The support device 1 uses a specific model as described above. The specific model identifies the coping method of nursing for the patient according to the condition of the patient and its change. Hereinafter, the coping method for the patient specified by the specific model may be referred to as a first coping method.
An example of the coping method specified by the specific model is shown below. The coping method specified by the specific model may be a part of the above, or may include a coping method other than the above. The particular model may identify how to treat patients with other conditions.
The specific model may be recorded in advance in the support device 1 or may be generated by another device that is communicatively connected to the support device 1. The support device 1 may also generate a specific model. As an example of this case, the support apparatus 1 deals with the patient's nursing care based on the nursing information acquired from the database of the electronic medical record system 31 and the correct answer label of the coping method information for the change in the condition indicated by the nursing information. Generate a specific model used to identify the method. In either case, the specific model is generated in advance.
In addition, in the support device 1, a condition estimation model may be used. The condition estimation model estimates changes in the patient's condition. The patient's condition estimated by the condition estimation model includes whether or not the patient's condition has changed, and the degree to which the condition has changed. The estimated patient condition is not limited to these. The condition estimation model may be recorded in advance in the support device 1 or may be generated by another device that is communicatively connected to the support device 1. In addition, the support device 1 may generate a condition estimation model. As an example of this case, the support apparatus 1 estimates the condition based on the nursing information acquired from the database of the electronic medical chart system 31 and the correct answer label of the information indicating that the condition of the patient has changed in the nursing information. A model may be generated. Here, the nursing information refers to information that the medical staff should understand in nursing a patient, such as the patient's condition, the content of the patient's care, and changes in the patient's condition in the nursing work for the patient. The change in the patient's condition may be varied, such as the patient's condition getting worse, the patient's condition improving, the patient's food amount increasing, the patient's food amount decreasing, and the like. The condition estimation model may be a model that can specify the degree of change in the patient's condition divided into a plurality of parts. In any case, the condition estimation model is generated in advance.
The specific model may be generated using the nursing information stored in the database of the electronic medical record system 31 and the information stored in the database of the resource management system 32. As a result, the specific model can specify the coping method according to the nursing information of the patient and the situation of the resource for nursing. The resource for nursing refers to a human resource such as working hours of a medical staff who cares for a patient and a facility resource such as medical equipment near the patient.
The above-mentioned condition estimation model and specific model of coping method may be generated for each nursing type of the patient or each attribute of the patient. For example, the nursing type is an identifier that identifies a mode of nursing such as bone fracture and stroke. The patient attribute is an identifier that identifies a patient category such as age and sex. The support device 1 records each model in the database 104. The condition estimation model and the specific model of the coping method include parameters such as the weight of the neural network for calculating the output with respect to the input. The condition estimation model and the specific model of the coping method may be models other than the neural network.
FIG. 5 is a first diagram showing a processing flow of the support device 1.
Next, the details of the processing of the support device 1 will be described with reference to FIG.
In the following process flow, it is assumed that the support device 1 includes the estimation unit 14. However, as described above, the support device 1 may not include the estimation unit 14.
The database 104 of the support apparatus 1 acquires and stores a patient ID and nursing information from the electronic medical chart system 31 in advance. As information recorded in the database of the electronic medical record system 31, nursing information is added or updated at any time. Therefore, the acquisition unit 12 of the support apparatus 1 communicates with the electronic medical record system 31 at a predetermined time interval, acquires the patient ID and nursing information associated with the patient ID from the database of the electronic medical record system 31, and acquires the database. Record at 104.
患者の身体的容態を示す情報は、体温、血圧、血液検査結果の各項目、“Subjective(主観的情報)”、“Objective(客観的情報)”、“Assessment(評価情報)”などの情報であってもよい。 Then, the estimation unit 14 identifies the patient ID of the first patient from the database 104 (step S101). The estimation unit 14 acquires the nursing information recorded in the database 104 in association with the identified patient ID. A plurality of pieces of nursing information associated with the patient ID may exist in time series. The estimation unit 14 acquires information indicating the physical condition of the patient from the nursing information (step S102). Information indicating the physical condition of the patient includes information such as body temperature, blood pressure, each item of blood test results, “Subjective (subjective information)”, “Objective (objective information)”, and “Assessment (evaluation information)”. It may be.
The estimation unit 14 acquires a condition estimation model (step S103). The estimation unit 14 may identify the nursing type or attribute for the patient indicated by the patient ID, and acquire the condition estimation model from the database 104 according to the nursing type or attribute. The estimation unit 14 inputs information indicating the physical condition of the patient a predetermined time before the current time and information indicating the physical condition at the current time into the condition estimation model, and determines whether the patient's condition has changed. Estimate (step S104). As a result, the estimation unit 14 acquires information on whether or not the estimated condition has changed. The estimation unit 14 compares the information indicating the past physical condition with the information indicating the physical condition at the current time without using the condition estimation model, and based on the difference between the information indicating the physical condition of the patient, The presence or absence of change may be estimated. For example, the estimation unit 14 may estimate that the condition has changed when the difference in body temperature included in each of the acquired pieces of information indicating the physical condition is equal to or more than a predetermined threshold value. Further, for example, the estimation unit 14 estimates that the condition has changed from the acquired information indicating the physical condition when the component value of a certain item of blood identified based on the nursing attribute of the patient is equal to or greater than a predetermined threshold value. You may. Specific examples of changes in the patient's condition may include deterioration of the patient's condition, improvement of the patient's condition, increase of the patient's dietary amount, decrease of the patient's dietary amount. In the above example, the estimation unit 14 determines whether the patient's condition has changed. However, the present invention is not limited to such an example, and the input of information indicating that the condition of the patient has changed may be received from the medical staff, the patient himself, or the like.
When the estimation unit 14 estimates that there is a change in the patient's condition, the coping method specifying unit 15 specifies the first coping method (step S105). If the estimating unit 14 is not provided, the coping method specifying unit 15 may specify the first coping method in response to a request for specifying a coping method from the medical staff via the terminal 2. Good. As an example of identifying the coping method, the coping method identifying unit 15 identifies a nursing type or attribute for the patient indicated by the patient ID, and acquires a specific model of the handling method from the database 104 according to the nursing type or attribute. The coping method identifying unit 15 inputs information indicating the physical condition of the patient a predetermined time before the current time and information indicating the physical condition at the current time into the coping model, and the first of the results is input. Information on a coping method is acquired (step S106). The first coping method may be output as information such as identification information such as an ID that identifies the first coping method. When the coping method specifying unit 15 acquires the identification information that specifies the first coping method, the coping method specifying unit 15 outputs the patient ID of the processing target and the specified first coping method to the UI management unit 18.
The UI management unit 18 acquires the patient ID and the first coping method, and generates presentation information of the coping method including them (step S107). This presentation information includes the content information of the coping method showing the first coping method in detail to the medical staff. The presentation information includes a presentation ID for identifying the presentation information. The content information corresponding to the first coping method may be stored in the database 104 of the support device 1 in advance. The UI management unit 18 acquires the medical staff ID recorded in the database 104 in association with the patient ID. Further, the UI management unit 18 acquires the terminal ID recorded in the resource management system 32 in association with the medical staff ID. This terminal ID is the terminal 2 used by the medical staff indicated by the medical staff ID specified based on the patient ID. The UI management unit 18 acquires an ID indicating the ward and the number of floors based on the patient ID, and based on the ID, based on the work schedule of the resource management system 32, the ID of the medical staff working at the current time. Identify. The UI management unit 18 may acquire the terminal ID associated with the ID of the caregiver from the resource management system 32. The UI management unit 18 transmits the presentation information to the terminal 2 having the specified terminal ID (step S108). The UI management unit 18 may temporarily record the presentation information in a memory or a storage unit.
The terminal 2 that has received the presentation information by the above processing outputs the presentation information indicating at least the patient ID included in the presentation information and the coping method for the change in the patient's condition. Note that the presentation information may be output to another output destination device. Thereby, the medical staff can deal with the patient whose condition has changed based on the information displayed on the terminal 2 by the appropriate coping method.
In the above processing, the electronic chart system 31 may automatically record the patient's vital measurement value acquired by a sensor or the like as nursing information. Vital measurements include, but are not limited to, patient temperature, blood pressure, electrocardiogram data, and the like. Each time the vital measurement value is registered as nursing information, the sensor or electronic medical chart system 31 notifies the support apparatus 1 that the nursing information is registered, and the estimation unit 14 of the support apparatus 1 changes the patient's condition. You may judge whether it did. The vital measurement value of the patient is automatically recorded as nursing information, and based on this record, when the support device 1 determines that the condition has changed based on the change in the condition of the patient, the presentation information of the coping method is generated. By transmitting to the terminal 2, it is possible to take prompt and appropriate measures for the patient.
In the above-mentioned processing, the estimation unit 14 stores the human resources and facility resources available for nursing the patient in the database of the resource management system 32 based on the ward ID, the room ID, the bed ID, etc. specified based on the patient ID. This resource may be input to the specific model along with information indicating the physical condition of the patient. In this case, the estimation unit 14 stores information regarding resources as the input information included in the model input / output information described above. Then, the coping method specifying unit 15 specifies the first coping method for the patient based on the change in the physical condition of the patient and the human resource or facility resource. As a result, the support apparatus 1 can identify an appropriate coping method for a patient based on the human resources of the medical staff identified based on the work schedule at the current time and the facility resources available for nursing the patient. it can. For example, when the doctor in charge of the patient is absent, there are coping methods that can and cannot be carried out among coping methods for the patient. In addition, depending on the available nursing facilities, there are treatments that can be performed and treatments that cannot be performed among the treatment methods for patients. The support device 1 can specify an appropriate coping method in consideration of such human resources and facility resources.
FIG. 6 is a second diagram showing a processing flow of the support device 1.
The medical staff refers to the presentation information displayed on the terminal 2 to take care of the patient. In this case, the first coping method included in the presentation information may be executed. Also, when the medical staff determines that the first coping method is not appropriate or there is a more appropriate coping method, a coping method different from the first coping method may be executed. . Unlike the first coping method, the coping method performed on the patient may be hereinafter referred to as a second coping method.
Then, the medical staff inputs the content of the treatment to the terminal 2 during or after the treatment for the patient. When the first coping method is performed, the first coping method included in the presentation information is input as the coping content. Further, when the second coping method different from the coping method of the first coping method is performed, the detailed content thereof is input as the coping content. A user such as a medical staff can input the information of the handling contents into the terminal 2 by voice input or character input. The terminal 2 transmits the coping result including the presentation ID, the patient ID, and the coping contents to the support device 1.
Assistance device 1 receives the response result. The acquisition unit 12 of the support device 1 acquires a coping result (step S201). The acquisition unit 12 outputs the coping result to the coping result processing unit 16. The coping result processing unit 16 acquires, from the UI management unit 18, the presentation information transmitted in the past to the terminal 2 that has transmitted the coping result (step S202). The handling result processing unit 16 determines whether the handling method included in the presentation information and the handling content indicated by the handling result match (step S203).
When the coping method included in the presentation information and the coping content indicated by the coping result match (Yes in step S203), the first coping method has been executed. The coping result processing unit 16 records in the database 104 the first coping result including at least the information indicating that the first coping method has been performed, the presentation ID, and the patient ID (step S204). The first coping result is a coping result recorded when the first coping method is performed.
When the coping method included in the presentation information and the coping content indicated by the coping result do not match (No in step S203), the second coping method is executed instead of the first coping method included in the presentation information. It has been done. The coping result processing unit 16 includes information indicating that the second coping method is performed, coping details of the second coping method executed instead of the first coping method, a presentation ID, and a patient ID. The second coping result including at least is recorded in the database 104 (step S205). The second coping result is a coping result that is recorded when the second coping result is performed instead of the first coping method.
Then, the support device 1 receives from the terminal 2 the evaluation information of nursing according to the coping method presented to the user by the presentation information. For example, the medical staff registers the evaluation information in the terminal 2 together with the input of the handling content, or one hour after the handling of the handling method is performed. The evaluation information mainly indicates the quality of the first coping method. The evaluation information may be, for example, detailed contents regarding evaluation of a coping method represented by a numerical value, evaluation of a state of a patient after one hour, and the like, but is not limited thereto. The evaluation information may be any information indicating whether the first coping method is good or bad.
The evaluation information includes the presentation ID designated by the user and acquired by the terminal 2. The user may input the evaluation information into the terminal 2 by voice input or operation input. The support device 1 acquires the evaluation information (step S206). The acquisition unit 12 of the support device 1 outputs the evaluation information to the handling evaluation unit 17.
The coping evaluation unit 17 refers to the evaluation information and determines whether or not the coping method is appropriate (step S207). As an example, a case will be described in which the evaluation information is represented by a numerical value having a larger value when the coping method is appropriate. In this case, the handling evaluation unit 17 determines whether the evaluation included in the evaluation information is less than the threshold value. In this case, when the evaluation is less than the threshold value, the coping evaluation unit 17 determines to transmit a specific model update determination request for the coping method. The handling evaluation unit 17 outputs an update determination request for the specific model including the presentation ID included in the evaluation information to the model updating unit 13. When the evaluation included in the evaluation information is equal to or more than the threshold, the handling evaluation unit 17 determines that the presented processing is appropriate and ends the processing.
The model update unit 13 acquires an update determination request. When the coping method included in the acquired coping result includes information indicating that the second coping method is performed, the coping method specified by the specific model is not an appropriate coping method for the patient, or It is inferred that the coping method of may be more appropriate. Therefore, the model updating unit 13 advances the model updating process. In this case, the model update unit 13 inputs and acquires the input information input to the acquired model and the output information (information indicating the physical condition of the patient, resource information, etc.) output from the acquired model. In addition to the relationship with the second coping method included in the coping result, when the same information as the input information is input to the particular model, it is specified so that the same information as the output information is output The model is updated (step S208). That is, the model updating unit 13 updates the specific model by relearning the second coping method to the specific model. The model updating unit 13 updates the specific model by re-learning that the evaluation of the first coping method is low together with or instead of re-learning of the second coping method. Good.
The process of the model updating unit 13 is one mode of the process of updating the specific model when the second coping method performed in place of the first coping method is evaluated to be more appropriate. The support device 1 determines whether to end the process (step S209). The support apparatus 1 repeats the above-described steps S201 to S209 each time the coping result is acquired.
That is, when the second coping method is more appropriate than the first coping method, re-learning regarding the second coping method is performed for the specific model. As a result, it is possible to improve the accuracy of the specific model that specifies the coping method.
In the processing described above, the coping evaluation unit 17 determines whether the evaluation of the second coping method performed in place of the first coping method is less than the threshold value based on the evaluation information acquired from the user's terminal 2. is doing. However, the coping evaluation unit 17 replaces the first coping method with the second coping method, which is performed based on the nursing information of the patient registered by the medical staff after the medical staff has dealt with the patient. You may judge whether the evaluation of is more than a threshold value.
For example, the coping evaluation unit 17 of the support device 1 detects the elapse of a predetermined time after recording the first coping result in step S204 and recording the second coping result in step S205, and starts the coping evaluation process. To do. The coping evaluation unit 17 acquires the presentation ID and the patient ID included in the first coping result and the second coping result. The coping evaluation unit 17 acquires the nursing information including the presentation ID and the patient ID from the nursing information recorded in the database 104. The coping evaluation unit 17 also acquires, from the database 104, the nursing information of the patient having the patient ID registered for a predetermined time before the registration time of the nursing information including the presentation ID and the patient ID. In addition, in the registration of nursing information to the electronic medical chart system, the medical staff uses the electronic medical chart system 31 as the nursing information including information such as the condition of the patient and the presentation ID and the patient ID after executing the coping method indicated by the presentation information. It is assumed that you have registered. The nursing information registered in the electronic medical chart system 31 by the processing of the support device 1 is copied to the database 104.
The coping evaluation unit 17 of the support device 1 calculates an evaluation based on the acquired nursing information. Specifically, the coping evaluation unit 17 acquires predetermined information from the information on the physical condition of the patient included in the acquired nursing information. The predetermined information may be, for example, a body temperature or a consciousness level value. The coping evaluation unit 17 calculates the evaluation of the coping method indicated by the presentation information based on the increase or decrease in the value of the body temperature or the consciousness level included in each acquired time-series nursing information. For example, the coping evaluation unit 17 calculates a low evaluation when the body temperature is rising and the consciousness level is decreasing. Further, the coping evaluation unit 17 calculates a high evaluation when the body temperature is lowered and the consciousness level is increased. This evaluation calculation method is an example, and any calculation method may be used. Then, the support device 1 similarly performs the processing from step S207 onward to update the specific model.
Although the embodiment of the present invention has been described above, according to the above-described processing, the support device 1 can output information for performing appropriate coping in accordance with a change in the condition of the patient in the nursing work. .
