Abstract

Prepackaged wipes are becoming the standard of bathing patients, especially the bed ridden. The comprehensive skin care, especially in the ICU, deals with cleaning the skin, stopping infections and preventing skin lesions due to incontinence. For that, the wipes should include the following four elements: 2% Chlorhexidine, no-rinse surfactant, moisturizer and skin protectant (3% Dimethicone). All in a balance solution of PH = 5.5: (Safetex Plus - 4 in 1).

Description
The long-held tradition of using a basin, soap and water to bathe bed-bound hospitalized patients is no longer the recommended standard of practice, according to the American Association of Critical-Care Nurses (AACN) updates on 8- April- 2013.
Bathing a patient deals with the skin cleaning. However, the objective is not only cleaning the skin but we are after the skin integrity, also. On the contrary, cleaning the skin in the traditional way will cause further problems to the skin: skin dryness, skin irritation, loss of hydration, easy tear, loss of acid mantle against microorganisms, change of the normal flora ...etc. Moreover, it is time consuming and not standardized, as it differs from a caregiver to another.
The center of disease control recommended bathing with Chlorhexidine gluconate (CHG) primarily in critical care to prevent central line-associated bloodstream infections and infections due to multidrug-resistant organisms.
The prepackaged wipes with chlorhexidine helped decrease the infections in hospitalized patients. However, it increased irritant dermatitis and skin dryness.
On the other hand, the skin integrity problem was still there. The skin-irritating materials, namely the stools and urine in incontinent patients, cause incontinence associated dermatitis (IAD). The recommendations by the Wound Ostomy and Continence Nurses Society™ (WOCN) about IAD in the publication of the Best Practice for Clinicians, on Oct, 14th, 2011, said: (-use skin care products that cleans, moisturize and protect). The third element in the recommendation was to protect. This is achieved by the Dimethicone 3%, which leaves a protective layer of silicone oil on the skin.
The Safetex Plus (4 in 1) comprehensive skin care is extended over the whole day, making it ideal to be used for the patients in the ICU . It helps to reduce skin infections, IAD and pressure ulcers. It provides the high level of care deserved for the patients and saves the time and efforts of the caring nurses.

A package of 10 non-woven, antiseptic bathing wipes of 30*32 cm in size, immersed in a balanced solution of PH = 5.5, alcohol-free, fragrance-free, containing a unique mixture of a no-rinse cleanser of surfactant (PEG-7-Olivate), glycerin, olive extracts, Vitamin E, 3% Dimethicone and 2% chlorhexidine. (Net weight 320 gm).

Claims
The unique combination of the solution in Safetex Plus (4 in 1) provides a comprehensive care for the skin. It is made of alcohol-free, fragrance-free, balanced acidity solution that maintains the acid mantle of the skin (PH=5.5).
Protection (Patency)
I applied to WIPO for patency rights and protection of the Safetex Plus (4 in 1) in regard to the combination of 2% Chlorhexidine with the 3% Dimethicone in the balanced solution of the wipes.
As per this combination, the Safetex Plus (4 in 1) is unique in the world.
The ingredients of the solution:
1- Surfactant (PEG-7- olivate) as a no-rinse skin cleanser.
2- Emollient-based moisturizing ingredients of glycerin, olive extracts and vitamin E, to maintain the skin natural barrier.
3- Insulating, protective material of silicone oil (Dimethicone 3%), to isolate irritant materials away from the skin (esp. urine, stools or over moisture). This helps to prevent incontinence associated dermatitis (IAD), a well-known risk factor in development of pressure ulcers in incontinent hospitalized adults and nursing home residents. It can be used in children older than 2 months for diaper care to prevent napkin dermatitis. Also, it is easier to use in bariatric patients.
4- Antiseptic material of 2% Chlorhexidine, provides additional protection against skin infections, cross contamination and to decreases chances of central line associated blood stream infections (CLABSI).
