Abstract
MuradBridge is a dental bridge that contains a "Muradpontic". Muradpontic is a pontic-made of acryl-used in the dental metal porcelain bridges. So it is a modification on the pontic used in the traditional metal porcelain dental bridge. The modification is done by making the pontic of acryl instead of porcelain to provide several advantages: The bridge will be less weight, the occlusal forces will be reduced on the abutments and tissue support will be providedfor the abutments. Muradpontic is used as an alternative solution for the treatment of some special clinical cases: Lack of bone support of dental abutments, Long bridges, Cantilever bridge, Free Edentulous and dental implant. lt is used to provide fixed prosthodontics that reduce the occlusal forces on the abutments and reduce the cost compared with the cost of dental implants. So Muradpontic is a potential treatment solution for such special clinical cases.
Description
Name of invention :MuradBridge in Dentistry
Description of the invention:
1. Earlier technical condition:
The earlier technical condition of the invention is the traditional metal ceramic dental bridge: it is a bridge -consists usually of tow retainers and aponticthat all made of porcelain on metal- fixed on tow dental abutments to restore a missing tooth.
This type of bridges is widely used to restore missing teeth. Using this kind of bridges over years around the world proved its effectiveness and success in restoring missing teeth in dental patients.
But thereis a need to make amodification on the traditional metal porcelain dental bridge in some special clinical cases that characterized with a lack of dental and/ or bone support to make the dental bridgeless weight and reduce the occlusal forces on its abutments.
This is where the idea of Murad Bridge came from.
2. Detailed description of the invention:
Definition:
MuradBridge is a dental bridge that containsa"Muradpontic".
Muradpontic is a pontic-made of acryl- used in the dental metal porcelain bridges whatever theirdesigns were.So it is a modification on the pontic used in the traditional metal porcelain dental bridge.
The modification is done by making the pontic of acryl instead of porcelain to provide several advantages:
• The bridge will be less weight.
• The occlusal forces will be reduced on the abutments.
• It will provide tissue support for the abutments. The aim of inventing MuradBridge in Dentistry:
MuradBridge can be used as an alternative solutionfor thetreatmentof some special clinical cases that need a reduction in the occlusal forces on the abutments. Advantages of MuradPontic:
MuradPontic has the characteristic that should be in any other pontic; so it is aesthetics, easy to keep hygiene and resistance to mastication forces.
In addition, it has its own characteristic because it is made of acryl:
Light weight.
Less cost.
Do not cause abrasion to the opposite teeth.
Reduce the weight of the bridge and reduce the occlusal forces on the abutments. l
Indications of MuradBridge in Dentistry:
1. Lack of bone support of dentalabutments.
2. Long bridges\ lack of dental support (few remaining teeth).
3. Cantilever bridge.
4. Free Edentulous.
5. Dental implants.
Lack of bone support of the dental abutments:
Mostly, dentists in this situation have to increase the number of the abutments but sometimes it is not possible to do thatbecause of missing most of the teeth, Alsopatients refuse to have removable denturesor the economicalconditions of the patient don't provide the cost of dental implant. So what is the solution? Long bridges:
Patients usuallypreferdental bridges on removable denturesin cases of missing most of the teeth. But most of these cases there is a lack of bone supportof the dental abutments to make fixed prosthodotics and the economical conditions of the patient don't provide the cost of dental implant. So what is the solution? Free edentulous:
Many patients refuse to make removable denture in this case and the economical conditions of the patient don't provide the cost of dental implant. And usually dentist refuses to make a Cantilever bridgebecause of the awareness of its negativities. So what is the solution?
Dental implant:
It is known that the occlusion is the main factor affecting the dental implants. This is why reducingocclusal forces on the dental implants is required; especially when the number of dental implants is few and\or there is a lack of bone support of implants. So what is the solution?
We notice from all the former cases that the issue is the lack of dental and /or bone support;that is the most important factor in makinga fixed prosthodontic. But patients always prefer fixed prosthodontics on removable dentures. That make patients in need for dental implants but sometimes the economical conditions of the patient don't provide the cost of dental implant or the systemic condition of the patient is a contraindication for dental implant.
This is why we need a solution to provide fixed prosthodontics that reduce the occlusal forces on the abutments and reduce the cost compared with the cost of dental implants; that make MuradBridge a potential treatment solution for such special clinical cases.
So the solution of all the former cases is Murad Bridge
How to make Murad Bridge in Dentistry:
The steps of making Murad Bridge clinically in the dental office do not differ from the steps of making the traditional metal porcelain bridge.
But the steps of making Murad Bridge in the dental laboratory are different from the steps of making traditional metal porcelain bridge.
And here are the steps of making Murad Bridge in the dental laboratory:
1. Wax the retainers of the bridgeas usually done in the traditional metal porcelain bridge.
2. Put the waxed dye in its place and wax between them firmly to make a bar.
3. Then cast all the parts (retainers andbar) as usually done in the traditional method and finish the metal as usually done in the method of making traditional metal porcelain bridge.
4. Then start making the layers of porcelain by putting the first layer on the metal retainers and bar.
5. Build the other porcelain layers of the retainersas known in making traditional metal porcelain bridge. Then finish t and glaze the abutments as known in the making of traditional metal porcelain bridge.
6. Wax the pontic in the required shape to make a waxed dye, and then cook the acryl at temperature 100.
7. Then finish the acryl pontic and clean the bridge.
And the bridge is made and it is ready to be tried in the patient's mouth.(Fig re 1) MuradBridge after fixing it on the abutments in the patient's mouth. (Figure 2)
Clinical cases:
1st clinical case:
A 67 year male, had uncontrolled diabetes type 2 and blood hypertension. He wanted a fixed prosthodontic and does not have the cost of dental implants. MuradBridge is made in the lower jaw using six remaining teeth as abutments. This bridge consists of 13 pieces to restore the lower missing teeth to help the patient increase his ability of chewing food, increase aesthetic of the patient and reduce occlusal forces on the dental abutments. As well a removable upper denture is made for the patient and a crown is made for the canine.
Because we wanted to make a comparison between the traditional metal porcelain dental bridge and MuradBridge we weighted the tow bridges:
In this clinical case, each bridge consists of 13 pieces. The traditional metal porcelain bridge weight was 18 grams but MuradBridge weight was 9 grams. Reducing the weight of the bridge is an important role in reducing the occlusal forces on the dental abutments and this is the main advantage of MuradBridge. And we notice from the photos of the tow bridges that both bridges are similar from all aspects but what makes MuradBridge special is its light weight which is an important factor in reducing occlusal forces on the abutments. AlsoMurad bridge provide a fixed prosthodonticthat is less expensive than dental implants. Alsowe notice from the next tow x-rays the difference between the look of Murad bridge and the look of the traditional metal porcelain bridge on the x-ray. The pontics of Murad bridge look like a bar shape but the pontics of the traditional metal porcelain bridge look like teeth s ape.(Figure 3)
Murad Bridge and Traditional metal porcelain bridge/E/gwg 4)
2nd clinical case:
A 51 year femalemissed many of her upper teeth. She wanted a fixed prosthodonticnot a removable denture.
Damaged upper teeth were extracted except the upper left canine and the right upper molar. Then a dental implant was done in the area of the upper right canine. After the healing period a Murad dental bridge was done- which consisted of 11 pieces fixed on 3 abutments(the dental implant , upper left canine and upper right molar)- to restore the missing upper teeth for the patient with a fixed prosthodontic that reduce the forces on the dental abutemnets and the dental implant. (Figure 5)
3™clinical case:
A 40year male, haduncontrolled diabetes mellitus type 2.He wanted a fixed prosthodontic to restore his missing teeth which are(right lower molar, left lower premolar, left lowr first molar and left lower second molar). He did not want dental implants because of the high cost.
Cantilever Murad DentalBridge was done to restore the lower right molar fixed on tow abutments (first and second right premolars). In the left side a Murad dental bridge was done to restore the missing teeth (second premolar and tow lower molars) despite the long adentulous area.Using Murad bridge inthis case
helped reducing the occlusal forces on the dental abutments and also reduced thetreatment cost compared to dental implants cost.(Figure 6)
4thclinical case:
A 30 yearfemale, had an embedded second premolar. She wants a fixed prosthodontic to restore the space of the second premolar and could not offer the cost of a dental implan A cantilever Murad dental bridge was done to restore the embedded second premolar to reduce the treatment cost compared to the dental implant cost. The bridge consisted of 3 pieces fixed on 2 abutments (first and second lower molars).(FzgMre 7)
5thclinical case:
A 60 year female, had diabetes mellitus type 2. She hadmissed most of her teeth and wanted a fixed prosthodontic.
Dental implants were made in both upper and lower jaws. After 4 months Muraddental brigdes were made over the implants in both jaws to restore missing teeth. In the lower jaw cantileverMuradbridge was made in both sides. MuradBridge as previously described is less weight so it reduces the occlusal forces on the implants which is a very important factor for the success of the implants. (Figure 8)
6thclinical case:
A 45 year female, had a free edentulous in the right lower jaw. She wanted a fixed prosthodontic.A dental implant in the edentulous area was done. Muraddental bridgewas made. Murad bridge in this case consisted of 5 pieces on 2 abutments (one is an implant and the other is the canine). We notice that the edentulous area was long this is why we need Murad bridge because it reduces the occlusal forces on the abutments. (Figure 9)
7thclinical case:
A 60 year female, hada lower edentulous lower jaw. She wanted a fixed prosthodontic to restore her missed lower teeth.
Four dental implants were made inthe lower jaw. After the healing period aMuraddental bridge was made over the implants in the lower jaw to restore the missing lower teeth with tow cantilevers on both sides (right and left). So the bridge in this case consisted of 10 pieces fixed on 4 dental implants to restore the lower missed teeth.
Muraddental bridge as previously described is less weight so it reduces the occlusal forces on the implants which is a very important factor for the success of the implants. (Figure 10)
8 clinical case:
A 50 year male, hada bad oral hygiene. We noticed the caries on the six upper anterior teeth. He missed the upper premolars and molars on both sides. He wanted a fixed prosthodontic to restore his missed upper teeth. He did not want dental implants because of the high cost. We noticed also the bone desorption in the upper jaw and the closed upper sinus which indicate more procedures like bone grafting and sinus lifting that the patient couldn't afford.
A Muraddentalbridge was done with tow cantilevers on both sides to restore the upperteeth. The bridge in this case consisted of 1 1 pieces fixed on six dental abutments (after endodontic treatment for the abutments was done).
As well periodontal treatment was done to the lower teeth and the patient was given the oral hygiene instruction to improve his oral hygiene. (Figure 11)
9thclinical case:
A 50 year female. She had missed her lower anterior teeth and the right lower premolar. She wanted a fixed prosthodontic to restore her missed teeth but she could not afford the cost of dental implants.
A Muraddental brigdewasdone to restore her missed teeth with a fixed prosthedentic reduces the treatment cost. The bridge in this case consisted of 9 pieces fixed on 3 abutments (lower left canine , right first premolar and right lower oXax). (Figure 12)
10thclinical case:
A 60 yearmale, hadan upper edentulous jaw. He wanted a fixed prosthodontic to restore his missed upper teeth. He missed some of his lower teeth also and wanted a fixed prosthodontic to restore them.
Four dental implants were made intheupper jaw (tow implants in each side).
After the healing period aMuraddental bridge was made over the implants in the upper jaw to restore the missing upperteeth with tow cantilevers on both sides
(right and left). So the bridge in this case consisted of 10 pieces fixed on 4 dental implants to restore the upper missed teeth .
Muraddental bridge as previously described is less weight so it reduces the occlusal forces on the implants which is a very important factor for the success of the implants.f gwre 13)
llthclinical case:
A 40 year male. He missed his upper molar in both side and the right upper right lateral inciser. He wanted a fixed prosthodontic to restore his missed upper teeth without dental implants because he could not afford their cost.
AMuraddental bridge was made.lt consisted in this case of 12 pieces fixed on 8 dental abutments to restore the upper missed teeth.Murad dental bridge reduces the occlusal forces on the dental abutments and decreases the treatment cost compared with the dental implants. (Figure 14)
