Abstract

The present invention ensures to create the triage system, wherein the triage application is carried out completely and in an effective manner within the framework of measurable criteria and standards, priority of examination is given to the patients who are presented to the emergency service by considering the urgency of patients, priority situations are listed, it can serve as a guide and provide support to the practitioner since it is a triage application that does not vary based on the institutions and practitioners.

Description

TRIAGE DECISION SUPPORT METHOD AND THE SYSTEM USING THIS
METHOD
Technical Field of the Invention
The invention relates to a triage decision support method that provides easy application of the triage method applied in the presentations to the emergency service within the framework of measurable criteria and standards by the auxiliary health staff, performing the same in a standardized and effective manner, determining the time and order of the emergency service to be given to the patients that are presented to the emergency service, does not vary according to institutions and practitioners providing emergency service and gives decision support to the practitioner and relates to a system that uses this method.
State of the Art
Emergency services of the hospitals are clinics wherein generally more than one emergency case must be dealt simultaneously and thus the number of patients in emergency services increases day by day. In such emergency presentations that increase the workload of the emergency services, the most effective method adopted in the world is the triage system. In this context, triage is a dynamic process wherein the instant parameters are considered so as to provide treatment to the right patient at the right time by classifying the patients according to the priority of emergency in order to provide the patients to receive effective and proper emergency health services by making classification according to the types of injuries of the patients and injured individuals instantly.
In Turkey, triage application in the emergency services is required with the “ Declaration on the Application Procedures and Principles of Emergency Services in Inpatient Health Facilities ” which is issued by the T.R. Ministry of Health and published in the Official Gazette No. 27378 on October 16, 2009 and published the colour coding table that shows the urgency of the patient for triage practice. Moreover, it has been reported that in hospitals, doctors, nurses, health officers and emergency medical technicians can perform the triage.
Triage has been started to be implemented in the emergency services of hospitals within this framework. The triage practitioners attempt to define the level of urgency of the patient (triage colour code) within the scope of the triage colour table, depending on their experience, know-how and interpretations of the parameters that instantly occur based on their own experience and know how. Since the evaluation of the parameters is in possession of said characteristics of the triage practitioner in practice, mostly the triage practice is carried out by arranging the emergency presentations in order based on their arrival time. It is seen that different triage practitioners define different levels of urgency in the same patient. Moreover, since the doctor is also included in the triage practitioners, it creates a disadvantage in terms of cost effectiveness and workload of the doctor, reduces the effectiveness of the intervention of the doctor to the emergency patients.
Due to the above mentioned reasons, in the triage application, it is seen that many health institutions and triage practitioners examine patients mostly by taking the triage scale and the colour condition of them into consideration, however it varies based on the know how between them. This application leads to life threatening situations not to be noticed, to provide delayed service to the emergency patients, sometimes to provide service and treatment to the non-emergency patients according to the order of arrival. Application of the triage system as required is vitally important when intense patient presentations are available or when a situation occurs where immediate aid requirement is intense. Priority shall be given to the patients with high urgency, or patients with low urgency should be kept within the safe waiting period and an accurate order of urgency shall be made.
When methods of the state of the art are considered, there are some studies on triage decision support systems.
In the United States patent application numbered US20150036551, there is a triage system that can be accessed over a communication network and comprises a central command server network having at least one communication application module so as to assist in the first intervention in the diagnosis, treatment and care of an injured patient. It comprises the communication of different systems in the hospital with each other for the benefit of the triage process, rather than how to evaluate the patients in the scope of the triage. In this system, triage is carried out in the pre-hospital period with the telemedicine method and the transfer of the patient to a suitable hospital is provided. The Chinese patent application numbered CN103632227A relates to a pre -triage paediatric emergency service system that contains a hospital information system, database system, pre- scanning station and call number and triage station.
The United States patent application numbered US20130030825 relates to a triage method that is automatically triggered by the wristbands to be given to patients in the emergency service and automatically receives data from the patient evaluation device.
The patients must have wristbands mentioned specifically in the method and the emergency service must have a patient evaluation device so as to apply the method.
The United States patent application numbered US20060047188 is a prioritization system so as to receive the patients in an emergency care center and comprises a system monitoring station. This system monitoring station comprises various sensors that automatically receive data from the patients.
Moreover, the internationally recognized study named “Canadian Triage System - CTAS” and “Manchester Triage System - MTS” are used as a guideline in the triage practice in many countries and also many countries create their own triage methods by revising similar studies regarding their own cultures. It is seen that the triage practices determined in Turkey vary based on the health institutions and the know-how of the triage practitioner, different triage practitioner determine different emergency levels in the same patient, the patients being presented to the emergency service are examined by ordering the same based on order of arrival in line with the triage colour table of the same. The most important difference between the determined triage methods and the method that we have developed is the provision of effective and secure emergency health service according to the risk point determined within the framework of measurable objective criteria, not the emergency level determined according to the knowledge between the triage practitioners and the arrival order of the patient to emergency.
Aim of the Invention
The triage decision support method regarding the triage application aims to arrange the triage order by providing support to the health personnel (doctor, nurse, emergency medical technician, and paramedic) who apply triage regarding the objective decision making. The inventive triage decision support method will create the recommendations regarding the final decision to be made by the practitioner so as to define the triage colour code suitable for the patient demanding help and ordering the patients included within the same colour code by taking the assessment criteria of the patients as the basis. Thus, the patients with more critical conditions will be protected from the subjectivity of human evaluations and will be treated without time loss.
Carrying out the triage application completely within the framework of measurable criteria and standards and performing an effective and standardized triage application, giving priority of examination to the patients who are presented to the emergency service by considering the urgency of patients, ranking among urgency levels, creating a triage decision support system that is a guide with a triage application that does not vary according to institutions and triage practitioners are the aims of the present invention.
Detailed Description of the Invention
The triage decision method performed so as to fulfil the aims of the invention is shown in the attached figures, wherein;

A novel triage method is formed with a novel triage decision support system operation algorithm (A) so as to calculate the patient order required for the triage with the protocols created for the complaints of the patients who are presented to the emergency service for help. This decision support method can make order among the patients with the same level of urgency.
In the inventive triage decision support system operation algorithm (A), as the first step, the triage personnel meets the patient who is presented to the emergency with a request for help (101) and makes an assessment of the general situation (102). The patient who requests help is subjected to the triage decision support method application (103). As a result of the assessment of the general situation, the patient who is defined (104) as a critical patient by the triage staff is transferred to the RED 1 colour code very urgently. Patients with the RED 1 colour code are immediately taken to the red area and are provided to have emergency health care (105). The situations mentioned as critical here are defined as the situations that threaten life and require immediate, aggressive approach and urgent simultaneous evaluation and treatment. As a result of the assessment of the general situation, data are continued to be collected in the triage decision support system for the patients who are identified as non- critical patients.
In the application of the inventive triage decision support method application, first of all the identity of the patient is received, the information regarding the current status (general appearance, isolation requirement, judicial case, oxygen use history and similar parameters) of the patient and complaints of the patients regarding their arrival to the emergency service are entered into the system. The critical questions included in the literature and matched with this complaint are shown to the triage staff with the entry of the complaint findings in the system. Thus, the information in terms of the patient’s complaint and information regarding this complaint that may pose a risk are obtained from the patient (106). The triage decision support method shows the decision about whether the patient’s status is critical or not by comparing the obtained data with the data previously obtained from the literature to the triage staff (107). The patient determined as a critical patient is defined as RED 1 by the triage staff and is immediately directed to the red area so as to receive emergency service (105). The non- critical patient is continued to be evaluated by the triage decision support method.
The operation algorithm (A) of the inventive triage decision support method continues with the measurement of vital findings of the patient (respiration rate, fever, pulse rate, blood pressure, oxygen saturation in blood and the like) and entering the same into the system (108). The vital data entered into the system are compared with the data previously obtained from the literature by the triage decision support method; it makes the decision of whether the situation of the patient is critical nor not (109) and shows this to the triage staff. The patient determined as a critical patient is defined as RED 1 by the triage staff and is immediately directed to the red area so as to receive emergency service (105).
If the operation algorithm (A) of the inventive triage decision support method does not define the patient as a critical patient, it processes the step of determining the colour code required for the triage and order of examination (110). All data and triage information regarding the patient are shown to the triage staff and the approval of the triage staff is taken (111). The data of the patient whose triage process is completed, Triage Colour Code and examination order are displayed on the doctor’s screen (112). In case the determined safe waiting period for the patients waiting for examination order (114) ends, the triage staffs follow the reassessment process of the relevant patient (115). The patient determined as a critical patient during this evaluation, is defined as RED 1 by the triage staff and is immediately directed to the red area so as to receive emergency service (105).
The following patient is invited for examination (113). The triage process and operation for the patient who is subjected to the examination is terminated (116).
In the application of the inventive triage decision support method, after the complaint about the patient who is presented to the emergency service for help and the information that may pose risk for this complaint are obtained from the patient (106), in the step of making decisions regarding whether the status of the patient is critical or not (107), different protocols will be applied for adult patients over 18 and children under 18. The risk factors of the complaint finding will be determined and prioritized based on the opinions of the experts during the creation of these protocols.
In the application of the inventive triage decision support method, after the values regarding the findings of the patient and the vital parameters are obtained from the patient (108), in the step of making decisions regarding whether the status of the patient is critical or not (109), different protocols will be applied for adult patients over 18 and children under 18. The evaluation and prioritization of the vital parameters will be realized based on the opinions received from the literature researches and experts in the field during the preparation phase of these protocols.
The parameters to be used for determining the patient order during the step (110) of determining the colour code required for triage and the order of examination in the operation algorithm (A) of the inventive triage decision support method will be decided depending on the scoring system made “National Early Warning Score - NEWS” for adults patients and depending on the scoring system named “Paediatric Early Warning Score PEWS” for the paediatric patients. It is planned to determine the level or urgency of the data input to the computer and carry out the patient order simultaneously by means of the triage decision support method arranged so as to give service to the patient demanding help and not to pose a health risk during this service provision.

Claims

1. A triage decision support method, wherein the required information so as to create the recommendations regarding the final decision to be made by the practitioner so as to define the triage colour code suitable for the patient demanding help and ordering the patients included within the same colour code are obtained by;
Meeting the patient who is presented to the emergency service for help (101); Assessing the general status of the patient (102);
Taking the ID information of the patient,
Obtaining information in terms of the patient’s complaint and information regarding this complaint that may pose a risk from the patient (106), characterized by comprising the following step;
Making a decision about whether the patient’s situation is critical or not by comparing the obtained data with the data previously obtained from the literature (107).
2. A triage decision support method according to claim 1; wherein it comprises the step of measuring the vital findings of the patient (respiration rate, fever, pulse rate, blood pressure, oxygen saturation in blood and the like) and entering the same into the system (108) and characterized by; comprising the step of making decision (109) regarding whether the status of the patient is critical or not by comparing this data with the data obtained previously from the literature.
3. A triage decision support method according to any of the preceding claims, characterized by comprising the step of determining (110) the required colour code and examination order for the triage by evaluating the data obtained from the previous claims.
4. A triage decision support method according to claim 3, characterized by comprising the step of re-applying (115) the triage process in case the determined safe waiting period for the patients waiting for examination order (114) ends.
5. A triage decision support system characterized in that; it has at least one microprocessor in which the triage decision support operation algorithm (A) can be run or operates through wired or wireless connection to the device where at least one microprocessor is operated.
