多功能旋臂开口器 技术领域
本实用新型涉及一种多功能旋臂开口器。
背景技术
开口器是临床进行咽部和口腔全麻手术时,使患者的口腔保持张开状态的常用器具,是耳鼻咽喉头颈外科和口腔科的必备手术器械。
技术问题
传统常用带压舌板的开口器有Davis-Boyle开口器、Dingmann开口器等,这些开口器主要由金属固定框架和直角压舌板两部分构成,使用时通过对安装在固定框架上的压舌板的牵拉和提举达到开口效果。然而,由于开口器的固定框架的限制,压舌板开口牵开方向只能与框架平面一致,即只能在一个方向受力,这使得只能牵开口腔和口咽部,难以显露舌根及喉咽深部。而为了达到更好的口腔或咽部暴露效果,经常需要勉强将压舌板手柄向上提拉,这一过程的副作用是把受力点转移到上列牙齿及上颌骨,甚至使头位改变颈部扭曲,易造成附带损伤,也难以达到期望效果。有鉴于此,有必要设计一种新的开口器,以有效克服现有开口器的缺点。
技术解决方案
本实用新型针对上述现有技术存在的问题做出改进,即本实用新型所要解决的技术问题是提供一种多功能旋臂开口器,结构设计合理,不仅提高使用灵活性,而且实现口腔咽喉深部更好的暴露。
为了实现上述目的,本实用新型采用的技术方案是:一种多功能旋臂开口器,包括三边固定框架和通过旋臂与三边固定框架的开口相铰接的压舌组件,所述三边固定框架的两侧边沿其长度方向分别设有若干个并排分布的调节孔,所述旋臂的两端分别与三边固定框架的两侧边上的调节孔相连接。
进一步的,所述三边固定框架包括Ω形的框顶边和两个直条状的框侧边,所述框顶边的中间弧形段上滑动连接有至少一个齿钩,两个框侧边水平设置且分别与框顶边两端的水平段垂直连接,所述框侧边上沿其长度方向设有若干个并排分布的调节孔。
进一步的,所述框顶边的中间弧形段两端分别螺接有用于对齿钩进行限位的防滑脱螺钉。
进一步的,所述旋臂包括贯穿两个框侧边上其中两个位置相对应的调节孔的轴心螺杆,所述轴心螺杆的两端通过固定螺母与框侧边锁紧固定,轴心螺杆的外侧可转动的套设有轴套;所述压舌组件与轴套的外侧壁固定连接。
进一步的,所述压舌组件包括压舌板导槽、与压舌板导槽滑动配合的压舌板或牵开器,所述压舌板导槽与旋臂相垂直,压舌板导槽靠近旋臂的一端与轴套的中部外侧壁固定连接。
进一步的,所述牵开器包括喉咽牵开器和颈前间隙牵开器,所述喉咽牵开器、颈前间隙牵开器以及压舌板的形状与手术部位相适应且均为直角形状。
进一步的,所述旋臂远离框顶边的一侧平行设有旋臂限向螺杆,所述旋臂限向螺杆位于压舌板导槽的下方,旋臂限向螺杆贯穿两个框侧边,旋臂限向螺杆的其中一端经由紧固螺母与框侧边锁紧。

本实施例中,所述三边固定框架1包括Ω形的框顶边5和两个直条状的框侧边6,所述框顶边5包含向上凸起的中间弧形段501和设于中间弧形段501两端的水平段502,框顶边5的中间弧形段适应上齿槽生理解剖曲度,框顶边5的中间弧形段上滑动连接有两个齿钩7,两个框侧边6水平设置且分别与框顶边5两端的水平段502垂直连接,所述框侧边6上沿其长度方向设有3~5个并排分布的调节孔3。应说明的是,齿钩为现有成熟技术,其在现有的开口器中广泛使用,此处不再对其具体构造做过多赘述。齿钩经框顶边两端工字形缩窄处套入,滑动安装于框顶边的中间弧形段上,使用时勾扣于上列牙齿,成为开口器框架的关键受力点。通过在两个框侧边上均开设3~5个调节孔,可根据年龄个体及手术区域的不同,旋臂安装于不同的调节孔内,三边固定边框与旋臂形成四边形框架,实现对框架空间大小的简易调节,达到最佳手术区域暴露效果。
本实施例中,为了对齿钩的滑动进行限位,所述框顶边5的中间弧形段501顶面两端分别开设有螺纹孔27,每个螺纹孔内螺接有用于对齿钩7进行限位的防滑脱螺钉8。
本实施例中,所述旋臂4包括贯穿两个框侧边6上其中两个位置相对应的调节孔3的轴心螺杆9,所述轴心螺杆9的两端通过固定螺母10与框侧边6锁紧固定,轴心螺杆9的外侧设有可转动的轴套11;所述压舌组件2与轴套11的外侧壁中部固定连接,以实现压舌组件的转动连接。
本实施例中,所述压舌组件2包括现有的压舌板导槽12、与压舌板导槽12滑动配合的压舌板13或牵开器,所述压舌板导槽12与旋臂4相垂直,压舌板导槽12靠近旋臂4的一端与轴套11的中部外侧壁固定连接。压舌板导槽为现有的成熟技术,此次不对其具体构造做过多赘述,其与压舌板或牵开器滑动配合,这样方便根据需要调节压舌板或者牵开器的位置。

(1)利用三边固定框架与可调的悬臂组合形成可调金属框架,且压舌板导槽可旋转,即能在金属框架的同一平面牵拉开口,也可在与之垂直的方向牵开显露咽喉深部,安装方便,结构简洁,使用效果好;
(2)现有的直角形压舌板、牵开器结合框架空间大小调整,除了可用于口腔口咽常规手术外,还可用于下咽喉部内镜手术和机器人辅助咽喉区手术,以及用于经口腔路径的颈前间隙手术,实现多用途功能;
(3)三边固定框架的两个框侧边上的夹持组件,安装简单,使用方便,可根据需要夹持颊部侧向拉钩、吸引头、内镜等器械,使用时不会脱落;
(4)压舌板导槽通过旋臂与固定框架实现转动连接,以便根据需要实现压舌板不同平面角度与力度调节,使用时可有效缓解上牙列和上颌部的受力,因此可使上牙列和颈部副损伤的概率减小。
本实施例中,还应说明的是,在使用时,为了能达到更理想的效果,该多功能旋臂开口器可与开口器支撑装置28(专利申请号:202022045548.1)相配合使用,如图6所示,具体的使用方式为:将与压舌板导槽12滑动配合的直角形压舌板或者牵开器远离三边固定框1的一端与开口器支撑装置28的持握结构29的C形卡槽卡接配合,之后通过C形卡槽顶面的螺栓30将持握结构29与压舌板13或牵开器锁紧固定,最后利用该开口器支撑装置28将压舌板或牵开器固定支撑于所需的提拉角度与力度。

  4. 根据权利要求2所述的多功能旋臂开口器,其特征在于:所述旋臂包括贯穿两个框侧边上其中两个位置相对应的调节孔的轴心螺杆,所述轴心螺杆的两端通过固定螺母与框侧边锁紧固定,轴心螺杆的外侧可转动的套设有轴套;所述压舌组件与轴套的外侧壁固定连接。
  5. 根据权利要求4所述的多功能旋臂开口器,其特征在于:所述压舌组件包括压舌板导槽、与压舌板导槽滑动配合的压舌板或牵开器,所述压舌板导槽与旋臂相垂直,压舌板导槽靠近旋臂的一端与轴套的中部外侧壁固定连接。
  6. 根据权利要求5所述的多功能旋臂开口器,其特征在于:所述牵开器包括喉咽牵开器和颈前间隙牵开器,所述喉咽牵开器、颈前间隙牵开器以及压舌板的形状与手术部位相适应且均为直角形状。
  7. 根据权利要求5所述的多功能旋臂开口器,其特征在于:所述旋臂远离框顶边的一侧平行设有旋臂限向螺杆,所述旋臂限向螺杆位于压舌板导槽的下方,旋臂限向螺杆贯穿两个框侧边,旋臂限向螺杆的其中一端经由紧固螺母与框侧边锁紧。
  8. 根据权利要求2所述的多功能旋臂开口器,其特征在于:还包括夹持组件,所述夹持组件包含分别与两个框侧边形状相匹配的滑座,所述滑座的下端与框侧边滑动连接,滑座的上端滑动连接有与框侧边相垂直的滑杆,所述滑杆位于三边固定框架内侧的一端固联有螺接有夹持件。
  9. 根据权利要求8所述的多功能旋臂开口器,其特征在于:所述滑座的下端设有以利于框侧边滑动贯穿的第一滑动通孔,滑座的上端设有以利于滑杆滑动贯穿的第二滑动通孔,滑座的顶部开设有与第二滑动通孔相连通的锁紧螺孔,所述锁紧螺孔内螺接有用于将滑座与滑杆锁紧固定的锁紧螺钉。
  10. 根据权利要求1所述的多功能旋臂开口器,其特征在于:所述三边固定框架由金属材料制成。
