      前記健康関連項目グループを参照して、一又は複数の健康関連データベースから、前記健康関連項目グループに属する健康関連項目に対応する健康関連データを抽出する抽出部と、
      前記抽出された健康関連データに対応する個人特定情報に対してサブIDを決定する決定部と、
      前記個人特定情報と前記サブIDとの対応関係を示した対応情報を作成して前記第1の記憶部に記憶させる作成部と、
      前記抽出された健康関連データ及び前記決定されたサブIDを送信する送信部と、
     を備え、
     前記集積装置は、
     前記統一項目名に対応した統一データベースを記憶した第2の記憶部と、
     前記送信された健康関連データ及びサブIDを受信する受信部と、
     前記受信した健康関連データを集積し、前記統一項目名と関連付けて前記統一データベース上に反映させると共に、前記受信した健康関連データと関連付けて、前記サブIDを前記統一データベースに付加する集積部と、

    A creation unit that creates correspondence information indicating a correspondence relationship between the personal identification information and the sub ID and stores the correspondence information in the first storage unit;
    A transmission unit for transmitting the extracted health-related data and the determined sub-ID;
