La présente invention concerne un système et un procédé de gestion de patient, le procédé faisant appel : à la réception, sur une interface de données et en provenance d'un médecin, des données d'activité correspondant à plusieurs activités ; à la mémorisation des données d'activité dans un journal numérique partagé ; à la réception, en provenance du patient et par le biais du journal numérique partagé, d'un score d'une activité parmi la pluralité d'activités ; et à la production, par un processeur, d'un compte-rendu indiquant le score de la pluralité d'activités. Chaque activité est une activité à effectuer au moins en partie par un patient et est associée à une date d'échéance. Le journal numérique partagé est accessible par le médecin et par le patient. Chaque entrée dans le journal est associée à la date d'échéance d'une activité correspondante. Le score de chaque activité est au moins en partie associé à un niveau d'achèvement de l'activité.
