thérapie ciblée combinée. Le traitement par un iBTK en monothérapie est efficace mais conduit à une maladie résiduelle mesurable le plus souvent détectable, nécessitant un traitement à long terme et induisant un coût médico-économique significatif. La combinaison d’un iBTK (Ibrutinib) et du RTX conduit à une MRD négative à 12 mois dans 8,3 % des cas. La combinaison d’un iBTK (Ibrutinib) et de l’OBZ conduit à une MRD négative dans 30% des cas (étude ILLUMINATE). La combinaison d’un iBTK (Ibrutinib) et du Vénétoclax conduit à une MRD négative dans 75 % des cas à 12 cycles de traitement (étude CAPTIVATE).
L’enjeu actuel de la recherche en immunothérapie est de définir la meilleure combinaison d’immunothérapie et/ou de thérapie(s) ciblée(s), sans adjonction de chimiothérapie et sans apport de toxicité additionnelle. De plus, dans le cadre de l’utilisation des AcMs thérapeutiques, la question est posée du meilleur isotype (IgG, IgA, IgM) de l’anticorps monoclonal à utiliser au lit du malade. A ce jour, les lgG1 , à structure divalente, représentent la majorité (80%) des anticorps approuvés et autorisés pour les différentes indications hématologiques et oncologiques.
Les AcM d’isotype IgM ont fait leur entrée sur la scène thérapeutique. En raison de leur format naturel qui peut être soit pentamérique soit hexamérique, leur bioactivité et leur puissance anti-tumorale peuvent être considérées comme potentiellement supérieures à celles des IgG, en raison d'une cytotoxicité dépendante du complément (CDC) supérieure et de leur multivalence, puisqu’elles présentent l'avantage d'avoir de nombreux sites de liaison pour leur cible (10 ou 12 sites, respectivement). Cette multivalence des IgM est capable d'induire une forte multimérisation des cibles épitopiques à la surface cellulaire, des capacités d'agglutination cellulaire et moléculaire, et d'induire ainsi des voies de transduction du signal intra-cellulaire (telle la voie ERK1/2) et/ou des réponses cytokiniques (interféron-gamma, TNF-alpha, IL-12...) conduisant à une réduction importante de la viabilité cellulaire des cellules tumorales et à la mort de celles- ci.
L’isotype IgM a ainsi été introduit dans la génération d’anticorps monospécifiques et bispécifiques contre les tumeurs malignes solides et hématologiques (IgM- 8444, agoniste de réticulation du récepteur DR5) et contre les lymphomes non hodgkiniens réfractaires et résistants et la LLC (anti-CD20 x CD3 IgM} (IgM BioSciences}. Le PAT-SM6 a également été évalué dans le cadre d'un essai de phase 1 dans le myélome multiple récidivant ou réfractaire [11-15].
Il existe des pathologies lymphocytaires B résistantes aux anticorps monoclonaux anti-CD20 (RTX ou OBZ) ou présentant un besoin médical non satisfait, telles que : • la LLC à délétion du gène P53 et doublement réfractaire (LLC 2R) (risque génétique élevé et risque élevé de transformation de Richter) ou malades ne tolérant pas les traitements de référence et/ou ayant présenté des effets indésirables majeurs, et patients âgés ne pouvant pas les tolérer et /ou à risques de complications avec ces traitements (cardiaques, infectieuses...), encore dénommés malades « unfit ».
• Les lymphomes non hodgkiniens à double ou triple hit (DHL/THL) et à double expression (DEL), ces deux pathologies entraînant une amplification de l’expression de l’oncogène c-Myc (un facteur de prolifération avec Ki-67 > 70 %), ainsi que les patients âgés atteints d’un lymphome diffus à grandes cellules B récidivant, réfractaire ou résistant.
Cas particuliers des patients réfractaires :
Dans la LLC, l’allogreffe de moelle osseuse peut être considérée :
- chez des malades réfractaires aux thérapies immunologiques et/ou aux thérapies ciblées et /ou à l’immunochimiothérapie et/ou aux malades présentant une mutation TP53 ou del(17p). ou
- chez des malades porteurs de LLC évoluant en un syndrome de Richter après une réponse isotopique après traitement immunochimiothérapeutique du syndrome de Richter.
Dans ces deux cas, les anticorps bispécifiques (BsAbs) et les CART-cells (cellules T à récepteur antigènique chimérique) peuvent constituer une alternative à l’allogreffe de moelle osseuse. Le caractère curateur de ces approches est incertain à moyen et à long-terme. Ces approches immunologiques peuvent être considérées chez des malades présentant des comorbidités interdisant l’allogreffe de moelle osseuse. Néanmoins, Ils ne sont pas dénués de toxicité : syndrome de relargage cytokinique (SRC), encéphalopathie reliée à l’utilisation des CAR-T cells [16].
Les BsAbs sont des anticorps possédant deux sites de liaison dirigés contre deux antigènes différents ou deux épitopes différents d’un même antigène. Avec le traitement par l’epcoritamab (IgG 1 bispécifique anti-CD3xanti-CD20), le SRC est observé chez 90% des malades (40% de grade 1 et 50% de grade 2). Un premier BsAb à base de scFv anti-CD3/anti-CD19 (BiTE, blinatumomab) a eu son autorisation de mise sur le marché (AMM) mais cette molécule possède une demi-vie très courte (doit être utilisée en perfusion continue) et peut présenter des effets secondaires notables. Des événements neurologiques, y compris des tremblements, des convulsions et des modifications de l'état mental, peuvent être observés chez 52% des patients, principalement de grade 1 ou 2, avec 11 % de grade 3 et 4 (2 %) de grade 4 [17-19]. Les doses inférieures à 60 pg/m2/jour ont entraîné des taux de réponse médiocres et une dose de 90 pg/m2/jour a été limitée par la neurotoxicité [17-19]. Dans la LLC, l’efficacité du blinatumomab a été analysée dans une phase II en combinaison avec le nivolumab (Ac anti-PD- 1 ) et l’ibrutinib (iBTK) et a montré des résultats décevants (taux de réponse de 22% à deux mois du traitement).
Dans la LLC et le syndrome de Richter, un autre AcM BsAb est en phase Ib/ll dans l’essai EPCORE-CLL : il s’agit de l’ebcoritamab (Tepkinly*), un Ac bivalent CD20/CD3 BiTe [20]. Son indication clinique future s’oriente vers une synergie ebcoritamab-vénétoclax. L’AcM BsAb, le glofitamab, un Ac bivalent CD20/CD3 BiTe (Columvi*), pourrait également trouver une place future dans le traitement de la LLC réfractaire [21-22],
Les cellules CAR-T représentent un autre domaine d'investigation prometteur dans les thérapies cellulaires adoptives, combinant les forces des cellules T et des anticorps pour stimuler l'activité anti-tumorale des cellules T. Les cellules CAR-T CD19 ont été largement utilisées dans les hémopathies lymphoïdes malignes B et il y a peu de recul de son utilisation dans la LLC. Dans la LLC, leur efficacité est incertaine et controversée en raison du phénotype épuisé ou “exhausted” des lymphocytes T et de la perte possible du CD19 via un phénomène de « shaving », créant une résistance à la thérapie CAR-T cells.
Nous avons évalué une combinaison immunologique d’un anticorps lgG1 anti- CD5 en combinaison avec une lgG1 anti-HLA-DR. La protéine HLA-DR est un récepteur de surface cellulaire du CMH de classe II codé par le complexe d’antigènes leucocytaires humains sur la région 6p21 .31 du chromosome 6. Sa fonction principale est de présenter des antigènes peptidiques, potentiellement d'origine étrangère, au système immunitaire dans le but d’induire des réponses des lymphocytes T CD4+ (lymphocytes T auxiliaires ou helper) qui conduisent potentiellement à la production d'anticorps contre le même antigène peptidique. L’anticorps IgG 1 anti-HLA-DR a été testé vis-à-vis de tumeurs malignes à cellules B (Demande internationale WO2010145895) [23]. Des expérimentations ont été réalisées in vivo, avec l’injection de cellules JOK-1 (lignée cellulaire), exprimant la protéine CD5 humaine (cellules transfectées de façon stable). Les résultats obtenus ont été très prometteurs dans la mesure où les souris traitées avec la combinaison d’anticorps monoclonaux anti-CD5 + anti-HLA-DR n’ont pas développé de maladie, contrairement aux souris traitées avec d’autres combinaisons : anti-HLA-DR x anti-CD71 ou anti-CD20, mAb RTX seul.
Description de l’invention
Les anticorps anti-CD20 sont très efficaces mais une maladie résiduelle minimale (MRD) est encore détectable et mesurable. Les Inventeurs ont donc cherché à améliorer les immunothérapies basées sur la molécule CD20 en évitant la formation des mécanismes de résistance de la maladie contre ce type d’agent (e.g. résistance à l’utilisation d’anticorps d’isotype lgG1 anti-CD20 tels que le RTX et l’OBZ) tout en réduisant les risques de récidive de la maladie, dont le risque peut être évalué selon une mesure de la maladie résiduelle minimale en RT-qPCR (ASO-PCR, ou immunophénotypage du clone tumoral en cytométrie en flux, ou plus récemment, séquençage de nouvelle génération, c’est-à-dire en étude NGS) des cellules du sang et/ou issues de la moelle osseuse.
Pour ce faire, les Inventeurs ont cherché un partenaire immunothérapeutique afin de renforcer la bio-activité anti-CD20 et induire une maladie résiduelle non- mesurable et mettre en place un traitement non-toxique et de durée limitée. L’objectif principal est d’augmenter l’efficacité d’AcMs IgG 1 anti-CD20, de type I (e.g. RTX) pour les tumeurs malignes lymphoïdes à cellules B non-LLC et de type Il (e.g. OBZ, anticorps de référence) pour la LLC à cellules B et/ou d’AcMs IgM anti-CD20 ; et de lever la résistance aux anticorps anti-CD20.
Les applications médicales sont : - Hémopathies lymphoïdes à cellules B en général, mais en particulier pathologies résistantes aux anticorps monoclonaux anti-CD20 (RTX ou OBZ) ou présentant un besoin médical non satisfait, telles que :
• LLC avec délétion du gène P53 et double-réfractaire (LLC 2R) (risque génétique élevé et risque élevé de transformation de Richter) ou les malades ne tolérant pas les traitements de référence et/ou ayant présenté des effets indésirables majeurs, et les patients âgés ne pouvant pas les tolérer et /ou à risques de complications avec ces traitements (cardiaques, infectieuses...), encore dénommés malades « unfit ».
• Lymphomes non hodgkiniens à double ou triple hit (DHL/THL) et à double expression (DEL), ces deux pathologies entraînant une amplification de l’expression de l’oncogène c-Myc (un facteur de prolifération avec Ki-67 > 70 %), ainsi que les patients âgés atteints d’un lymphome diffus à grandes cellules B récidivant, réfractaire ou résistant. Les Inventeurs ont mis en évidence de manière tout à fait inattendue que l’utilisation d’un anticorps monoclonal dïsotype IgM anti-HLA-DR (chaine lourde VHantiHLADR-Cmu, SEQ ID NO : 1 ; chaine légère VLantiHLADR-Ck, SEQ ID NO : 2 ; VHanti-HLADR, SEQ ID NO : 3 ; VLanti-HLADR, SEQ ID NO : 4) en combinaison avec un anticorps monoclonal dïsotype lgG1 anti-CD20 de type I (le rituximab, RTX) ou II, (l’Obinutuzumab, OBZ), ou avec un anticorps monoclonal dïsotype IgM anti-CD20, permet d’améliorer l’efficacité d’AcM anti-CD20. L’IgM anti-HLA-DR, même à faible dose ou isodose par rapport à une IgG anti-HLA-DR, associé aux anti-CD20, est capable de compléter la réponse anti-tumorale anti-cellule B des anti-CD20 et obtenir une maladie résiduelle non-mesurable in vivo. L’IgM anti-HLA-DR n’a présenté aucune toxicité in vitro, sur les cellules HUVEC HLA-DR+, contrairement à d’autres anticorps dïsotype lgG1 anti-HLA-DR tels que l’anticorps IMMU-114 (L243). Aucune toxicité n’a été observée durant les expérimentations réalisées in vivo dans le modèle murin CLL-PDX NGS ou l’embryon de poulet.
Les Inventeurs proposent donc un nouveau traitement de la LLC et des hémopathies lymphoïdes malignes B (lymphomes non hodgkiniens B) et en particulier dans des situations cliniques d’hémopathies malignes lymphoïdes B présentant un besoin médical non satisfait (cf. ci-dessus), sur la base d’une combinaison de deux anticorps, anti-CD20 (e.g. OBZ ou RTX) et IgM anti-HLA- DR, qui conduit à une maladie résiduelle qui n’est plus mesurable contrairement au traitement de référence avec l’ IgG 1 anti-CD20 (OBZ ou RTX) et une meilleure gestion des hémopathies lymphoïdes B et des patients, en particulier lorsque la maladie progresse et que le patient devient résistant à l’ IgG 1 OBZ/RTX.
La présente invention a donc pour objet un anticorps monoclonal dïsotype IgM anti-HLA-DR, sous forme pentamérique ou hexamérique. Par exemple, l’AcM dïsotype IgM anti-HLA-DR reconnaît le même épitope ou fragment d’épitope que l’anticorps monoclonal murin dïsotype lgG1 IMMU357 (Beckman Coulter), mais avec une meilleure affinité pour la molécule HLA-DR. Lïsotype IgG 1 IMMU357 est un AcM utilisé en diagnostic.
Selon un mode particulier de la présente invention, l’anticorps monoclonal dïsotype IgM anti-HLA-DR est sous forme pentamérique ou hexamérique, de préférence pentamérique, et présente une plus grande efficacité que lïsotype lgG1 anti-HLA-DR.
Selon un mode particulier de la présente invention, l’anticorps monoclonal dïsotype IgM anti-HLA-DR selon la présente invention a pour fonction de potentialiser l’activité d’anticorps monoclonaux dïsotype lgG1 anti-CD20 et/ou dïsotype IgM anti-CD20.
La présente invention a encore pour objet une composition pharmaceutique comprenant l’anticorps monoclonal dïsotype IgM anti-HLA-DR selon la présente invention, au moins un anticorps monoclonal dïsotype lgG1 anti-CD20 et/ou au moins un anticorps monoclonal dïsotype IgM anti-CD20, et un excipient pharmaceutiquement acceptable.
Selon un mode particulier de la présente invention, ledit au moins un anticorps monoclonal dïsotype lgG1 anti-CD20 est un anti-CD20 de type I.
Selon un mode particulier de la présente invention, ledit au moins un anticorps monoclonal dïsotype lgG1 anti-CD20 est un anti-CD20 de type II.
La présente invention a encore pour objet un anticorps monoclonal dïsotype IgM anti-HLA-DR selon la présente invention ou une composition pharmaceutique selon la présente invention, pour une utilisation comme médicament. La présente invention a encore pour objet un anticorps monoclonal d’isotype IgM anti-HLA-DR selon la présente invention ou une composition pharmaceutique selon la présente invention, pour une utilisation dans le traitement d’un lymphome, par exemple d’une hémopathie maligne lymphoïde B, d’un lymphome non hodgkinien (de phénotype B) ou d’une leucémie lymphoïde chronique de la lignée B (LLC) et également dans des situations cliniques d’hémopathies malignes lymphoïdes B présentant un besoin médical non satisfait (cf. ci-dessus).
BREVE DESCRIPTION DES FIGURES
[Figure 1] représente la carte épitopique de l’anticorps monoclonal dïsotype IgM anti-HLA-DR. Les résidus prédits comme appartenant à l'épitope sont indiqués sur la séquence et sur la structure de la cible, les sous unités alpha (HLA-DRA) et bêta (HLA-DRB) de la molécule HLA-DR (SEQ ID NO : 5). Ils sont répartis en quatre catégories en fonction de leur probabilité brute d'appartenir à l'épitope, en dégradé de gris du plus clair au plus foncé en fonction de la probabilité faible à élevée. Les régions d’interactions faibles (0, gris clair) à fortes (3, gris foncé) sont représentées par des traits sous les séquences d’acides aminés.
[Figure 2] représente la comparaison de la spécificité de reconnaissance antigénique de l’anticorps monoclonal dïsotype IgM anti-HLA-DR avec 4 anticorps commerciaux anti-HLA-DR dïsotype IgG utilisés pour le diagnostic. Les histogrammes représentent l’overlay des MFI obtenues par analyse cytométrique pour des cellules B de la lignée Daudi (HLA-DR positives) marquées avec les anticorps commerciaux dïsotype IgG 1 anti-HLA-DR (LN3 et L243 de Biolegend ; et IM1638U et IMMU-357 de Beckman) couplés au FITC. Pour chaque clone testé, les cellules sont incubées au préalable ou non, avec les AcMs anti-HLA- DR aux formats Fab’2, IgG et IgM. La ligne verticale en pointillé représente la médiane de fluorescence du clone commercial seul.
[Figure 3] représente la répartition membranaire comparée de l’épitope HLA-DR, en fonction de lïsotype de l’anticorps monoclonal agoniste anti-HLA-DR, isotype IgM ou IgG. Les cellules de LLC sont incubées avec les AcMs anti-HLA-DR IgG 1 et IgM conjugués au FITC à 2 pg/ml pendant 4 heures sur des lamelles de verre dans un milieu RPMI sans sérum. Après fixation les cellules sont recouvertes d’un milieu de montage contenant du DAPI (marquage nucléaire) et analysées en microscopie à fluorescence. La figure montre des images représentatives obtenues avec les AcMs IgG ou IgM anti-HLA-DR, les noyaux (DAPI) et la fusion des deux fluorescences (MERGE). Barre d’échelle 10 pm.
[Figure 4A] représente la mortalité cellulaire B comparée des lymphocytes B clonaux primaires et issus de malades, mortalité B observée après 6 heures (H6) de traitement par les isotypes IgG et IgM anti-HLA-DR. Les cellules de LLC ont été incubées pendant 6 heures avec les AcMs anti-HLA-DR isotype IgG et IgM à une concentration isodose de 2 pg/ml. La cytotoxicité sur les cellules B a ensuite été analysée par cytométrie en flux en utilisant un anticorps anti-CD19-AF700 pour la détection spécifique des lymphocytes B et l’annexine V couplée au FITC pour la détection des cellules mortes. La mortalité normalisée des cellules B est représentée pour chaque condition. Les barres représentent la moyenne ± SEM. La signification statistique des différences entre les groupes a été déterminée à l'aide du test t non apparié (*p < 0,05).

[Figure 5A] représente l’impact du microenvironnement tumoral sur la bioactivité des anticorps monoclonaux de type IgG 1 ou IgM anti-CD20 et IgM anti-HLA-DR. Le sérum autologue (SA) reflète l’activité immunosuppressive du microenvironnement tumoral (MET). Le SA contient des molécules à activité immunosuppressives (molécules HLA-DR solubles inductrices de tolérance immunologique, TGF-béta, CD48 soluble...) et des facteurs de survie dérivés des cellules nurse-like. La viabilité cellulaire lymphocytaire B après traitement par les AcMs anti-CD20 rituximab (RTX) d’isotype lgG1 et IgM anti-CD20 seuls ou en association de l’IgGI et IgM RTX avec l’isotype IgM anti-HLA-DR en sérum de veau foetal (SVF) et en SA des malades a été déterminée après incubation des cellules de LLC pendant 6 heures avec les AcMs anti-CD20 RTX lgG1 et IgM, l’IgM anti-HLA-DR ou l’association des deux, tous à une concentration isodose de 2 pg/ml. La cytotoxicité sur les cellules B a ensuite été analysée par cytométrie en flux (CF) en utilisant un anticorps anti-CD19-AF700 pour la détection spécifique des lymphocytes B et l’annexine V couplée au FITC pour la détection des cellules mortes. La mortalité normalisée des cellules B est représentée pour chaque condition. Les barres représentent la moyenne ± SEM.
[Figure 5B] représente l’impact du microenvironnement tumoral sur la bioactivité des anticorps monoclonaux de type IgG 1 ou IgM anti-CD20. Les PBMCs de LLC ont été pré-incubés pendant 16 heures, sans ou avec CpG ODN (1.5 pg/ml) et CD40 ligand (CD40L) (1 pg/ml), afin d’induire un microenvironnement favorable à la survie des cellules clonales B, à l’image de l’effet de survie du microenvironnement tumoral in vivo. A l’issue, les cellules ont été traitées par les AcMs pendant 5 heures, à la dose de 2 pg/ml. La mortalité cellulaire B a été évaluée par le marquage en annexine V.
[Figure 6A] représente l’étude de l’impact de la chronobiologie du traitement par les IgG 1 anti-CD20 et IgM anti-HLA-DR sur l’activité synergique de l’association IgG plus IgM. La mortalité cellulaire induite par les AcMs anti-CD20 RTX et OBZ d’isotype lgG1 en association avec l’IgM anti-HLA-DR a été déterminée après incubation des cellules de CLL pendant 6 heures avec les AcMs à 2 pg/ml en déposant les IgG 30 minutes avant l’IgM ou inversement. La cytotoxicité comparative sur les cellules B tumorales a ensuite été analysée par cytométrie en flux en utilisant un anticorps anti-CD19-AF700 pour la détection spécifique des lymphocytes B et l’annexine V couplée au FITC pour la détection des cellules mortes. L’indice de cytotoxicité (%) est représenté pour chaque condition. [Figure 6B] représente les effets de la chronobiologie lgG1 anti-CD20 et anti- HLA-DR IgM sur l’agrégation homotypique cellulaire B-B. La modification des cellules en culture induite par les AcMs anti-CD20 RTX et OBZ d’isotype IgG 1 en association avec l’IgM anti-HLA-DR a été observée après 1 jour et 3 jours d’incubation des PBMC de LLC avec les AcMs à 2 pg/ml en déposant l’IgG 30 minutes avant l’IgM ou inversement.
[Figure 7A] représente l’impact du traitement in vivo par le rituximab (RTX) sur la maladie résiduelle minimale (« mesurable residual disease » ou MRD), en comparaison avec son association avec l’IgM anti-HLA-DR. La détection des cellules tumorales humaines (PBMC de LLC) dans le modèle de xénogreffe de souris a été évaluée chez la souris CLL-PDX NGS (n= 5 par groupe). Groupe 1 : contrôle (NaCI 0,9 %). Groupe 2 : Rituximab RTX. Groupe 3 : IgM anti-HLA-DR, Groupe 4 : Rituximab RTX/lgM anti-HLA-DR. Les animaux ont été sacrifiés 5 jours après le traitement par les AcMs (J5). La présence de cellules clonales B de LLC dans la rate a été réalisée par RT-qPCR en temps réel avec des amorces spécifiques pour les séquences Alu humaines (hAlu) normalisées sur l'expression de la [3-Actine de souris (m[3actine) sur des extraits d'ADN total (a). En utilisant une courbe étalon réalisée avec un nombre de cellules connu, la maladie résiduelle minimale (MRD) (b) et un ratio de cellules détectées normalisées sur le contrôle a été calculé (c). Les données sont exprimées en moyenne ± SD, un point représentant une souris. La signification statistique des différences entre les groupes a été déterminée par un test ANOVA à sens unique (ns = non significatif, *p < 0,05, **p < 0,01 , ***p < 0,001 , ****p < 0,0001 ).
[Figure 7B] représente l’impact du traitement in vivo par l’anticorps obinutuzumab (OBZ) sur la MRD, en comparaison avec son association avec l’IgM anti-HLA- DR. La détection des cellules tumorales humaines (PBMC de LLC) dans le modèle de xénogreffe de souris a été évaluée chez la souris CLL-PDX NGS (n= 5 par groupe). Groupe 1 : contrôle (NaCI 0,9 %). Groupe 2 : Obinutuzumab OBZ. Groupe 3 : IgM anti-HLA-DR. Groupe 4 : Obinutuzumab OBZ/lgM anti-HLA-DR. Les animaux ont été sacrifiés 5 jours après le traitement par les AcMs (J5). La présence de cellules clonales B de LLC dans la rate a été réalisée par RT-qPCR en temps réel avec des amorces spécifiques pour les séquences Alu humaines (hAlu) normalisées sur l'expression de la P-Actine de souris (m actine) sur des extraits d'ADN total (a). [Figure 8A] représente la viabilité cellulaire B post-traitement (à H6) par les anticorps monoclonaux thérapeutiques anti-CD20, IgG et IgM anti-HLA-DR, utilisés seuls ou en combinaisons. La viabilité est identifiée par la déplétion des lymphocytes B (% LB) pour des combinaisons anti-CD20 IgG de type I ou II (anticorps RX et OBZ) et anti-HLA-DR IgM, les mêmes combinaisons avec des isotypes inversés, et des combinaisons « double IgM » ; au regard des anticorps utilisés seuls ou d’un contrôle négatif sans anticorps.
[Figure 8B] représente la mortalité cellulaire des LB « restants » (identifiés par une taille/structure restant normale), pour des combinaisons anti-CD20 IgG de type I ou II (anticorps RX et OBZ) et anti-HLA-DR IgM, les mêmes combinaisons avec des isotypes inversés, et des combinaisons « double IgM », au regard des anticorps utilisés seuls ou d’un contrôle négatif sans anticorps.
[Figure 8C] représente l’importance des épitopes dans la bio-activité des anticorps anti-CD20 IgG sur les LB tumoraux primaires de patients atteints de LLC.
L’évaluation du rôle des isotypes IgG et IgM dans les combinaisons d'anticorps anti-CD20 et anti-HLA-DR a été réalisée sur PBMC de LLC. Les cellules de LLC ont été mises en suspension à 1 million par millilitre dans le milieu de culture RPMI, 10% de SVF. Les anticorps d’isotype IgG et IgM anti-HLA-DR et anti-CD20 RTX et OBZ ont été incubés avec les cellules à une concentration de 2 pg/ml.
L’administration des AcMs a été réalisée en alternant les combinaisons anti- CD20 (RX ou OBZ) et anti-HLA-DR selon la description ci-dessous : - Traitement IgG + IgM - Traitement IgM + IgG - Traitement IgM + IgM Des isotypes contrôles IgG et IgM ont également été évalués.
Le pourcentage (%) de cible B [Fig. 8A], la mortalité cellulaire B [Fig. 8B] ainsi que l’effet des isotypes contrôles [Fig. 8C] ont été évalués après 6 heures (H6) d’incubation en cytométrie en flux en utilisant un anticorps anti-CD19-AF700 pour la détection spécifique des lymphocytes B et l’annexine V couplée au FITC pour la détection des cellules mortes (Excitation: 488 nm; Émission: 350 nm). Les données ont été collectées et analysées avec le logiciel Kaluza.
[Figure 9] représente la structure de l’ IgM hexamérique et pentamérique.
[Figure 10] représente la réponse anti-tumorale comparée de l’IgM hexamérique et pentamérique par la technique Incucyte après 48h, pour 7 patients atteints de LLC. L’analyse cinétique de la cytotoxicité et du stress oxydatif a été réalisée à l’aide du système d’imagerie en temps réel Incucyte® Live-Cell Analysis System (Essen BioScience, Ann Arbor, Ml, USA), à 37 °C avec 5 % de CO2. Les cellules mononucléées du sang périphérique (PBMC) ont été ensemencées en plaques de culture 96 puits (300000 cellules/puits dans 200 pL de milieu complet RPMI). Pour l’analyse de la viabilité cellulaire, le milieu a été supplémenté avec 5 pM du réactif Incucyte® Cytotox Green, qui pénètre spécifiquement les cellules mortes. Le stress oxydatif intracellulaire a été quantifié à l’aide de la sonde CelIROX™ Deep Red (5 pM). Le taux de mortalité a été calculé en croisant les signaux de cytotoxicité et de stress oxydatif, normalisés par la surface cellulaire détectée. Les anticorps ont été testés en duplicat sur les PBMC. Les acquisitions ont été réalisées pendant 48 à 72 heures, toutes les heures à l’aide d’un objectif *20. Quatre images ont été collectées par puits à chaque point temporel. L’imagerie en contraste de phase a permis de suivre la dynamique morphologique des populations cellulaires au cours du traitement.
[Figure 11] représente l’analyse comparative de courbes dose-réponse sur la déplétion lymphocytaire B de 3 patients atteints de LLC (LLC1 , LLC2, LLC3). La cytotoxicité des IgM hexamérique et pentamérique a été évaluée en effet dose (de 1 à 5 - 10 pg/ml) sur PBMC de 3 LLC. Les cellules de LLC ont été mises en suspension à 1 million par millilitre dans le milieu de culture RPMI, 10% de SVF. Les AcMs ont été déposés en doses croissantes et incubées 6 heures. Le pourcentage (%) de cible B après traitement a été mesuré en cytométrie en flux en utilisant un anticorps anti-CD19-AF700. Les données ont été collectées et analysées avec le logiciel Kaluza.
[Figure 12] représente l’analyse comparative de la bio-activité des IgM pentamérique et hexamérique après induction de la résistance : a) étude de la déplétion lymphocytaire B ; b) étude de la mortalité cellulaire. La bioactivité des anticorps anti-HLA-DR hexamérique et pentamérique a été évaluée selon deux conditions de culture en environnement de résistance, oligodésoxynucléotides CpG (0.25 pM) sans ou avec du CD40L (1 pg/ml) (n=2 à 8 LLC selon les conditions). Les cellules de LLC ont été mises en suspension à 1 million par millilitre dans le milieu de culture RPMI, 10% de SVF. Les AcMs ont été incubés avec les cellules à une concentration de 2 pg/ml. Le pourcentage (%) de cible B [Fig. 12a] ainsi que le % de mortalité cellulaire B [Fig. 12b] ont été évalués après 6 heures d’incubation en cytométrie en flux en utilisant un anticorps anti-CD19- AF700 pour la détection spécifique des lymphocytes B et l’annexine V couplée au FITC pour la détection des cellules mortes (Excitation: 488 nm; Émission: 350 nm). Les données ont été collectées et analysées avec le logiciel Kaluza.
[Figure 13] représente l’étude de la toxicité endothéliale des IgM pentamérique et hexamérique sur : a) les cellules primaires HUVEC en absence ou présence de PBMCs de patients atteints de LLC ; b) la lignée endothéliale EA.hy926. Dans les deux expériences, les cellules ont été stimulées avec 10 ng/ml d’interféron gamma (IFNy) pendant 24 heures afin d’induire l’expression membranaire de la protéine HLA-DR. La toxicité endothéliale a été évaluée sur les cellules primaires HUVEC (PromoCell) en plaque 12 puits (2.105 HUVEC/puits/ml) [Fig. 13a], Pour l’évaluation de la toxicité directe, les AcMs ont été ajoutés à 5 pg/ml. Pour l’évaluation de la toxicité indirecte, les HUVEC ont été co-cultivées avec des PBMC de LLC (6 millions / ml) au moment de l’ajout des AcMs. Après 6 heures d’incubation, les PBMC (cellules non adhérentes) ont été éliminées par lavage puis les HUVEC sont décollées par action de la trypsine. La mortalité a été évaluée en cytométrie en flux avec le marquage annexine V /IP (apoptose : annexine V+ ; nécrose : annexine V+ /IP+). La toxicité endothéliale sur la lignée EA.hy926, dérivée de cellules endothéliales de la veine ombilicale humaine transformées, a été évaluée en plaque 96 puits (2.5.105 EA.hy926 / ml) [Fig. 13b]. Les AcMs ont été ajoutés à 5 pg/ml en présence de sérum humain à 20%. L’anticorps anti-HLA-DR IgG L243 (Biolegend) a été testé en comparaison des IgM hexamérique et pentamérique. Après 24 heures de traitement, la viabilité cellulaire a été évaluée avec le test MTS (Promega) après 1 h d’incubation. La capacité des cellules à réduire le MTS en formazan est mesurable à 490 nm. Cette absorbance représente l’activité mitochondriale induite par le traitement par l’anticorps monoclonal considéré (IgM hexamérique, IgM pentamérique, anticorps anti-HLA-DR L243). L’incubation 1 heure avec de l’H2O2 à 10 mM a été utilisée comme contrôle positif, compatible avec le test MTS. Les résultats représentent la valeur d’absorbance des puits tests moins l’absorbance obtenue pour le milieu sans cellules (DO 490 : test - blanc).
[Figurel 4] représente l’étude de la cytotoxicité anti-lymphocytaire B induite par la combinaison IgG/lgM dans le modèle lymphome non-hodgkinien: a) Cellules de patients atteints de lymphomes non hodgkiniens B (LNH-B) : lymphomes de la zone marginale (LZM) et lymphome du manteau (LM) ; b) et c) La lignée cellulaire Raji a été utilisée comme modèle d’étude du lymphome de Burkitt en culture 3D sphéroïde. [Fig. 14a] La toxicité sur PBMCs de LNH a été évaluée après 4 heures de traitement en plaque 96 puits (1 millions / ml) en cytométrie en flux en utilisant un anticorps anti-CD19-AF700 avec le marquage annexine V (apoptose). Les AcMs ont été ajoutés à 2 pg/ml en présence de SVF 10%. Les données ont été collectées et analysées avec le logiciel Kaluza. Les résultats représentent le % de la cible tumorale B et le % de mortalité sur ces mêmes cellules. [Fig. 14b] La toxicité des AcMs sur la lignée Raji a été évaluée dans le modèle 3D sphéroïde en absence et présence d’effecteurs lymphocytaires T d’un donneur sain. Les cellules ont été ensemencées à 1 .104 cellules par puits de plaques fond rond ULA (Face 11 i tate) et cultivées 3 jours, avant ou non, ajout des effecteurs T (LT) à 3.104 cellules par puits et ajouts en parallèle des AcMs à 2 pg/ml. L’acquisition a été effectuée en utilisant l’Incucyte™ et le module d’analyse Sphéroïde. L’histogramme de droite représente la surface des sphéroïdes après traitements J4 en pm2. Les résultats de l’histogramme de gauche représentent la surface de cellules vivantes après traitement à J4 (= MRD en pm2), soit la surface totale du sphéroïde « masque brightfield » moins la surface de mort cellulaire « masque CytotoxGreen ». [Fig. 14c] Les images correspondantes aux sphéroïdes après 4 jours de traitement ont été obtenues avec l’Incucyte™, la surface blanche dans les sphéroïdes correspond à la surface de cellules vivantes (MRD). La barre d’échelle représente 1 ,3 mm.
EXEMPLES
EXEMPLE 1 : MATERIEL ET METHODES
- Production de l’IgM anti-HLADR recombinante [24-25].
L’anticorps monoclonal anti-HLA-DR d’origine a été généré chez la souris en utilisant comme antigène des cellules humaines exprimant le HLADR. Les ADN complémentaires (ADNc) codant les domaines variables VH et VL de cet anticorps ont été isolés à partir de l’hybridome murin (Demande internationale WO201 0/145895) [23]. Ces ADNc ont permis la production de cet anticorps sous une forme recombinante sous les isotypes IgG 1 et IgM.
Un AcM dïsotype IgM anti-HLA-DR sous sa forme hexamérique (12 domaines Fab au lieu de 2 avec une lgG1 ) a été produit grâce au système d’expression baculovirus/cellules d’insectes. Pour cela, les ADNc codant les régions variables VH et VL de l’anticorps anti-HLADR ont été clonés dans 2 vecteurs de transfert spécifiques (pVT chaîne légère et pVT chaîne lourde) comprenant déjà les ADN complémentaires codant les régions constantes kappa ou mu comme décrit dans Juliant et al. 2013 [24], Des virus double recombinants ont été générés après co- transfection des cellules Sf9 (ATCC CRL1711 ) avec les 2 vecteurs de transfert chargés et de l’ADN viral selon la technologie décrite dans la Demande internationale WO2019/081858 [25]. Les virus recombinants ont été clonés par la méthode des plages de lyse. Le génome des virus recombinants a été contrôlé par southern blot et les séquences intégrées dans l’ADN viral ont été vérifiées par séquençage après amplification PCR. Après 3 jours d’incubation à 28°C, les anticorps recombinants sécrétés au cours de l’infection de cellules Sf9 cultivées en milieu sans sérum ont été purifiés à partir du surnageant de culture sur une colonne de protéine A. La pureté des anticorps a été vérifiée par électrophorèse en gel de polyacrylamide et coloration argentique.
Un AcM d’isotype IgM anti-HLA-DR sous sa forme pentamérique (10 domaines Fab au lieu de 2 avec une lgG1 ) a été produit grâce au système d’expression baculovirus / cellules d’insectes. Pour cela, les ADNc codant les régions variables VH et VL de l’anticorps anti-HLADR ont été clonés dans 2 vecteurs de transfert spécifiques (pVT chaîne légère et pVT chaîne lourde) comprenant déjà les ADN complémentaires codant les régions constantes kappa ou mu comme décrit dans Juliant et al. 2013 [24]. Un 3ème vecteur de transfert a été généré en clonant l’ADNc codant pour la chaîne J de séquence suivante : MKNHLLFWGVLAVFIKAVHVKAQEDERIVLVDNKCKCARITSRIIRSSEDPNEDI VERNIRIIVPLNNRENISDPTSPLRTRFVYHLSDLCKKCDPTEVELDNQIVTATQ SNICDEDSATETCYTYDRNKCYTAVVPLVYGGETKMVETALTPDACYPD (SEQ ID NO : 10). - Les cellules de LLC
Toutes les expérimentations ont été réalisées avec des cellules primaires tumorales de malades (avec leur consentement éclairé), in vitro et chez la souris in vivo, et non avec des lignées de cellules tumorales établies (lignées de cellules tumorales B). Le tableau clinique de chaque patient atteint de LLC est connu avec précision, la connaissance des paramètres phénotypiques, cytogénétiques et moléculaires permet des études de corrélation entre le traitement d'intérêt et la réponse à celui-ci.
- Les cellules de lymphomes B
Les expérimentations sur cellules de lymphomes ont été réalisées in vitro avec des cellules primaires tumorales lymphocytaires B et une lignée de cellule tumorale. Les cellules primaires proviennent de patients atteints de lymphomes non hodgkiniens B (LNH-B) : deux lymphomes de la zone marginale (LZM) et un lymphome du manteau (LM) (avec leur consentement éclairé). La lignée cellulaire Raji a été utilisée comme modèle d’étude du lymphome de Burkitt en absence ou présence de lymphocytes T d’un donneur sain.
- Identification des résidus participant à l’épitope reconnu par l’anticorps monoclonal d’isotype IgM anti-HLA-DR utilisé.
La détermination de l’épitope reconnu par l’anticorps anti-HLADR a été effectuée par la société Mabsilico (https://www.mabsilico.com) par une approche bioinformatique.
S’il n’existe qu’une seule chaine a (HLADRA UNIPROT P01903.DRA_HUMAN), il y a de nombreux variants de la chaine p. Les travaux effectués pour l’identification de l’épitope ont été réalisés en utilisant comme modèle un hétérodimère composé de la chaine a et de la chaine P HLA-DRB1 (HLADRB UNIPROT P01911 ,DRB1_HUMAN) qui est la plus représentée (plus de 80%). À partir de modèles 3D de la cible et de l’anticorps, des positions d'amarrage identifiées par bio-informatique sont classées en fonction de leur probabilité brute d'appartenir à l'épitope, de gris foncé pour la probabilité la plus élevée, au gris le plus clair pour la probabilité la plus faible (mais toujours significative).
- Comparaison de la spécificité et l’individualité de reconnaissance antigénique de l’anticorps monoclonal d’isotype IgM anti-HLA-DR utilisé avec 4 anticorps commerciaux.
Quatre anticorps commerciaux d’isotype IgG anti-HLA-DR (LN3 et L243 de Biolegend ; et IM1638U et IMMU-357 de Beckman) couplés au FITC (isothiocyanate de fluorescéine) ont été incubés à une dose saturante avec des cellules de la lignée B Daudi (400.000 Daudi par test, en PBS1X 4°C) préalablement incubées 15 minutes à 4°C avec une forte dose (4 pg) de l’IgM anti HLA-DR, l’IgG anti-HLA-DR, un Fab’2 anti-HLA-DR (dérivé de l’IgM ou de l’IgG), ou rien. Après incubation avec les anticorps commerciaux 15 minutes, les cellules sont lavées et analysées en cytométrie en flux (Navios, Beckman; Laser FL1 pour la détection du FITC).
Les résultats montrent que les trois formats d’anticorps gênent la fixation des anticorps commerciaux selon la signification décroissante suivante : IMMU- 357»IM1638U>LN3>L243 (Fig. 2). Par conséquent, l’IgM anti-HLA-DR a un épitope identique ou chevauchant avec l’IMMU-357.
Il est à noter que l’anticorps L243 (IMMU114) a été utilisé pour des essais cliniques [27-34],
- Immunofluorescence sur lame.
Les cellules de LLC sont mises en suspension à 2 millions par millilitre dans le milieu de culture RPMI sans SVF (sérum de veau foetal) sur des lamelles de verre (16 mm de diamètre) placées dans des puits de plaque de culture 12 puits avec les AcMs anti-HLA-DR IgG et IgM couplés au FITC à 2 pg/ml. Après 4 à 6 heures d’incubation, le contenu des puits est rincé avec du tampon phosphate (PBS 1x) et les cellules sont fixées avec 4% de paraformaldéhyde (PFA) pendant 15 minutes à température ambiante. Après deux lavages avec du PBS 1x, les préparations sont recouvertes de milieu de montage contenant du DAPI pour la coloration nucléaire. Les lames ont été examinées avec l’Axio Imager 2 (Zeiss). Les images ont été collectées avec un objectif *63 et analysées à l'aide du logiciel ImageJ.
- Bioactivité : effet des AcMs sur la viabilité des lymphocytes B tumoraux et détermination de l’activité cytotoxique des anticorps.
Les cellules de LLC ou LNH sont mises en suspension à 1 million par millilitre dans le milieu de culture RPMI, 10% de SVF. Les anticorps anti-HLA-DR d’isotype IgG et IgM et les anti-CD20 RTX et OBZ sont incubés avec les cellules à une concentration de 2 pg/ml.
Selon les expériences, la viabilité lymphocytaire B a été comparée en suivant 3 schémas d’administration des AcMs permettant l’analyse chronobiologique des traitements :
- Traitement IgG 1 et IgM synchrone
- Traitement par l’IgG 1 30 minutes puis IgM
- Traitement par l’IgM 30 minutes puis lgG1 .
Dans le but d’évaluer l’impact du microenvironnement, dans certaines expériences les PBMCs de LLC ont été pré-incubées pendant 16 heures, sans ou avec CpG ODN (CpG oligodeoxynucleotides) (1.5 pg/ml) et CD40 ligand (CD40L) (1 pg/ml), afin de fournir un microenvironnement favorable à la survie des cellules clonales B, à l’image du microenvironnement tumoral in vivo. A l’issue, les cellules ont été traitées par les AcMs pendant 6 heures, à la dose de 2 pg/ml. La mortalité cellulaire B a été évaluée par le marquage en annexine V. L’analyse de la mort cellulaire est effectuée après 6 heures d’incubation (à H6), en cytométrie en flux en utilisant un anticorps anti-CD19-AF700 pour la détection spécifique des lymphocytes B et l’annexine V couplée au FITC pour la détection des cellules mortes (Excitation: 488 nm; Émission: 350 nm). Les données sont collectées et analysées avec le logiciel Kaluza. La mort cellulaire normalisée est calculée en utilisant cette formule (I): (I) % de mort cellulaire normalisée =100 - ((%T*100) / %C) avec “% T”, le % de cellules vivantes dans le tube test et le “%C”, le % de viabilité dans la condition contrôle.
L’index de cytotoxicité associant 3 paramètres représentatifs de la cytotoxicité est calculé selon la formule (II):
(II) Index de cytotoxicité (IC) = (% MTT - % MTC) + (% BC - % BT) + (%NCC- %NCT) avec % MTT et MTC, les % de mortalité totale dans le tube test et contrôle (sans anticorps); les % BC et BT, le % de LB dans les tubes contrôle et le test ; les % NCC et NCT, le % de cellules de taille/structure normale dans les tubes test et contrôle. - Détection de la MRD en chronobiologie in vivo : le modèle murin CLL-PDX NGS.
Le jour 0, des souris CLL-PDX NGS ont été injectées par voie intraveineuse (IV) avec des PBMC fraîches de LLC (3><107 cellules/souris). Le premier jour, les souris ont été réparties au hasard en quatre groupes (5 souris par groupe) et l'immunothérapie a été réalisée avec une dose en IV de chaque AcM. Deux expériences ont été menées en associant les AcMs anti-CD20 IgG 1 et l’IgM anti- HLA-DR, d'abord avec le RTX et ensuite avec l’OBZ. Les souris CLL-PDX NGS ont reçu par voie IV du NaCI 0,9 % (groupe 1 ), 7,5 mg/kg de RTX ou 7,5 mg/kg d'OBZ (groupe 2), 7,5 mg/kg IgM anti-HLA-DR (groupe 3) et du RTX (7,5 mg/kg) ou OBZ (7,5 mg/kg) suivi IgM anti-HLA-DR (7,5 mg/kg) I.V < 2 heures après l’ IgG 1 (groupe 4). Les souris CLL-PDX NGS ont été sacrifiées le cinquième jour après le traitement réalisé en dose unique à JO.
La présence de cellules B tumorales de la LLC a été évaluée par RT-qPCR dans le foie, la rate, les poumons et la moelle osseuse, en utilisant des amorces spécifiques pour les séquences Alu humaines.
- Détection quantitative des cellules humaines par q-PCR.
La détection de cellules humaines tumorales B primaires dans la rate, la moelle osseuse, les poumons et le foie de souris était basée sur la détection quantitative de séquences Alu humaines dans des extraits d'ADN d'organes de souris à l'aide d'un protocole mis au point par Funakoshi et al, 2017 [35]. Pour l’extraction d’ADN cent microlitres d'organe homogénéisé dans du PBS ont été utilisés. Le kit BioSprint 15 DNA Blood & Tissue de QIAGEN (Ref : 940017) a été utilisé conformément aux instructions du fabricant. Pour détecter les cellules humaines dans les tissus de souris, des amorces spécifiques des séquences Alu humaines (forward : 5'- GGTGAAACCCCGTCTACT-3' (SEQ ID NO : 6); reverse : 5'- GGTTCAAGCGATTCTCGC-3' (SEQ ID NO : 7)) ont été utilisées pour amplifier les séquences Alu humaines présentes dans l'ADN génomique extrait des échantillons. Des amorces -actine spécifiques de la souris (mp-actine) ont été utilisées comme gène de ménage (forward : 5'-AAGGCCAACCGTGAAAAGAT- 3' (SEQ ID NO : 8); reverse : 5'-GTGGTACGACCAGGGATAC-3' (SEQ ID NO : 9) [36]. La RT-qPCR en temps réel pour amplifier et détecter les séquences huAlu a été réalisée sur 40 ng d'ADN génomique avec 0,1 pM de chaque amorce et 5pl de Master mix SYBR green (Thermofisher) dans un volume final de 10 pl. Un système QuantStudio 6 Flex Real Time System (Applied Biosystems) a été utilisé pour le suivi des acquisitions dans les conditions suivantes : 50°C pendant 2 min et 95°C pendant 10 min suivi de 50 cycles à 95°C pendant 30 sec, 60°C pendant 1 min, une étape de courbe de fusion à 95°C pendant 15 sec, 60°C pendant 1 min et 95°C pendant 15 sec. Une mesure quantitative de l'ADN de souris amplifiable a été obtenue par amplification de la séquence d'ADN génomique de la p-actine de souris avec des amorces de m -actine (forward et revers), en utilisant les mêmes conditions de PCR. Le cycle seuil (Ct) de chaque échantillon a été enregistré comme mesure quantitative de la quantité de produit PCR dans l'échantillon. Lorsque cela est indiqué, le signal Alu a été normalisé par rapport à la quantité relative de mp-actine et exprimé sous la forme ACt = (Ctmp-actine- Ctalu). Les modifications du signal Alu par rapport à la quantité totale d'ADN génomique (c'est-à-dire les modifications de la quantité d'ADN humain dans le tissu de la souris) ont été exprimées comme AACt = ACtcontrôle - ACtraitement. Les changements relatifs dans la détection ont ensuite été calculés comme 2- AACt. Chaque essai comprenait un contrôle négatif, un contrôle positif, un contrôle sans modèle d'ADN et les échantillons expérimentaux en double. Le traitement des données et l'analyse statistique ont été effectués à l'aide de GraphPad Prism.
- Cytotoxicité des AcMs dans le modèle sphéroïde de lymphome : lignée Raji.
L’évaluation de la cytotoxicité anti-lymphocytaire B des AcMs dans le modèle 3D de cellules lymphomateuses Raji a été réalisée en culture sphéroïde, modèle couramment utilisé pour représenter les environnements tissulaires in vivo et le screening de molécules thérapeutiques. Les sphéroïdes ont été réalisés en cultivant 1.104 cellules Raji dans le milieu de culture RPMI en plaque non adhérente ULA (Facellitate) à 37 °C avec 5 % de CO2 pendant 72 heures. Les anticorps, avec ou sans lymphocytes T d’un donneur sain (30 000 cellules), ont ensuite été ajoutés à 2 pg/ml. Les anticorps ont été ajoutés dans du milieu RPMI contenant du CytotoxGreen à une concentration finale de 5 pM pour le suivi de la mortalité cellulaire en Incucyte™ avec l’objectif X4. L’analyse des images a été réalisée à l’aide du logiciel IncuCyte®, les valeurs de surface des sphéroïdes (masque brightfield, acquisitions en champ clair) et la surface de cellules mortes, CytotoxGreen positives, ont été utilisées pour calculer la surface de cellules vivantes après traitement en pm2 (= MRD).
EXEMPLE 2 : RESULTATS
- Identification des résidus participant à l’épitope reconnu par l’anticorps monoclonal d’isotype IgM anti-HLA-DR utilisé.
La molécule HLA-DR est composée de deux sous-unités alpha et bêta. À partir des positions d'amarrage les mieux classées, les résidus de la cible ont été notés selon leur probabilité d'appartenir à l'épitope. Les résidus censés appartenir à l'épitope sont indiqués sur la séquence et sur la structure de la cible (Fig. 1 ).
Ils sont divisés en 4 catégories en fonction de leur probabilité brute d'appartenir à l'épitope, de gris foncé pour la probabilité la plus élevée, au gris le plus clair pour la probabilité la plus faible (mais toujours significative). Pour des raisons techniques, la cible est renumérotée comme une seule chaîne. De plus, les acides aminés des chaînes de la cible ne sont pas tous visibles dans la structure 3D. Les résidus prédits comme appartenant à l'épitope et les peptides de validation sont indiqués sur la séquence de la cible.
A partir de ces résidus, quatre régions d’interaction principales ont été définies sur la cible. Les régions d'interaction sont indiquées sur la structure de la cible. Il est intéressant de noter que l’AcM anti-HLA-DR reconnaît préférentiellement un épitope de la chaîne alpha du HLA-DR, mais aussi certains acides aminés de la chaîne bêta.
- Effet de clustérisation membranaire (oligomérisation) de l’épitope HLA- DR induit par l’AcM anti-HLA-DR d’isotype IgM et effets biologiques liés au clustering épitopique. L’anticorps anti-HLADR sous un isotype IgM présente des propriétés très différentes comparé à l’IgGI de même spécificité, en particulier à l’oligomérisation de la cible non observée par l’agoniste IgG 1 anti-HLA-DR.
Observable : la bio-distribution membranaire de l’épitope HLA-DR après traitement avec les AcM anti-HLA-DR d’isotype IgG ou IgM a été analysée en immunofluorescence. Elle met en exergue la capacité et la spécificité inductive d’oligomérisation de l’épitope induite par l’isotype agoniste de l’ IgM anti-HLA-DR (Fig. 3).
Les résultats montrent que l'isotype I gG 1 anti-HLA-DR n’induit qu’une distribution aléatoire et non organisée de l’épitope HLA-DR au sein de la membrane des cellules B de LLC. En revanche, l’isotype IgM anti-HLA-DR induit rapidement (< 3 heures) le regroupement en clustering épitopique membranaire des molécules HLA-DR spécifiquement dans les zones de contacts intercellulaires.
Cette oligomérisation conduit au recrutement de HLA-DR au sein de radeaux lipidiques et à l'induction de nombreuses autres nouvelles activités biologiques liées à ce clustering épitopique :
- Interaction avec le cytosquelette, réorganisation de la F-actine et du cytosquelette ;
- Forte colocalisation de la cortactine et de la F-actine dans les structures membranaires de podosomes impliquées dans la forte adhésion homotypique lymphocytaire B-B ;
- Forte agrégation homotypique (HA) et clustering lymphocytaire de cellules B bien visibles en microscopie optique et en imagerie en temps réel avec l’Incucyte™.
- Interaction avec de nouvelles voies de transduction du signal inductrices de mort cellulaire lymphocytaire B (activation de la NOX2, production de radicaux libres de l’oxygène et création d’un stress oxydatif cellulaire B léthal, phosphorylation des protéines JNK1/2, ERK1-2 et Bcl-2, activation de la Bécline 1 , production importante intrinsèque B d’interféron-gamma);
- Tous ces événements biologiques conduisant à la mort de la cellule cible tumorale lymphocytaire B. Nous avons montré la corrélation qui existait entre agrégation homotypique et mortalité cellulaire lymphocytaire B ainsi que la corrélation entre le pourcentage de l’HA et la viabilité cellulaire réduite (induction de clusters de taille importante avec 100% de mortalité cellulaire B au sein de ces clusters).
- Bioactivité comparative de l’AcM d’isotype IgM anti-HLA-DR versus IgG anti-HLADR.
Observable : le traitement par l'isotype IgM anti-HLA-DR induit une plus grande efficacité anti-tumorale comparé à l’isotype IgG 1 anti-HLA-DR (Fig. 4A).
Lorsque l’AcM anti-HLA-DR a été utilisé sous l'isotype IgM hexamérique, une réduction importante et significative de la viabilité des cellules B tumorales a été observée. L'efficacité a été évaluée à 6 heures (H6) après une exposition à une dose unique d’AcM. L'isotype lgG1 anti-HLA-DR est moins efficace que l’IgM pour induire la mort des cellules B de la LLC : l'anticorps lgG1 anti-HLA-DR a induit à H6 la mort de 14,7± 2,8 % des cellules B de la LLC (n = 24) et on observe une augmentation significative jusqu'à 26,6 ± 3 % avec l'anticorps IgM anti-HLA- DR (*p < 0,05 ; n = 36).
- Combinaisons anticorps anti-CD20 de type I et de type II plus AcM anti- HLA-DR : l’AcM d’isotype IgM anti-HLA-DR potentialise l’activité antitumorale sur les cellules B de la LLC des AcM d’isotype lgG1 anti-CD20.
Observable : le traitement par une faible dose (< 2 pg/ml) d'IgM anti-HLA-DR est synergique avec les AcMs anti-CD20 (Fig. 4B) et amplifie la bioactivité d’un Ac monoclonal anti-CD20, que ce soit de type I ou de type II. Une telle amplification de l’activité de l’anticorps monoclonal anti-CD20 de type I et de type II par l’IgM anti-HLA-DR est observée jusqu’à des doses faibles de l’IgM à 0,5 pg/ml, témoignant d’une activité synergique à faibles doses de l’IgM in vitro et in vivo (dose de l’IgM < 2 pg/ml). Cette activité synergique observée de type « low dose IgM » permet d’envisager un bénéfice pharmacocinétique et pharmacodynamique in vivo.
L'efficacité a été évaluée à 6 heures (H6) après l'exposition à une dose unique d'AcMs. Par rapport aux contrôles H6, 2 pg/ml d'AcMs anti-CD20 ont induit une moyenne de mort normalisée des cellules B de la LLC de 10 ± 1 ,2 % pour RTX (n = 36) à 28,6 ± 5,1 % pour OBZ (n = 17). L'association d’IgGI anti-HLA-DR et d'IgGI anti- CD20 n'augmente pas la cytotoxicité cellulaire avec environ 20 % de mort cellulaire (données non représentées). A l'inverse, le traitement associé par l'isotype IgM anti-HLA-DR augmente significativement la mort cellulaire détectée à H6, par rapport à l’IgG 1 anti-CD20 seul (39,14 ± 4,14 % pour RTX + IgM ; 50,06 ± 5,18 % pour OBZ + IgM).
La combinaison lgG1 anti-CD20 plus IgM anti-HLA-DR crée une activité antitumorale amplificatrice vis-à-vis du clone lymphocytaire B, activité synergique et non additive. La convergence des bioactivités respectives de ces deux AcMs repose sur l'amplification de la voie NADPH oxydase via la protéine membranaire lymphocytaire B, la NOX2, dont l’activation synergique par l’IgG 1 et l’ IgM génère des espèces réactives de l'oxygène (ROS). Les ROS conduisent à une induction de la mort des cellules lymphocytaires B clonales néoplasiques, mortalité B indépendante de l’activation des caspases. Il s’agit d’une mort cellulaire nécrotique régulée, caspase-indépendante, induite par les ROS responsables d’un stress oxydatif léthal, qui occasionne un stress du réticulum endoplasmique, qui est associé à un transfert calcique intra-mitochondrial que nous avons mesuré par la sonde Rhod-2. L’importance du transfert calcique intra-mitochondrial à partir du réticulum endoplasmique via la protéine IP3R, est corrélée au traitement par l’ IgM et son intensité est corrélée à la mort lymphocytaire B observée. Stress oxydatif et transfert calcique intra-mitochondrial entraînent une dysfonction mitochondriale lymphocytaire B et des dommages morphologiques mitochondriaux létaux.
Nous avons régulièrement observé des lésions mitochondriales létales en étude en microscopie électronique, à H6 du traitement, par la combinaison des AcMs lgG1 puis IgM : ballonnement oedémateux de l’ensemble des mitochondries, involution sévère des crêtes mitochondriales, rupture mitochondriale.
Ces altérations mitochondriales morphologiques et fonctionnelles sont majeures et léthales (mortalité lymphocytaire B par nécrose cellulaire induite par la carence en ATP), constituent la propriété de la synergie IgG 1 anti-CD20 et IgM anti-HLA- DR, génèrent une mort lymphocytaire B par nécrose induite et régulée, précédée d’un oedème des mitochondries et de la membrane péri-nucléaire.
- Combinaisons anti-HLA-DR x anti-CD20 : AcM d’isotype IgM anti-HLA-DR potentialisant l’activité anti-tumorale d’un AcM d’isotype lgG1 ou IgM anti- CD20 (dérivé du RTX). Impact du microenvironnement tumoral et du sérum autologue.
Le SA reproduit avec une plus grande fidélité le microenvironnement tumoral (facteurs solubles d’immunodépression et facteurs de survie), en comparaison avec le SVF.
Observable : l’utilisation de sérum autologue (SA) de malades atteints de LLC diminue l’efficacité des AcMs seuls ou en combinaison lgG1 plus IgM anti-HLA- DR, par comparaison avec l’utilisation du SVF 10% (sérum de référence in vitro). De façon importante, l’utilisation d’une IgM anti-CD20 avec l’ IgM anti-HLA-DR, non seulement, rend la combinaison insensible à l’effet inhibiteur du SA mais majore sa bio-activité dans cette condition de résistance immune.
La bio-activité des AcMs est évaluée à 6 heures (H6) après l'exposition à une dose unique d'anticorps.
En SA, par rapport à utilisation du SVF 10%, les niveaux de perte d’activité sont les suivantes :
AcM RTX anti-CD20 : perte moyenne de - 2.5 % pour l’IgG 1 et perte moyenne de - 6.2 % pour l’ IgM anti-CD20.
IgM anti-HLA-DR : une diminution d’environ - 2.7 %.
Pour l’association RTX IgG + anti-HLA-DR IgM : une diminution d’environ - 9.5 %.
Une faible dose (< 2 pg/ml) d'IgM anti-HLA-DR induit une efficacité synergique avec les AcMs anti-CD20 lgG1 de type I et de type II, aussi bien en SVF 10 % qu’en sérum autologue de patient (n = 3) (Fig. 5A).
En SVF 10% par rapport aux contrôles H6, 2 pg/ml d'AcM RTX anti-CD20 induit un faible index de cytotoxicité vis-à-vis des cellules B tumorales de la LLC, soit une augmentation d’environ 5.4 % pour RTX lgG1 et 13.8 % pour le RTX IgM. En milieu SVF, l’ajout d'IgM anti-HLA-DR aux AcMs anti-CD20 augmente significativement la mort cellulaire B, par rapport à l’IgGI et l’IgM anti-CD20 seules : gain moyen de + 32.8 % pour RTX IgG + IgM anti-HLA-DR ; gain moyen de + 17.6 % pour RTX IgM + IgM anti-HLA-DR.
En SA, l’index de cytotoxicité anti-lymphocytaire B avec l’anti-CD20 RTX est d’environ 3.1 % pour l’IgGI et 7.6 % pour l’isotype IgM anti-CD20. De la même manière qu’en SVF 10%, en SA, l’association des AcMs anti-CD20 avec l’anti- HLA-DR IgM potentialise leur effet : gain moyen de + 25.6 % pour RTX IgG + IgM anti-HLA-DR ; gain moyen de + 32.5 % pour RTX IgM + IgM anti-HLA-DR. L’association RTX IgM avec l’AcM anti-HLA-DR IgM augmente l'efficacité de la synergie : gain moyen de 8.6 %.
Les cellules B sont pré-traitées pendant 16 heures par l’association CpG ODN (CpG oligodeoxynucleotides) et CD40 ligand (CD40L), pour fournir un environnement favorable à la survie cellulaire en induisant une résistance dans les cellules clonales B, puis celles-ci sont traitées par les anticorps monoclonaux lgG1 anti-CD20 et l’ IgM anti-HLA-DR, seuls ou en combinaison. La mortalité est ensuite évaluée en cytométrie et normalisée à la condition contrôle sans CpG /CD40L.
En comparant la mortalité cellulaire normalisée des LB sans pré-traitement par CpG ODN / CD40L, on observe une diminution de la mort cellulaire basale en présence de CpG ODN / CD40L (Fig. 5B). Le pré-traitement par CpG ODN et CD40L entraîne effectivement une meilleure survie des lymphocytes B tumoraux et une réduction importante de leur mortalité spontanée (contrôle positif expérimental). Une mortalité spontanée importante des lymphocytes B de LLC > 30% est observable après un cycle de congélation / décongélation cellulaire et permet bien de confirmer l’effet de résistance induite à la mort cellulaire B par le pré-traitement par CpG ODN et CD40L (voire contrôle positif des cellules non traitées dans la condition CpG/CD40L).
L’efficacité des AcMs avec ou sans pré-traitement est évaluée en parallèle. Les traitements AcMs anti-CD20 ne suffisent pas à lever la résistance induite par la condition de culture prétraitée. A l’inverse, l’AcM IgM anti-HLA-DR seule ou en association avec les anti-CD20 lgG1 permet l’induction significative d’une importante mort cellulaire en CpG/CD40L par comparaison à l’effet des IgG 1 anti- CD20, sans bénéfice de l’association avec une lgG1 (moyenne des conditions sans CpG/CD40L avec les anti-CD20 : 1.93 % ; avec IgM : 6.3 %) ; avec CpG/CD40L et anti-CD20 utilisée seuls: une diminution moyenne de -0.6 %, en condition CpG/CD40L avec traitement par une IgM anti-HLA-DR en combinaison avec un anti-CD20 : on observe une augmentation d’environ 15.9 %).
La bio-activité de l’anticorps monoclonal de type IgM reste inchangée et induit le même seuil de mortalité B, y compris dans les conditions de résistance tumorale, à l’inverse des anticorps anti-CD20 de type I et II. Ceci met en évidence le fait que l’ IgM est peu sensible au microenvironnement tumoral qui induit à contrario, une résistance au traitement par les IgG.
- Chronobiologie du traitement in vitro et in vivo IgM anti-HLA-DR x lgG1 anti-CD20 (OBZ).
Chronobiologie in vitro
L’étude de la viabilité cellulaire lymphocytaire B a été analysée en chronobiologie afin d’optimiser la réponse anti-tumorale. Ainsi, la viabilité lymphocytaire B a été comparée selon les 3 schémas suivants d’administration des AcMs : - Traitement IgG 1 et IgM synchrone
- Traitement par l’Igl 30 minutes puis IgM
- Traitement par l’IgM 30 minutes puis lgG1
Observable : nous montrons que le traitement par lgG1 puis le traitement par l’IgM anti-HLA-DR (après 30 minutes d’exposition à l’IgGI ) induit une forte agrégation homotypique B-B et une mortalité cellulaire significativement supérieure, par rapport aux autres formes chronobiologiques, traitement IgG 1 et IgM synchrone et IgM puis lgG1 (Fig. 6A) (Fig. 6B).
L'efficacité a été évaluée à 6 heures (H6) en cytométrie après l'exposition à une dose unique d'AcMs.
Une faible dose (< 2 pg/ml jusqu’à 0,5 pg/ml) d'IgM anti-HLA-DR a conduit à une efficacité synergique avec les AcMs anti-CD20 lgG1 (RTX et OBZ) (SVF 10 %), avec la chronobiologie IgG avant IgM (RTX, n = 3 ; OBZ, n = 4) (Fig. 6A).
Pour la combinaison RTX + IgM anti-HLA-DR, on augmente de + 21.5% l’indice de cytotoxicité anti-lymphocytaire B, en déposant l’IgG avant l’IgM (indice de cytotoxicité à H6 pour IgG puis IgM = 47.5 % et IgM puis IgG = 26 %). Pour la combinaison OBZ + IgM anti-HLA-DR, on augmente de + 11.2% l’indice de cytotoxicité anti-lymphocytaire B, en déposant IgG avant l’IgM (indice de cytotoxicité à H6 pour IgG puis IgM = 78 % et IgM puis IgG = 66.8 %).
Des évaluations en imagerie en temps réel avec captation des images à J1 et J3 (technique IncuCyte) montrent la nette différence entre les deux chronobiologies avec une nette augmentation de l’agrégation cellulaire homotypique B-B, avec la chronobiologie IgG 30 minutes avant IgM (Fig. 6B).
Evaluation de la MRD : Chronobiologie in vivo
L'objectif d'obtenir une maladie résiduelle non-mesurable est très important dans la prise en charge médicale des hémopathies malignes. En effet, les cellules tumorales qui survivent au traitement sont capables de nouvelles modifications génétiques, à l'origine de la récidive et de l'efficacité transitoire des AcMs dues au développement d'une résistance acquise.
Observable : Le traitement in vivo des souris CLL-PDX NGS, par la combinaison des AcMs RTX (ou AcMs OBZ) et IgM anti-HLA-DR, induit une forte réponse antitumorale vis-à-vis des lymphocytes B clonaux B de LLC. L’analyse de la MRD montre une réponse anti-tumorale induite par la combinaison IgG 1 RTX (ou OBZ) plus IgM significativement supérieure au traitement par le RTX (ou l’OBZ) utilisé en monothérapie.
En effet, en utilisant le modèle murin PDX-CLL NGS, nous montrons que la maladie résiduelle tumorale reste détectable après un traitement avec des AcMs d’isotype lgG1 anti-CD20 utilisés en monothérapie (RTX et OBZ), alors que l'association avec OBZ x IgM HLA-DR rend la maladie résiduelle indétectable et non-mesurable dans la rate (Fig. 7A et 7B).
L'isotype IgM anti-HLA-DR combiné à l'OBZ a induit in vivo une maladie résiduelle non-mesurable de la LLC. Ce résultat démontre l’efficacité de la réduction de la MRD d’une hémopathie lymphoïde B, en combinant l'IgGI anti- CD20 de type I ou de type II ceci avec une faible dose d'AcM dïsotype IgM anti- HLA-DR.
L'isotype lgG1 RTX combiné à lïsotype IgM anti-HLA-DR réduit, in vivo, de 17 fois le niveau de MRD dans la rate. L'isotype lgG1 OBZ combiné à lïsotype IgM anti-HLA-DR réduit, in vivo, de 2 fois le niveau de MRD dans la rate (Fig. 7A b-c et 7B b-c).
La diminution de la MRD a été confirmée in vivo dans 2 modèles différents, en xénogreffe tumorale de l’embryon de poulet et en la xénogreffe de souris par des PBMC de malades atteints de LLC (modèle murin PDX-CLL NGS).
- Comparaison de l’effet biologique des combinaisons IgG/lgM sur des cellules de patients atteints de leucémie lymphoïde chronique (LLC).
Ce test a pour objet de vérifier si l’association d’un anticorps monoclonal thérapeutique d’isotype IgG anti-CD20 (de type I et II) avec un anticorps monoclonal thérapeutique d’isotype IgM anti-HLA-DR (en isodose) est la règle : celle de la capacité d’une IgM d’amplifier la bio-activité d’une IgG.
Les PBMCs de patients atteints de LLC ont été traitées avec les anti-CD20 et anti-HLA-DR d’isotype IgG ou IgM pendant 6h (H6) et analysées en cytométrie en flux (repérage d’un marquage annexine V+, témoignant d’une mortalité cellulaire évolutive).
Deux fenêtres de lecture sont utilisées.
Elles reflètent toutes deux la bio-activité des anticorps monoclonaux seuls ou en combinaisons IgG + IgM : a- La capacité de déplétion de la population tumorale lymphocytaire B (LB) à T6 (évaluation de la lyse tumorale). Elle est identifiée dans la Fig. 8A par le symbole : %LB. b- La capacité d’induire une mortalité cellulaire lymphocytaire B des cellules gardant une taille / structure normale (dits « B restants ») à H6 également. La mortalité cellulaire est repérée par un marquage membranaire annexine V+. Elle est identifiée dans la Fig. 8B par le symbole : % mort cellulaire. c- Au temps d’analyse H6, la mortalité acquise (a- lyse lymphocytaire B complète et déplétion LB) et la mortalité cellulaire évolutive (b- lymphocytes B dits « restants » « annexine V+ ») s’additionnent pour donner une mortalité cellulaire LB globale.
La toxicité lymphocytaire T (LT) qui aurait pu être induite par les anticorps seuls ou en combinaisons a également été analysée. Cette toxicité a été examinée par le calcul de la proportion des LT avant et après le traitement par les anticorps. Aucune toxicité lymphocytaire T n’a jamais été observée, que ce soit avec les IgG et les IgM seules ou en combinaison (données non représentées).
La Fig. 8A montre que la déplétion des lymphocytes B (% LB) est maximale pour les associations « anti-CD20 IgG + IgM anti-HLA-DR » par comparaison avec les mêmes combinaisons avec les isotypes inversés. Les combinaisons « double IgM » sont également efficaces avec l’association « RX IgM + HLA-DR IgM » soit : - 19.7% de LB et « OBZ IgM + HLA-DR IgM » : - 29.4%.
La Fig. 8B montre que la mortalité cellulaire des LB « restants » est maximale avec « OBZ IgG + HLA-DR IgM », comme soutenu dans l’invention. Les associations « anti-CD20 IgG + anti-HLA-DR IgM » montent une cytotoxicité supérieure (53.61 % et 75,56%) que les mêmes combinaisons avec les isotypes combinés inversés. Ces derniers (anti-CD20 IgM + anti-HLA-DR IgG) permettent néanmoins d’induire une mortalité notable de 37.09% et 49.96% au sein des LB restants. Les combinaisons « double IgM » sont également efficaces avec « RX IgM + HLA-DR IgM » : + 34.3% et « OBZ IgM + HLA-DR IgM » : + 44.6%.
La Fig. 8C montre l’importance des épitopes pour la bio-activité des anticorps via l’utilisation d’isotype contrôle (isotype non pertinent, soit IgG soit IgM), sans reconnaissance de cible spécifique. Les IgG et IgM contrôles (iso IgG et iso IgM) ne présentent pas d’effet sur les LB de LLC. La combinaison « RX IgG + iso IgM » ne montre pas non plus d’effet synergique. Avec la combinaison « OBZ IgG + iso IgM », les mêmes valeurs de déplétion B et de mort cellulaire sont observés qu’avec l’utilisation de l’ IgG OBZ seule. Il n’y a donc aucun effet d’un isotype non pertinent sur l’activité des IgG anti-CD20 de type I ou II (contrôle négatif).
Ces résultats montrent l’importance des épitopes dans une association IgM/IgG. L’isotype contrôle IgM n’apporte aucun bénéficie aux anticorps anti-CD20 IgG.
- Comparaison de la bio-activité de l’IgM anti-HLA-DR pentamérique recombinante et de l’IgM anti-HLA-DR hexamérique.
Par co-expression du gène codant pour la chaine J et des gènes codant pour la chaine lourde et la chaine légère de l’IgM, une IgM pentamérique a été produite (Figure 9). L’analyse comparative de la bio-activité des IgM anti-HLA -DR pentamérique et hexamérique a été réalisée sur des cellules primaires tumorales lymphocytaires B de malades atteints de leucémie lymphoïde chronique (LLC) de la lignée B. Après 48h de suivi de la mortalité en temps réel avec la technique « Incucyte », les données ont été normalisées au contrôle et présentées en % de viabilité cellulaire (histogramme, Fig. 10A). Les données cumulées pour 7 patients atteints de LLC ne montrent pas de différence significative entre les deux IgM. Pour 3 patients atteints de LLC, une supériorité de l’IgM pentamérique a été mise en évidence (Fig. 10B). L’efficacité respective des IgM pentamérique et hexamérique semblent être malade-dépendant.
Sur les 3 patients atteints de LLC, les analyses dose-réponse ont montré une forte efficacité de déplétion B des deux IgM à très faibles doses (0.25-1 pg/ml) (Fig. 11 ). Une supériorité de l’IgM pentamérique à ces mêmes doses a été observée. L’effet est similaire entre les deux IgM pour des doses de 2 ou 4 pg/ml selon le patient. Pour les doses plus élevées (>4 pg/ml), une inhibition de l’effet des anticorps a été observée (effet dose de l’anticorps IgM favorisant la liaison du CH4 de son Fcp avec le FCMR lymphocytaire B responsable de l’induction d’un signal de survie cellulaire B). Les résultats montrent donc la capacité déplétive lymphocytaire B supérieure de l’IgM pentamérique ; son optimum étant atteint à une concentration de 4 pg/ml d’anticorps.
L’effet de la résistance induite sur la bio-activité des IgM a été testé à partir de 4 à 9 expériences selon les conditions à H6. Le milieu de culture a été supplémenté avec CpG-ODN (CpG oligodeoxynucleotide) en absence ou en présence de ligand CD40 (CD40L). Ces deux conditions de culture ont permis d’induire un environnement cellulaire connu pour réguler l’apoptose en induisant des signaux pro-survie (Granziero et al., 2001 ) [37], En condition de résistance, les IgM ont montré un bon effet sur la déplétion lymphocytaire B (Fig. 12a) ainsi que sur l’induction de la mort cellulaire sur les LB restants (Fig. 12b). La mort cellulaire étant un phénomène survenant en amont de la disparition de lymphocytes B tumoraux par lyse cellulaire (= déplétion B), la diminution du % de LB permet une interprétation cinétique de l’effet des anticorps. Les LB ayant survécu et conservé une taille/structure normale à ce temps de l’étude ont permis d’observer un effet persistant des anticorps si un marquage annexine V+ était observé (cellules présentant un signe biologique de mortalité et destinées à la lyse cellulaire). Ces deux paramètres ont été analysés à H6.
- Etude de la toxicité endothéliale des IgM anti-HLA-DR pentamérique et hexamérique.
La cytotoxicité des anticorps IgM anti-HLA-DR a été évaluée sur cellules endothéliales HUVEC et sur la lignée EA.hy926 après stimulation 24 heures avec de l’interféron gamma pour induire l’expression de la protéine HLA-DR (condition pro-inflammatoire). Une mortalité endothéliale serait prédictive de la survenue de vascularites in vivo. La cytotoxicité sur les cellules HUVEC a été évaluée en effet direct ou en présence de cellules (PBMCs) de patients atteints de LLC et de leur sérum respectif pour mimer les conditions physiologiques ainsi qu’une potentielle cytotoxicité indirecte. Les résultats ne montrent aucune toxicité de l’IgM anti-HLA- DR vis-à-vis des cellules endothéliales [Fig. 13a], ni d’induction de stress oxydatif dans les cellules endothéliales (données non représentées), que ce soit en présence ou non de PBMCs de patients atteints de LLC. La toxicité sur la lignée EA.hy926, protéine HLA-DR induite par lïnterféron-gamma, a été évaluée en sérum humain après 24 heures de traitement avec les anticorps anti-HLA-DR IgM. L’anticorps anti-HLA-DR IgG L243 a été utilisé comme contrôle commercial et I H2O2 comme contrôle positif de mort cellulaire. Les résultats ne montrent pas de toxicité de nos anticorps IgM anti-HLA-DR, pentamérique et hexamérique, avec un métabolisme mitochondriale fonctionnel, identique à la condition contrôle [Fig. 13b]. L’IgG anti-HLA-DR, le L243, montre une augmentation significative de l’activité mitochondriale (**p= 0.003), non retrouvée avec les IgM anti-HLA-DR.
- Comparaison de l’effet biologique des combinaisons IgG/lgM sur des cellules de lymphomes. Afin d’évaluer la toxicité des AcMs dans un autre modèle d’évolution maligne B, la bioactivité a été testée sur des cellules de patients atteints de lymphomes non hodgkiniens B (LNH-B) : lymphomes de la zone marginale (LZM) et lymphome du manteau (LM) [Fig. 14a]. La lignée cellulaire Raji a été utilisée comme modèle d’étude du lymphome de Burkitt en culture 3D sphéroïde [Fig. 14b] et [Fig. 14c]. Sur PBMCs de LNH-B (LZM et LM), la combinaison de I’IgGI anti-CD20 OBZ avec l’anti-HLA-DR IgM majore l’effet de l’OBZ sur la réduction du % de cible tumorale B ainsi que sur la mortalité lymphocytaire B des cellules restantes, ceci après un temps court d’exposition soit 4 heures de traitement. (LZM n=2 et LM n=1 ). Ces deux lymphomes présentent une sensibilité similaire aux traitements IgG/IgM. L’évaluation des combinaisons IgG/lgM dans le modèle sphéroïde Raji après 4 jours de traitement a été réalisée en absence ou présence de lymphocytes T d’un donneur sain pour évaluer l’effet médié par les effecteurs cellulaire (ADCC) et l’effet direct. L’acquisition a été réalisée avec le module sphéroïde de l’Incucyte™ en mesurant la surface des sphéroïdes ainsi que la surface de cellules morte (CytotoxGreen positives). Les résultats montrent une réduction globale de la taille des sphéroïdes après traitement, avec une réduction maximale observée dans la condition combinant OBZ/lgM hexamérique, en présence de lymphocytes T [Fig14. b] et [Fig 14. c]. Pour estimer plus précisément la viabilité cellulaire, la MRD (Maladie Résiduelle Détectable) a été déterminée comme étant la surface représentant les cellules vivantes au sein de chaque sphéroïde (pm2). Les analyses révèlent que le traitement avec OBZ seul entraîne déjà une réduction de la MRD, indiquant une perte de viabilité. Cette diminution est fortement accentuée par la combinaison synergique avec l’IgM hexamérique. La présence des LT renforce encore davantage cet effet, traduisant une implication de l’immunité cellulaire lymphocytaire T dans la bioactivité anti-lymphocytaire B observée. Les images [Fig 14. c] des sphéroïdes à J4 de traitement permettent à la fois d’observer la diminution de la taille des sphéroïdes ainsi que l’observation de la MRD, représentée par la surface de couleur blanche. Les valeurs correspondantes sont reportées en dessous des images avec une diminution de 3,7.106 pm2 dans le contrôle à 0.27.106 pm2 en présence de OBZ/lgM + LT. L’apport des LT permet de diminuer de 0,43.106 pm2 la MRD de l’OBZ /IgM. On observe également la présence d’un noyau central de cellules mortes, résultant d'une nécrose spontanée. Claims

REVENDICATIONS
1 ) Anticorps monoclonal d’isotype IgM anti-HLA-DR.
2) Anticorps monoclonal d’isotype IgM anti-HLA-DR selon la revendication 1 , où ledit anticorps est sous forme pentamérique ou hexamérique.
3) Anticorps monoclonal d’isotype IgM anti-HLA-DR selon la revendication 1 ou 2, où ledit anticorps potentialise l’activité d’anticorps monoclonaux d’isotype lgG1 anti-CD20 et/ou d’isotype IgM anti-CD20.
4) Composition pharmaceutique comprenant l’anticorps monoclonal d’isotype IgM anti-HLA-DR selon l’une quelconque des revendications 1 à 3, au moins un anticorps monoclonal d’isotype IgG 1 anti-CD20 et/ou au moins un anticorps monoclonal d’isotype IgM anti-CD20, et un excipient pharmaceutiquement acceptable.
5) Composition pharmaceutique selon la revendication 4, où ledit au moins un anticorps monoclonal d’isotype IgG 1 est un anti-CD20 de type I.
6) Composition pharmaceutique selon la revendication 4, où ledit au moins un anticorps monoclonal d’isotype IgG 1 est un anti-CD20 de type II.
