Abstract

System for monitoring and communication between patient and doctor, comprising a communication device (100) adapted to allow a dialogue between doctor and patient and to allow the patient to carry out controls by using the following devices connected to said communication device (100): an infrared temperature sensor (103), a pressure meter (104), a blood oxygen and heart rate meter (105), a high-definition video camera (110). The data detected by said devices is processed by a processor (106) and sent by means of Internet network connection (107). The payments are managed by means of a payment terminal (109) while the interaction with the user mainly occurs via voice by means of a suitable microphone (101) and speaker (102). The data is saved locally, for greater security and privacy, on an electronic archiving device with data encryption function (108).

Description

“System for monitoring and communication between patient and doctor”
Description
Field of the art The present invention operates in the field of digital medicine, which represents a set of medical and computer techniques that allow the remote treatment of a patient. Through a computerized system for monitoring the diagnostic data and a digital communication system, the present patent application proposes a personalized monitoring and an optimal management of the patients under remote treatment.
Prior art
The idea of the present patent has its origins --- made clear in a particularly strong manner with the explosion of the Covid-19 pandemic --- from the verification that often there is no suitable way for health centers to interface with patients in a remote manner, at the patient homes. It is not uncommon that the only means available for a family doctor to be able to remotely interact with a patient who is at home is via the telephone. Solutions have been proposed that allow an easy communication between doctor and patient directly from home, but not all of these solutions are able to cover the functions set forth by the present invention, and they do not consider problems that are instead brilliantly resolved by the idea described hereinbelow, especially regarding privacy and security of the patient data.
One solution is proposed in the patent CN103793607 A in which reference is made to a device for the monitoring of the health and the diagnostics by means of television, which comprises network connections and diagnostics devices for use by the user. This patent does not name any secure communication system with storage of encrypted data, of the patient, possessed by the same and used as required upon access of the physician by means of integrated server, said system thus adapted to ensure total privacy, fully controlled by the patient. In addition, no device is provided for managing the remote payment, connected for the whole family, and ail adherent to the system. Also the device proposed in the patent CN205451067U proposes a similar solution, adding a specific set of sensors and diagnostics instruments, and comprising a wireless and Bluetooth connection. Also in this case, there is no system that simultaneously ensures the remote medical assistance service and the security and privacy of the data collected by the devices as mentioned above. Here too there is no integrated payment function.
In summary, we recognize that there are solutions of remote medical assistance by means of electronic communication devices, but the latter do not comprise functions, especially regarding privacy and the centralized physical possession of the recorded data, which according to the present study are particularly of interest and of importance for the patients under remote treatment.
Therefore, there is no doubt that the solutions only partly confront the critical points set forth herein.
Description of the invention According to the present invention, a system is attained for monitoring and communication between patient and doctor, which effectively solves the abovementioned problems.
The sudden need to confront the Covid-19 epidemic has greatly stimulated - in Italy and not only - the clinical aid practices known as “digital medicine”, foremost among the medical consulting online. In such a manner, the bases were set for the construction of a treatment system that is predictive, participative and more personalized than ever.
Our health care system responded quickly to the growing demand of remote doctor visits and health guide, registering a. significant reduction of accesses to the emergency medical service areas, to doctors’ office and a general lessening of the health care operators’ activities. Digital medicine makes use of new processes and interactive clinical paths which use digital devices and virtual communication systems for providing medical consulting online of any type. In practice, medical consulting online signifies making remote doctor visits, providing opinions, making prevention and establishing therapies, with the advantage of considerably simplifying the communication between doctor and patient and of accelerating the diagnosis and treatment process with full respect of the securi ty of the transmitted data.
The present invention proposes a system for monitoring diagnostic data and a series of digital instruments which allow and facilitate the remote communication between doctor and patient. These digital instruments available to the doctor help for example in interacting with the patient through chat consultation in real time, in remote monitoring several health parameters of the patient always through digital devices (pressure meters, glucose meters, blood oxygen meters, etc.). This plurality of functions ensures the patient a diagnostic and therapeutic path without requiring a physical meeting with one’s own physician, which can be based on the report of the clinical history and display online the exams already made and the previous medical history' in general.
It is evident that this mode of communication reduces physical movements and ensures aid to those who have mobility difficulty, both due to physical problems and for people who are situated far away from their own doctor or off-site; it optimizes the work of the doctor who, through chat and tele-visits, can dedicate more time to his/her own patients, without having to manage paperwork and crowded offices; it decongests the flow of people at doctors’ offices and the request for aid in structures that are already overfilled, like the emergency medical service areas, it contributes to making a health model that is increasingly online and rationalizes the decisions through the remote consultation with other specialists, independent of the structure or country' in which they operate and finally, but not less important, it reduces the cost of treatment for each patient.

In order to substitute, in a complete and exhaustive manner, an actual medical visit in physical presence, the present invention provides that in the system of communication between patient and doctor, there is not only the presence of a speaker and a microphone that facilitates the verbal communication between the two, in any case preserving the privacy of the telephone number, but there is also a high-definition video camera connected in wireless mode, by means of, for example, Bluetooth. In this manner, the patient can take images of various parts of the body or generally of himself and send them in streaming to the doctor, who will be ab le to better judge the health state of the patient through the integration of the various information: the symptoms recounted verbally by the patient, the images relative to his/her actual physical state and the data coming from the analyses of the vital parameter sensors, since quite often the symptoms referred by the patient can be misleading, since they can be amplified by anxiety or underestimated by negligence. In this manner, the doctor can carry out an accurate and complete remote medical history and make an optimal basic diagnosis. In order to complete the entire medical visit operation online, the present patent provides for an electronic payment terminal that allows an easy payment by means of card or other electronic payment devices. The present invention can be applied not only in the field of generic basic medicine, but also in specialist fields such as for example denial medicine. The system, in the specific case of dental treatment, can compri se an intelligent electronic toothbrush that records the dental hygiene operations of the patient, i.e. the use time, the frequency and the mode of cleaning. This data can then be sent by means of Bluetooth connection to the electronic communication system with which the present system is equipped, in such a manner this recorded and stored data can be displayed both by the patient, who thus will have a report of the total use time, and by the specialist doctor who can remotely monitor the dental hygiene of his/her own patients, in order to possibly correct bad habits or give advice regarding a more functional and effective hygiene. In order to ensure an effective oral hygiene and consequently a healthy mouth, protected from the damage caused by negligence, the present system also comprises a specialized bacterial plaque detector. It is in fact by now known that a frequent dental cleaning is not sufficient, this must also be accurate and correctly executed otherwise --- notwithstanding the apparent cleaning - bacterial populations can be accumulated in the mouth, which can compromise the health of the mouth and of the teeth in particular. The aforesaid plaque detector allows monitoring the level of bacterial load accumulated in the oral cavity and communicates it both to the patient and to the phy sician, this constant monitoring thus allows modulating, correcting and if necessary intensifying the dental treatment in order to prevent permanent damage caused by the bacterial plaque. Associated with the bacterial plaque detector is also a sensor for the pH variation of the oral cavity, always functional for mouth and teeth health prevention, in particular for the problem of cavities due to the bacterial presence. Since the bacterial survive only within a certain range of acidity, the pH sensor transmits useful information to the physician, who can thus modulate the prevention treatment in the best possible manner. A dental treatment is often associated with an orthodontic treatment. The movable orthodontic apparatuses are often used as therapy assisting or substituting that of the fixed apparatuses; the difficulty that a patient encounters in the use of the movable apparatuses is that of repeating the daily actions, which can lead to forgetting or negligence. The present invention proposes a case for movable orthodontic apparatuses, which is intelligent and provided with a light sensor which sends light signals that warn the patient that it is time to wear the orthodontic apparatus; this is made possible due to a Bluetooth connection of the case of the apparatus to the central device of the monitoring system.
The present invention, in an even more advantageous embodiment thereof, can also monitor the number of hours in which the movable orthodontic therapy is worn by the patient, by means of a suitable sensor positioned within the movable orthodontic apparatus, allowing the signaling of moving away from the therapeutic plan and other parameters regarding the progression of the corrections induced by the orthodontic therapy.
The present invention can also be applied in the field of diet medicine such as, for example, the relationship between a patient and his/her nutritionist who, through a digital communication, can monitor the lifestyle, constructing a diet diary associated with a physical training sheet.
All the progresses and the data transmitted can be viewed by both parties, who can better understand the effectiveness of the lifestyle on a daily basis, in order to continue or improve it. This monitoring and communication system facilitates the remote relationship between patient and doctor since the communication software can be configured by the user by means of any one computer or mobile device. In addition, the present system also comprises the function of scanning and printing documents relative to the systems and processes regarding authorizations, exemptions and identification of the patient of the public health system, in this manner, where the health system still uses paper, it is possible to integrate the digital function of the present system with that of the conventional health care system, while still adhering to the practices still in force in the national health care system.
The advantages offered by the present invention are evident in light of the description set forth up to now and will be even clearer due to the enclosed figures and to the relative detailed description.

Said elements comprise: a device for communication between patient and physician characterized by different functional modules: a speaker 101 and microphone 102 adapted to allow' a voice interaction between patient and physician; an Internet network connection 107 for the communication and the sending of the data to the physician, a processor 106 adapted to perform all the calculation functions regarding the collection of data from the meters and the sending of the data and an electronic archiving device with data encryption function 108. In addition, said system provides for a plurality of modules with function of constant monitoring of the health conditions of the patient such as: an infrared temperature sensor 103 adapted to allow' the doctor to monitor, during a remote control, the body temperature of the patient; a pressure meter 104 adapted to allow the monitoring thereof, a blood oxygen and heart rate meter 105 adapted to measure the health state of the circulatory system and a sensor for pH variation 400 in the mouth adapted to allow constant monitoring of the health state of the mouth. All these parameters are then remotely consultable and monitorable by means of a software display client 111 adapted to allow the doctor to receive and analyze ail the data sent by said communication device. Finally, said Figure I show's devices adapted to facilitate the interaction between physician and patient by means of the use of a high-definition video camera 110 connected in wireless mode adapted to capture images regarding the parts under analysis of the patient and an electronic payment terminal 109 adapted to allow' an easy payment by means of card or other pay ment devices.
- FIGURE 2 instead illustrates several integrated devices which allow implementing the efficiency of the sy stem. The figure shows an intelligent toothbrush that records the dental hygiene operations and sends them by means of Bluetooth connection 200 to said communication device 100 adapted to allow the recording of the data regarding the use time, and a case for intelligent orthodontic apparatuses 300 connected by means of Bluetooth connection to said device 100 and provided with a light sensor 301. Figure 2 shows the sensor within the movable orthodontic apparatus 302 connected to the device 100, In order to monitor other important and essential parameters like the level of accumulation of the bacterial mass present in the oral cavity, the system provides for a specialized detector for the bacterial plaque 500 adapted to monitor and communicate, both to the patient and to the physician, the level of bacteria accumulated in the oral cavity and possibly to intervene or intensify the prevention treatment. Finally, the image represents a generic mobile device used for the configuration of said communication devi ce. Detailed description of the invention
The present invention will now be illustrated merely as a non-limiting, non-binding example with reference to the figures, which illustrate several embodiments relative to the present inventive concept.
With reference to Fig. 1 , the system is shown that i s adapted to ensure a sendee of personalized monitoring and of optimal managemen t of the pati ents under remote treatment. This compri ses a device for communication between patient and physician characterized by different functional modules:
- a speaker 101 and microphone 102, adapted to allow a voice interaction with the patient and to communicate directly with one’s doctor, preserving the privacy of the telephone number;
- an Internet network connection 107 for the communication and sending of the data to the physician;
- a processor 106 adapted to perform all the calculation functions regarding the collection of data from the meters and the sending of the data and an electronic archiving device with data encryption function 108 adapted to constitute a single true source for the patient data and to ensure data security and physical control.
In order to allow a constant and accurate monitoring of the health conditions of the patient, the invention also comprises specific modules. The doctor will be able to monitor, during a remote control, the body temperature of the patient through an infrared temperature sensor 103, his/her pressure with pressure meter 104 and the health state of the circulatory system by using a blood oxygen and heart rate meter 105. In addition, the patient, through a sensor for pH variation 400 inserted in the mouth, can allow a constant monitoring of the health state of the mouth in order to confirm possible prevention therapies for cavities or infections of the oral cavity. All these parameters are remotely consumable and monitorable by means of a specific software display client 111 adapted to allow the doctor to receive and analyze all the data sent by said communication device, which acts as a server for said data.
Finally, in order to facilitate the interaction between physician and patient, the system provides for the use of a high-definition video camera 110 connected in wireless mode, by way of example by means of Bluetooth, adapted to capture images of various parts of the body of the patient and to send them in streaming mode to the physician, who can analyze them together with the other data in order to be able to monitor the patient and give basic diagnoses and an electronic payment terminal 109 adapted to allow an easy payment by means of card or other payment devices.
With reference to Fig. 2, instead, several integrated devices of said system are represented which are adapted to implement the efficiency thereof Among said devices, there is an intelligent toothbrush that records the dental hygiene operations and sends them by means of Bluetooth connection 200 to said communication device 100, allowing a monitoring and a report of the total use time, and a case for intelligent orthodontic apparatuses 300 connected by means of Bluetooth connection to said device 100 and provided with a light sensor 301 adapted to send light signals at the time in which the patient, according to the prescription, must w'ear said orthodontic apparatus, and a sensor 302 within the orthodontic apparatus mobile connected to the device 100. Finally, in order to allow the monitoring of other parameters that are important and essential for a correct diagnosis and an efficient control, the invention also provides for a specialized bacterial plaque detector 500 adapted to monitor and communicate, both to the patient and to the physician, the level of bacteria accumulated in the oral cavity and possibly to intervene or intensify the prevention treatment. Finally, the image represents a generic mobile device used for the configuration of said communication device. Finally, it is clear that modifications, additions or variations that are obvious for a man skilled in the art can be made to the invention described up to now, without departing from the protective scope that is provided by the following claims.

Claims
1. System for monitoring and communication between patient and doctor characterized in that it monitors and manages, in a personalized manner, a plurality of patients under remote treatment; said system comprising at least a communication device (100) in turn comprising at least:
- a speaker (101) and microphone (102) adapted to allow' a voice interaction with the patient or to communicate directly with one’s own doctor, preserving the privacy of the telephone number,
- an infrared temperature sensor (103) adapted to allow the doctor to monitor, during a remote control, the body temperature of the patient;
- a pressure meter (104) adapted to allow monitoring the same,
- a blood oxygen and heart rate meter (105) adapted to measure the health state of the circulatory system;
- a processor (106) adapted to perform all the calculation functions regarding the collection of data from the meters and the sending of the data;
- an Internet network connection (107) for the communication and sending of the data to the physician;
- an electronic archiving device with data encryption function (108) adapted to constitute a single true source for the patient data and to ensure data security and physical control;
- an electronic payment terminal (109) adapted to allow easy payment by means of card or other payment devices,
- a high-definition video camera (110) connected in wireless mode, by way of example by means of Bluetooth, adapted to capture images of various parts of the body of the patient and to send them in streaming mode to the physician who can analyze it together with the other data in order to be able to monitor the patient and to give basic diagnoses, said communication device (100) thus having server function for the patient data at the time in which the physician consults the data of the latter; said system finally comprising a software display client (111) adapted to allow the doctor to receive and analyze all the data sent by said communication device (100) and limiting the possibility to have a copy thereof in order to maintain total privacy,

System for monitoring and communication between patient and doctor, according to the preceding claim 1, characterized in that it is adapted for dental treatment and it comprises an intelligent toothbrush that records the dental hygiene operations and sends them by means of Bluetooth connection (200) to said communication device (100), allowing a monitoring and a report of the total use time.

System for monitoring and communication between patient and doctor, according to any one of the preceding claims, characterized in that it comprises a case for intelligent orthodontic apparatuses (300) connected by means of Bluetooth connection to said device (100) and provided with a light sensor (301) adapted to send light signals at the time in which the patient, according to the prescription, must wear said orthodontic apparatus,

System for monitoring and communication between patient and doctor, according to any one of the preceding claims, characterized in that it comprises a sensor (302) positioned within a movable orthodontic apparatus, connected by means of Bluetooth connection to said device (100) in order to communicate the use time of the orthodontic therapy and signal possible moving away from the therapeutic plan and other parameters indicating the state of the corrections induced by the movable orthodontic therapy.

System for monitoring and communication between patient and doctor, according to any one of the preceding claims, characterized in that it comprises a sensor (400) for pH variation in the mouth adapted to allow constant monitoring of the health state of the mouth, and to confirm possible prevention therapies for cavities or infections of the oral cavity.

System for monitoring and communication between patient and doctor, according to any one of the preceding claims, characterized in that it comprises a bacterial plaque detector (500) adapted to monitor and communicate both to the patient and to the physician the level of bacteria accumulated in the oral cavity and possibly intervene or intensify the prevention treatment.
7. System for monitoring and communication between patient and doctor, according to any one of the preceding claims, characterized in that the patient can consult all his/her personal information online in a clinical file shared by the physician.
8. System for monitoring and communication between patient and doctor, according to any one of the preceding claims, characterized in that by means of said communication device (100), the patient can obtain personalized information regarding lifestyle such as, by way of a non-limiting example, a training sheet or diet diary provided by a nutritionist.
9. System for monitoring and communication between patient and doctor, according to any one of the preceding claims, characterized in that said communication device (100) can be configured by the user by means of any one computer or mobile device (900).
10. System for monitoring and communication between patient and doctor, according to any one of the preceding claims, characterized in that said payment terminal (109) comprises an additional function of scanning and printing documents relative to systems and processes regarding authorizations, exemptions and identification of the patient of the public health system.
