Abstract
MPDI provides a simple solution for managing crestal bone loss, at any stage, from just after osteintegration to long term implant use. This design is totally capable in managing crestal bone loss, increasing life of failing implant and enhancing esthetics, either used alone or with combination of bone augmentation procedures.
Description
MULTIPLE PLATFORM DENTAL IMPLANT (MPDI)
BACKGROUND
Technical Field of Invention
[001] The embodiments herein generally relate to a medical device and particularly relate to a dental implant having multiple platforms.
Description of Related Art
[002] The most important criterion for success of dental implant is the presence of good amount and quality of bone around the implants, especially the crestal bone.
[003] Once crestal bone loss starts, it can lead to increased bacterial accumulation, resulting in secondary peri-implantitis which further leads to bone loss and ultimately resulting in implant failure.
[004] Therefore, crestal bone preservation and augmentation should be an important part of treatment planning. Various approaches are in practice like guided bone regeneration using many types of bone grafts, membranes, PRF etc.
[005] But all these methods have their own limitations and results may or may not be as desired. So, keeping in mind that the crestal bone loss can happen even after using above stated methods, what would be its solution??
[006] Introducing a new type of dental implant which can provide the solution for bone loss, immediately after healing period or after failure of bone augmentation procedures or even after many years of implant supported prosthesis used by patient.
[007] The above-mentioned shortcomings, disadvantages and problems are addressed herein, as detailed below.
SUMMARY OF THE INVENTION
[008] The various embodiments herein disclose Multiple Platform Dental Implant (MPDI) is a modification of conventional endosteal dental implant, made of titanium alloy (same as conventional implant), to be surgically placed in maxilla or mandible, serves to support and retain artificial teeth or prosthesis. The MPDI comprises of a long cylindrical or tapered implant body of various diameter (3, 3.5, 4.2, 5 mm) and length (8,10,11.5, l3mm) having external threads (for mechanical retention and to increase the surface area for osteointegration). Apical end of this cylinder is closed and its crestal end having an internal threaded bore to receive the screw to tighten detachable part of MPDI or abutment. This internal bore opens at crestal end of implant body with a flare at top 2-3 mm, making a shape of hexagon or triangle in cross section. This part of implant body is called lst platform of implant.
[009] According to one embodiment herein, the MPDI further comprises small (2,3mm) cylinder of titanium alloy having same pattern of external threads like implant body and having internal bore throughout its length with internal threads. One end of this cylinder has features similar to lst platform and referred as 2nd platform, having exactly same design and dimensions as of lst platform (This feature differentiates MPDI from conventional dental implant which typically have only one platform). The other end of this small cylinder has non threaded polished hexagonal or triangular projection of same dimensions as of lst and 2nd platform or we can say that this projection is just a negative replica of implant platform, which exactly fits into the platform and secured with an internal screw that passes through internal bore of both parts of implant to tie them together and make them a single unit for implant placement.
[0010] According to one embodiment herein, the MPDI further comprises an internal screw, means a fixation screw having a long threaded cylindrical portion engagaable with threads of internal bore of both the main implant body and small detachable part. Screw tightens to secure both these parts during implant placement so that the small part does not separate during torque. Once the desired torque is achieved, this screw is removed and cover screw is placed.
[0011] According to one embodiment herein, the MPDI further comprises a cover screw having threaded portion receivable in internal bore, attached to a curved disc used to cover the cylindrical cavity and platform of implant during osteointegration period so that bone and soft tissue does not grow inside the platform and internal bore.
[0012] According to one embodiment herein, Abutment used in MPDI is similar to conventional abutments that can be fitted on lst or 2nd platform and tightened with a abutment screw.
[0013] According to one embodiment herein, the MPDI comprises an abutment screw which passes through abutment, internal bore of detachable part and implant body to tie them together during prosthetic phase. Whenever small detachable part of MPDI is removed to manage crestal bone loss, a new abutment screw is needed which is shorter in length because it has to tighten only two parts i.e. abutment and implant body.
[0014] These and other aspects of the embodiments herein will be better appreciated and understood when considered in conjunction with the following description and the accompanied drawings. It should be understood, however, that the following descriptions, while indicating preferred embodiments and numerous specific details thereof, are given by way of illustration and not of limitation. Many changes and modifications may be made within the scope of the embodiments herein without departing from the spirit thereof, and the embodiments herein include all such modifications.
BRIEF DESCRIPTION OF THE DRAWINGS
[0015] The other objects, features and advantages will occur to those skilled in the art from the following description of the preferred embodiment and the accompanied drawings in which:
[0016] FIG. 1 illustrates a multiple platform dental implant in comparison to conventional dental implants, according to one embodiment herein.
[0017] FIG. 2 illustrates a flow diagram of the MPDI for managing bone loss, according to one embodiment herein.
[0018] FIG. 3 illustrates a flow diagram of managing bone loss through conventional dental implants, according to one embodiment herein.
DETAILED DESCRIPTION OF THE DRAWINGS
[0019] In the following detailed description, a reference is made to the accompanied drawings that form a part hereof, and in which the specific embodiments that may be practiced is shown by way of illustration. The embodiments are described in sufficient detail to enable those skilled in the art to practice the embodiments and it is to be understood that the logical, mechanical and other changes may be made without departing from the scope of the embodiments. The following detailed description is therefore not to be taken in a limiting sense.
[0020] With respect to FIG. 2 and 3, the MPDI can be splitted into 2 or more parts, a longer apical part and a short detachable crestal part . Each of these parts have their separate platforms, where abutment can be fitted.
[0021] Example-
[0022] Suppose an immediate extraction and implant placement is done at upper central incisor region using Multiple platform Dental Implant of size l3mm (lOmm apical part +3mm crestal part). But after osteo- integration period of 4 months the crestal bone loss of 3 to 4 mm is noticed which may lead to implant failure. It may have esthetic complications if implant is exposed in oral cavity.
[0023] Because MPDI is used in this case, crestal 3 mm part of this implant can be detached and discarded, exposing its 2nd platform and leaving behind apical lOmm part of implant which is osteointegrated and fully surrounded by healthy bone.
[0024] Now 2nd platform serves for loading the prosthesis and good esthetics can be achieved. Here the crown length will be 3 mm more than the previous one. So crown to implant ratio should be considered during treatment planning, for choosing proper combination of length of both parts of MPDI.
[0025] These implants can be manufactured in many combinations in terms of length and diameter.
[0026] Example:
10+2 mm
10+3 mm
11+2 mm
11+3 mm
12+2 mm etc. For all diameters
[0027] More than 2 platforms can also be used i.e implant will have more than 2 parts .Example 10+2+2 mm.
[0028] Most important criterion for selection MPDI is the length of remaining apical part. It should be such that the minimum requirement of crown root ratio is fulfilled, even after removal of small crestal part.
[0029] It is to be understood that the phraseology or terminology employed herein is for the purpose of description and not of limitation. Therefore, while the embodiments herein have been described in terms of preferred embodiments, those skilled in the art will recognize that the embodiments herein can be practiced with modification within the spirit and scope of the claims.
