Description

生体内留置具と生体内留置具の製造方法
 本発明は、血管疾患部の血管に塞栓を形成するための生体内留置具とその製造方法に関する。
 頭頸部の動脈瘤、動静脈奇形、動静脈瘻、肺血管奇形、腎血管奇形、腎動脈、腹部動脈瘤等の血管病変の治療法の一つとして血管内治療が挙げられる。血管内治療では、塞栓形成用のコイルを有する生体内留置具を用いて、コイルを目的部位に留置し、血栓化を促進することによって例えば動脈瘤が破裂するのを防ぐ塞栓術が用いられる。
 特許文献1には、ワイヤー本体と、このワイヤー本体の先端部に水膨潤性材料からなる接続部材を介して接続された生体内留置部材とからなり、切断用電流が供給されることにより前記接続部材が加熱されて切断し、これによって前記生体内留置部材が離脱されることを特徴とする生体内留置部材を有する医療用ワイヤーにおいて、該接続部材が水溶性の塩類を含有することを特徴とする医療用ワイヤーが記載されている。また、特許文献1には、ガイドワイヤー1が、コアとなるワイヤーの外周面上に樹脂被覆層が形成されたワイヤー本体2と、これに続く先端造影部分7とを有してなること、先端造影部分7の先端部に接続部材3が接続されていることも記載されている。

 特許文献1に記載の医療ワイヤーでは、生体内に留置される部材に造影部分が配されているわけではないため、操作者が接続部材の切断操作を行った後は、生体内留置部材が生体内のどこに配置されたのか確認することができないという課題があった。また、操作者が接続部材の切断操作を行ったものの、実際には生体内留置部材の離脱が出来ていないこともある。そのような場合、特許文献1の医療ワイヤーでは、ワイヤー本体2を抜去して目視確認してはじめて離脱が成功したかを確認することができるため、離脱できていない場合の処置が遅れることも考えられる。そこで、本発明は、生体内に留置されるコイルの位置を把握しやすい生体内留置具とその製造方法を提供することを目的とする。
 上記課題を解決することのできた本発明の実施の形態に係る生体内留置具は、以下の通りである。
 [1] 長手軸方向を有するコイルと、
 前記長手軸方向において前記コイルよりも近位側に配されており、前記コイルを遠位側に向かって押すプッシャーと、
 前記コイルと前記プッシャーとを接続している接続部であって、前記コイルが前記プッシャーから離脱する離脱機構を備え、放射線不透過材料を含む接続部と、を有する生体内留置具。
 さらに実施の形態に係る生体内留置具は、以下の[2]~[6]のいずれかであることが好ましい。
 [2] 前記離脱機構では、前記プッシャーを介して供給される電気的または熱的エネルギーによって前記接続部が加熱されて前記コイルが前記プッシャーから離脱する[1]に記載の生体内留置具。
 [3] 前記接続部には、前記放射線不透過材料からなる粒子が存在している[1]または[2]に記載の生体内留置具。
 [4] 前記放射線不透過材料は、第1の放射線不透過材料からなる第1の粒子と、第2の放射線不透過材料からなる第2の粒子を含む、[3]に記載の生体内留置具。

 上記課題を解決することのできた本発明の実施の形態に係る生体内留置具の製造方法は、以下の通りである。
 [7] 長手軸方向を有するコイルと、前記コイルを遠位側に向かって押すためのプッシャーと、を準備する工程と、
 前記コイルと前記プッシャーとを接続する接続部であって、放射線不透過材料を含む接続部を形成する工程と、

 さらに実施の形態に係る生体内留置具の製造方法は、以下の[8]~[13]のいずれかであることが好ましい。
 [8] 前記接続部を形成する工程は、前記熱可塑性材料と前記放射線不透過材料からなる粒子とを混合する工程を含む[7]に記載の生体内留置具の製造方法。
 [9] 前記接続部を形成する工程は、さらに、前記熱可塑性材料と前記放射線不透過材料からなる粒子を混合して得られた材料を押出成形する工程を含む[8]に記載の生体内留置具の製造方法。
 [10] 前記接続部を形成する工程は、さらに、前記押出成形された材料を延伸する工程を含む[9]に記載の生体内留置具の製造方法。
 [11] 前記接続部を形成する工程において、前記熱可塑性材料と前記放射線不透過材料からなる粒子を混合して得られた材料を三次元積層造形法により積層造形する[8]に記載の生体内留置具の製造方法。

 上記生体内留置具によれば、接続部が放射線不透過材料を含んでいることによって、放射線透視下で接続部が可視化されるため、生体内での接続部の位置を把握できる。そして、プッシャーからのコイルの離脱後、コイルの近位部には接続部の一部が残留しやすくなるため、接続部の位置を手がかりにして生体内でのコイルの位置を把握できる。
 また、上記生体内留置具の製造方法によれば、放射線透視下で接続部が可視化されることで生体内での接続部の位置を把握できる生体内留置具が得られる。そのような生体内留置具は、プッシャーからのコイルの離脱後、コイルの近位部には接続部の一部が残留しやすくなるため、接続部の位置を手がかりにして生体内でのコイルの位置を把握できる。

 以下、下記実施の形態に基づき本発明をより具体的に説明するが、本発明はもとより下記実施の形態によって制限を受けるものではなく、前・後記の趣旨に適合し得る範囲で適当に変更を加えて実施することも勿論可能であり、それらはいずれも本発明の技術的範囲に包含される。なお、各図面において、便宜上、ハッチングや部材符号等を省略する場合もあるが、かかる場合、明細書や他の図面を参照するものとする。また、図面における種々部材の寸法は、本発明の特徴の理解に資することを優先しているため、実際の寸法とは異なる場合がある。
1.生体内留置具
 以下では、生体内留置具を単に留置具と称することがある。留置具の使用の例としては、脳動脈瘤、頭頸部の動脈瘤、動静脈奇形、動静脈瘻、肺血管奇形、腎血管奇形、腎動脈、腹部動脈瘤等の目的部位での血栓化を促進する塞栓術が挙げられる。中でも、留置具は、脳動脈瘤用の留置具であることが好ましい。瘤の形状としては紡錘状や嚢状が挙げられる。
 本発明の一実施形態における生体内留置具は、長手軸方向を有するコイルと、前記長手軸方向において前記コイルよりも近位側に配されており、前記コイルを遠位側に向かって押すプッシャーと、前記コイルと前記プッシャーとを接続している接続部であって、前記コイルが前記プッシャーから離脱する離脱機構を備え、放射線不透過材料を含む接続部と、を有する点に要旨を有する。上記生体内留置具によれば、接続部が放射線不透過材料を含んでいることによって放射線透視下で接続部が可視化されるため、生体内での接続部の位置を把握できる。そして、プッシャーからのコイルの離脱後、コイルの近位部には接続部の一部が残留しやすくなるため、接続部の位置を手がかりにして生体内でのコイルの位置を把握できる。
 塞栓術にはFraming、Filling、Finishingのフェーズがある。留置具はそれぞれいずれか1つのフェーズに用いることもできるし、いずれか2つまたは3つのフェーズに亘って用いることもできる。留置具は、Finishing用でもよいが、Framing用および/またはFilling用であることが好ましい。
 図1~図8を参照しながら、本発明の一実施形態に係る生体内留置具を説明する。図1は本発明の一実施形態に係る生体内留置具の側面図である。図2は図1に示した生体内留置具の長手軸方向に沿った断面図(一部側面図)である。図3は図2に示した接続部を拡大した模式図である。図4~図8は図3に示した接続部の変形例を示す模式図である。図1~図3に示すように、留置具1は、コイル10と、プッシャー30と、接続部40と、を有している。接続部40は、離脱機構を備え、放射線不透過材料46を含んでいる。
 図2から理解できるように、コイル10は長手軸方向xを有する。コイル10は周方向pと径方向を好ましく有する。コイル10は長手軸方向xに遠位端と近位端を好ましく有する。コイル10の近位側とは、コイル10の長手軸方向xに対して使用者または術者の手元側の方向を指し、遠位側とは近位側の反対方向すなわち処置対象側の方向を指す。なお、図2において、図の右側が近位側であり、図の左側が遠位側である。コイル10の径方向とはコイル10の半径方向を指し、コイル10の径方向において内方とはコイル10の長手軸中心側に向かう方向を指し、径方向において外方とは内方と反対側に長手軸中心から放射状に延びる方向を指す。コイル10の周方向pとは長手軸周りの方向を指す。
 コイル10は、一または複数の線材21がらせん状に巻回されて構成される。線材21としては、単線、撚線、コイル状に巻回されたコイル線が挙げられ、中でも単線が好ましい。また、線材21はコイル線でないことが好ましい。
 線材21がらせん状に巻回されて構成されたものを一次コイルということがある。一次コイルをさらにらせん状や三次元形状に癖付けしたものを二次コイルということがある。本明細書では特に説明がない場合、コイル10は一次コイルの状態での構成を示している。なお、図2に示すような一次コイルのコイル10が癖付けされて図1に示すような二次コイルが構成されることが好ましい。図1では、一次コイルが巻回されることで立体的な二次コイル形状が構成されている。
 線材21は生体適合性および可撓性を有していることが好ましい。線材21を構成する材料としては、例えば、白金、金、チタン、タングステンおよびこれらの合金、ステンレス鋼等の金属材料またはこれらの組み合わせを挙げることができる。中でも線材21は白金-タングステン合金により構成されていることがより好ましい。
 線材21は長手軸方向を有し、線材21の長手軸方向に遠位端と近位端を有している。線材21は遠位端から近位端まで単一の線状部材から構成されていてもよく、長手軸方向において互いに連結された複数の線状部材から構成されてもよい。線材21の長手軸方向に垂直な断面の形状は、円形状、長円形状、多角形状、またはこれらの組み合わせであってもよい。線材21の長手軸方向に垂直な断面の形状は、線材21の長手軸方向の全体に亘って同じであってもよく、長手軸方向の位置によって異なっていてもよい。
 線材21の外径は特に限定されないが、例えば、25μm以上、30μm以上、または35μm以上であってもよく、75μm以下、または70μm以下であっても許容される。
 線材21の外径は、線材21の長手軸方向において同じ大きさであってもよく、線材21の長手軸方向の位置によって異なる大きさであってもよい。なお、線材21の断面が円形以外の場合、線材21の外径は円相当径を指すものとする。
 図2に示すように、コイル10はその長手軸方向xに延びている内腔11を好ましく有する。コイル10は外周面12と内周面13を好ましく有する。コイル10の外周面12はコイル10の外側、すなわち径方向の外側に面しており、コイル10の内周面13は内腔11に面している。内腔11には後述する伸張抵抗部材25が配置されることが好ましい。
 コイル10は、単層コイルであってもよく、複数の層を有している多層コイルであってもよい。コイル10の長手軸方向xの一部が単層であり、残りの部分が複層であってもよい。
 コイル10の密度、つまり巻き間隔は特に限定されず、密巻き、ピッチ巻き、またはこれらを組み合わせることができる。コイル10は長手軸方向xにおいて隣り合う線材21どうしが接触していてもよい。コイル10は、長手軸方向xの一部のみで隣り合う線材21どうしが接触していてもよく、長手軸方向xの全体に亘って隣り合う線材21どうしが接触していてもよい。また、コイル10は長手軸方向xにおいて隣り合う線材21どうしが接触していなくてもよい。接触していない状態とは、コイル10の長手軸方向xで隣り合う線材21どうしの間が開いている状態をいう。
 コイル10の長手軸方向xに垂直な断面の外縁形状は、円形状、長円形状、多角形状、またはこれらの組み合わせであってもよい。長円形状には楕円形状、卵形状、角丸長方形状が含まれるものとする。以降の説明においても同様である。
 コイル10の最大外径や最小外径は特に限定されず、手技のフェーズに応じて適宜選択することができるが、例えば、150μm以上、180μm以上、または200μm以上であってもよく、400μm以下、380μm以下、あるいは350μm以下であることも許容される。
 コイル10の外径および/または内径は、コイル10の長手軸方向xにおいて同じ大きさであってもよく、コイル10の長手軸方向xの位置によって異なる大きさであってもよい。なお、コイル10の断面が円形以外の場合、コイル10の外径は円相当径を指すものとする。またコイル10の内腔断面が円形以外の場合、コイル10の内径は円相当径を指すものとする。
 コイル10は長手軸方向xにおいて外径が一定であってもよい。外径が一定であるとは、コイル10の長手軸方向xの全体に亘ってコイル10の外径が実質的に一定の大きさであることを意味し、長手軸方向xの全体に亘ってコイル10の外径変化が±5%以内の範囲に収まっているものを含む。
 図示していないが、コイル10は遠位側に向かって外径が小さくなっている遷移部を有していてもよい。これにより、コイル10の柔軟性を遠位側から近位側に向かって徐々に高めることができる。遷移部では、コイル10の外径が遠位側に向かってテーパー状に小さくなっていてもよい。ここでテーパー状とはコイル10を構成する線材21が一巻き毎に外径が小さくなっている結果、遷移部においてコイル10の外径の包絡線がテーパー状になっている形態を含む。遷移部では、コイル10の外径が遠位側に向かって階段状に小さくなっていてもよい。ここで階段状とはコイル10を構成する線材21が少なくとも2巻き以上の巻き数毎に外径が小さくなっている形態を含む。
 コイル10を長手軸方向xにおいて遠位部と近位部に二等分割したときに、コイル10は近位部での外径の平均値が遠位部での外径の平均値よりも小さくてもよい。近位部で外径が小さいことにより、コイル10の遠位部の柔軟性を高めることができるため、操作性を確保することができる。
 プッシャー30は、留置具1を保持し、遠位側へ押し出すために用いられる部材である。プッシャー30は、長手軸方向xにおいてコイル10よりも近位側に配されており、コイル10を遠位側に向かって押すものである。プッシャー30は接続部40を介してコイル10と接続されている。
 プッシャー30は長手軸方向xに延びている棒状または線状の部材であることが好ましい。プッシャー30は、一または複数の部材から構成することができる。例えば、プッシャー30は、ワイヤー部材、コイル部材またはこれらの組み合わせから構成することができる。
 プッシャー30を介して接続部40には電気的または熱的エネルギーが供給されることが好ましい。そのため、プッシャー30は、生体適合性を有する導電性材料、中でも金属から構成されていることが好ましい。そのような材料としては、白金、タングステン、チタン、金、イリジウム、パラジウム、タンタルおよびこれらの合金、ステンレス鋼等を例示することができる。
 留置具送達用のカテーテル等の他の部材との摺動性を高めるために、プッシャー30には保護層31が配されていてもよい。例えば、プッシャー30の外面に保護層31がコーティングされていてもよい。保護層31は、プッシャー30の外面の周方向全体を覆っていてもよい。保護層31は、絶縁性材料から構成されていることが好ましく、例えば、ポリテトラフルオロエチレン(PTFE)等のフッ素系樹脂や親水性樹脂から構成することができる。
 図1~図2に示すように、プッシャー30の遠位端部30Dには保護層31が配されていないことが好ましい。これにより、プッシャー30の遠位端部30Dが露出するため、遠位端部30Dを加熱用の電極として使用しやすくなる。
 図1に示すように、プッシャー30の近位端部30Pに電気接続用の端子32が配されていることが好ましい。これにより、プッシャー30の近位端部30Pを高周波電源装置に接続することができるため、プッシャー30の遠位端部30Dに対してジュール熱を発生することができる。プッシャー30の近位端部30Pには保護層31が配されていないことが好ましい。このような構成により、保護層31が配されていない部分を端子32として機能させることができる。
 長手軸方向xにおけるプッシャー30の長さは、例えば0.1m以上1.8m以下とすることができる。プッシャー30の外径は、例えば0.1mm以上2.0mm以下とすることができる。
 図示していないが、プッシャー30には、位置特定用の一または複数のマーカーが配されていてもよい。そのようなマーカーとしては、例えば放射線不透過マーカー、具体的にはX線不透過マーカーが挙げられる。これにより、体内でのプッシャー30の位置を特定することができる。
 マーカーは放射線不透過物質を含んでいることが好ましい。放射線不透過物質としては、後述の放射線不透過材料46の説明を参照することができる。
 マーカーは、プッシャー30の遠位端部30Dに取り付けられていることが好ましい。プッシャー30が中空形状を有している場合、マーカーはプッシャー30の内腔に配されていてもよい。また、マーカーは、プッシャー30の外面に取り付けられていてもよい。
 図1~図2に示すように、接続部40は、コイル10とプッシャー30とを接続しているものである。図3~図8に示すように、接続部40は、コイル10がプッシャー30から離脱する離脱機構を備え、放射線不透過材料46を含む。接続部40が離脱機構を有することによって、手技中、操作者が離脱機構を作動させる操作を行うことで、プッシャー30からコイル10を離脱することができる。これにより生体内にコイル10を留置することができる。ここで、接続部40が放射線不透過材料46を含んでいることによって放射線透視下で接続部40が可視化されるため、生体内での接続部40の位置を把握できる。そして、プッシャー30からのコイル10の離脱後、コイル10の近位部には接続部40の一部が残留しやすくなるため、接続部40の位置を手がかりにして生体内でのコイル10の位置を把握できる。特に、複数のコイルを順番に動脈瘤内に配置する場合に好適である。例えば、放射線透視下で動脈瘤内に先行留置されるコイルの遠位端部と近位端部の位置の把握が困難であったが、留置具1によれば先行留置したコイルの近位端の位置を留置後にも確認できる。通常、コイルは近位端側の方に接着剤が配されていたり、コイルの内腔に離脱のための金属を含むことがあるため、遠位端側に比べて近位端側の方が硬い。このことから、後から追加で挿入される、より柔軟なコイルを留置する際に、その遠位端部が先行留置したコイルの近位端部に当たり、柔軟なコイルが瘤外に跳ね返される場合がある。放射線透視下で先行留置されたコイルの近位端部と、後から留置されるコイルとの位置関係を把握することで、先行するコイルの近位端部と後から留置されるコイルの遠位端部が接触することを避けるよう、操作者は、動脈瘤内に配置した、留置具1を送達するためのカテーテル、例えばマイクロカテーテルの挿入位置を調整することが可能となる。
 離脱機構としては、例えば、接続部40を化学的、熱的または電気的な方法で切断する方法、接続部40を水圧により押し出す方法、機械的な係合によって接続部40が構成されている場合には当該係合を解除する方法、接続部40を電気分解する方法等が挙げられる。
 接続部40は、生体適合性を有する材料から構成されていることが好ましい。接続部40を構成する材料は、離脱機構の種類に応じて適宜選択できるが、樹脂、エラストマー、ゴム、金属、またはこれらの組み合わせから構成されていることが好ましい。樹脂は、熱可塑性材料であることが好ましい。熱可塑性材料としては、熱可塑性樹脂や熱可塑性エラストマーが挙げられる。接続部40を構成する具体的な材料としては、例えば、ポリビニルアルコール(PVA)、PVA架橋重合体、PVA吸水ゲル凍結解凍エラストマー、エチレンビニルアルコール共重合体等のポリビニルアルコール系樹脂、ポリテトラフルオロエチレン、パーフルオロアルコキシアルカン、エチレンテトラフルオロエチレンコポリマー、ポリビニリデンフルオライド等のフッ素系樹脂、ポリエチレン、ポリプロピレン等のオレフィン系樹脂、ポリエチレンテレフタレート等のポリエステル樹脂、ナイロン等のポリアミド樹脂やポリイミド、ポリシリコーン、白金、金、ロジウム、パラジウム、レニウム、金、銀、チタン、タンタル、タングステン、マグネシウムおよびこれらの合金、ステンレス鋼等が挙げられる。なお、熱可塑性材料は、放射線不透過材料を含まないことが好ましい。
 離脱機構では、プッシャー30を介して供給される電気的または熱的エネルギーによって接続部40が加熱されてコイル10がプッシャー30から離脱することが好ましい。接続部40を加熱することでコイル10をプッシャー30から離脱させることができるため、コイル10の留置操作時間を短くすることができる。図1から理解できるように、プッシャー30の遠位端部30Dが加熱用電極として機能し、プッシャー30の遠位端部30Dと対極との間に供給される高周波電流によって接続部40が加熱されることが好ましい。
 離脱機構では、電気的または熱的エネルギーによってジュール熱が供給されることにより、接続部40が加熱されることが好ましい。加熱により接続部40が変形することが好ましく、加熱により接続部40が溶解または融解して接続部40が切断されることが好ましい。接続部40は熱可塑性材料45を含むことが好ましく、接続部40は熱可塑性樹脂または熱可塑性エラストマーを含むことがより好ましい。
 接続部40を構成する材料は、親水性を有していることが好ましい。また、接続部40は、膨潤性材料を含むことが好ましい。これにより、接続部40が電解液や生理食塩水等の溶媒を含有して膨張した状態で、接続部40が加熱されることにより、接続部40が容易に切断される。接続部40は、親水性樹脂を含むことが好ましく、例えば、ポリビニルアルコール(PVA)、PVA架橋重合体、PVA吸水ゲル凍結解凍エラストマー、エチレンビニルアルコール共重合体等のポリビニルアルコール系の重合体、ポリメタクリル酸ヒドロキシエチル、ポリアクリル酸、ポリアクリル酸ナトリウム、ポリアクリルアミド、ポリ-N-ビニルピロリドン、ポリビニルメチルエーテル、ポリエチレンオキシド、ポリエチレングリコール、ポリグリコール酸、メチルビニルエーテル無水マレイン酸共重合体、ポリヒドロキシエチルフタル酸エステル、ポリジメチロールプロピオン酸エステル、メチルイソプロピルケトンホルムアルデヒド、ポリエチレンイミン、ポリスチレンスルホネート、水溶性ナイロンなどの合成高分子物質;カルボキシメチルデンプン、ジアルデヒドデンプン、セルロース類(CMC,MC,HEC,HPC)、タンニン、リグニン、アルギン酸、アラビアゴム、グアーガム、トラガントガム、ゼラチン、カゼイン、にかわ、コラーゲンなどの天然高分子物質を含むことが好ましい。
 留置具1を体内に留置する前の状態で、接続部40が好ましく有する膨潤性材料が膨張していることが好ましい。膨潤性材料を膨張させるためには、接続部40を膨潤用液体に接触させればよい。例えば、膨潤用液体が入った容器に接続部40を浸漬することによって、接続部40を膨潤用液体に接触させることができる。このように接続部40を膨張状態にすることで、接続部40の加熱時に、接続部40を好ましく構成する親水性材料が溶解しやすくなり、接続部40が切断されやすくなる。
 膨潤用液体としては、例えば生理食塩水、純水、超純水、イオン交換水、ジメチルスルホキシドなどの化合物の水溶液、血液、またはこれらの組み合わせを挙げることができる。
 接続部40は、水溶性の塩類を含んでいてもよい。これにより、接続部40の強度を確保でき、加熱時の切断時間や膨潤処理の時間を短縮することができる。水溶性の塩類としては、例えば、25℃における水に対する溶解度が10wt%以上の金属のハロゲン化物、炭酸塩、硫酸塩、硝酸塩、塩素酸塩、次亜塩素酸塩、酢酸塩、カルボン酸塩、リン酸塩等の塩類を用いることができ、これらは結晶水を有していてもよい。
 接続部40に水溶性の塩類を含ませる方法は特に限定されない。例えば、膨潤用液体に水溶性の塩類を溶解させた溶液を接続部40に接触させる方法、または膨潤用液体とは別の水溶性の塩類を溶解させた溶液を接続部40に接触させる方法を採用してもよい。
 接続部40の形状は特に限定されず、線状、棒状、円柱状、多角柱状、円筒状、多角形筒状、円錐台状、多角錐台状、またはこれらの組み合わせであってもよい。
 本明細書において、放射線不透過材料46とは放射線不透過性を有する材料であり、放射線不透過性とは、放射線、特にX線が体内組織に比べて通過しにくく、放射線透視画像上で視認できる程度であることを意味する。放射線不透過性は、一般に、当該材料の原子番号と密度および当該材料からなる部材の厚さにより決まる。なお、X線透視画像では、放射線不透過材料46は白色または不透明に見える。また、本明細書において、放射線透過材料47とは、放射線不透過材料46に比べて、放射線、特にX線が通過しやすい材料であることを意味する。
 接続部40に含まれる放射線不透過材料46としては、例えばX線不透過材料が挙げられる。放射線不透過材料46としては、例えば、鉛、バリウム、ヨウ素、タングステン、金、銀、白金、イリジウム、白金イリジウム合金、ステンレス、チタン、コバルトクロム合金、パラジウム、タンタル等が挙げられる。また、接続部40に放射線不透過材料46が含まれることで、電気的または熱的エネルギーによってジュール熱が供給される際に、接続部40の熱可塑性樹脂または熱可塑性エラストマーへジュール熱が伝導し、コイル10の離脱が容易となる。
 放射線不透過材料46は、接続部40内に存在していてもよく、接続部40の表面に存在していてもよい。
 放射線不透過材料46は、バルク材料として接続部40に含まれていてもよい。図3に示すように、接続部40が放射線不透過材料46を含む金属製の棒状部材であってもよく、接続部40が放射線不透過材料46からなる棒状部材であってもよい。図示していないが、接続部40が放射線不透過材料46と放射線透過材料47とを含む金属製の棒状部材であってもよい。
 図4に示すように、接続部40は、長手軸方向xに延在している接続部本体41と、接続部本体41に取り付けられている一または複数のマーカー42とを有しており、マーカー42が放射線不透過材料46から構成されていてもよい。このように接続部40が放射線不透過材料46から構成されているマーカー42を有していることにより、体内での接続部40の位置を特定することができる。例えば、図4では、2つのリング形状のマーカー42が接続部本体41に取り付けられている。マーカー42は、長手軸方向xにおいて接続部本体41の任意の位置に取り付けることができるが、接続部本体41の遠位端部、近位端部の少なくともいずれかに取り付けられていることが好ましい。
 接続部本体41の形状は特に限定されず、線状、棒状、帯状、円柱状、多角柱状、円筒状、多角形筒状、円錐台状、多角錐台状、またはこれらの組み合わせであってもよい。
 接続部40が、棒形状を有する接続部本体41と、接続部本体41に取り付けられているマーカー42とを有している場合、プッシャー30を介して供給される電気的または熱的エネルギーによって接続部本体41が加熱されて接続部本体41が変形することが好ましい。また、加熱により接続部本体41が溶解または融解して接続部40が切断されることが好ましい。接続部本体41は樹脂から構成されていることが好ましい。図4に示すように、接続部本体41は熱可塑性材料45を含むことが好ましく、熱可塑性材料45は熱可塑性樹脂を含むことがより好ましい。接続部本体41を構成する材料は、親水性を有していることが好ましい。また、接続部本体41は、膨潤性材料を含むことが好ましい。接続部本体41は、水溶性の塩類を含んでいてもよい。接続部本体41の具体的な構成材料としては、上記の接続部40の構成材料の説明を参照することができる。
 プッシャー30が複数のマーカー42を有している場合、複数のマーカー42は長手軸方向xにおいて並んで配されていてもよく、周方向pにおいて並んで配されていてもよい。
 マーカー42の形状は特に限定されず、円形状、長円形状、多角形状、板形状、柱形状、錐形状、錐台形状、球形状、L字形状、T字形状、リング形状、リングに切れ込みが入った断面C字の形状、線材21を巻回したコイル形状、またはこれらを組み合わせた形状などであってもよい。
 マーカー42は、接続部本体41に対して溶接、溶着、接着、縫合、かしめ等の方法で固定することができる。
 図5に示すように、接続部40には、放射線不透過材料46からなる粒子が存在していることが好ましい。これにより、放射線不透過材料46がマーカー42等のバルク部材として構成されている場合と比較して、接続部40の引張強度を大きくすることができる。
 接続部40は、基材と放射線不透過材料46からなる粒子とを含んでいてもよい。基材を構成する材料としては、熱可塑性材料が挙げられる。また、基材を構成する材料としては、樹脂、エラストマー、ゴム等が挙げられる。例えば、接続部40は、樹脂と放射線不透過材料46からなる粒子とを含む樹脂組成物であってもよい。図5に示すように、接続部40は、熱可塑性材料45と、放射線不透過材料46からなる粒子とを含んでいてもよい。
 接続部40が、基材と放射線不透過材料46からなる粒子を含む場合、放射線不透過材料46からなる粒子の含有量は、基材100質量部に対して、1質量部以上、2質量部以上、3質量部以上、5質量部以上、10質量部以上であってもよく、30質量部以下、25質量部以下、20質量部以下、15質量部以下であってもよい。
 放射線不透過材料46からなる粒子は、接続部40の長手軸方向xの一部のみに存在していてもよく、接続部40の長手軸方向xの全体に亘って存在していてもよい。
 放射線不透過材料46からなる粒子は、接続部40に内包されていることが好ましく、接続部40内に分散していることが好ましい。例えば、接続部40は、樹脂と樹脂中に分散された放射線不透過材料46からなる粒子とを含んでいることが好ましい。放射線不透過材料46からなる粒子は、接続部40中に均一に分散していてもよく、局所的に分散していてもよい。なお、放射線不透過材料46からなる粒子は、接続部40の表面に露出していてもよく、接続部40の表面に固着されていてもよく、接続部40の表面を被覆していてもよい。
 放射線不透過材料46からなる粒子は、中実状でもよいし、中空状でもよい。また、放射線不透過材料46からなる粒子の形状は特に限定されないが、例えば、球状、楕円球状、円盤状、ラグビーボール状、不定形状等が挙げられる。
 放射線不透過材料46からなる粒子の粒径は特に限定されないが、例えば、0.010μm以上、0.050μm以上、0.10μm以上、0.20μm以上であってもよい。また、放射線不透過材料46からなる粒子の粒径は、5.0μm以下、4.0μm以下、3.0μm以下、2.0μm以下、1.0μm以下であってもよい。粒径が上記範囲にあることにより、接続部40を構成する材料中に放射線不透過材料46からなる粒子を均一に分散させやすくなる。また、粒径が上記範囲にあることにより、接続部40の形成に際し、形成用材料の押出成形や積層等の製造工程が行いやすくなる。
 「粒径」とは、動的光散乱法などで得られた粒径分布におけるメジアン50%径での体積平均粒子径(D50)を意味する。放射線不透過材料46からなる粒子としては市販品を用いてもよく、その場合、カタログ記載の粒径を採用できる。
 接続部40に放射線不透過材料46からなる粒子を存在させる方法としては、例えば、押出機などを用いて、接続部40を構成する材料を混合(好ましくは混錬)する際に放射線不透過材料46からなる粒子を加える方法や、接続部40の表面に放射線不透過材料46からなる粒子を付与する方法等が挙げられる。詳細は「2.生体内留置具の製造方法」の記載を参照することができる。
 図6に示すように、長手軸方向において接続部40の長さを遠位部40Dと近位部40Pに二等分割したときに、近位部40Pでの放射線不透過材料46からなる粒子の個数密度は、遠位部40Dでの放射線不透過材料46からなる粒子の個数密度よりも高いことが好ましい。接続部40の遠位部40Dの個数密度を低くすることにより、放射線透視画像において、接続部40の遠位側に位置しているコイル10と、接続部40の遠位部40Dとの濃淡差(コントラスト)が大きくなりやすいため、生体内での接続部40とコイル10の位置の特定が行いやすくなる。
 近位部40Pでの放射線不透過材料46からなる粒子の個数密度は、遠位部40Dでの放射線不透過材料46からなる粒子の個数密度の1.2倍以上であることが好ましく、1.5倍以上であることが好ましく、2.0倍以上であることが好ましい。また、近位部40Pでの放射線不透過材料46からなる粒子の個数密度は、遠位部40Dでの放射線不透過材料46からなる粒子の個数密度の10.0倍以下、8.0倍以下、5.0倍以下にすることも許容される。
 放射線不透過材料46からなる粒子の個数密度は、接続部40の遠位側から近位側に向かって漸次高くなることが好ましい。これにより、生体内での接続部40とコイル10の位置の特定がより一層行いやすくなる。
 図7に示すように、放射線不透過材料46は、第1の放射線不透過材料46Aからなる第1の粒子と、第2の放射線不透過材料46Bからなる第2の粒子を含むことが好ましい。ここで、第1の粒子と第2の粒子は、互いに異なる材料から構成されていることが好ましい。第1の粒子は第1の種類の粒子、第2の粒子は第2の種類の粒子と言い換えることもできる。接続部40が有する放射線不透過材料46が複数の種類の粒子を含むことにより、放射線透視画像での明瞭度を調整しやすくなる。第1の粒子と第2の粒子の種類としては、上述した放射線不透過材料46を任意に選択することができ、例えば、第1の粒子の材料はビスマスであり、第2の粒子の材料はプラチナであってもよい。第1の粒子の粒径と第2の粒子の粒径は同じであってもよく、いずれか一方が他方よりも大きくてもよい。
 図8に示すように、接続部40には、放射線透過材料47からなる粒子が存在していてもよい。このように放射線透過材料47からなる粒子を存在させることによっても、接続部40の引張強度を調整することができる。また、接続部40中の放射線透過材料47からなる粒子の個数密度によって、放射線透視画像の明瞭度も調整することができる。放射線透過材料47からなる粒子を構成する材料としては、ポリエチレン、ポリスチレン、ポリアクリル酸、ポリ乳酸、ポリグリコール酸等の樹脂、二酸化ケイ素等のガラス、チタン、銀、銅、鉄、アルミニウム等の非磁性の金属等が挙げられる。放射線透過材料47からなる粒子としては、例えば、ガラス製マイクロスフィア、ポリマーマイクロスフィア、シリカナノ粒子等のナノ粒子、チタン粉末等の金属粉末、ポリエチレン粉末等の樹脂粉末等が含まれる。中でも、細胞毒性が低く医療材料として広く使用されているチタン粉末は金属材料のなかで放射線透過性が高いことから好ましい。放射線不透過材料と放射線透過材料との透過度の差が大きいほど放射線透視画像の陰影に伴う明瞭度が大きくなる。
 接続部40は、基材と放射線不透過材料46からなる粒子と放射線透過材料47からなる粒子とを含んでいてもよい。基材を構成する材料としては、熱可塑性材料が挙げられる。また、基材を構成する材料としては、樹脂、エラストマー、ゴム等が挙げられる。接続部40は、樹脂と放射線不透過材料46からなる粒子と放射線透過材料47からなる粒子とを含む樹脂組成物であってもよい。図8に示すように、接続部40は、熱可塑性材料45と、放射線不透過材料46からなる粒子と、放射線透過材料47からなる粒子とを含んでいてもよい。
 接続部40が、樹脂と放射線透過材料47からなる粒子を含む場合、放射線透過材料47からなる粒子の含有量は、樹脂100質量部に対して、1質量部以上、2質量部以上、3質量部以上、5質量部以上、10質量部以上であってもよく、30質量部以下、25質量部以下、20質量部以下、15質量部以下であってもよい。
 接続部40において、放射線不透過材料46からなる粒子の個数密度は、放射線透過材料47からなる粒子の個数密度よりも高いことが好ましい。これにより、放射線透視画像において生体内での接続部40とコイル10の位置の特定が行いやすくなる。
 接続部40において、放射線不透過材料46からなる粒子の個数密度は、放射線透過材料47からなる粒子の個数密度の1.2倍以上であることが好ましく、1.5倍以上であることがより好ましく、2.0倍以上であることがさらに好ましく、また、10.0倍以下、8.0倍以下、5.0倍以下とすることも許容される。
 放射線透過材料47からなる粒子は、接続部40の長手軸方向xの一部のみに存在していてもよく、接続部40の長手軸方向xの全体に亘って存在していてもよい。
 放射線透過材料47からなる粒子は、接続部40に内包されていることが好ましく、接続部40内に分散していることが好ましい。例えば、接続部40は、樹脂と樹脂中に分散された放射線透過材料47からなる粒子とを含んでいてもよい。放射線透過材料47からなる粒子は、接続部40中に均一に分散していてもよく、局所的に分散していてもよい。なお、放射線透過材料47からなる粒子は、接続部40の表面に露出していてもよく、接続部40の表面に固着されていてもよく、接続部40の表面を被覆していてもよい。
 放射線透過材料47からなる粒子は、中実状でもよいし、中空状でもよい。また、放射線透過材料47からなる粒子の形状は特に限定されないが、例えば、球状、楕円球状、円盤状、ラグビーボール状、不定形状等が挙げられる。
 放射線透過材料47からなる粒子の粒径は特に限定されないが、放射線不透過材料46からなる粒子と同様の理由から、例えば、0.010μm以上、0.050μm以上、0.10μm以上、0.20μm以上であってもよく、5.0μm以下、4.0μm以下、3.0μm以下、2.0μm以下、1.0μm以下であってもよい。なお、粒径については、放射線不透過材料46からなる粒子の説明を参照することができる。放射線透過材料47からなる粒子の粒径は、放射線不透過材料46からなる粒子の粒径と同じであってもよく、いずれか一方が他方よりも大きくてもよい。
 接続部40に放射線透過材料47からなる粒子を存在させる方法については、接続部40に放射線不透過材料46からなる粒子を存在させる方法の説明を参照することができる。
 本発明の実施の形態に係る生体内留置具の送達システムは;長手軸方向を有する第1のコイルと、長手軸方向において第1のコイルよりも近位側に配されており、第1のコイルを遠位側に向かって押す第1のプッシャーと、第1のコイルと第1のプッシャーとを接続している第1の接続部であって、第1のコイルが第1のプッシャーから離脱する第1の離脱機構を備え、放射線不透過材料からなる粒子を含む第1の接続部と、を有する第1の生体内留置具と;長手軸方向を有する第2のコイルと、長手軸方向において第2のコイルよりも近位側に配されており、第2のコイルを遠位側に向かって押す第2のプッシャーと、第2のコイルと第2のプッシャーとを接続している第2の接続部であって、第2のコイルが第2のプッシャーから離脱する第2の離脱機構を備え、放射線不透過材料からなる粒子を含む第2の接続部と、を有する第2の生体内留置具と;を有していてもよい。その場合、第1の接続部に含まれる放射線不透過材料からなる粒子の個数密度が、第2の接続部に含まれる放射線不透過材料からなる粒子の個数密度よりも大きくてもよい。このようにコイル毎に放射線不透過材料からなる粒子の個数密度を変えることによって、留置するコイルの種別を放射線透視画像から判断することができる。また、操作中に留置中のコイル種別や長さを再認識できるだけでなく、再開通した動脈瘤の治療など過去に留置したコイルの種別や長さを把握することもできる。とりわけ、他院で過去に実施したコイル塞栓術におけるコイル種別を把握することは患者のカルテ情報を参照できない場合が多いことから難しかったが、このようなシステムによれば、放射線透視下でコイル種別を確認することができる。なお、図6から理解できるように、第1のコイルと第2のコイルのうち少なくともいずれかは、当該コイルの長手軸方向の位置によって放射線不透過材料からなる粒子の個数密度が異なっていてもよい。
 本発明の実施の形態に係る生体内留置具の送達システムは;長手軸方向を有する第1のコイルと、長手軸方向において第1のコイルよりも近位側に配されており、第1のコイルを遠位側に向かって押す第1のプッシャーと、第1のコイルと第1のプッシャーとを接続している第1の接続部であって、第1のコイルが第1のプッシャーから離脱する第1の離脱機構を備え、放射線不透過材料からなる粒子と放射線透過材料からなる粒子とを含む第1の接続部と、を有する第1の生体内留置具と;長手軸方向を有する第2のコイルと、長手軸方向において第2のコイルよりも近位側に配されており、第2のコイルを遠位側に向かって押す第2のプッシャーと、第2のコイルと第2のプッシャーとを接続している第2の接続部であって、第2のコイルが第2のプッシャーから離脱する第2の離脱機構を備え、放射線不透過材料からなる粒子と放射線透過材料からなる粒子とを含む第2の接続部と、を有する第2の生体内留置具と;を有していてもよい。その場合、第1の接続部に含まれる放射線不透過材料からなる粒子に対する放射線透過材料からなる粒子の個数密度の比が、第2の接続部に含まれる放射線不透過材料からなる粒子に対する放射線透過材料からなる粒子の個数密度の比と異なることが好ましい。このようにコイル毎に放射線不透過材料からなる粒子に対する放射線透過材料からなる粒子の個数密度の比を変えることによって、留置するコイルの種別を放射線透視画像から判断することができる。また、操作中に留置中のコイル種別や長さを再認識できるだけでなく、再開通した動脈瘤の治療など過去に留置したコイルの種別や長さを把握することもできる。とりわけ、他院で過去に実施したコイル塞栓術におけるコイル種別を把握することは患者のカルテ情報を参照できない場合が多いことから難しかったが、このようなシステムによれば、放射線透視下でコイル種別を確認することができる。なお、第1のコイルと第2のコイルのうち少なくともいずれかは、当該コイルの長手軸方向の位置によって放射線不透過材料からなる粒子に対する放射線透過材料からなる粒子の個数密度の比が異なっていてもよい。
 第1の接続部に含まれる放射線不透過材料からなる粒子に対する放射線透過材料からなる粒子の個数密度の比が、第2の接続部に含まれる放射線不透過材料からなる粒子に対する放射線透過材料からなる粒子の個数密度の比よりも低いことが好ましい。このように個数密度の比を設定することにより、留置するコイルの種別を放射線透視画像から判断することができる。
 長手軸方向において、第1のコイルは、第2のコイルよりも長いことが好ましい。コイルの全長が長いほど放射線不透過材料からなる粒子の個数密度を高くすることにより、第1コイルと第2コイルの双方で接続部をコイルと誤認することを防ぐことができる。コイルの長さは特に限定されないが、長手軸方向において、第1のコイルの長さは例えば15cm以上30cm以下、第2のコイルの長さは例えば5cm以上10cm以下とすることができる。
 長手軸方向において、第1のコイルは、第2のコイルと同じ長さを有していてもよい。その場合、第1のコイルの平均コイル径(一次コイル径)は、第2のコイルの平均コイル径(一次コイル径)よりも小さいことが好ましい。このようにコイル径を設定することにより、第1のコイルと第2のコイルのコイル長が同じであっても、第2のコイルを第1のコイルよりも柔軟に形成することができるため、第1のコイルの後に第2のコイルを体内に留置する操作が行いやすくなる。
 第1の接続部の近位部での放射線不透過材料からなる粒子の個数密度と遠位部での放射線不透過材料からなる粒子の個数密度との差が、第2の接続部の近位部での放射線不透過材料からなる粒子の個数密度と遠位部での放射線不透過材料からなる粒子の個数密度との差よりも大きいことが好ましい。これにより、放射線透視画像において、第2の接続部よりも第1の接続部での遠位部と近位部の濃淡差(コントラスト)を大きくすることができるため、留置するコイルの種別を放射線透視画像から判断することができる。
 第1のコイルは、第2のコイルよりも先に体内に挿入されるものであることが好ましい。第1の生体内留置具がFraming領域とFilling領域の少なくともいずれかで用いられるものであり、第2の生体内留置具が、Filling領域とFinishing領域の少なくともいずれかで用いられるものであることが好ましい。また、第1の生体内留置具がFraming領域で用いられるものであり、第2の生体内留置具が、Filling領域で用いられるものであることが好ましい。第1の生体内留置具がFilling領域で用いられるものであり、第2の生体内留置具がFinishing領域で用いられるものであることが好ましい。
 第1の生体内留置具、第2の生体内留置具の構成については、生体内留置具1の説明を参照することができる。第1のコイル、第2のコイルの構成については、コイル10の説明を参照することができる。第1のプッシャー、第2のプッシャーの構成については、プッシャー30の説明を参照することができる。第1の接続部、第2の接続部の構成については、接続部40の説明を参照することができる。
 図1~図2に示すように、コイル10はその遠位端部にヘッド部23を有していてもよい。ヘッド部23は、線材21の遠位端が生体内壁面に直接接触することを避けるために線材21の一部を覆うものである。ヘッド部23は、伸張抵抗部材25と接していてもよく、接していなくてもよい。
 ヘッド部23の形状はそれぞれ特に限定されないが、例えば、半球状、半長円球状、円柱状、多角柱状であってもよい。
 ヘッド部23はコイル10の外周面12と内周面13の少なくともいずれかに接合されてもよい。またヘッド部23の抜け落ちを防ぐために、ヘッド部23の一部はコイル10の遠位端部の内腔11に配されてもよい。ヘッド部23の近位端が線材21の遠位端よりも遠位側に位置していてもよく、ヘッド部23の近位端が線材21の遠位端よりも近位側に位置していてもよい。
 ヘッド部23は金属材料または樹脂から構成されてもよい。ヘッド部23を構成する樹脂としては熱可塑性樹脂や紫外線硬化樹脂が挙げられる。ヘッド部23を構成する樹脂としては、エポキシアクリレート系樹脂、ウレタンアクリレート系樹脂、ポリエステルアクリレート系樹脂、ポリエチレンテレフタレート系樹脂等のエステル系樹脂、ポリプロピレン等のオレフィン系樹脂を用いることができる。ヘッド部23を構成する金属としては、線材21の説明で挙げた金属を用いることができる。線材21とヘッド部23の材料は、同じでもよく異なっていてもよい。
 留置具1は、コイル10の内腔11に配置されている伸張抵抗部材25を有していることが好ましい。伸張抵抗部材25は、操作中にコイル10が長手軸方向xに伸張することを抑制する。伸張抵抗部材25は、単線または撚線からなる長尺部材であってもよい。伸張抵抗部材25は長手軸方向を有し、その長手軸方向に第1端と第2端を有している。伸張抵抗部材25は長手軸方向に垂直な径方向において単層から構成されていてもよく、複数の層から構成されていてもよい。伸張抵抗部材25は、複数の線材からなる撚線から構成される内層と、内層の外側に設けられ樹脂組成物を含む外層と、を有していてもよい。伸張抵抗部材25はコイル10の内腔11に1本のみ配置されていてもよく、複数本配置されていてもよい。
 伸張抵抗部材25は樹脂または金属から構成されてもよい。伸張抵抗部材25を構成する樹脂としてはポリエチレンテレフタレート等のポリエステル樹脂、ナイロン等のポリアミド樹脂、ポリエチレン、ポリプロピレン等のポリオレフィン樹脂を挙げることができる。樹脂であることで柔軟性が高まり、留置具1のデリバリー性能が向上する。また、伸張抵抗部材25が樹脂であることで、デリバリー時の金属疲労による破断をなくすことができる。伸張抵抗部材25をコイル10よりも長くすることおよび/または伸張抵抗部材25に伸びやすい材料を用いることにより、コイル10を瘤内に配置したときに伸張抵抗部材25の長さが不足してコイル10の端部が直線状に伸びることによる突っ張りを緩和できる。伸張抵抗部材25を構成する金属としては、白金、金、ロジウム、パラジウム、レニウム、銀、ニッケル、チタン、タンタル、タングステンおよびこれらの合金、ステンレス鋼を挙げることができる。
 伸張抵抗部材25はコイル10を構成する線材21とは異なる材料から構成されてもよい。例えば、コイル10が白金-タングステン合金により構成され、伸張抵抗部材25がポリプロピレン樹脂から構成されてもよい。
 伸張抵抗部材25はその長手軸方向と垂直な断面の形状が、円形状、長円形状、多角形状、またはこれらの組み合わせであってもよい。
 コイル10の内腔11に伸張抵抗部材25を配置しやすいように、伸張抵抗部材25の外径は、コイル10の内径の2分の1よりも小さいことが好ましく、3分の1以下であることがより好ましい。伸張抵抗部材25の破断防止のためには、伸張抵抗部材25の外径は、コイル10の内径の15分の1以上であることが好ましく、10分の1以上であることがより好ましい。
 伸張抵抗部材25は、直線状、波形状、らせん形状またはこれらの組み合わせにすることができる。
 伸張抵抗部材25の第1端側は、コイル10の遠位端部、具体的にはコイル10を構成する線材21の遠位端部に接続されていてもよい。伸張抵抗部材25の第2端側は、コイル10の近位端部、具体的にはコイル10を構成する線材21の近位端部に接続されていてもよい。伸張抵抗部材25の第2端側は、コイル10とプッシャー30を接続している接続部40に接続されていてもよい。伸張抵抗部材25が、伸張抵抗部材25の長手軸方向の途中で折り返された状態でコイル10の内腔11に配置されていてもよい。その場合、伸張抵抗部材25の折り返し部26がコイル10の遠位端部または近位端部に接続され、第1端側と第2端側がコイル10の近位端部または遠位端部、あるいは接続部40の遠位端部に接続されていることが好ましい。例えば図2では伸張抵抗部材25が長手軸方向の途中で折り返された折り返し部26を有し、折り返し部26がコイル10の遠位端部に接続され、第1端側と第2端側が後述する接続部40に接続されている。
 伸張抵抗部材25と他の部材との接続方法としては、溶着、溶接、かしめ等の圧着、接着剤による接着、係合、連結、結着、結紮等の物理的な固定等の方法、またはこれらの組み合わせを挙げることができる。ここで「接続」とは2つの要素が直接接続されている形態も、2つの要素が一以上の他の要素を介して間接的に接続されている形態も含むものとする。
 図2に示すように、コイル10の表面には薬剤50が配されていてもよい。薬剤50は、有効成分(原薬)単独であってもよいし、他の添加物との混合物であってもよい。添加物として好ましくは、基材、可塑剤、安定化剤、界面活性剤等が挙げられる。
 薬剤50の種類は患部の予防や治療に必要なものであれば特に限定されない。薬剤50は、抗炎症化作用、抗酸化作用、降圧作用およびずり応力感知抑制作用のうちの少なくともいずれか1つを有していてもよい。薬剤50としては、例えば、選択的セロトニン再取り込み阻害薬(SSRI)、DPP-4阻害薬、HMG―CoA還元酵素阻害薬、非ステロイド性抗炎症薬、アンジオテンシンII受容体拮抗薬、トコフェロール酢酸エステル、アスコルビン酸、エダラボン、NアセチルLシステイン、カルシウム拮抗薬、利尿薬、アンジオテンシンII受容体拮抗薬(ARB)、アンジオテンシン変換酵素阻害薬(ACE)、β遮断薬、α遮断薬、αβ遮断薬等が挙げられる。
 薬剤50は、コイル10の表面に付着していてもよいし、生体接着剤を介して間接的にコイル10の表面に付着していてもよい。生体接着剤の材料の種類は特に限定されないが、例えばコラーゲン、キトサン、ゼラチン等の多糖類接着剤、ポリエチレングリコールベースのヒドロゲル接着剤、フィブリン、コラーゲン等のタンパク質接着剤等を用いることができる。
 薬剤50は、薬剤層としてコイル10の表面に保持されていてもよい。薬剤50は、マイクロカプセル等のカプセルに内包された形態でコイル10の表面に保持されていてもよい。カプセルに内包された薬剤50が層状になってコイル10の表面に保持されていてもよい。
 薬剤50はカプセルに内包されていることが好ましい。コイル10が生体内の患部に送達されたときに効果的に薬剤50が放出されるようにすることができる。カプセルに内包された薬剤50が、コイル10の表面に付着していてもよいし、生体接着剤を介して間接的にコイル10の表面に付着していてもよい。
 カプセルのサイズは好ましくは10nm以上、より好ましくは50nm以上、さらに好ましくは100nm以上、また、好ましくは500nm以下、より好ましくは400nm以下、さらに好ましくは200nm以下である。
 薬剤50は、生分解性の材料を含むカプセルに内包されていることが好ましい。生分解性の材料としては、生体吸収性高分子、天然高分子、脱細胞化処理した生体組織や細胞またはそれらの組み合わせを用いることができる。生体吸収性高分子としてはポリ乳酸(PLA)、ポリL-乳酸(PLLA)、ポリグルコール酸(PGA)、乳酸とグリコール酸の共重合体(PLGA)、ポリカプロラクトン(PCL)、ポリジオキサノン(PDS)のうちの少なくともいずれか一種が好ましく用いられる。また、天然高分子としてはコラーゲン、ラミニン、フィブロイン、ゼラチン、グリコサミノグリカン、キチン、キトサン、ヒアルロン酸、ポリペプチドのうちの少なくともいずれか一種が好ましく用いられる。生分解性の材料としては中でもPLGAが好ましい。
 カプセルは、複数個の糸状の生体吸収性高分子が球状に凝縮された構造を有することが好ましい。糸状の生体吸収性高分子の数は、好ましくは10,000個以上、より好ましくは20,000個以上、さらに好ましくは30,000個以上、また、糸状の生体吸収性高分子の数は100,000個以下、90,000個以下、80,000個以下であってもよい。中でも、カプセルは、10,000個以上100,000個以下の糸状のPLGAが球状に凝縮された構造を有することが好ましい。
 カプセルを構成する材料のガラス転移温度は特に限定されないが、例えば40℃以上、41℃以上、42℃以上であってもよく、50℃以下、49℃以下、48℃以下とすることも許容される。
 カプセルの表面は被覆材によって覆われていることが好ましい。被覆材により、生体内への留置具1の送達中に薬剤50が血液中に溶出したり脱落することを抑制することができる。被覆材を構成する材料は、薬剤50の初期バーストを防止する観点から水溶性高分子であることが望ましい。被覆材の材料としては、カルボキシルメチルセルロース、ヒドロキシプロピルセルロース、メチルセルロース、ヒドロキシエチルセルロース、ポリビニルアルコール、アルギン酸、ペクチン、アラビアガム、ジェランガム、グアガム、キサンタンガム、カラギーナン、ゼラチンなどを挙げることができる。
 カプセルの表面には界面活性剤が付与されていてもよい。界面活性剤が付与されていることにより、カプセルの細胞膜流動性が高まり、細胞膜に対する薬物の透過性を向上させることができる。界面活性剤の種類は特に限定されないが、非イオン性界面活性剤であることが好ましく、例えばポリオキシエチレンソルビタンモノラウレート、好ましくはTween(登録商標)20、Tween(登録商標)80等が挙げられる。
 薬剤50は、コイル10の外周面12に配されていてもよいし、コイル10の内周面13に配されていてもよいし、コイル10の外周面12と内周面13に配されていてもよい。薬剤50は、コイル10の長手軸方向xの一部のみに配されていてもよく、コイル10の長手軸方向xの全体に亘って配されていてもよい。薬剤50は、コイル10の周方向pの一部のみに配されていてもよく、コイル10の周方向pの全体に亘って配されていてもよい。
 薬剤50は、伸長抵抗部材25の表面に配されていてもよい。薬剤50は、伸長抵抗部材25の長手軸方向の一部のみに配されていてもよく、伸長抵抗部材25の長手軸方向の全体に亘って配されていてもよい。コイル10と同様に、薬剤50は、伸張抵抗部材25の表面に付着していてもよく、生体接着剤を介して間接的に伸張抵抗部材25の表面に付着していてもよい。また、コイル10と同様に、カプセルに内包された薬剤50は、伸張抵抗部材25の表面に付着していてもよいし、生体接着剤を介して間接的に伸張抵抗部材25の表面に付着していてもよい。生体接着剤の種類としては、コイル10に付着される生体接着剤の種類についての説明を参照することができる。カプセルの構造については、コイル10に付着されるカプセルの構造についての説明を参照することができる。
2.生体内留置具の製造方法
 本発明の一実施形態における生体内留置具の製造方法は、長手軸方向を有するコイルと、前記コイルを遠位側に向かって押すためのプッシャーと、を準備する工程と、前記コイルと前記プッシャーとを接続する接続部であって、放射線不透過材料を含む接続部を形成する工程と、前記コイルと前記プッシャーとを前記接続部で接続する工程と、を有する点に要旨を有する。上記生体内留置具の製造方法によれば、放射線透視下で接続部が可視化されることで生体内での接続部の位置を把握できる生体内留置具が得られる。そのような生体内留置具は、プッシャーからのコイルの離脱後、コイルの近位部には接続部の一部が残留しやすくなるため、接続部の位置を手がかりにして生体内でのコイルの位置を把握できる。
 図9~図14を参照しながら、本発明の一実施形態に係る生体内留置具の製造方法を説明する。図9~図14は本発明の一実施形態に係る生体内留置具の製造方法を示すフローチャートである。本明細書に記載の製造方法を実施することで、図1~図8に示した生体内留置具を得ることができる。
 図9に示すように、上記製造方法は、長手軸方向を有するコイルと、コイルを遠位側に向かって押すためのプッシャーと、を準備する工程(ステップS1)を有する。コイルは周方向と径方向を好ましく有する。コイルは長手軸方向に遠位端と近位端を好ましく有する。コイルの近位側とは、コイルの長手軸方向に対して使用者または術者の手元側の方向を指し、遠位側とは近位側の反対方向すなわち処置対象側の方向を指す。コイルの径方向とはコイルの半径方向を指し、コイルの径方向において内方とはコイルの長手軸中心側に向かう方向を指し、径方向において外方とは内方と反対側に長手軸中心から放射状に延びる方向を指す。コイルの周方向とは長手軸周りの方向を指す。
 コイルは、一または複数の線材がらせん状に巻回されて構成される。線材としては、単線、撚線、コイル状に巻回されたコイル線が挙げられ、中でも単線が好ましい。また、線材はコイル線でないことが好ましい。
 線材がらせん状に巻回されて構成されたものを一次コイルということがある。一次コイルをさらにらせん状や三次元形状に癖付けしたものを二次コイルということがある。ステップS1において準備するコイルは一次コイルでも二次コイルでもよい。
 プッシャーは、留置具を保持し、遠位側へ押すために用いられる部材である。プッシャーは長手軸方向に延びている、棒状または線状の部材であることが好ましい。プッシャーは、一または複数の部材から構成することができる。例えば、プッシャーは、ワイヤー部材、コイル部材またはこれらの組み合わせから構成することができる。
 プッシャーを介して接続部には電気的または熱的エネルギーが供給されることが好ましい。そのため、プッシャーは、生体適合性を有する導電性材料、中でも金属から構成されていることが好ましい。そのような材料としては、白金、タングステン、チタン、金、イリジウム、パラジウム、タンタルおよびこれらの合金、ステンレス鋼等を例示することができる。
 留置具送達用のカテーテル等の他の部材との摺動性を高めるために、プッシャーは保護層が配されていてもよい。例えば、プッシャーの外面に保護層がコーティングされていてもよい。保護層は、プッシャーの外面の周方向全体を覆っていてもよい。保護層は、絶縁性材料から構成されていることが好ましく、例えば、ポリテトラフルオロエチレン(PTFE)等のフッ素系樹脂や親水性樹脂から構成することができる。
 プッシャーの遠位端部には保護層が配されていないことが好ましい。これにより、プッシャーの遠位端部が露出するため、遠位端部を加熱用の電極として使用しやすくなる。
 プッシャーの近位端部に電気接続用の端子が配されていることが好ましい。これにより、プッシャーの近位端部を高周波電源装置に接続することができるため、プッシャーの遠位端部に対してジュール熱を発生することができる。プッシャーの近位端部には保護層が配されていないことが好ましい。このような構成により、保護層が配されていない部分を端子として機能させることができる。
 長手軸方向におけるプッシャーの長さは、例えば0.1m以上1.8m以下とすることができる。プッシャーの外径は、例えば0.1mm以上2.0mm以下とすることができる。
 プッシャーには、位置特定用の一または複数のマーカーが配されていてもよい。そのようなマーカーとしては、例えば放射線不透過マーカー、具体的にはX線不透過マーカーが挙げられる。これにより、体内でのプッシャーの位置を特定することができる。マーカーの具体的な構成については、「1.生体内留置具」で記載したマーカーの説明を参照することができる。
 図9に示すように、上記製造方法は、コイルとプッシャーとを接続する接続部であって、放射線不透過材料を含む接続部を形成する工程(ステップS2)を有する。ステップS2を行うことにより、放射線不透過材料によって放射線透視下で接続部が可視化されることで生体内での接続部の位置を把握できる生体内留置具が得られる。そのような生体内留置具は、プッシャーからのコイルの離脱後、コイルの近位部には接続部の一部が残留しやすくなるため、接続部の位置を手がかりにして生体内でのコイルの位置を把握できる。
 接続部を形成する工程(ステップS2)では、バルク材料として放射線不透過材料を含む接続部を形成してもよい。例えば、ステップS2では、放射線不透過の金属の棒状部材を準備することによって接続部を形成してもよい。これにより、図3に示すような接続部40が得られる。ステップS2では、放射線不透過の金属の粉末を鋳造、射出成形、圧結の少なくともいずれかを行うことによって接続部を形成してもよい。ステップS2では、金属線材を巻回することによってコイル状の接続部を形成してもよい。ステップS2では、放射線不透過の金属板を丸め、両端を接合することによって管状の接続部を形成してもよい。また、接続部を形成する工程(ステップS2)では、樹脂、エラストマー、ゴム等から構成された接続部本体に、放射線不透過材料を含むマーカーを取り付けることによって接続部を形成してもよい。これにより、図4に示すような接続部40が得られる。
 接続部を形成する工程(ステップS2)では、放射線不透過材料からなる粒子を含む接続部を形成してもよい。この方法については、図10~図14を用いて後述する。
 図9に示すように、上記製造方法は、コイルとプッシャーとを接続部で接続する工程(ステップS3)を有する。ステップS3は、コイルと接続部を接続する工程と、プッシャーと接続部を接続する工程とを有していてもよい。ステップS3では、コイルの近位端部と接続部の遠位端部とを接続することが好ましい。また、ステップS3では、接続部の近位端部とプッシャーの遠位端部とを接続することが好ましい。
 ステップS3において、コイルと接続部、また接続部とプッシャーとを接続する方法としては、溶着、溶接、かしめ等の圧着、接着剤による接着、係合、連結、結着、結紮等の物理的な固定等の方法、またはこれらの組み合わせを挙げることができる。ここで「接続」とは2つの要素が直接接続されている形態も、2つの要素が一以上の他の要素を介して間接的に接続されている形態も含むものとする。
 ステップS1、ステップS2、ステップS3はこの順に行われることが好ましい。
 図10を参照しながら、図9に示した生体内留置具の製造方法の変形例について説明する。図10に示す製造方法では、図9に示した製造方法のうち接続部を形成する工程の具体的な構成が示されている。
 図10に示す製造方法は、長手軸方向を有するコイルと、コイルを遠位側に向かって押すためのプッシャーと、を準備する工程(ステップS11)と、接続部を形成する工程(ステップS12)と、コイルとプッシャーとを接続部で接続する工程(ステップS13)を有する。ステップS11は、ステップS1と同様の方法で行うことができる。ステップS13は、ステップS3と同様の方法で行うことができる。以下ではステップS12について詳しく説明する。
 図10に示すように、上記製造方法において接続部を形成する工程(ステップS12)は、熱可塑性材料と放射線不透過材料からなる粒子とを混合する工程(ステップS12-1)を含むことが好ましい。混合することにより、接続部の全体に放射線不透過材料が行き渡りやすくなるため、放射線透視下で接続部が可視化されやすくなり、生体内での接続部の位置を把握しやすくなる。これにより、図5~図8に示すような接続部40が得られる。
 ステップS12-1では、熱可塑性材料と放射線不透過材料からなる粒子とを混練することが好ましい。混練することにより、熱可塑性材料中に放射線不透過材料からなる粒子が分散されやすくなる。すなわち、接続部の全体に放射線不透過材料が均一に行き渡りやすくなることによって、接続部がより一層可視化されやすくなる。混練には、公知の混練押出機等を用いることができる。ステップS12-1では、熱可塑性材料を含む材料は、放射線不透過材料を含まないことが好ましい。
 ステップS12-1において、放射線不透過材料は、第1の放射線不透過材料からなる第1の粒子と、第2の放射線不透過材料からなる第2の粒子を含んでいてもよい。これにより、図7に示すような接続部40が得られる。ステップS12-1において、熱可塑性材料および放射線不透過材料としては「1.生体内留置具」で説明した材料を用いることができる。
 図10に示すように、上記製造方法において接続部を形成する工程(ステップS12)は、さらに、熱可塑性材料と放射線不透過材料からなる粒子を混合して得られた材料を押出成形する工程(ステップS12-2)を含むことが好ましい。押出成形により、任意の形状を有する接続部を形成することができる。また、押出成形を採用することで製品の大量生産や品質の確保等にも対応することができる。押出成形には公知の押出成形機等を用いることができる。
 図10に示すように、上記製造方法において接続部を形成する工程(ステップS12)は、さらに、押出成形された材料を延伸する工程(ステップS12-3)を含むことが好ましい。この工程により、コイルとプッシャーを接続するのに適した形状に接続部を変形させることができる。また、用いる材料によっては、材料中の分子鎖が整列して結晶化されることによる接続部の強度向上、応力の緩和等の効果も得られる。延伸には公知の延伸機等を用いることができる。
 ステップS12-3では、押出成形された材料を加熱せずに延伸することが好ましく、押出成形された材料を加熱せずに室温環境下(例えば23℃±2℃)で延伸することがより好ましい。接続部を加熱、冷却するための時間が不要となるため、生産効率の向上が図られる。
 ステップS12-3では、押出成形された材料を加熱延伸してもよい。加熱延伸により、材料の結晶化が促進されるため、接続部の強度を高めることができる。押出成形された材料の加熱には公知の加熱装置を用いることができる。
 図11を参照しながら、図10に示した生体内留置具の製造方法の変形例について説明する。図11に示す製造方法が、図10に示した製造方法と異なっているのは、接続部を形成する工程で三次元積層造形法を用いている点である。
 図11に示す製造方法は、長手軸方向を有するコイルと、コイルを遠位側に向かって押すためのプッシャーと、を準備する工程(ステップS21)と、接続部を形成する工程(ステップS22)と、コイルとプッシャーとを接続部で接続する工程(ステップS23)を有する。ステップS21は、ステップS1、ステップS11と同様の方法で行うことができる。ステップS23は、ステップS3、ステップS13と同様の方法で行うことができる。以下ではステップS22について詳しく説明する。
 図11に示すように、接続部を形成する工程(ステップS22)は、熱可塑性材料と放射線不透過材料からなる粒子を混合する工程(ステップS22-1)を含むことが好ましい。ステップS22-1は、ステップS12-1と同様に行うことができるため、説明を省略する。
 図11に示すように、上記製造方法において接続部を形成する工程(ステップS22)は、熱可塑性材料と放射線不透過材料からなる粒子を混合して得られた材料を三次元積層造形法により積層造形する工程(ステップS22-2)を含むことが好ましい。積層造形により接続部を形成することで、接続部の形状設計の自由度が高められるとともに、オーダーメイドや小ロットでの生産が可能となる。このような方法によっても、例えば図5に示すような接続部40が得られる。
 ステップS22-2では、例えば以下の手順で積層造形を行うことができるが、これらの方法に限定されるものではない。まず、接続部の三次元の設計データ(3Dモデル)を、CADソフトウェアやモデリングソフトウェアを用いて作成する。3Dモデルを層状にスライスする。スライスには、公知のスライスソフトウェアを用いることができる。設計データに基づき、混合して得られた材料を積層する。ここで、混合して得られた材料の形状としては、糸状のフィラメント、粉末状、液体状、ペースト状等が挙げられる。混合して得られた材料を積層する工程では、該材料を加熱しながら積層してもよい。接続部の造形中の形状保持のために、土台等のサポート材の上に、混合して得られた材料を積層してもよい。積層には、熱溶解積層法、光造形法、パウダーベッド法(粉末床溶融結合法)、インクジェット法等を用いることができる。積層後は、材料を固化することが好ましい。一層を形成する毎に材料を固化してもよいし、複数の層を積み重ねた後に材料を固化してもよい。材料の固化方法は特に限定されないが、材料の加熱、紫外線光やレーザービーム等の照射等を行ってもよい。材料の固化後に、接続部を支持していたサポート材を除去してもよい。積層造形には公知の積層造形装置、例えば3Dプリンターを用いることができる。
 必須ではないが、ステップS22-2では、積層造形された材料を延伸してもよく、その際に加熱延伸してもよい。ステップS22-2では、ステップS12-3と同様の方法で延伸することができる。
 図11に示す製造方法では、ステップS21、ステップS22、ステップS23がこの順に行われることが好ましい。また、ステップS22は、ステップS22-1、ステップS22-2の順に行われることが好ましい。
 図12を参照しながら、図11に示した生体内留置具の製造方法の変形例について説明する。図12に示す製造方法が、図11に示した製造方法と異なっているのは、接続部の材料として、放射線不透過材料からなる粒子と放射線透過材料からなる粒子とを用いている点である。
 図12に示す製造方法は、コイルとプッシャーとを準備する工程(ステップS31)と、接続部を形成する工程(ステップS32)と、コイルとプッシャーとを接続部で接続する工程(ステップS33)を有する。ステップS31は、ステップS1、ステップS11、ステップS21と同様の方法で行うことができる。ステップS33は、ステップS3、ステップS13、ステップS23と同様の方法で行うことができる。以下ではステップS32について詳しく説明する。
 図12に示すように、上記製造方法において接続部を形成する工程(ステップS32)は、放射線不透過材料からなる粒子と、放射線透過材料からなる粒子とを混合する工程(ステップS32-1)を含むことが好ましい。接続部を形成する工程(ステップS32)において、熱可塑性材料と、放射線不透過材料からなる粒子と、放射線透過材料からなる粒子とを混合してもよい。これにより、図8に示すような接続部40が得られる。ステップS32-1において材料を混合する方法は、ステップS12-1、ステップS22-1と同様に行うことができるため、説明を省略する。放射線透過材料としては「1.生体内留置具」で説明した材料を用いることができる。
 図12に示すように、上記製造方法において接続部を形成する工程(ステップS32)は、放射線不透過材料からなる粒子と、放射線透過材料からなる粒子とを混合して得られた材料を三次元積層造形法により積層造形する工程(ステップS32-2)を含むことが好ましい。ステップS32-2は、ステップS22-2と同様の方法で行うことができる。
 ステップS32-2に代えて、放射線不透過材料からなる粒子と、放射線透過材料からなる粒子とを混合して得られた材料を押出成形する工程を行ってもよい。また、ステップS32-2に代えて、放射線不透過材料からなる粒子と、放射線透過材料からなる粒子とを混合して得られた材料を押出成形する工程と、押出成形された材料を延伸する工程とを行ってもよい。押出成形する工程については、ステップS12-2と同様の方法で行うことができる。押出成形された材料を延伸する工程については、ステップS12-3と同様の方法で行うことができる。
 図13を参照しながら、図11に示した生体内留置具の製造方法の変形例について説明する。図13に示す製造方法が、図11に示した製造方法と異なっているのは、接続部を形成する工程において、互いに異なる材料からなる第1素材部と第2素材部を形成し、これら2つの素材部を接合する点である。
 図13に示す製造方法も、コイルとプッシャーとを準備する工程(ステップS41)と、接続部を形成する工程(ステップS42)と、コイルとプッシャーとを接続部で接続する工程(ステップS43)を有する。ステップS41は、ステップS1、ステップS11、ステップS21、ステップS31と同様の方法で行うことができる。ステップS43は、ステップS3、ステップS13、ステップS23、ステップS33と同様の方法で行うことができる。以下ではステップS42について詳しく説明する。
 図13に示すように、ステップS42は、熱可塑性材料を含む材料から第1素材部を形成する工程(ステップS42-1)を有することが好ましい。
 ステップS42-1において、熱可塑性材料は、熱可塑性樹脂または熱可塑性エラストマーであることが好ましい。ステップS42-1では、熱可塑性材料を混練することで第1素材部を形成することが好ましい。材料を混練することにより、材料の密度や材料の粒子の形状が均一化されやすくなるため、接続部の品質を向上させることができる。ステップS42-1では、熱可塑性材料を含む材料は、放射線不透過材料を含まないことが好ましい。
 ステップS42-1では、熱可塑性材料を含む材料と放射線透過材料からなる粒子とから第1素材部を形成してもよい。その場合、第2素材部における放射線不透過材料からなる粒子の個数密度よりも、第1素材部における放射線透過材料からなる粒子の個数密度が低いことが好ましい。これにより、図6に示すような接続部40が得られる。
 ステップS42は、熱可塑性材料と放射線不透過材料からなる粒子を混合し、第2素材部を形成する工程(ステップS42-2)を有することが好ましい。
 ステップS12-1と同様に、ステップS42-2では、熱可塑性材料と放射線不透過材料からなる粒子とを混練することが好ましい。ステップS42-2において、熱可塑性材料は、熱可塑性樹脂であることが好ましい。
 図示していないが、ステップS42-2では、熱可塑性材料と放射線不透過材料からなる粒子と放射線透過材料からなる粒子とを混合して第2素材部を形成してもよい。
 ステップS42-1で形成される第1素材部は固体であることが好ましい。ステップS42-2で形成される第2素材部は固体であることが好ましい。
 ステップS42-1で形成される第1素材部の形状と、ステップS42-2で形成される第2素材部の形状は、線状、棒状、円柱状、多角柱状、円筒状、多角形筒状、円錐台状、多角錐台状、またはこれらの組み合わせであってもよい。第1素材部と第2素材部の形状は互いに異なっていてもよいが、同じであることが好ましい。
 図13に示すように、ステップS42は、第1素材部と第2素材部とを接合する工程(ステップS42-3)を有することが好ましい。互いに異なる材料から構成された第1素材部と第2素材部を接合することによって、第1素材部と第2素材部が並んでいる方向において、接続部中の放射線不透過材料からなる粒子の個数密度を変化させることができる。その結果、放射線透視画像において、例えば、コイルと接続部との濃淡差(コントラスト)を調整することができ、生体内での接続部とコイルの位置の特定が行いやすくなる。
 ステップS42-3において第1素材部と第2素材部とを接合する方法は特に限定されないが、例えば、熱溶着、接着剤による接着、超音波接合、溶接、圧接またはこれらの組み合わせを挙げることができる。
 図13に示すように、ステップS42は、ステップS42-3で接合した部材を押出成形する工程(ステップS42-4)を有することが好ましい。また、図13に示すように、ステップS42は、ステップS42-4で得られた、押出成形された材料を延伸する工程(ステップS42-5)を有することが好ましい。押出成形する工程(ステップS42-4)については、ステップS12-2と同様の方法で行うことができる。押出成形された材料を延伸する工程(ステップS42-5)については、ステップS12-3と同様の方法で行うことができる。
 図14を参照しながら、図11に示した生体内留置具の製造方法の変形例について説明する。図14に示す製造方法が、図11に示した製造方法と異なっているのは、接続部を形成する工程において、第3素材部を形成した後に第3素材部に放射線不透過材料からなる粒子を付与する点である。
 図14に示す製造方法も、コイルとプッシャーとを準備する工程(ステップS51)と、接続部を形成する工程(ステップS52)と、コイルとプッシャーとを接続部で接続する工程(ステップS53)を有する。ステップS51は、ステップS1、ステップS11、ステップS21、ステップS31、ステップS41と同様の方法で行うことができる。ステップS53は、ステップS3、ステップS13、ステップS23、ステップS33、ステップS43と同様の方法で行うことができる。以下ではステップS52について詳しく説明する。
 図14に示すように、ステップS52は、第3素材部を形成する工程(ステップS52-1)を有することが好ましい。ステップS52-1では、熱可塑性材料を含む材料から第3素材部を形成することが好ましい。ステップS52-1では、熱可塑性材料を含む材料は、放射線透過材料を含まないことが好ましい。また、第3素材部は、放射線不透過材料を含まないことが好ましい。ステップS52-1は、ステップS42-1と同様の方法で行うことができる。
 ステップS52-1において、熱可塑性材料は、熱可塑性樹脂または熱可塑性エラストマーであることが好ましい。ステップS52-1では、材料を混練することで第3素材部を形成することが好ましい。材料を混練することにより、材料の密度や材料の粒子の形状が均一化されやすくなるため、接続部の品質を向上させることができる。
 必須ではないが、ステップS52は、ステップS52-1で得られた材料を押出成形する工程(ステップS52-2)を有していてもよい。また、ステップS52は、押出成形された材料を延伸する工程(ステップS52-3)を有していてもよい。押出成形する工程(ステップS52-2)については、ステップS12-2と同様の方法で行うことができる。押出成形された材料を延伸する工程(ステップS52-3)については、ステップS12-3と同様の方法で行うことができる。ステップS52-1、ステップS52-2、ステップS52-3、ステップS52-4はこの順に行われることが好ましい。
 第3素材部の形状としては、第1素材部および第2素材部の形状を参照することができる。
 図14に示すように、ステップS52は、第3素材部に、放射線不透過材料からなる粒子を付与する工程(ステップS52-4)を有することが好ましい。ステップS52-4では、第3素材部の表面に、放射線不透過材料からなる粒子を付着させることが好ましい。ステップS52-4において、粒子の付与方法としては、例えば刷毛塗り、ロールコーティング、ディップコーティング、スプレーコーティング等が挙げられる。
 図示していないが、第3素材部が長手方向を有し、第3素材部が長手方向の長さを二等分割した第1側部と第2側部を有している場合、ステップS52-2において、第1側部のみに放射線不透過材料からなる粒子を付与し、第2側部には放射線不透過材料からなる粒子を付与しなくてもよい。これにより、第3素材部の長手方向において放射線不透過材料からなる粒子の個数密度を変化させることができる。その結果、放射線透視画像において、例えば、コイルと接続部との濃淡差(コントラスト)を調整することができ、生体内での接続部とコイルの位置の特定が行いやすくなる。
 上述した製造方法の各工程の実施に際しては「1.生体内留置具」に記載の構成や方法を適宜参照することができる。

Claims (13)

  1.  長手軸方向を有するコイルと、
     前記長手軸方向において前記コイルよりも近位側に配されており、前記コイルを遠位側に向かって押すプッシャーと、
     前記コイルと前記プッシャーとを接続している接続部であって、前記コイルが前記プッシャーから離脱する離脱機構を備え、放射線不透過材料を含む接続部と、を有する生体内留置具。
  2.  前記離脱機構では、前記プッシャーを介して供給される電気的または熱的エネルギーによって前記接続部が加熱されて前記コイルが前記プッシャーから離脱する請求項1に記載の生体内留置具。
  3.  前記接続部には、前記放射線不透過材料からなる粒子が存在している請求項1または2に記載の生体内留置具。
  4.  前記放射線不透過材料は、第1の放射線不透過材料からなる第1の粒子と、第2の放射線不透過材料からなる第2の粒子を含む請求項3に記載の生体内留置具。
  5.  前記長手軸方向において前記接続部の長さを遠位部と近位部に二等分割したときに、前記近位部での前記放射線不透過材料からなる粒子の個数密度は、前記遠位部での前記放射線不透過材料からなる粒子の個数密度よりも高い請求項3に記載の生体内留置具。
  6.  前記粒子の粒径は0.010μm以上5.0μm以下である請求項3に記載の生体内留置具。
  7.  長手軸方向を有するコイルと、前記コイルを遠位側に向かって押すためのプッシャーと、を準備する工程と、
     前記コイルと前記プッシャーとを接続する接続部であって、放射線不透過材料を含む接続部を形成する工程と、
     前記コイルと前記プッシャーとを前記接続部で接続する工程と、を有する生体内留置具の製造方法。
  8.  前記接続部を形成する工程は、前記熱可塑性材料と前記放射線不透過材料からなる粒子とを混合する工程を含む請求項7に記載の生体内留置具の製造方法。
  9.  前記接続部を形成する工程は、さらに、前記熱可塑性材料と前記放射線不透過材料からなる粒子を混合して得られた材料を押出成形する工程を含む請求項8に記載の生体内留置具の製造方法。
  10.  前記接続部を形成する工程は、さらに、前記押出成形された材料を延伸する工程を含む請求項9に記載の生体内留置具の製造方法。
  11.  前記接続部を形成する工程において、前記熱可塑性材料と前記放射線不透過材料からなる粒子を混合して得られた材料を三次元積層造形法により積層造形する請求項8に記載の生体内留置具の製造方法。
  12.  前記接続部を形成する工程は、前記放射線不透過材料からなる粒子と、放射線透過材料からなる粒子とを混合する工程を含む請求項7に記載の生体内留置具の製造方法。
  13.  前記接続部を形成する工程において、前記放射線不透過材料からなる粒子と放射線透過材料からなる粒子とを混合して得られた材料を三次元積層造形法により積層造形する請求項12に記載の生体内留置具の製造方法。
