Abstract
The invention relates to medical diagnostic equipment, and more particularly to systems for screening the state of a person's cardiovascular system in real time with automatic diagnosis of abnormalities on an electrocardiogram (ECG) for the early detection, warning and diagnosis of cardiovascular diseases. A hardware/software system comprises a housing that is attachable to a user and contains: a microcontroller, an analog-to-digital converter, a data storage means, a wireless communication module, electrodes, a storage battery, and a socket for the connection of external electrodes for recording an ECG. The frequency band of the amplitude/frequency characteristics for all of the channels of the analog interfaces is 0-4000 Hz with a permissible amplitude variation of from 0 to 2000 Hz±10% and a sampling frequency of 8000 Hz. The microcontroller and the data storage means are configured for recording, reading and transmitting ECG data in parallel streams with priority for the process of recording. The microcontroller implements diagnostic algorithms based on rules and decision trees, and connects to diagnostic units that implement machine-learning methods. The invention provides highly accurate, automatic, pre-clinical, nosological, syndromic diagnosis of abnormalities on ECGs of varying duration.

Развитие методов автоматизированной обработки данных в сочетании с прогрессом в области телемедицинских технологий и использованием портативной медицинской аппаратуры открывает новые возможности для совершенствования методов ранней диагностики болезней системы кровообращения и дистанционного контроля состояния здоровья пациентов. Одним из перспективных направлений является создание интеллектуального электрокардиографа, т. е. автоматизированной системы расшифровки сигналов электрокардиограммы (ЭКГ) с получением заключения, максимально приближенного к врачебному. Для достижения этой цели в известных нам прототипах кардиорегистраторов используются прямые алгоритмы диагностики, основанные на правилах и деревьях решений. В целях повышения точности диагностики авторами была разработана система двух уровневой автоматической диагностики сигнала ЭКГ. Первым этапом использовались стандартные прямые методы диагностики. Примеры таких методов приведены на фиг. 4, 5, 6. На фиг. 4 показан пример дерева принятия решений для определения, является ли ритм синусовым. На фиг. 5 показан пример дерева принятия решений для определения синусовой тахикардии и брадикардии. На фиг. 6 показан пример дерева принятия решений для определения отклонения электрической оси сердца. Вторым этапом - методы машинного обучения, включающий обучение и тестирование программного обеспечения на больших массивах базы данных ЭКГ и на структурированных врачебных заключениях. The development of methods for automated data processing, combined with progress in the field of telemedicine technologies and the use of portable medical equipment, opens up new opportunities for improving methods for early diagnosis of diseases of the circulatory system and remote monitoring of patients' health. One of the promising directions is the creation of an intelligent electrocardiograph, i.e. an automated system for decoding electrocardiogram (ECG) signals with obtaining a conclusion as close as possible to a medical one. To achieve this goal, the prototypes of cardiac recorders known to us use direct diagnostic algorithms based on rules and decision trees. In order to improve the accuracy of diagnostics, the authors developed a system of two-level automatic diagnostics of the ECG signal. At the first stage, standard direct diagnostic methods were used. Examples of such methods are shown in Fig. 4, 5, 6. In Figs. 4 shows an example decision tree for determining if the rhythm is sinus. In FIG. 5 shows an example decision tree for defining sinus tachycardia and bradycardia. In FIG. 6 shows an example decision tree for determining the deviation of the electrical axis of the heart. The second stage is machine learning methods, including training and testing of software on large arrays of the ECG database and on structured medical reports.
Использовались методы машинного обучения - алгоритмы, автоматически строящиеся по большой выборке «размеченных данных», т.е. базе ЭКГ с известными заключениями. При этом в методах машинного обучения алгоритм постановки диагноза (решающая функция) не закладывался явным образом. Происходила «настройка» модели по данным обучающей выборки - набору характеристик и атрибутов ЭКГ пациента с известным заключением.
Был разработан базовый вариант программы анализа ЭКГ "Диагностика», позволяющий далее проводить её обучение. Интерфейсная часть программы «Диагностика» написана на языке С#, математические методы - на языке Python с использованием библиотеки матричных вычисления NumPy (www.numpy.org), библиотеки вейвлет-анализа PyWavelets (github.com/PyWavelets) и библиотеки методов машинного обучения Scikit-leam (scikit-leam.org/stable). Machine learning methods were used - algorithms that are automatically built on a large sample of "labeled data", i.e. ECG database with known findings. At the same time, in the methods of machine learning, the algorithm for making a diagnosis (the decisive function) was not laid down explicitly. The model was "adjusted" according to the data of the training sample - a set of characteristics and attributes of the patient's ECG with a known conclusion. A basic version of the "Diagnostics" ECG analysis program was developed, which allows further training. The interface part of the "Diagnostics" program is written in C#, mathematical methods are written in Python using the NumPy matrix calculation library (www.numpy.org), the library wavelet analysis PyWavelets (github.com/PyWavelets) and machine learning library Scikit-leam (scikit-leam.org/stable).
Была создана база данных «Кардиобаза», включающая 36652 записей стандартной 12-канальной ЭКГ покоя в цифровом формате EDF. ЭКГ были получены от взрослых пациентов в возрасте 17 - 80 лет, Записи ЭКГ были независимо описаны врачами-экспертами (кардиологами и врачами функциональной диагностики) с формированием структурированного врачебного заключения. Далее все записи ЭКГ были подвергнуты автоматическому анализу с использованием метода детектирования ключевых точек (ДКТ), сегментированы и автоматически описаны в том же формате доврачебного заключения разработанной авторами программой по классическим критериям анализа ЭКГ. - ДКТ сигнала - детектирование начала, пика и конца комплекса QRS, Р- и Т-волн и определение их морфологии; - DCT signal - detection of the beginning, peak and end of the QRS complex, P- and T-waves and determination of their morphology;
- автоматическая генерация заключения. - Automatic conclusion generation.
Использовались стандартные функции для вычисления числовых характеристик (средняя продолжительность и высота комплексов сигнала ЭКГ, их стандартные отклонения и т.д.). В итоге для описания каждого отведения ЭКГ были получены признаки (см. фиг. 7). ПРОМЫШЛЕННАЯ ПРИМЕНИМОСТЬ INDUSTRIAL APPLICABILITY
Было проведено обучение программы «Диагностика» с использованием ЭКГ, имеющихся в «Кардиобазе», с формализованными заключениями врачей. Были использованы методы машинного обучения: метод опорных векторов (SVM), дерево решений, методы квадратичного и линейного дискриминантного анализа, метод случайных подпространств, AdaBoost, случайный лес, логистическая
регрессия (модель нейрона МакКаллока-Питтса) с предварительной предобработкой признаков с помощью слоя BatchNormalization, графовый метод. При анализе результатов учитывалась точность диагностики относительно основных признаков и классов ЭКГ. Проводилась комплексная оценка с сопоставлением результатов. Оценка качества происходила с помощью пятикратного перекрестного контроля по метрике ROC-AUC с помощью методов машинного обучения на доступных международных базах данных: Arryhthmia, PTBDB, PhysioNetCompetition 2017. Чувствительность заключений, выполненных программой «Диагностика», оказалась равной 85,8%- 100,0% по различным классам в сравнении с заключениями врачей-диагностов. Специфичность метода составила 90,2-99,0%, точность - 86,9-95,1%. A comprehensive assessment was carried out with a comparison of the results. Quality assessment was carried out using five-fold cross-checking according to the ROC-AUC metric using machine learning methods on available international databases: Arryhthmia, PTBDB, PhysioNetCompetition 2017. The sensitivity of the conclusions made by the Diagnostics program turned out to be 85.8% - 100.0 % for different classes in comparison with the conclusions of diagnosticians. The specificity of the method was 90.2-99.0%, the accuracy was 86.9-95.1%.

Результаты проведённого исследования показывают, что методы машинного обучения могут успешно применяться в задачах автоматического анализа ЭКГ, при этом наиболее перспективными является сочетание методов логистической регрессии и случайного леса. Достигается технический результат, заключающийся в реализации автоматической доврачебной нозологической посиндромальной высокоточной диагностики патологических отклонений ЭКГ различной длительности.

Claims
1. Аппаратно-программный комплекс электрокардиографических (ЭКГ) измерений, содержащее корпус, закрепляемый на пользователе, в котором установлены: микроконтроллер, выполняющий управление электронным устройством; аналого-цифровой преобразователь, обеспечивающий преобразование в цифровой вид сигналов, поступающих с электродов; средство хранения данных, выполненное с возможностью хранения данных ЭКГ ; модуль беспроводной связи, обеспечивающий передачу данных из средства памяти на внешнее вычислительное устройство; первый электрод, совмещенный с емкостной кнопкой, обеспечивающей активацию функции снятия ЭКГ устройством; второй и третий электроды, расположенные на тыльной стороне корпуса и обеспечивающие совместно с первым электродом регистрацию ЭКГ ; аккумулятор; разъем для подключения внешних электродов, обеспечивающих регистрацию ЭКГ, отличающийся тем, что полоса частот амплитудно-частотных характеристик для всех каналов аналоговых интерфейсов составляет 0-4000 Гц, с допустимыми отклонениями амплитуд от 0 до 2000Гц ±10%, частота дискретизации сигнала 8000Гц; при этом микроконтроллер и средство хранения данных выполнены с возможностью записи, чтения и передачи данных ЭКГ в параллельных потоках с приоритетом у процесса записи; микроконтроллер также выполнен с возможностью реализации прямых алгоритмов диагностики, основанных на правилах и деревьях решений; микроконтроллер также выполнен с возможностью подключения к блокам диагностики, реализующим методы машинного обучения. 1.
