Abstract
最小侵襲骨接合術において、骨折部に筋からの血液の供給を阻害しない形状の非平板型骨プレートを提供する。この非平板型骨プレートは、第一固定板11、第一連結部12、支持板13、第二連結部14及び第二固定板15の順に長手方向に配列し、一体化されてなる。第一固定板11及び第二固定板15は、各々、ネジ貫通孔16を持ち、骨に当接する曲面11a及び15aを持つ。支持板13は筋に当接する曲面を持つ。第一連結部12は第一固定板11と支持板13とを繋ぎ、第二連結部14は支持板13と第二固定板15とを繋ぐ。支持板13の筋に当接する曲面は、第一連結部12と第二連結部14によって、第一固定板11及び第二固定板15の骨に当接する曲面よりも上方に位置せしめられている。
Description
本発明は、犬や猫等の動物の骨折部の骨接合術に用いる非平板型骨プレートに関し、特に、動物の四肢を構成している長管骨の骨折部を非開創で骨接合を図るために用いる非平板型骨プレートに関するものである。
従来より、動物の骨折部の骨接合術としては、創外固定法や骨プレートを用いる方法が知られている。創外固定法に用いる創外固定器は、体外に創外固定器が設けられるため、動きにくいといった欠点や、創外固定器に偶発的に外力が負荷され、骨折部外の骨を損傷させるといった欠点があった。
このため、骨折部の骨接合術に骨プレートを用いることが多くなっている。骨プレートは、チタン合金等の各種合金よりなるネジ貫通孔を持つ平板型骨プレートであり、この平板型骨プレートを骨折部に当接し、ネジ貫通孔を通して骨にネジを螺着して用いるものである。したがって、平板型骨プレートは筋と骨(骨折部の骨)との間に存在し、創外固定器のように体外に存在しないため、創外固定法の欠点を回避することができる。このため、骨折部の骨接合術に骨プレートが多用され、骨プレートに関して種々の工夫が提案されている(特許文献1)。
しかしながら、平板型骨プレートを骨折部の骨に沿って当接するには、骨折部全体を開創しなければならず、骨折部の筋全体を侵襲してしまうという欠点があった。このため、骨折部全体を開創するのではなく、骨折部の両側(近位側と遠位側)の筋のみを侵襲し、その部位から平板型骨プレートを滑り込ませ、近位側と遠位側のみでネジを螺着する最小侵襲骨接合術が提案されている。
最小侵襲骨接合術は、筋の侵襲が少なく好ましいものである。しかしながら、最小侵襲骨接合術においても、用いる骨プレートは平板型骨プレートであるため、以下のような欠点を回避することができない。すなわち、平板型骨プレート1は、図1に示すように骨折部の骨に沿って設けられるため、骨2と筋3との間に平板型骨プレート1が存在し、筋3からの血液が骨2の表面の骨芽細胞からなる骨膜4に供給されず、骨芽細胞が壊死してしまうという欠点があった。つまり、骨芽細胞は筋3からの血液により産生し増殖してゆくのであり、この骨芽細胞が壊死すると、平板型骨プレート1の存在側では骨芽細胞の発達による仮骨5が産生せず、平板型骨プレート1の非存在側でのみ仮骨2が産生することになる。したがって、骨折部の癒合が弱くなり、骨折部の治癒が遅れるという欠点があった。
そこで、本発明の課題は、最小侵襲骨接合術において、骨折部に筋からの血液の供給を阻害しない形状の骨プレートを提供することにある。
本発明は、従来の平板型骨プレートを、特異な形状の非平板型とすることにより、上記課題を解決したものである。すなわち、本発明は、第一固定板、第一連結部、支持板、第二連結部及び第二固定板の順に長手方向に配列し一体化されてなる非平板型骨プレートであって、前記第一固定板及び前記第二固定板は、ネジ貫通孔を持つ共に骨に当接する曲面を持ち、前記支持板は筋に当接する曲面を持ち、前記第一連結部は前記第一固定板と前記支持板とを繋ぎ、前記第二連結部は前記支持板と前記第二固定板とを繋ぎ、前記支持板の筋に当接する曲面は、前記第一連結部と前記第二連結部によって、前記第一固定板及び前記第二固定板の骨に当接する曲面よりも上方に位置せしめられていることを特徴とする非平板型骨プレートに関するものである。
本発明に係る非平板型骨プレートを用いれば、最小侵襲によって施術を行うことが可能になると共に、以下のような作用により、骨折部の早期治癒が可能となる。すなわち、骨折部の周囲には、骨プレートが当接していないため、骨折部の骨芽細胞に筋からの血液が十分に供給され、骨芽細胞が壊死することなく、産生及び増殖する。この結果、骨折部の周囲全体において、仮骨が産生し骨折部の癒合が強くなって、骨折部が早期に治癒するという効果を奏する。
本発明に係る非平板型骨プレートは、第一固定板11、第一連結部12、支持板13、第二連結部14及び第二固定板15の順に長手方向に配列しており、一体的に形成されているものである。第一固定板11等の各部は、一般的に金属製であり、たとえばチタン合金やコバルトクロム合金が採用される。
第一固定板11及び第二固定板15は、骨折部を跨いで、骨の近位及び遠位に沿って、骨の表面に当接されるものである。したがって、第一固定板11及び第二固定板15は、骨に当接する曲面11a及び15aを持っている。動物の四肢の骨は長管骨と呼称されるものであり、略円柱形状となっているため、骨表面に当接する曲面11a及び15aも曲面は、一般的には凹曲面となっている。なお、長管骨は複雑な表面形状をしていないため、曲面11a及び15aは、同一面に存在しているのが好ましい。また、第一固定板11及び第二固定板15は骨折部外の骨表面に当接されるが、短期間であれば、筋からの血液が骨芽細胞に供給されなくても、骨折部の治癒には差し支えない。
第一固定板11及び第二固定板15は、骨折部を跨いで、骨の近位及び遠位にネジによって固定されるものであるため、ネジ貫通孔16を持っている。ネジ貫通孔16は、各固定板11及び15に、一個乃至数個設けられている。このネジ貫通孔16にネジを貫通させて、ネジを骨に螺着することにより、第一固定板11及び第二固定板15は骨に沿って固定される。
支持板13は、第一固定板11と第二固定板15の間に存在し、骨折部を支持するためのものである。この支持板13は、骨折部を支持するが、骨に沿って当接されるものではない。支持板13は、骨折部に対応する筋表面に当接する。したがって、支持板13は、筋に当接する曲面13aを持っている。筋も長管骨等の骨と同様に、略円柱状となっているため、筋表面に当接する面13aも、一般的には凹曲面となっている。
第一固定板11と支持板13とは、第一連結部12で繋がれている。また、第二固定板15と支持板13とは、第二連結部14で繋がれている。第一連結部12及び第二連結部14は、支持板13を第一固定板11及び第二固定板15の上方に押し上げており、この結果、支持板13の筋に当接する曲面13aは、第一固定板11及び第二固定板15の骨に当接する曲面11a及び15aよりも上方に位置せしめられている。第一連結部12及び第二連結部14は円弧状となって、支持板13を上方への押し上げるのが好ましい。
第一連結部12及び第二連結部14の幅は、第一固定板11、支持板13及び第二固定板15の幅よりも狭くなっているのが好ましい。この理由は、骨折部近傍位の開創を小さくしうるからである。
本発明に係る非平板型骨プレートを用いて、最小侵襲骨接合術を施す方法の一つを説明する。まず、骨折部の近位21及び遠位22を開創する。そして、近位21に第一固定板11を滑り込ませと共に、遠位22に第二固定板15を滑り込ませて、骨2の表面に第一固定板11及び第二固定板15を当接する。支持板13は、皮膚6のみを開いて、筋3表面に当接する。この際、第一連結部12及び第二連結部14の進入箇所のみを開創する。そして、第一固定板11及び第二固定板15のネジ貫通孔16にネジ17を貫通し、骨2に螺着して固定すれば、施術は終了し、図3に示した状態となる。したがって、開創部位は、骨折部の近位21、遠位22及び第一連結部12及び第二連結部14のみであり、最小侵襲によって施術を行うことができる。
本発明に係る非平板型骨プレートは、単体で製品として提供されてもよいし、施術で用いるネジと共にキットとして提供されてもよい。
本発明に係る非平板型骨プレートは、骨折した人や動物の施術に利用することができる。

- 第一固定板、第一連結部、支持板、第二連結部及び第二固定板の順に長手方向に配列し一体化されてなる非平板型骨プレートであって、
前記第一固定板及び前記第二固定板は、ネジ貫通孔を持つ共に骨に当接する曲面を持ち、
前記支持板は筋に当接する曲面を持ち、
前記第一連結部は前記第一固定板と前記支持板とを繋ぎ、前記第二連結部は前記支持板と前記第二固定板とを繋ぎ、
前記支持板の筋に当接する曲面は、前記第一連結部と前記第二連結部によって、前記第一固定板及び前記第二固定板の骨に当接する曲面よりも上方に位置せしめられていることを特徴とする非平板型骨プレート。 - 第一連結部及び第二連結部の幅は、第一固定板、支持板及び第二固定板の幅よりも狭い請求項1記載の非平板型骨プレート。
- 第一固定板及び第二固定板の骨に当接する曲面が、同一面に存在する請求項1又は2記載の非平板型骨プレート。
