Abstract
Implante intradiscal para el tratamiento de la escoliosis, cifosis, estenosis y fracturas de la columna vertebral, que comprende un elemento biocompatible, flexible e insuflable, de configuración sensiblemente cilindrica, con una morfología dependiente del problema a tratar, adecuado para introducirse en el espacio discal intervertebral y de ser inflado mediante un sistema externo de presión a fin de permitir la elevación del disco lesionado y de rellenarse con un material (2) que, una vez solidificado, tiene una dureza elástica similar a la del disco intervertebral sano del nivel vertebral en el que se va a introducir, siendo mayor en la zona de las vértebras lumbares y menor en la zona dorsal o cervical.

DESCRIPCION
Implante intradiscal para el tratamiento de la escoliosis, cifosis, estenosis y fracturas de la columna vertebral.
Sector de la técnica
El sector en el que se encuadra la invención es el de los instrumentos, dispositivos o procedimientos quirúrgicos para el tratamiento de los huesos o articulaciones.
La invención se refiere a un implante intradiscal para el tratamiento de deformidades y otros trastornos de la columna vertebral, que consiste en un globo insuflable de diferentes formas y con capacidad de adaptarse al espacio discal al ser insuflado. El llenado de este globo se realiza con materiales que presentan una dureza elástica tal que, resulta ser similar a la del disco intervertebral sano del nivel vertebral en el que se introducen. Con su implantación se logra elevar el disco intervertebral y estabilizar el segmento vertebral correspondiente, con consecuente descompresión del canal medular y de los forámenes, recuperando la movilidad vertebral normal. El uso de este implante encuentra su campo de aplicación en el tratamiento de la escoliosis, cifosis, estenosis, y fracturas de la columna vertebral.
Estado de la técnica
Entre cada dos vértebras se encuentra el disco intervertebral, que es el elemento elástico móvil, que tiene la función de transmitir, distribuir y amortiguar las cargas de peso producidas por la gravedad y el movimiento, evitando de esta manera solicitaciones mecánicas elevadas sobre áreas limitadas de las vértebras que podrían producir fracturas o sobrecargas vertebrales con repercusiones negativas e irreversibles.
El disco intervertebral tiene tres componentes:
1. El cartílago del platillo terminal, que se mantiene laxamente unido al platillo vertebral óseo, conformando una barrera que evita el paso del núcleo pulposo al interior del cuerpo vertebral.
2. El ánulo fibroso, que es una banda gruesa fibrosa que rodea el núcleo pulposo y está constituido por vahas láminas de fibras colágenas.
3. El núcleo pulposo, que está rodeado por los platillos terminales y el ánulo fibroso y es de naturaleza semi-gelatinoso a fin de distribuir las cargas hidráulicas vertebrales en todas las direcciones evitando la concentración de fuerzas sobre áreas limitadas.
La degeneración del disco intervertebral de cualquier nivel de la columna vertebral cervical, dorsal y lumbar produce disminución de la tensión intradiscal del núcleo pulposo y consecuentemente pérdida de la altura discal con abombamiento de las partes blandas que lo envuelven, esto es el ánulo fibroso discal y los ligamentos vertebrales adyacentes. Esta disminución de la altura discal también afecta a las articulaciones facetarías posteriores de las dos vértebras adyacentes al disco que pierden su congruencia con subluxaciones. El abombamiento de los elementos periféricos del disco y la subluxación facetaría introduciéndose los extremos facetados en los forámenes comprimen las estructuras nerviosas, la médula y las raíces nerviosas, que se encuentran en contacto con el disco intervertebral abombado.
Hasta la fecha, para conseguir descomprimir los nervios y la médula de la columna vertebral, estabilizar la columna inestable y recuperar la movilidad vertebral, los cirujanos utilizan abordajes quirúrgicos amplios que obligan a seccionar estructuras anatómicas, abrir la cavidad abdominal y/o torácica en muchas ocasiones y a la manipulación de estructuras nerviosas y vasculares con elevado número de complicaciones para los pacientes, especialmente en los de más edad.
Los documentos US2020113705A1 y US2013131808A1 divulgan sendos implantes en forma de globos, capaces de inflarse mediante un sistema externo de presión y de poderse rellenar con materiales elásticos. Ambos globos están destinados a reemplazar los discos intervertebrales degenerados y más concretamente el núcleo pulposo de un disco intervertebral con una prótesis nuclear implantable percutáneamente.
En el documento W02009117459A2 se describe un dispositivo para restauración de la altura de un disco intervertebral, que incluye un cuerpo que se inserta en el espacio intervertebral, que define sendas superficies superior e inferior que acoplan en el espacio existente entre dos vértebras consecutivas.
Por otro lado, los implantes referenciados no permiten su uso en el tratamiento de la escoliosis, cifosis, estenosis, o fracturas de la columna vertebral, ya que no presentan ni la morfología, ni la estructura adecuadas para ello, ni el material de relleno ofrece la dureza y elasticidad requeridas para las distintas zonas de las vértebras en las que se implantan estos globos.
Tampoco se conoce en la literatura médica una descripción del uso de globos de este tipo para el tratamiento de la escoliosis, cifosis, estenosis, o fracturas de la columna vertebral.
Explicación de la invención
Con el fin de alcanzar los objetivos propuestos, mencionados en el apartado anterior, la invención propone un implante intradiscal para el tratamiento de la escoliosis, cifosis, estenosis, o fracturas de la columna vertebral, que tiene las características de la reivindicación 1. Más concretamente y como indicábamos en el punto anterior, este implante consiste en un globo biocompatible, flexible e insuflable, con una configuración sensiblemente cilindrica, de forma que resulta adecuado para introducirse en el espacio discal intervertebral y al inflarse mediante un sistema externo de presión permite elevar el disco intervertebral lesionado y estabilizar el segmento vertebral correspondiente, al tiempo que descomprimir el canal medular y los forámenes recupera la movilidad vertebral. La principal característica del implante de la invención es que dicho globo presenta, tanto perimetralmente, como en sus bases, una morfología dependiente del problema a tratar; e interiormente un material de relleno que, una vez solidificado, tiene una dureza elástica similar a la del disco intervertebral sano del nivel vertebral en el que se va a introducir.
Una de las grandes ventajas del uso de este tipo de globos intradiscales es que se introducen por la vía percutánea, empleando trocares que a través de la piel se colocan en el disco intervertebral. Su introducción por la piel provoca pequeñas heridas quirúrgicas que facilitan la rápida recuperación del paciente y el alta hospitalaria a las pocas horas de la intervención.
Con nuestra metodología se consigue efectuar una elevación discal insuflando el balón una vez introducido en el interior del disco y de esta manera recuperaremos la altura discal original, quitando el abombamiento de las partes blandas periféricas del disco. También con esta elevación discal se recupera la congruencia de las articulaciones facetarías posteriores. En definitiva, se consigue la descompresión de los elementos nerviosos: la médula y las raíces nerviosas. También se logra la estabilización vertebral mediante “ánulotaxis y ligamentotaxis” conseguida con la elevación del disco intervertebral, al tiempo que se evitan movimientos anormales de ese segmento vertebral discal. Esta elevación discal tensa los elementos ligamentosos periféricos del disco, el ánulo fibroso y los ligamentos vertebrales adyacentes al nivel del disco afectado y de esta manera, se consigue la estabilidad de este segmento vertebral. De igual manera se corrige la cifosis focal producida por el descenso de la altura discal y mejora la funcionalidad muscular al incrementar su brazo de palanca, especialmente en los casos que se tratan varios niveles.
Con nuestra propuesta de uso de este tipo de implante se corrigen los desequilibrios vertebrales producidos por las lesiones del disco o discos de la columna vertebral, cualquiera que sea la causa que provoque la pérdida de altura discal, da lugar a la cifosis de este nivel vertebral, y esta corrección tiene unas consecuencias muy importantes para el paciente porque le permite recuperar la verticalidad y mejorar la funcionalidad.
Nuestra propuesta terapéutica también consigue resultados espectaculares en aquellos pacientes con discopatías múltiples que les obliga a realizar sobrecargas musculares muy importantes para mantenerse erguidos y mantener la verticalidad fisiológica. Esta situación de desequilibrio vertebral provoca cansancio fácil y dolor
especialmente en los ancianos con importantes limitaciones funcionales, que son los más frecuentemente afectados.
Con la elevación discal y el mantenimiento de la altura discal que se logra con este implante elástico es posible la recuperación de la congruencia de las articulaciones facetarías posteriores y la movilidad vertebral fisiológica. Además, este elemento elástico evitan las fracturas de las vértebras adyacentes que se presentan en un elevado porcentaje de casos, a consecuencia del uso de cemento óseo que se implanta actualmente en las vértebras fracturadas. Esta complicación es aún más frecuente en los casos de discoplastia donde se introduce el cemento en el disco intervertebral, y este cemento óseo intradiscal frecuentemente se hunde en el frágil hueso vertebral adyacente (subsidencia). Aunque se ha recomendado para evitar esta complicación realizar vertebroplastias en las vértebras adyacentes a la tratada; es decir, introducir cemento óseo también en las vértebras adyacentes “para reforzarlas”; pero esta metodología no evita la subsidencia del cemento en la vértebra adyacente a las discoplastias, porque el cemento óseo colocado en el interior del disco intervertebral fractura el platillo vertebral que se encuentra junto al cemento y aún más frecuentemente en los casos con osteoporosis, tan frecuente en los pacientes de más edad.
La subsidencia del cemento en los cuerpos vertebrales adyacentes a la discoplastia o la aparición de fracturas vertebrales secundarias al uso de cemento provoca la pérdida de la corrección conseguida y la reaparición de la situación preoperatoria o incluso la empeora. Esta complicación, casi inevitable con el uso de cemento óseo en la columna, se evita con la aplicación de material elástico en los discos con una elasticidad superior a las vértebras adyacentes. El uso de material elástico acortará el tiempo de la cirugía y el número de niveles a tratar, porque no será necesario realizar vertebroplastias adyacentes para evitar la aparición de fracturas de las vértebras vecinas.
Descripción de los dibujos
Para complementar la descripción que se está realizando y con objeto de facilitar la comprensión de las características de la invención, se acompaña a la presente
memoria descriptiva un juego de dibujos en los que, con carácter ilustrativo y no limitativo, se ha representado lo siguiente:
Fig. 1 es una sección desde el lateral del implante de la invención.
Fig. 2 y 3 muestran sendas secciones, sagital y axial respectivamente, en un implante con el globo (1) situado e insuflado en el lugar correspondiente al antiguo núcleo pulposo.
Fig. 4 representa una sección axial de un disco lumbar de un paciente que presenta una escoliosis, tal y como se muestra en la Fig. 5.
Las Fig. 5 y 6 muestran una zona de la columna vertebral con una escoliosis degenerativa toraco-lumbar, respectivamente antes y después de colocar los implantes de la invención.
Fig. 7 muestra una sección axial de un implante colocado en un caso de cifosis con desequilibrio vertebral anterior o espondilolistesis.
Fig. 8 es una vista lateral de un caso de estenosis de canal lumbar siguiendo al tratamiento con implantación discal del dispositivo que proponemos.
Fig. 9 muestra un implante colocado en caso de fractura severa de la vértebra L1 (Kummel), sin afectación del platillo terminal T12.
La Fig. 10 muestra un implante colocado en un caso de fractura severa de la vértebra L1 (Kummel), con fractura asociada del platillo terminal caudal de T12.
Realización de la invención
El implante intradiscal de la presente invención, destinado a la recuperación y tratamiento de la escoliosis, cifosis, estenosis, o fracturas de la columna vertebral, tal y como se aprecia en las Fig. 1-3, comprende un balón (1) biocompatible, flexible, de configuración sensiblemente cilindrica, que es adecuado para introducirse en el espacio discal intervertebral, que está provisto de una conexión (3) a través de la cual
se puede conectar con un tubo (31) que permite la introducción de líquido con ayuda de un sistema de presión externo, consiguiéndose de esta manera su estabilización en el disco y la elevación discal, y consecuentemente la descompresión de los elementos neurológicos lesionados.
De acuerdo a la invención, en este balón (1) al menos una de sus bases (11) es rígida o semirrígida y ambas bases (11) presentan perimetralmente una morfología que depende del problema a tratar. Por su parte la envolvente perimetral (12) es flexible y su altura está determinada por la separación que ha de existir entre discos intervertebrales, una vez que es insuflado dicho globo. De esta forma se consigue adaptar el balón (1) a la recuperación y tratamiento de las afecciones antes citadas.
Según otra importante característica de la invención, el interior de dicho balón (1) se rellena con un material (2) que, una vez solidificado, tiene una dureza elástica similar a la del disco intervertebral sano del nivel vertebral en el que se va a introducir. En términos generales, la dureza elástica del material de relleno (2) del globo (1) está comprendida entre Shore 50A a Shore 90A.
En una realización preferential, el balón (1) cuenta con unos marcadores radio-opacos (17) en ambas bases que permiten objetivar la situación exacta del mismo cuando se coloca entre dos vértebras (4).
El material (2) con el que se rellena el balón (1) puede ser cualquier tipo de material elástico que se inyecta en forma de líquido o de gel, y se solidifica una vez inyectado en el interior del globo (1) adquiriendo la dureza elástica similar a la del disco intervertebral sano del nivel vertebral en el que se va a introducir.
Seguidamente se describen ejemplos de la morfología y estructura de estos balones (1) para diversos tratamientos. Comenzando por las deformaciones vertebrales, como la escoliosis degenerativa, por ejemplo la que se observa en el paciente de la Fig. 5. En este caso, se inserta un implante como el representado en la Fig. 4, en un corte axial de un disco lumbar, donde se evidencia como la morfología del implante tiene la mayor superficie de contacto en la parte anterior (15) y en la concavidad de la escoliosis, que es justamente donde deben aplicarse las mayores fuerzas correctoras de la deformidad y donde sufrirá mayores exigencias mecánicas el implante.
En esta patología no existen descripciones en la literatura médica de tratamiento quirúrgico percutáneo y con la metodología que pasamos a presentar. Tampoco se tiene conocimiento de patentes dirigidas al tratamiento percutáneo de esta patología, ni tampoco con la metodología y el tipo de implante que proponemos realizándolo de manera abierta.
La introducción del trocar en el disco (5) se realiza preferentemente desde el lado de la convexidad, donde técnicamente es muy fácil alcanzar el disco y colocar adecuadamente el implante en situación anterior y en la concavidad de la escoliosis. Teniendo en cuenta que esta patología afecta con frecuencia a pacientes de edad avanzada y que los cambios degenerativos discales son muy severos, con casi ausencia total de tejido intradiscal y frecuente esclerosis de los platillos terminales, el implante está adaptado a estas características. En el diseño de este implante además se ha tenido en cuenta la importancia de aplicar las fuerzas correctoras de la escoliosis en el nivel adecuado que es la concavidad de la escoliosis y la parte anterior del cuerpo vertebral para corregir la cifosis. El implante esta realizado de manera que en esos dos niveles será donde las superficies de contacto del implante con los cuerpos vertebrales serán más extensas.
El implante, es decir el globo conformado, mantiene las superficies cefálica y caudal indurada para mantenerse en contacto con los platillos terminales vertebrales generalmente esclerosos. El implante se rellena de cemento de características elásticas como se describió previamente, teniendo en cuenta la frecuente esclerosis asociada de los platillos terminales.
Para el tratamiento de cifosis, desequilibrios vertebrales anteriores y espondilolistesis se utiliza el mismo implante descrito para el tratamiento de la escoliosis degenerativa pero posicionado en manera diferente. El área donde se realizarán la máximas fuerzas correctoras y donde se mantendrán las mayores solicitaciones mecánicas tras la corrección es en la parte anterior del cuerpo vertebral y es en este área anterior y
central donde debe colocarse el implante (ver Fig. 7). Para ello el punto de entrada percutáneo en el disco debe hacerse más posteriormente utilizando un trocar que permita la angulación de 60-90° del extremo distal con control externo, para permitir el posicionamiento del implante en la parte anterior y central del cuerpo vertebral y esa posición es donde debe insuflarse el implante.
En el tratamiento de la degeneración discal, ya sea en la columna cervical, dorsal o lumbar; de la estenosis del canal lumbar se emplea el implante representado en la Fig.
8. En este caso, el implante está constituido por un globo (1) de material flexible biocompatible susceptible de ser insuflado, y que es posible introducirlo en el disco intervertebral a través de un trocar. Este globo (1) al insuflarlo aumenta de tamaño en el interior del disco adoptando una forma sensiblemente cilindrica. Este cilindro central está en contacto y rodeado por un segundo globo (16), que también puede insuflarse alrededor del primer cilindro (1) y de forma independiente de aquel.
Cuando se usa este implante, en casos de degeneración discal o de estenosis del canal lumbar, con la finalidad de incrementar la estabilidad del implante y dar mayor movilidad discal produciendo una transición más suave al ánulo fibroso, el cilindro central (16) se rellena con un cemento elástico (21) de menor o igual elasticidad el cemento (2) con el que se rellena el cilindro periférico (1).
El globo (1) lleva unos marcadores circulares radio-opacos (17) en ambas bases que permite objetivar la situación exacta de los implantes en relación con los platillos vertebrales terminales que es donde debe situarse para facilitar su función trasmisora de carga, estos marcadores son visualizadles al colocar el globo en el espacio intradiscal bajo control radioscópico, navegación o cuando se colocan por robótica.
Las fracturas del cuerpo vertebral se asocian a hundimientos de los platillos terminales que producen una alteración de la mecánica funcional normal del disco, es decir la trasmisión de carga de peso al perderse el contacto del disco con los elementos óseos adyacentes porque para que el disco transmita adecuadamente la carga es necesario su contacto exacto con ambos platillos terminales de las vértebras adyacentes y en las
fracturas con hundimiento óseo vertebral el contacto o no existe o no es uniforme. Consecuentemente también se alteran las incurvaciones sagitales normales de la columna dando lugar a cambios cifóticos con consecuencias biomecánicas negativas para el paciente.
Incluso en fracturas de escasa cuantía, especialmente en niveles críticos como la transición toraco-lumbar o los niveles lumbares bajos donde las solicitaciones mecánicas y de movilidad son mayores, se pierde la funcionalidad discal, se producen compresiones de las estructuras neurológicas vecinas y solicitaciones mecánicas muy elevadas sobre los elementos ligamentosos de la columna media y posterior que precipitan su degeneración progresiva. Consecuentemente es de gran importancia corregir la inestabilidad discal anterior del nivel fracturado, recuperar la funcionalidad discal y mantener la movilidad vertebral del segmento lesionado de manera percutánea como se hace en esta propuesta.
La lesión discal o lesiones discales producidas con la fractura vertebral se tratarán percutáneamente corrigiendo la cifosis post-fractura mediante la sustitución del disco por una prótesis discal elástica que permita corregir y mantener la corrección de la deformidad vertebral manteniendo la movilidad del nivel fracturado.
En la Fig. 9 se aprecia la reparación de una fractura severa de la vértebra L1 (Kummel), sin afectación del platillo terminal de T12, tratada percutáneamente y empleando implantes como los de la presente invención. Para ello se realiza primero una cifoplastia del cuerpo de L1 con un balón (1) para elevar lo más posible la altura del cuerpo vertebral de L1. A continuación se corrige y estabiliza la deformidad cifótica de este nivel introduciendo en el disco T12-L1 un balón (6) preformado con una superficie plana adaptada al platillo terminal distal de T12 que se conforma como rígida o semirrígida al rellenarse con un cemento de consistencia ósea. La insuflación de este balón eleva la columna vertebral anterior y consigue la corrección de la deformidad vertebral cifótica. El llenado del balón con cemento elástico consigue mantener la corrección y permite la movilidad vertebral.
Como se ha citado anteriormente, en este tipo de fractura se utilizan dos globos diferentes: un primer globo (6) que se coloca en el interior del cuerpo vertebral fracturado y es totalmente elástico y un segundo globo (1) que se introduce en el espacio fracturado y también en el espacio donde se situaba el disco antes de la fractura. Este globo preformado (1) tiene dos compartimentos un compartimento en forma de disco (18), que se adapta al platillo terminal vertebral que no está hundido, y otro inferior que se adapta al platillo vertebral hundido. El compartimento (18) es cilindrico ya que representa la morfología discal y se rellenara con un cemento óseo con la consistencia y elasticidad del hueso que va a sustituir, mientras que el otro compartimento se rellenara con un cemento elástico (2) que sea equiparable a la elasticidad discal normal.
Este globo (1) se insufla después de introducir el primer globo (6) en el interior del cuerpo vertebral para realizar la corrección de la altura de esa vértebra y rellenarlo con el material elástico. La insuflación de este globo (1) permite la corrección de la cifosis focal de este nivel bajo control radioscópico.
Una fractura de los dos platillos vertebrales adyacentes al disco (Fig. 10) requiere emplear tres globos en su reparación:
Sendos globos (6), que se insertan en cada una de las dos vértebras fracturadas para conseguir, tras su insuflación, la corrección de la altura de los cuerpos vertebrales siguiendo la metodología indicada previamente.
El tercer globo (1) se introduce a la altura del disco lesionado. En el ejemplo de la Fig. 10, el tercer globo (1) está divido en tres compartimentos según planos sensiblemente horizontales, susceptibles de ser insufladas de forma independiente. El compartimento intermedio conforma el disco intervertebral y se rellena en primer lugar con un cemento elástico (21) de consistencia similar al del disco normal, finalmente se rellenan los dos compartimentos, superior e inferior, con un cemento (2) de mayor o igual elasticidad que el cemento (21) a fin de que el globo (1) se adapte superior e interiormente a las dos superficies fracturadas irregulares de las vértebras adyacentes.
llna vez descrita la naturaleza de la invención, así como un ejemplo de realización preferente, resulta de manera evidente que la invención es susceptible de aplicación industrial, en el sector indicado. Asimismo se hace constar a los efectos oportunos que los materiales, forma, tamaño y disposición de los elementos descritos podrán ser modificados, siempre y cuando ello no suponga una alteración de las características esenciales de la invención que se reivindican a continuación:
Claims
1.- Implante intradiscal para el tratamiento de la escoliosis, cifosis, estenosis y fracturas de la columna vertebral, que comprende un elemento biocompatible, flexible e insuflable, de configuración sensiblemente cilindrica, adecuado para introducirse en el espacio discal intervertebral y de ser inflado mediante un sistema externo de presión a fin de permitir la elevación del disco lesionado, en el que al menos una de dichas bases (11) es rígida o semirrígida y ambas bases (11) presentan perimetralmente una morfología dependiente del problema a tratar; mientras que la envolvente perimetral (12) es flexible y su altura está determinada por la separación que ha de existir entre discos los intervertebrales una vez que es insuflado dicho globo; y en el que interiormente se rellena con un material (2) que, una vez solidificado, tiene una dureza elástica similar a la del disco intervertebral sano.
2.- Implante, según la reivindicación 1, en el que la envolvente perimetral (12), una vez insuflado, adquiere una altura similar a la del disco intervertebral, caracterizado porque ambas bases (11) son semirrígidas y su morfología se corresponde con una sección irregular en la que al menos una zona (15) presenta forma de sector circular, que tiene mayor superficie de contacto en la zona del ánulo fibroso (5) que es necesario levantar para reparar deformidades vertebrales tales como escoliosis degenerativa, cifosis, desequilibrios vertebrales anteriores y espondilolistesis.
3.- Implante, según la reivindicación 1 , caracterizado por que ambas bases (11) son semirrígidas y la envolvente perimetral (12) cuenta con un cilindro central (16) y otro periférico (1) ambos insuflables y rellenables de forma independiente, con un cemento (2) de igual o menor elasticidad en el cilindro periférico (1) que el cemento (21) con el que se rellena el cilindro central (16), cuando se va a usar en casos de degeneración discal o de estenosis del canal lumbar, para incrementar la estabilidad del implante y dar mayor movilidad discal produciendo una transición más suave al ánulo fibroso.
4.- Implante, según la reivindicación 1, caracterizado por que comprende un globo preformado (1) dividido en dos compartimentos, un compartimento en forma de disco (18), que se adapta al platillo terminal vertebral que no está hundido, que se rellenara con un cemento elástico con la consistencia y elasticidad del disco que va a sustituir,
conformando una base semirrígida, y un segundo compartimento que se rellenara con un cemento óseo (2) que sea equiparable a la elasticidad del hueso adyacente, y que se adapta a la superficie irregular de una vértebra fracturada, en la que previamente se ha introducido un segundo globo (6), en el interior del cuerpo vertebral, para realizar la corrección de la altura de esa vértebra, e introducido un material elástico en su interior que permita la corrección de la cifosis focal de este nivel; en caso de fractura de uno de los platillos vertebrales adyacentes al disco.
5.- Implante, según la reivindicación 1, caracterizado por que comprende un globo preformado (1) para introducirse en el disco lesionado, que está divido en tres compartimentos según planos sensiblemente horizontales, susceptibles de ser insufladas de forma independiente, en el que el compartimento intermedio conforma el disco intervertebral y se rellena en primer lugar con un cemento elástico (21) de consistencia similar al del disco normal, mientras que los dos compartimentos, superior e inferior, se rellenan con un cemento (2) de mayor o igual elasticidad similar a la del huesos adyacente (21) a fin de que el globo (1) se adapte superior e interiormente a las dos superficies fracturadas irregulares de las vértebras adyacentes, en las que previamente se han colocado sendos globos (6) para conseguir, tras su insuflación, la corrección de la altura de los cuerpos vertebrales.
6.- Implante, según cualquiera de las reivindicaciones anteriores, caracterizado por que cuenta con unos marcadores radio-opacos (17) en ambas bases que permiten objetivar la situación exacta del mismo cuando se coloca entre las dos vértebras.
7.- Implante, según cualquiera de las reivindicaciones anteriores, caracterizado por que la dureza elástica del material de relleno (2) del globo (1) está comprendida entre Shore 50A a Shore 90A.
8.- Implante, según cualquiera de las reivindicaciones anteriores, caracterizado por que el material con el que se rellena es una silicona, caucho, cemento o cualquier otro material elástico, inicialmente líquido o en forma de gel, y que se solidifica una vez inyectado en el interior del globo (1) adquiriendo la dureza elástica similar a la del disco intervertebral sano del nivel vertebral en el que se va a introducir.

9.- Uso de un globo intradiscal que comprende un elemento biocompatible, flexible e insuflable, de configuración sensiblemente cilindrica, adecuado para introducirse en el espacio discal intervertebral y de ser inflado mediante un sistema externo de presión a fin de permitir la elevación del disco lesionado, en el que las bases (11) son rígidas o semirrígidas y presentan perimetralmente una sección irregular en la que al menos una zona (15) presenta forma de sector circular, que tiene mayor superficie de contacto en la zona del ánulo fibroso (5) que es necesario levantar, para la reparación de deformidades vertebrales tales como escoliosis degenerativa, cifosis, desequilibrios vertebrales anteriores y espondilolistesis.
10.- Uso de un globo intradiscal que comprende un elemento biocompatible, flexible e insuflable, de configuración sensiblemente cilindrica, adecuado para introducirse en el espacio discal intervertebral y de ser inflado mediante un sistema externo de presión a fin de permitir la elevación del disco lesionado, en el que ambas bases (11) son semirrígidas y la envolvente perimetral (12) cuenta con un cilindro central (16) y otro periférico (1), ambos insuflables y rellenables de forma independiente, con cementos de menor o igual elasticidad en el cilindro periférico (1) que en el cilindro central (16), en casos de degeneración discal o de estenosis del canal lumbar, para incrementar la estabilidad del implante y dar mayor movilidad discal produciendo una transición más suave al ánulo fibroso.
11.- Uso de un globo intradiscal que comprende un elemento biocompatible, flexible e insuflable, de configuración sensiblemente cilindrica, adecuado para introducirse en el espacio discal intervertebral y de ser inflado mediante un sistema externo de presión a fin de permitir la elevación del disco lesionado, que interiormente está dividido en dos compartimentos, un compartimento en forma de disco (18), que se adapta al platillo terminal vertebral que no está hundido, que se rellenara con un cemento óseo con la consistencia y elasticidad del hueso que va a sustituir, y un segundo compartimento que se rellenara con un cemento elástico (2) que sea equiparable a la elasticidad discal normal, que se adapta a la superficie irregular de una vértebra fracturada, en la que previamente se ha introducido un segundo globo (6), en el interior del cuerpo vertebral, para realizar la corrección de la altura de esa vértebra y se introduce un material elástico en su interior; en la reparación de la fractura de uno de los platillos vertebrales adyacentes al disco de una vértebra fracturada.

12.- Uso de un globo intradiscal que comprende un elemento biocompatible, flexible e insuflable, de configuración sensiblemente cilindrica, adecuado para introducirse en el espacio discal intervertebral y de ser inflado mediante un sistema externo de presión a fin de permitir la elevación del disco lesionado, que interiormente está divido en tres secciones según planos sensiblemente horizontales, susceptibles de ser insufladas de forma independiente, conformando la sección intermedia un disco que se rellena en primer lugar con un cemento elástico (21), de mayor o igual elasticidad que el cemento (2) con el que se rellenan posteriormente las dos secciones superior e inferior, para recuperar la altura vertebral perdida tras la fractura, de las vértebras adyacentes, en las que previamente se han colocado sendos globos (6) para conseguir, tras su insuflación, la corrección de la altura de los cuerpos vertebrales
