Description

TARGETED LIGHT SYSTEM AND RELATED DEVICES AND METHODS
CROSS-REFERENCE TO RELATED APPLICATIONS
[0001] The present application claims the benefit of co-pending United States Provisional Patent Application Serial No. 61/920,109, filed December 23, 2013, which application is incorporated herein by reference in its entirety.
BACKGROUND
[0002] There are many circumstances where it is helpful to assure a clean or sterile light source that does not interfere with the airborne environment in a room. An obvious example is in hospitals, and especially in operating rooms of hospitals. In the operating room setting, it is the wound site of the patient where sterility is of most importance. Unfortunately, in current approaches, the location of the light sources within the room are not coherently coordinated with the air filtration elements of the room.
[0003] Thus, there has gone unmet a need for improved methods of providing light sources, including highly flexible light sources configured to selectively provide all of variable directionality of light, variable intensity of light and variable wavelengths of light all without physically touching or moving the physical light source and without interfering with either the air filtration units for the room nor with the air flow at or near the target site such as a surgical site.
[0004] The present systems and methods, etc., provide one or more of these and/or other advantages. SUMMARY
[0005] The present systems, methods, devices, etc., relate to highly flexible targeted light systems that provide emitted light sources configured to selectively provide all of variable directionality of light, variable intensity of light and variable wavelengths of light all without moving the physical light source and without interfering with either the air filtration units for the room nor with the air flow at or near the target site such as a surgical site. [0006] The targeted light systems comprise an array of individually addressable light source elements such as LEDs, and a light controller configured to individually address each of the light source elements in the array, wherein the targeted light system emits light focused on a target site and wherein the targeted light system is configured to alter the direction of emitted light exiting the targeted light system and onto the target site without moving the targeted light system and wherein the light source elements of the array are individually selectively manipulated such as selectively chosen, by the controller to emit a desired light color frequency. The array can have any suitable geometry such as a simple concave, double concave or convex shape array of individually addressable light source elements. [00028] The targeted light systems herein can stand-alone or mount substantially flush into the ceiling or wall of a room such as an enclosure or surgical suite, ICU, exam room, dental exam, patient room or work table surrounding two or more sides of a clean air zone. The clean air zone may be formed by an air filtration system and in a manner as shown, for example in US Patent Publication No. 2012/0031271 Al, or in co-owned pending Patent Cooperation Treaty patent application no. PCT/US2014/062450.
[00029] The targeted light systems may surround two or more sides of the air filtration system for a room with high intensity light from the targeted light system 14 directed towards an intended target area within the clean air zone. In one embodiment, there can be an even number of targeted light systems above the target work surface (such as an operating table), the targeted light systems split between two or more opposing sides. There can also be targeted light systems in the ceiling directed at either or both the head or foot of a patient lying on the operating table. These targeted light systems can be installed permanently or can be temporarily mounted in the ceiling or upper wall directed at any prescribed target site (such as a surgical site) to intensify the brightness of the work area for safe, conditioned work light; or when all lights are turned on, of the entire work surface within the combined light area. By means of multiple light sources from different overhead angles, the shadow occultation effect of primary worker, doctor, assisting personnel or equipment may be minimized. These targeted light systems may be utilized independently or integrated with centrally mounted air filtration systems of the type, for example, described in the above-mentioned published application, as an integrated single system controlled by a single hand held and/or wall mounted controller.
[00030] Turning to the figures, Fig. 1 depicts an exemplary embodiment of the targeted light system 14 discussed herein. In the embodiment shown, such systems include a multiplicity of light source elements 15 comprising one or more individual light sources 16 such as LEDs 3. There are typically at least 30, 50, 90 or more light source elements 15 arranged in an array such as the single or double-concave compound curved array 1. Other shapes for the array are also possible such as flat or convex. The array 1 is configured to direct light emitted from the multiple light source elements 15 to form a "Focused Clear Light Zone" (FCLZ) at a target site such as a surgical site.
[00031] By selectively turning on or off individual light source elements 15, the concave curvature can selectively direct or "steer" the output light (emitted light) to different areas of the target site without mechanically moving the light box (housing) 4 of the targeted light system 14. The individual light sources 16 can be mounted in any desired shape, such as a circular, oval, square or rectangular array and can be made of any desired material such as metal, plastic or FR4 printed circuit board material 17. The correct polarity contact (36 in Fig. 2) for the light source elements 15 contacts circuit board traces 18 that are embossed upon the array 1. The circuit board traces 18 have conductors that lead to surface mounted LED drivers and further to the edge of the board where an edge connector can attach a cable bundle of wires that lead to the light controller 19. [00032] A gross adjustment for aiming the direction for the housing 4 of the may be set at installation using a hingepin 2 and spring detent 5 or other mechanism to account for different ceiling heights or target distance. Inserted in the multiplicity of holes in the array 1, the multiple light source elements 16 can be implemented in a regular array or otherwise as desired. The light sources can comprise super bright light emitting diodes (LEDs) 3 or any other suitable light source. LEDs 3 as the light sources for the light source elements 15 can be advantageous in some situations due to a high light output per unit of electrical power consumption coupled with a low infrared and/or ultraviolet emission and low radiant heat transmission; the low radiant heat reduces thermal gradients in nearby airflows.
[00033] The targeted light system 14 may provide bright white light or any other color(s), or alternately, may be any combination of white or color LED array to tailor the combined light spectrum emitted from the targeted light system 14. This allows for selecting the light spectrum for the most "warm" natural light desired. This light color frequency shifting also has advantages such as in targeted fluorescence surgery to emphasize certain tissues and vascular structures for the visual aid of the surgeon or worker. Additionally, the color of the light can be manipulated to provide light to disinfect the direct surgical surface or work area for a programmed period of time.
[00034] The entire front of the targeted light system 14 may be covered with a gasketed smooth, sealed, transparent cover 7 (e.g., glass or plastic) covering and a light bezel 9 that allows for easy wipe down cleaning. The targeted light system 14 housing can incorporate a sealed lip or flange 10 around the outer edge to assure the targeted light system 14 seals fully and flush with the ceiling, wall, etc. to facilitate cleaning and mitigating bacterial growth, holding the targeted light system 14. This can be achieved, for example, with two skinned sponge light gaskets 8 to fill any gap between the light bezel 9 and the cover glass 7. The light bezel 9 can be attached to the rest of the system, such as at flange 10, via screws 11 so that the bezel 9 and transparent cover 7 can be removed at will for maintenance, etc. The backside of this flange 10 also provides a room- side surface to mount the targeted light system 14 flush to the finished ceiling or upper wall surface by means of a T-bar light gasket 12 which provides a stop to position the targeted light system 14 at the correct flush depth. [00035] Fig. 2 depicts a heat sink 30 suitable for use with an LED light source in array 1. In the embodiment depicted, it is physically solder attached and thermally (typically not electrically) attached to the targeted light system 14 via center pad heat sink 31 which is central to the fin radiator 32 to conduct heat away from the LED light source 37. Two die pin conductors 33 located on either side of the center pad heat sink 31 conduct the electrical plus and minus polarity from the LED light source 37 to the positive and negative contacts 36, respectively, through the electrical insulator separator 34. These contacts mate with the curved circuit board into a quarter turn insertion socket for easy replacement or reconfiguration. Additionally, the LED light source 37 may be adjustable longitudinally along its axis, either individually or in concert with the other LEDs in the targeted light system 14.
[00036] LED shroud 35 acts as a lens to direct and concentrate the light into a narrow or wide emitted angle as desired. The LED shroud 35 also acts as a baffle for the light beam to collimate the light. At the end of the LED shroud 35, distal to and in front of the LED light source 37, is a lens. This can be an individual lens or single collimator 13 as shown in Fig. 1. The individual lens or collimator 13 focuses the emitted light from a LED light source 37 source via axial adjustment. Also this individual lens or collimator 13 can act as a notch filter to selectively transmit only a certain wavelength or wavelength band of light. Alternately, the LED light source 37 may be fixed and the individual lens or collimator 13 may or may not be moveable either individually or in concert with other lenses. This allows for precise, even light coverage of the intended area by focusing the emitted light.
[00037] Referring to Fig. 3, it depicts a targeted light system 14 installed on a column mount 21. The targeted light system 14 may be cooled using a conventional flat fan 23 for cooling of the housing 4. This embodiment can be particularly useful where the targeted light system 14 is installed outside the sterile field such as by hanging from the ceiling by the column mount 21 (Fig. 3 and 6). The column mount 21 can be removed from the ceiling socket with a quarter-turn twist, disconnecting the electrical contacts 22 to be set aside when not in use.
[00038] Referring to Figs. 4 and 5, the targeted light system 14 may be used individually or in any combination with an air filtration system 40 such as those described in the aforementioned patent applications. The targeted light system 14 may be cooled using either natural or forced convection from the air filtration device. As shown in the Figures, the targeted light system 14 can be operably connected to the air filtration system 40 to receive forced convection air via cool air inlet 41 from the air filtration system 40 to cool the light source elements 15 of the targeted light system 14. The targeted light system 14 can also be operably connected back to the air filtration system 40 via hot air outlet 42. Typically, the targeted light system 14 and the air filtration system 40 are connected such that no air from the targeted light system 14 enters the room being illuminated and provided with air unless the air first goes through the filters of the air filtration system 40.
Light Controller
[00039] The aim, on/off status, intensity, etc., of the LEDs (whether individually or in groups as light source elements 15) of the targeted light system 14 can be addressed by the light controller 19. The light controller can be located in any desired position either in or outside the room, including in or on a wall, as a part of a surgical bed, or handheld (typically wireless and held by an assistant to a surgeon so as not to interfere with the surgeon's surgical work). This allows for the light source elements 15 to act in concert to smoothly steer the combined light to different areas of the surgical site. The controller may be arranged to gradually turn on or off the lights. In addition, various preprogrammed functions such as simulating sunrise or sunset or a "twinkling star" field with changing colors may be programmed in the controller to be used as "mood lighting". This embodiment provides a fail-safe design wherein, among other things, the internal temperature may be monitored for fire safety by the controller processor. Fig. 7 depicts a block diagram for an exemplary light controller. The light controller may be located in any desired location such as in a nearby wall and provides a power switch and directional positioning buttons. The power switching provides isolation of all power to the unit to allow maintenance of the system without exposure to 120 VAC.
[00040] The power supply can be configured to utilize pulse width modulation so no waste energy goes to heating, and the system can be configured to be energy efficient (Energy Star rated) so dimming the light intensity during the pulse width modulation doesn't just dump excess intensity to heating, but instead reduces the power, typically at a frequency imperceptible to the human eye. Light Target Device
[00041] As shown in Fig. 6, the system can also comprise a light target device 61 comprising a light sensor that detects features of the light emitted by the targeted light system 14 and then provides data to automate control of such features. The light target device 61 can be used to determine where the light is directed (e.g., is the light hitting the target site such as the rectangle on operatory table 62?), the wavelength(s) of the light emitted, and/or the intensity(ies) of the emitted light. The light target device 61 may be battery powered and enclosed in a sterile soft enclosure and can be placed for example over a surgical field. It may incorporate light sensors on its top surface or elsewhere as desired. Alternately, the target may be a tablet PC 63 with software that detects and controls the light coming from the targeted light system 14. In some embodiments, where ever the light target device 61 is placed, the targeted light system 14 can focus its combined light output by turning on or off individual light sources 16 within targeted light system 14. The light target device 61 may then be removed, having adjusted the light source automatically.
Integrated Video Camera
[00042] As shown in Fig. 6, one or more integrated cameras 64 may be incorporated behind the cover glass of each targeted light system 14 (or in other locations as desired). In another embodiments, one or more of the light source elements 15 in Fig. 1 can be replaced by a camera. Providing multiple channels of video allows multiple angles of viewing/recording simultaneously. Optionally a night vision type camera can be incorporated to record or monitor action in a room when the room is darkened. One or more wireless audio microphones inside the room can integrate the audio with the video and stored offsite. This streaming A/V output may be broadcast over the intranet of the facility comprising the room, and/or stored for future review or projected live for instructional purposes. The overhead video may also be streamed to a tablet version of the light target device 61. The tablet may then be used to direct the direction of the lights in real time by touching the video screen of the tablet. Alternately, the system can include motion capture to allow gesture commands that control the specific location or direction of the emitted light within the target site without the surgeon touching any non-sterile surface. Information can be stored anywhere desired such as in facilities storage, for example to record usage information and for reporting through a wired or wireless network reporting system or monitoring. This would allow the facility management to have instant information such as the status of the various components of the system including component failure.

What is claimed is:
A sealed targeted light system comprising a housing comprising a transparent cover, the housing containing an array comprising a plurality of light source elements disposed to emit light comprising at least one color frequency through the transparent cover, the array operably connected to a light controller configured to individually address each of the light source elements in the array, wherein the array is configured to emit light that focuses on a target site and wherein the controller is configured to individually address each of the light source elements such that a) the light source elements of the array are individually selectively turned on/off by the controller to alter a direction of emitted light exiting the targeted light system without moving the housing and b) the light source elements of the array are individually selected by the controller to emit a desired light color frequency emitted by the targeted light system without moving the housing, and wherein the targeted light system is sealed to prevent transmission of air through or around the transparent cover.
The targeted light system of claim 1 wherein the housing is configured to standalone on a column mount.
The targeted light system of claim 1 wherein the targeted light system further comprises a flush-mount configured to mount the targeted light system substantially flush with a wall or ceiling within a room.
The targeted light system of claim 3 wherein the targeted light system is sealed to prevent transmission of air from the targeted light system into the room and wherein the flush mount is configured to prevent transmission of air from the targeted light system to the room.
The targeted light system of claim 4 wherein the flush mount comprises a gasket that seals the transparent cover within the housing.
6. The targeted light system of any one of claims 1 to 5 wherein the plurality of light source elements each comprise at least one LED light source. 7. The targeted light system of claim 6 wherein the plurality of light source elements each comprise a plurality of LED light sources configured to emit light of a different color and wherein the plurality of LED light sources is individually addressed by the controller to selectively to vary intensity of each of, and turn on/off each of, the LED light sources.

The targeted light system of any one of claims 3 to 7 wherein the targeted light system is a sealed targeted light system configured to be integrated with an air filtration system configured to form a clean air zone, wherein the targeted light system is configured to not disrupt the clean air zone when integrated with the air filtration system.
9. The targeted light system of claim 8 wherein the sealed targeted light system is configured to be operably connected to the air filtration system to receive forced convection air from the air filtration system to cool the light source elements of the sealed targeted light system.
10. The targeted light system of claim 9 wherein the sealed targeted light system is configured to be flush-mounted within a wall or ceiling also containing the air filtration system and wherein the targeted light system is disposed adjacent the air filtration system.
11. The targeted light system of any one of claims 1 to 10 wherein the array is configured in a concave curve. 12. The targeted light system of any one of claims 1 to 10 wherein the array is configured in a double-concave curve.
13. The targeted light system of any one of claims 1 to 10 wherein the array is configured in a convex curve.

The targeted light system of any one of claims 1 to 13 wherein the targeted light system further comprises a light target device comprising at least one light sensor that detects features of the emitted light and wherein the light target device is configured to transmit data identifying the features to the controller.
15. The targeted light system of claiml4 wherein the features comprise at least two of where the emitted light is directed, wavelength(s) of the emitted light, and/or intensity of the emitted light.
16. The targeted light system of claim 15 wherein the controller is configured to modify the features of the emitted light based on the data transmitted from the light target device.
17. The targeted light system of any one of claims 1 to 16 wherein the emitted light consists essentially of one of visible light, ultraviolet light or infrared light. 18. The targeted light system of any one of claims 1 to 17 wherein the array further comprises a collimator configured to collimate the emitted light.

a) a plurality of targeted light systems wherein each of the targeted light systems comprises a housing comprising a transparent cover, the housing containing an array of light source elements each disposed to emit light comprising at least one color frequency through the transparent cover and wherein the array is configured to emit light that focuses on a target site,
wherein the arrays of each of targeted light systems are all operably connected to a single light controller configured to individually address each of the light source elements in each of the arrays such that a) the light source elements of the array are individually selectively turned on/off by the controller to alter a direction of emitted light exiting the targeted light system without moving any of the plurality of targeted light systems, and b) the light source elements of the array are individually selected by the controller to emit a desired light color frequency emitted by the targeted light system without moving any of the plurality of targeted light systems; and
b) the target site;
wherein i) the plurality of targeted light systems are each disposed substantially flush within at least one of a wall or ceiling within the room such that the plurality of targeted light systems illuminate the target site from substantially different angles to reduce shadows covering the target site; ii) the plurality of targeted light systems are each configured within the wall or ceiling such that no air is transmitted through or around the targeted light systems into the room. 26. The room of claim 25 wherein room is an operating room and the target site is a surgical site.
27. The room of claim 25 wherein room further comprises at least one additional targeted light system located on a column mount.
28. The room of claim 27 wherein the targeted light systems are sealed to prevent transmission of air from the targeted light systems to the room and wherein the flush mount is configured to prevent transmission of air from the targeted light systems into the room.
29. The room of claim 28 wherein the flush mount comprises a gasket that seals the transparent cover within the housing.
30. The room of any one of claims 25 to 29 wherein the plurality of light source elements each comprise at least one LED light source.

The room of any one of claims 25 to 42 wherein the arrays of each of the targeted light systems further comprise a printed circuit board material comprising circuit board traces embossed thereon wherein the circuit board traces operably electrically connect the light source elements to the controller.
44. The room of any one of claims 25 to 43 wherein the controller comprises preprogrammed functions configured to provide a selected mood lighting.
45. The room of claim 44 wherein the selected mood lighting comprises at least one of a sunrise, sunset or twinkling star field with changing colors
46. The room of any one of claims 25 to 45 wherein the targeted light system further comprises at least one video camera disposed within the housing. 47. A method comprising manufacturing a room according to any of claims 25-46.
48. A method comprising using a room according to any of claims 25-46.
49. A method of claim 48 wherein the method further comprises performing a surgery in the room.
