Abstract

The invention relates to a prosthesis which is used in orthopedic reverse shoulder operations, provides the patient to move his/her shoulder more effectively in his/her daily life by means of turning the shoulder inwardly by making a connection between the muscle that turns the shoulder inwardly (subscapularis) and the prosthesis. The invention is an orthopedic product which can be used for the reconstruction of the defects that occur due to any reason in the humerus proximal. The invention particularly relates to an orthopedic prosthesis to be used in the medical field in severe shoulder structure disorders such as insufficient shoulder rotator muscles (rotator cuff), calcification (arthritis) and complex shoulder fractures etc.

NOVELTY IN REVERSE SHOULDER PROSTHESES
Technical Field
The invention relates to a prosthesis which is used in orthopedic reverse shoulder operations, ensures the patient to move his/her shoulder more effectively in his/her daily life by means of turning the shoulder inwardly by making a connection between the muscle that turns the shoulder inwardly (subscapularis) and the prosthesis. The invention is an orthopedic product which can be used for the reconstruction of the defects that occur due to any reason in the humerus proximal.
The invention particularly relates to an orthopedic prosthesis to be used in the medical field in severe shoulder structure disorders such as insufficient shoulder rotator muscles (rotator cuff), calcification (arthritis) and complex shoulder fractures etc.
Prior Art
The reverse shoulder prosthesis is applied to the patients with irreparable rotator cuff (shoulder rotator muscles) tear and arthritis (calcification), complex fractures and painful movement restriction. The reverse shoulder prosthesis which is an important and favorable option in the surgical treatment of the shoulder problems has been used for more than 30 years.
In the state of the art, there is no structure that will enable to engage the muscle that turns the shoulder inwardly (subscapularis) to the prosthesis. This condition leads to vulnerability in turning the arm inwardly. It leads to the incapability of the patients in some activities such as reaching to the back pocket of the trousers, lacing the upper underwear by women.
In the application known in the art, in the Turkish Patent and Trademark Office document with registration number 2014-12306, it is mentioned that there is an incompatibility between the humeral insert and the lower part of the glenoid bone during adduction and thus a novel reverse shoulder prosthesis is developed in order to eliminate this incompatibility. Moreover, in another known state of the art, since the proximal humerus prosthesis is totally embedded in the bone, the suture holes in the proximal humeral component are not sufficient in sending suture materials.
Aim of the Invention
Novel reverse shoulder prosthesis is developed for solving the above mentioned and existing problems in the state of the art. The aim of this developed orthopedic product is to use the same for the reconstruction of the defects that occur due to any reason in the humerus proximal.
The invention includes the structure that will allow for mounting the rotator muscles (rotator cuff tendons) in the shoulder to the prosthesis by means of using suture materials. A more efficient functional result is obtained from the prosthesis by attaching the relevant muscle or muscles to the prosthesis again. The vulnerability that occurs during the rotation of the arm is eliminated by means of engaging the muscle that rotates the shoulder inwardly (subscapularis) to the prosthesis. Thus, the patients is capable of carrying out some activities such as reaching to the back pocket of the trousers, lacing the upper underwear of women.
Another aim of the invention is to use this prosthesis having multiple suture holes in proximal bone loss cases. Required amount of suture materials can be sent from the locations seem necessary by means of the extra suture holes mentioned in the invention.
Detailed Description of the Invention
In order to fulfill the aim of this invention, this product which is an orthopedic prosthesis and its components to be used in the reconstruction of the defects occurs as a result of the irreparable rotator cuff (shoulder rotator muscles) tear and arthritis (calcification), complex fractures and painful movement restriction, are shown in the attached figures.

Figure 6 - is the passage of the suture materials from the suture holes.

The present invention is related to a reverse shoulder prosthesis which consists of the following elements; metaglene (2) comprising lock screw (1), glenoid sphere (3), humeral insert (4), proximal humeral component (5), humeral stem (6). In Figure 1 , side section views of the elements of the invention are given in disassembled form. In Figure 2, side section views of the elements of the invention are given in assembled form.
Metaglene (2) which has different size options compatible with glenosphere is an element which comprises a centering stem, screw hole (tapped hole) and channel shaft to make the initial fixation easy. Metaglene (2) enables the prosthesis to be placed by means of centering the same to the glenoid bone. Lock screw (1) is used for maximum fixation and stability. Moreover, there are 2 types of metaglene and these can be implanted without cement.
The size of the glenoid sphere (3) is determined according to the size of metaglene (2) used to fix the same to the glenoid bone. The selected size glenoid sphere (3) is located on the metaglene (2). The notch located on the humeral insert (4) is positioned such that it is in line with the marking line located on the back part of the proximal humeral component (5) and the humeral insert (4) is located manually to the proximal humeral component (5) freely. After positioning, the humeral insert (4) is nailed on the humeral component (5) by means of a driver. Then, when the patient is in a comfortable position, the humeral insert (4) is placed on the glenoid sphere (3). While the bone cement continues to be cured, the shoulder is slowly examined and the determined motion range and joint stability are controlled. In case the humeral insert (4) is separated from the glenoid sphere (3) while the arm is pulled in the distal humerus direction, it shows that the deltoid tension is insufficient. After all controls are performed, the joint gaps are washed and the residues are cleaned.
In the proximal humeral component (5) as the main element of the present invention, the peripheral suture holes (5.1) are found as located perpendicular to the proximal front surface different from the suture holes that are embedded to the bone (Figures 3, 4, 5). The rotator muscles in the shoulder are mounted to the prosthesis through suture holes (5.1) by using suture materials (Figure 6). Moreover, there are peripheral suture holes (5.1) opened in a parallel manner to the proximal side surfaces and front surface and equal number of peripheral groove (5.2) in order to pass the suture materials easily. The peripheral suture hole (5.1) openings and channel peripheries (5.3) are rounded in order not to give damage to the suture materials (Figure 5). Required amount of suture materials can be sent from the locations seem necessary by means of said peripheral suture holes (5.1). (Figure 6)

Claims

1. Prosthesis used in the orthopedic reverse shoulder operations, characterized in that;
it comprises a proximal humeral component (5) which has at least one peripheral suture hole (5.1) and at least one peripheral groove (5.2).
2. Proximal humeral component (5) according to claim 1 , characterized in that; the peripheral suture holes (5.1) are located perpendicular to the proximal front surface.
3. Proximal humeral component (5) according to claim 1 , characterized in that; the peripheral grooves (5.2) are opened parallel to the proximal side surfaces and front surface.
4. Proximal humeral component (5) according to claim 1 , characterized in that; the peripheral suture hole (5.1) openings and channel peripheries (5.3) are rounded.
