Description
Device for performing medical and/or diagnostic procedure The present invention relates to a device for performing medical and/or diagnostic procedure in an outpatient setting. More particularly, an object of the invention is a device for performing a semi-automatic or automatic cervical conization procedure, enabling precise administration of an anesthetic agent, excision of a portion of the cervix, and collection of samples for further testing.
At present, the cervical conization procedure is performed using a combination of simple surgical and gynecologic instruments. However, such simple manual tools make it impossible to precisely perform the procedure and collect material for histopathological examination, while sample size determination is subject to significant error and often results in taking too small of a sample for further analysis and diagnosis.
Several more complex tools for performing such procedure are also known in the state of the art. For instance, document CN106974709A discloses a tool for performing a cervical conization procedure, with a blade assembly, a central tube, a cutting blade shaft and a drive mechanism. The blade shaft can be moved and rotated. The device is equipped with a vacuum suction cup located in the absorption cavity. The invention, which is not fully automated, though, does not eliminate all the problems that occur in this type of procedure, including the time required by the tool to perform a complete medical procedure, or the extensive experience and manual skills of the person performing the procedure.
In a turn, document US6309388B1 discloses a device for performing electrocoagulation of cervical tissue. The tool includes a housing, inside of which is a rotating mechanism that supplies electrical current. Electrical energy is delivered to an electrocoagulation head comprising an electrocoagulation wire functionally electrically coupled to a connector and rotating about a longitudinal axis parallel to a second longitudinal axis, causing electrocoagulation of patient tissue as it rotates about the longitudinal axis. However, it is worth noting that in the case of sampling for diagnostic purposes, electrocoagulation may lead to sample deterioration or destruction.
The purpose of the invention is therefore to solve the limitations of the tools available in the state of the art for performing medical and/or diagnostic procedure, in particular cervical conization, especially by increasing the automation of the device, reducing the time required by the device to perform a complete medical procedure and ensuring the precision and quality of the sample. The device according to the invention should also make it possible to perform all the required operations in a comprehensive manner and in a single procedure.

A detailed description of a preferred embodiment of the invention
A device for performing medical and/or diagnostic procedure, in particular for performing a cervical conization procedure, according to a preferred embodiment of the invention, is shown in detail in Fig. 1.
The device generally comprises a vacuum cup 1, a guide lance 2, a centering device 3, a pusher mechanism with needles 4, an automatic blade 5, and a diathermy apparatus 6.
The vacuum cup 1 is oriented with its opening in a distal direction and connected to a guide lance 2 passing axially through it, the guide lance 2 being in the form of a tube, the distal end of which is located within the vacuum cup 1 and the proximal end of which is
outside the vacuum cup 1 . Vacuum cup 1 is made of transparent material approved for contact with the human body such as, for example, PP, PE, PS. The use of transparent plastic facilitates the operation of the device and visual inspection of the procedure. The guide lance 2 generates a vacuum in the vacuum cup 1 . Preferably, the guide lance 2 is made of acid-resistant (stainless) steel. At the distal end of the guide lance 2, the centering device 3 is provided which terminates within the opening of the vacuum cup 1. The centering device 3 is cylindrical in shape ending in a cone and is made of surgical steel. A pusher mechanism with needles 4 located within a vacuum cup 1 is connected to the guide lance 2, which is connected to an anaesthetic agent (anaesthetic) dosing system. This automatically moves the pusher, punctures the tissue, and delivers the anaesthetic. After the anaesthetic is injected, the pusher retracts.
A diathermy apparatus 6 is connected to the guide lance 2, terminating with, located within the vacuum cup 1 , automatic blade 5 connected to the guide lance 2. The blade 5 is in the form of a leaf spring made of surgical steel. The blade 5 is in a shape of a circle (ring) and is articulatedly mounted to the guide lance 2. The blade 5 rotates and cuts off a portion of the cervix, which lands after excision at the bottom of the vacuum cup 1. The blade 5, during the cutting operation, performs a movement along a portion of the circle within a range of about 180°. It will be obvious to an expert in the field that the construction of the blade 5 may be different. The blade may be differently connected to the device, may have a different shape and geometry, and may perform movement within a different range. The cutting automation is provided by a drive unit (not included in the figure for clarity) causing movement of the automatic blade 5 enabling control of speed and force. For the drive unit, a person skilled in the art unit may select, for instance, a stepper motor, a servo drive, a pneumatic actuator or many others from the state of the art.
For a clearer representation of the invention, the figures of the drawing do not include additional components of the device according to the invention. In particular, these are a control unit configured to control the device and a viewing device such as an endoscope, the vision probe of which is placed in the bottom of the wall of the vacuum cup IThe control unit further enables the device according to the invention to be fully automated, in particular by further automation and coordination of, for instance, vacuum generation, anesthesia using the needle pusher mechanism 4, or cutting with the automatic blade 5.
While, the viewing device, in addition to the transparency of the vacuum cup (1 ), further facilitates device operation, visual inspection of the procedure, and provides video documentation capabilities.
The principle of operation of the device according to the invention, is illustrated in Fig. 2. The device operates on the principle that it is inserted into the vagina A of a patient. The opening of the vacuum cup 1 is adjacent to the treatment site, and the centering device 3 positions the device at the center of the cervix. After reaching the desired position, the person performing the treatment presses the start button and a vacuum is created in the vacuum cup 1. Thereafter, the needle pusher mechanism 4 having a connection to the (unmarked in the figure) anaesthetic dosing system delivers a predetermined dose of the local anaesthetic agent, after which the needle pusher mechanism 4 retracts to the initial position. After the time required for the anesthetic to take effect has elapsed, the automatic blade 5 rotates and cuts off a portion of the cervix, which, when cut off, lands at the bottom of the vacuum cup 1 due to the vacuum created therein. After the tissue is cut off, the blade 5 makes a return motion and the system turns on the voltage in the diathermy apparatus 6, which coagulates the cervical wound created during the removal of the tissue fragment. Coagulation occurs only on the cervical side, because the cut section is at the bottom of the vacuum cup 1 and coagulation does not occur in the cut tissue. This ensures that the cut off fragment is intact and allows for better analysis and histopathological examination. When the blade 5 returns, the examiner removes the tool from the patient's vagina A.
