Abstract

【課題】網膜下腔内へ薬剤を投与することができ、かつ網膜に形成された穿孔を簡便かつより確実に閉塞させることができる医療器具を提供する。 【解決手段】医療器具100は、患者の網膜Rに穿孔Pを形成するための内針120と、内針を収容可能なルーメン131を備える外針130と、を有し、内針は、患者の網膜下腔A内に投与される薬剤d1を送達することが可能なルーメン121を備え、かつ外針のルーメンに沿って移動可能に構成されており、外針は、穿孔を閉塞するための閉塞剤d2を吐出可能に構成されている。

Description

医療器具
 本発明は、網膜下腔内への薬剤の投与に使用される医療器具に関する。
 網膜剥離、網膜裂孔、糖尿病網膜症、黄斑円孔、黄斑上膜、網膜静脈分枝閉塞症、加齢黄斑変性、網膜の遺伝性疾患等に代表される網膜硝子体疾患の治療方法として「網膜硝子体手術」が知られている。網膜硝子体手術では、疾患の症状や進行等に応じて、網膜下腔(網膜と網膜色素上皮細胞の間の空間)内への薬剤の投与や網膜下腔内で治療器具を使用した所定の処置を実施する。
 特許文献1では、患者の眼の外部から網膜下腔内へのアクセスを可能にするカテーテルデバイスが提案されている。また、特許文献1では、カテーテルデバイスを使用した手技の一例として、カテーテルデバイスにより網膜の一部に穿孔(開口部)を形成することで網膜下腔内へのアクセスを可能とし、網膜下腔内での薬剤の投与を終えた後、穿孔を閉塞する、といった手順を開示している。網膜下腔内に薬剤を投与した後、網膜に形成した穿孔を閉塞させずに放置した場合、網膜下腔内の細胞が眼内で拡散して異常増殖し、網膜前膜が発症したり、硝子体液が穿孔から網膜下腔内へ流入し、網膜剥離を発症させたりする虞がある。したがって、網膜に穿孔を形成した場合、その穿孔を適切かつ確実に閉塞させることが重要である。
 特許文献1では、網膜に形成した穿孔を閉塞させる方法として、レーザー照射や冷却によって穿孔の周辺組織を変性させる方法を挙げている。ただし、これらの方法を採用した場合、次のような点が課題となる。

 レーザー照射により穿孔周辺の組織を変性させる方法を採用した場合、網膜下腔内に投与した薬剤がレーザーの熱で変性してしまう可能性がある。また、穿孔付近の網膜下腔内に液体(網膜下液)が存在すると、穿孔を確実に閉塞させることが難しくなる。それにより、薬剤の漏れを十分に防ぐことができなくなる。このような課題は、冷却手段を採用して穿孔を閉塞させる場合にも生じうる。さらに、薬剤を投与する箇所が黄斑付近である場合、レーザーの熱で黄斑を損傷させてしまうことのないように細心の注意を払う必要があり、手技が煩雑なものとなる。
 なお、網膜に形成した穿孔を閉塞させる方法として、タンポナーデガスを硝子体腔内に注入して、網膜を付着させる方法を採用することも可能である。しかしながら、この方法を採用した場合、入院期間の長期化、患者の姿勢維持の強制、予後が悪い、白内障を誘発する等の別の課題が生じる。
 本発明は、上述した課題を解決するためになされたものであり、網膜下腔内へ薬剤を投与することができ、さらに網膜に形成された穿孔を簡便かつより一層確実に閉塞させることができる医療器具を提供することを目的とする。
 上記目的を達成する医療器具は、患者の網膜に穿孔を形成するための内針と、前記内針を収容可能なルーメンを備える外針と、を有し、前記内針は、前記患者の網膜下腔内に投与される薬剤を送達することが可能なルーメンを備え、かつ前記外針の前記ルーメンに沿って移動可能に構成されており、前記外針は、前記穿孔を閉塞するための閉塞剤を吐出可能に構成されている。
 上記の医療器具を使用した手技において、術者(例えば、医師等の医療従事者)は、外針のルーメン内に内針を収容した状態で外針と内針を網膜まで送達し、外針の先端部を網膜の所定の位置付近に配置した後、内針を外針から突出させることで、網膜に穿孔を形成することができる。術者は、内針の先端部を網膜下腔内に配置した状態で、内針のルーメンを介して薬剤を送達することにより、網膜下腔内に薬剤を投与することができる。術者は、内針を外針のルーメン内に退避させた状態で、外針を介して閉塞剤を吐出させることができる。術者は、穿孔を形成する前後において、外針の先端部を網膜の所定の位置付近に位置決めした状態で保持することができる。そのため、術者は、閉塞剤を網膜の適切な位置に塗布することができ、閉塞剤によって簡便かつより一層確実に穿孔を閉塞させることができる。

 以下、添付した図面を参照しながら、本発明の実施形態を説明する。なお、以下の記載は特許請求の範囲に記載される技術的範囲や用語の意義を限定するものではない。また、図面の寸法比率は説明の都合上誇張されており、実際の比率とは異なる場合がある。
 <第1実施形態>
 本実施形態に係る医療器具100は、図7~図9に示すように、患者の網膜下腔A内に薬剤d1を投与する手技に使用することができる。薬剤d1の投与は、例えば、網膜硝子体疾患の治療方法である網膜硝子体手術の一部として実施することができる。ただし、医療器具100を使用した手技の目的、治療対象となる具体的な疾患等について特に制限はない。
 図1、図2を参照して、概説すると、医療器具100は、患者の網膜Rに穿孔Pを形成するための内針120と、内針120を収容可能なルーメン131を備える外針130と、を有する。
 内針120及び外針130は患者の眼内に挿入される挿入部110を構成する。術者は、図5に示すように、網膜Rまで挿入部110を送達する際、内針120を外針130のルーメン131内に収容させることができる。これにより、術者は、内針120が患者の眼の不要な箇所に誤って穿刺されることや、網膜Rまでの送達途中に内針120が折れたり、キンクすることを防止できる。
 図1において、内針120及び外針130の長手方向(延在方向)を矢印X1-X2で示す。矢印X1で示す方向は医療器具100の先端部側であり、患者の眼内への挿入方向を指している。矢印X2で示す方向は医療器具100の基端部側を指している。
 図1、図2、図7に示すように、内針120は、患者の網膜下腔A内に投与される薬剤d1を送達することが可能なルーメン121を備える。また、内針120は、外針130のルーメン131に沿って移動可能に構成されている。
 図1に示すように、外針130の基端部側には手元ハブ140が配置されている。手元ハブ140は、外針130に対する内針120の進退移動を操作するためのスライド操作部143を備える。内針120の基端部側の一部は手元ハブ140の内部でスライド操作部143と連結されている。
 術者は、図1の矢印f1で示すようにスライド操作部143を前進させることにより、その前進に連動させて矢印f2で示すように内針120を前進させることができる。内針120を所定の距離だけ前進させることにより、内針120の先端部を外針130の先端部よりも先端側へ突出させることができる。
 術者は、図1の矢印b1で示すようにスライド操作部143を後退させることにより、その後退に連動させて矢印b2で示すように内針120を後退させることができる。内針120を所定の距離だけ後退させることにより、内針120の先端部を外針130のルーメン131内に収容させることができる。
 術者は、上記のように手元ハブ140に設けられたスライド操作部143をスライドさせる簡単な操作によって内針120の突出及び収容を切り替えることができる。
 図1、図2、図8に示すように、外針130は、網膜Rに形成された穿孔Pを閉塞するための閉塞剤d2を吐出可能に構成されている。
 図2に示すように、外針130のルーメン131内には、閉塞剤d2を供給するためのチューブ250を挿入することができる。医療器具100は、チューブ250を介して外針130の先端部側へ閉塞剤d2を吐出させることができる。
 医療器具100は、図3に示す変形例のように、外針130のルーメン131内にチューブ250が挿入されていなくてもよい。このように構成する場合、医療器具100は、外針130のルーメン131を介して閉塞剤d2を直接吐出させることができる。
 手元ハブ140の基端部には、図1に示すように、薬剤d1を供給するためのシリンジ210を接続することが可能なポート141を配置している。内針120のルーメン121は、手元ハブ140の内部を介してポート141の内部と連通している。図2に示すように、外針130のルーメン131内において、内針120のルーメン121は隔離されている。そのため、医療器具100は、内針120のルーメン121内を流れる薬剤d1と、内針120のルーメン121の外側を流れる閉塞剤d2とが混ざり合うことを防止できる。
 内針120及び外針130は、図1に示すように、長手方向に略直線状に延びた形状を有する。ただし、後述する実施形態で説明するように、内針120及び外針130は湾曲針で構成することも可能である(図10を参照)。
 外針130は金属針で構成することができる。また、内針120は、外針130よりも柔軟な樹脂製の針で構成することができる。外針130を金属針で構成することにより、各針120、130を網膜Rまで送達する際、各針120、130が折れたり、キンクしたりすることを防止できる(図5を参照)。また、内針120を柔軟な樹脂製の針で構成することにより、内針120で網膜Rを穿刺する際、眼内を突き破ることを防止することができる(図7を参照)。
 内針120は、例えば、ポリウレタン、テフロン(登録商標)、ポリエステル、ポリイミド等で構成することができる。外針130は、例えば、ステンレス、ニチノール、タングステン等で構成することができる。
 内針120及び外針130は、例えば、鈍針で構成することができる。ただし、内針120及び/又は外針130は、鋭針で構成してもよい。
 内針120及び外針130の外径、針長、及びルーメン径等について特に制限はなく、任意の構成を適宜採用することが可能である。
 薬剤d1の材質は特に限定されないが、例えば、加齢黄斑変性症や遺伝性網膜疾患といった網膜の治療に使用される低分子、ペプチド、抗体、核酸、細胞薬やウイルスベクター薬等、具体的にはアフリベルセプトやラニビズマブ等のVEGF阻害薬を使用することができる。なお、薬剤d1の性状(液状、ゲル状等)についても特に制限はない。
 閉塞剤d2の材質は特に限定されないが、例えば、二液混合型の生分解性を備える接着剤を使用することができる。二液混合型の接着剤を閉塞剤d2として使用する場合、例えば、第1液(一方の組成剤)としてフィブリノーゲンを用いることができ、第2液(他方の組成剤)としてトロンビンを用いることができる。なお、閉塞剤d2の性状や粘度について特に制限はない。
 図1に示すように、医療器具100は、閉塞剤d2を供給するための各シリンジ220、230と接続されるチューブ250が接続可能に構成されている。本実施形態では、シリンジ220に接着剤を構成する第1液が保持されており、シリンジ230に接着剤を構成する第2液が保持されている。
 各シリンジ220、230には、各シリンジ220、230の押し子の移動を同期させるための補助部材260を接続している。術者は、補助部材260を操作することにより、各シリンジ220、230の押し子の移動量を略同一に調整することができる。それにより、第1液と第2液の分量を均等に調整することができる。
 第1液を保持するシリンジ220の先端部にはチューブ231を接続している。また、第2液を保持するシリンジ230の先端部にはチューブ232を接続している。各チューブ231、232の先端部には合流ポート240を配置している。各シリンジ220、230から送液された第1液及び第2液は、合流ポート240で混合されて接着剤を形成した後、チューブ250を介して外針130(図2を参照)へ送液される。
 次に、医療器具100を使用した手技手順の一例を説明する。
 図4には、手技手順の概略をフローチャートで示している。なお、以下では、医療器具100を使用した薬剤d1の投与及び閉塞剤d2による穿孔Pの閉塞等以外の各手順(一般的な網膜硝子体手術で採用される各手順、例えば、麻酔やポートの配置等)についての説明は省略する。ただし、網膜硝子体手術において公知の内容は本実施形態の手技においても任意に組み込むことができる。
 図4に示すように、本実施形態に係る手技は、概説すると、挿入部110(内針120及び外針130)を眼内へ挿入すること(S11)と、内針120を網膜Rに穿刺すること(S12)と、内針120を介して網膜下腔A内へ薬剤d1を投与すること(S13)と、内針120を網膜下腔Aから抜去すること(S14)と、外針130を介して閉塞剤d2を吐出させて、網膜Rに形成された穿孔Pを閉塞させること(S15)と、を有する。以下、具体的に説明する。
 術者は、図5に示すように、挿入部110を患者の眼内に挿入する。術者は、外針130のルーメン131内に内針120を収容した状態で、挿入部110の先端部を網膜Rの所定の位置まで送達する。
 術者は、外針130の先端から内針120を突出させ、内針120を網膜Rに穿刺する。内針120を網膜Rに穿刺することにより、網膜Rに穿孔Pが形成される。
 術者は、図7に示すように、内針120のルーメン121を介して、網膜Rと網膜色素上皮細胞Eの間に区画された網膜下腔A内へ薬剤d1を投与する。
 術者は、薬剤d1を投与した後、内針120を網膜下腔Aから抜去する。術者は、内針120を外針130のルーメン131内に退避させる。
 術者は、図6、図8に示すように、外針130を介して閉塞剤d2を吐出させる。閉塞剤d2を網膜Rに形成した穿孔P及びその周辺部に塗布する。なお、外針130のルーメン131内にチューブ250を配置している場合(図2の構成例の場合)には、閉塞剤d2はチューブ250のルーメン251を介して吐出させることができる。一方、外針130のルーメン131内にチューブ250を配置していない場合(図3の構成例の場合)には、閉塞剤d2は外針130のルーメン131を介して吐出させることができる。
 術者は、図7、図8に示すように、内針120の穿刺、内針120を介した薬剤d1の投与、及び外針130を介した閉塞剤d2の吐出を行う間、外針130の先端部(針先)の位置を穿孔P付近で保持することができる。そのため、術者は、内針120により穿孔Pを形成した後、穿孔Pの位置を見失うことなく、穿孔P及びその周辺部に閉塞剤d2を適切に塗布することができる。
 術者は、図9に示すように、網膜Rに形成した穿孔P及びその周辺部に吐出させた閉塞剤d2により、穿孔Pを閉塞させることができる。術者は、穿孔Pを閉塞させた後、挿入部110を患者の眼から抜去する。
 以上のように、本実施形態に係る医療器具100を使用した手技において、術者は、外針130のルーメン131内に内針120を収容した状態で外針130と内針120を網膜Rの所定の位置まで送達する。術者は、外針130の先端部を網膜Rの所定の位置付近に配置した状態で、内針120を外針130から突出させることで、網膜Rに穿孔Pを形成することができる。術者は、内針120の先端部を網膜下腔A内に配置した状態で、内針120のルーメン121を介して薬剤d1を送達することにより、網膜下腔A内に薬剤d1を投与することができる。術者は、薬剤d1を投与した後、内針120を外針130のルーメン131内に退避させる。術者は、内針120を外針130のルーメン131内に退避させた状態で、外針130を介して閉塞剤d2を吐出させることができる。術者は、穿孔Pを形成する前後において、外針130の先端部の位置を穿孔P付近に位置決めした状態で保持することができる。そのため、術者は、穿孔P及びその周辺部に対して適切に閉塞剤d2を塗布することができる。それにより、術者は、穿孔Pを閉塞剤d2によって簡便かつより確実に閉塞させることができる。
 上述した手技において、術者は、下記手順(i)及び(ii)を任意かつ選択的に組み込むことができ、また一部の手順と置き換えることもできる。
 (i)術者は、薬剤d1を投与するのに先立ち、網膜下腔A内に膨隆剤を注入して、膨疹を形成することができる。術者は、膨隆剤を注入することにより、網膜下腔A内に薬剤d1を注入する十分なスペースが存在するか否かを確認することができる。また、術者は、膨疹を薬剤d1を投与する位置の目印に用いることができる。なお、膨隆剤の材質は特に限定されず、公知のものを適宜使用することができる。
 上記(i)の手順を手技に組み込む場合、術者は、膨隆剤を注入するために、医療器具100を使用することができる。術者は、医療器具100の内針120を使用して、穿孔Pの形成と、網膜下腔A内への膨隆剤の注入を行うことができる。また、術者は、膨隆剤を注入した後、前述した手順と同様の手順によって、穿孔Pを閉塞剤d2により閉塞させることができる。これらの具体的な手順は、前述した薬剤d1を投与する場合と同様である。術者は、膨疹を形成した後、医療器具100を患者の眼から一旦抜去する。術者は、その後、薬剤d1を投与するために準備された所定のポートを介して、各針120、130を膨疹が形成された位置まで送達し、膨疹が形成された位置の周辺部での穿孔Pの形成、膨疹が形成された位置への薬剤d1の投与、閉塞剤d2による穿孔Pの閉塞を実施することができる。なお、後述する実施形態で説明する医療器具100D(図18を参照)を使用した場合、一つの医療器具で薬剤d1の投与と膨隆剤の注入を実現することができる。
 (ii)術者は、網膜Rに穿孔Pを形成する前に、穿孔Pを形成する予定の位置に閉塞剤d2を予め塗布しておくことができる。術者は、閉塞剤d2を塗布した位置に穿孔Pを形成することにより、穿孔Pからの内針120の抜去に伴って、穿孔Pを閉塞剤d2によって閉塞させることができる。このような手順を採用する場合、穿孔Pからの内針120の抜去に伴って穿孔Pを自動的に閉じることができるようにするために、閉塞剤d2の粘度等の物性は適切に調整される。
 <第2実施形態>
 次に、図10~図16を参照して、第2実施形態に係る医療器具100Aを説明する。第2実施形態の説明では、前述した第1実施形態で既に説明した内容(装置構成や手技手順等)は適宜省略する。また、特に言及されていない内容については、前述した第1実施形態と同様のものとすることができる。
 医療器具100Aは、図10に示すように、内針120A及び外針130Aが湾曲針で構成されている。なお、内針120A及び外針130Aの具体的な曲率や形状について特に制限はない。
 内針120Aの基端部には針ハブ125が配置されている。針ハブ125には、薬剤d1を供給するためのシリンジ210を接続することができる。
 外針130Aの基端部には針ハブ133が配置されている。針ハブ133には、閉塞剤d2を供給するためのチューブ250が接続されている。
 内針120Aは、外針130Aの針ハブ133を介して外針130のルーメン131内に挿入されている(図11を参照)。術者は、針ハブ133の基端部側で内針120Aを手指等で把持することにより、内針120Aの移動を操作することができる。
 医療器具100Aは、図12に示す変形例のように、外針130Aのルーメン131内にチューブ250を挿入しなくてもよい。このように構成する場合、医療器具100Aは、外針130Aのルーメン131を介して閉塞剤d2を吐出させることができる。
 次に、医療器具100Aを使用した手技の手順例を説明する。
 術者は、図13に示すように、内針120A及び外針130Aを網膜Rの所定の位置まで送達した後、内針120Aを外針130Aから突出させて、網膜Rに穿孔Pを形成する。術者は、内針120Aが湾曲針で構成されているため、内針120Aを網膜Rと網膜色素上皮細胞Eの間に区画された網膜下腔A(図7を参照)に沿って移動させることができる。そのため、術者は、内針120Aが網膜色素上皮細胞Eに穿刺されることを防止しつつ、穿孔Pから離れた位置まで内針120Aの先端部を移動させることができる。それにより、術者は、穿孔Pから離れた位置で薬剤d1を投与することができる。
 また、術者が内針120Aを網膜Rに穿刺する際、内針120Aは、湾曲針で構成された外針130Aによって穿刺方向がガイドされる。術者は、内針120Aが網膜Rに対して直線的に穿刺されることを防止でき、網膜色素上皮細胞Eに穿孔が形成されることを防止できる。
 術者は、薬剤d1を網膜下腔A内に投与した後、図14に示すように、外針130Aを介して閉塞剤d2を吐出させる。術者は、薬剤d1の投与位置から離れた位置に形成された穿孔Pを閉塞剤d2によって閉塞させることができる。
 医療器具100Aを使用した手技では、図15、図16に示す手技を実施することも可能である。
 術者は、図15に示すように、穿孔Pを形成した位置から所定の距離だけ内針120Aを網膜下腔Aに沿って移動させることができる。黄斑付近に薬剤d1を投与する場合、黄斑の損傷等を未然に防止するために、黄斑から離れた位置に穿孔Pを形成することが好ましい。術者は、医療器具100Aを使用することにより、穿孔Pを薬剤d1の投与位置から所定の距離だけ離れた位置に形成することができる。そのため、黄斑等の損傷が発生することをより確実に防止することができる。
 術者は、図16に示すように、薬剤d1を投与した後、外針130Aから閉塞剤d2を吐出させることにより、穿孔Pを閉塞させることができる。
 なお、図13~図16で説明した手技を実施する場合、外針130Aが湾曲針である必要はない。そのため、内針120Aのみを湾曲針で構成し、外針130Aを直線形状の針で構成してもよい。
 <第3実施形態>
 次に、図17を参照して、第3実施形態に係る医療器具100Cを説明する。第3実施形態の説明では、前述した各実施形態で既に説明した内容(装置構成や手技手順等)は適宜省略する。また、特に言及されていない内容については、前述した各実施形態と同様のものとすることができる。
 図17に示すように、医療器具100Cは、内針120の基端部に延長チューブ150が接続されている。延長チューブ150の基端部には薬剤d1を供給するためのシリンジ210を接続可能な針ハブ153が配置されている。医療器具100Cは、内針120の基端部に接続された延長チューブ150により、患者の眼の外部での内針120の取り扱いが容易なものとなる。そのため、医療器具100Cを使用した手技をより一層円滑に実施することができる。
 <第4実施形態>
 次に、図18~図20を参照して、第4実施形態に係る医療器具100Dを説明する。第4実施形態の説明では、前述した各実施形態で既に説明した内容(装置構成や手技手順等)は適宜省略する。また、特に言及されていない内容については、前述した各実施形態と同様のものとすることができる。
 図18、図19に示すように、医療器具100Dは、膨隆剤を注入するためのシリンジ270と接続されたチューブ160を内針120のルーメン121内に挿入している。チューブ160の基端部にはシリンジ270を接続するためのハブ163が配置されている。
 術者は、医療器具100Dを使用した手技において、薬剤d1を網膜下腔A内に投与する前に膨疹を形成する場合、チューブ160のルーメン161を介して膨隆剤を網膜下腔A内に注入することができる。術者は、医療器具100Dを他の医療器具に取り換えることなく、膨疹を形成する作業に続いて膨疹付近に薬剤d1を注入することができる。したがって、医療器具100Dを使用することにより、手技をより一層円滑に進めることができる。
 医療器具100Dは、図20に示す変形例のように、外針130のルーメン131内にチューブ250を挿入しなくてもよい。このように構成する場合、医療器具100Dは、外針130のルーメン131を介して閉塞剤d2を吐出させることができる。
 <他の実施形態>
 前述した各実施形態では、閉塞剤d2を網膜Rの外側から塗布した例を説明した。ただし、閉塞剤d2は、網膜Rの内側(網膜下腔A側)から塗布してもよい。このような方法を採用する場合、内針を網膜下腔Aから抜去する直前に、網膜Rの内側に配置した内針の先端部から閉塞剤d2を吐出させる。閉塞剤d2は、粘度を所定の大きさに調整しておくことにより、内針の先端部に留めて保持することができる。内針の先端部に閉塞剤d2を留めて保持した状態で内針を網膜Rから抜去することにより、網膜Rの内側に閉塞剤d2を貼り付けることができる。このような手順を採用することにより、網膜Rからの抜去を完了させたときに、穿孔Pを網膜Rの内側から閉塞させることができる。
 なお、内針から閉塞剤d2を吐出させる場合、外針に挿入された内針のルーメンを介して、薬剤d1、閉塞剤d2、膨隆剤を選択的に吐出させるように構成することができる。この場合、内針はシングルルーメンで構成してもよいし、それぞれの材料を個別に吐出させることができるマルチルーメンで構成してもよい。内針をシングルルーメンで構成する場合、各材料を吐出させる際に、薬剤d1の専用ルート、閉塞剤d2の専用ルート、膨隆剤の専用ルートを付け替える作業を行う。内針をマルチルーメンで構成する場合、上記と同様に各専用のルートを付け替えてもよいし、閉塞剤d2を吐出させるタイミングで閉塞剤d2のルートを薬剤d1のルート又は膨隆剤のルートと付け替える作業を行ってもよい。
 以上、複数の実施形態及び変形例に基づいて本発明に係る医療器具を説明したが、本発明は、明細書で説明した実施形態及び変形例のみに限定されず、特許請求の範囲の記載において種々の変更が可能である。
 例えば、各実施形態で説明した構成は任意に組み合わせることができる。一例として、第3実施形態及び第4実施形態で説明した医療器具の内針及び/又は外針は、湾曲針で構成することが可能である。
 また、医療器具の各部の構造、剤質、大きさ、及び形状等は、網膜の穿刺、網膜下腔内への薬剤の投与、網膜に形成した穿孔の閉塞を行いうる限り、任意に変更することが可能である。

Claims (7)

  1.  患者の網膜に穿孔を形成するための内針と、
     前記内針を収容可能なルーメンを備える外針と、を有し、
     前記内針は、前記患者の網膜下腔内に投与される薬剤を送達することが可能なルーメンを備え、かつ前記外針の前記ルーメンに沿って移動可能に構成されており、
     前記外針は、前記穿孔を閉塞するための閉塞剤を吐出可能に構成されている、医療器具。
  2.  前記内針は、湾曲針で構成されている、請求項1に記載の医療器具。
  3.  前記外針は、湾曲針で構成されている、請求項1又は請求項2に記載の医療器具。
  4.  前記外針は、金属針で構成されており、
     前記内針は、前記外針よりも柔軟な樹脂製の針で構成されている、請求項1~3のいずれか1項に記載の医療器具。
  5.  前記内針は、前記薬剤を供給するためのシリンジを接続可能な針ハブを備える、1~4のいずれか1項に記載の医療器具。
  6.  前記外針の基端部側に配置された手元ハブを有し、
     前記手元ハブは、前記内針の移動を操作するためのスライド操作部を有する、請求項1~5のいずれか1項に記載の医療器具。
  7.  前記閉塞剤は、二液混合型の生分解性を備える接着剤である、請求項1~6のいずれか1項に記載の医療器具。
