Abstract
This newly invented system is intended for cannulating arteries and advancing a guidewire to allow inserting arterial sheaths, plastic tubes or devices that need to be inserted into the arteries. The system is consisted of multiple hollow needles and multiple plastic tubing system which are on the same plane and parallel to each other and the plastic tubes exactly fit on the needles, with the help of this device, arteries are easily cannulated and a guidewire is advanced. With this new device, the procedure is done easily and more quickly, hence it eases the work of the doctors and nurses besides reducing the stress of the patients by decreasing the procedural time.
Description
DESCRIPTION
Multiple Needle System With A Multiple Plastic Tubing To Cannulate Narrow Arteries
The invention explained here is a medical device with multiple plastic tubing system over a multiple hollow needle system that is intended to be used for arterial cannulation to monitor blood pressure, frequent blood sampling or introducing medical devices (guidewires, catheters) through it.
In medicine, plastic tubes, called sheath in English, are inserted and placed in narrow arteries and veins during some procedures and also in angiography. Inserting narrow plastic cathethers is not only necessary for angiography but for also some procedures like continious arterial blood pressure monitoring or drawing arterial blood gas samples frequently. In such procedures where these plastic tubes are used, first the arteries are cannulated by a needle and then a guide wire is advanced through the needle and finally plastic tubes or sheaths are inserted into the arteries over the guide wire.
It is not always easy to get the hollow needle into the artery to advance the plastic tubing and/or advance the tiny guidewire especially if the artery is narrow and such a procedure may result with a waste of time, exhaustion of the doctor and pain and distress for the patient.
In this invention, to overcome the abovementioned difficulties, this new device will especially ease the insertion of the needle, guidewire and the 'sheath' into the artery of the adults and children with narrow arteries during cathetherization and angiography. Besides that, recently angiography and percuteneous coronary interventions via the wrist artery
(Radial artery) has increased in number and will continue to do so in the future as well, as it is more convinient for the patient and in many studies it is shown to carry a lower complication rate.

The invented and newly designed system, which consists a multiple hollow needle system (1) with a multiple plastic tubing (2), uses multiple plastic tubing (2) that exactly fits on the needle (1) system, all in one equipment that causes the main difference from the ones that have single needle and plastic tube and used presently (Figures 1 and 2). This new device system can be consisted of 2, 3, 4, 5 or more hollow needles and corresponding number of plastic tubing system on the needle system, however, triple needle with a triple plastic tubing will be more convinient and easy to use, so the figures basically will show the triple hollow needle system.
In Figure-1 the system is seen in an upper view and in Figure-2 the system is seen in a side view, in Figure-1 A: The triple plastic tubing (2) system is fit on the triple hollow needle system (1) (upperview), in Figure-1 B: The triple plastic tubing (2) system is seen seperately (upperview), in Figure 1 C: The triple hollow needle (1) system is seen seperately (upperview), in Figure-2 A: The triple plastic tubing (2) system is fit on the triple hollow needle (1) system (sideview), in Figure-2 B: The triple plastic tubing (2) system is seen seperately (sideview), in Figure 2 C: The triple hollow needle (1) system is seen seperately (sideview). In all sideview scenes (Figure-2) as the needles (1) and plastic tubing (2) legs
are parallel to each other and are on the same plane, only the needle and plastic tube in front are seen.
The three hollow needles (1) that stands next to one and other are attached to each other at the very proximal edge (8) (far most from the tip) which is made of plastic. The three plastic tubes (2) that surround and fit the three needles (1) are transparent and they are attached to each other at the very proximal edge (7) as stated before and can easily be seperated by pulling slightly as they are bounded to each other at a very small point. The plastic tubing (2) could also be produced from non-transparent material if desired. These three plastic tubes (2) over the three needles (1) could also be manufactured unattached to each other if required.
The system could be manufactured in different needle sizes (thickness and length) with the appropriate plastic tubes in those sizes. The distance between the hollow needles (6) is the same as the thickness of the needle and the plastic tube (2) covering it together which means thicker needles will have more distance (6) between them, however, if needed in the manufactured system, the distance (6) between needles could be set less or more than the thickness of one needle and tubing unit, this distance could vary according to the anatomic area that the system is intended to be used. The thickness of the needles will be measured by the international measuring system called "Gauge". Hence, systems that are 14-16-18-20-22-24-26-28-30-32-34 G thick and other systems measured in different thicknesses could also be manufactured.
The system will and could only be used by authorized personnel. It is advisable that each one of the three (or more) hollow needles with the plastic tubing unit be filled with normal saline or with a likewise sterile liquid before used.
The operator using the system should aim the central needle to the region where he/she palpates the artery (9) with his/her finger tip (Figure-3) and inserts the unit all the way down in the anatomic area until where the operator thinks it is deep enough. This way the aimed artery (9) is punctured and passed through, both from the front and back by one or more of the three hollow needles (1) and the plastic tubing (2) on it (Figure-3). At this point the three hollow needle (1) unit that is inside triple plastic tubing (2) is pulled back and the only device that stays in the artery is triple plastic tube (2) system (Figure 4). Then the triple plastic tube (2) system in the artery of the patient is pulled back slowly and when the tube(s) fall into the lumen of the artery (9) a pulsatile blood stream comes back from the tubing system (Figure-5). At this point of the procedure, the tube is no longer pulled back and the guidewire (5) is advanced through the plastic tube (2) unit in which the blood comes back (Figure-6), but if blood comes from two of the tubes then the guidewire (5) is adviced to be sent through the one with more pulsatile blood flow. If another equipment will be used the guidewire (5) is left inside the artery (9) (Figure 7) and the desired equipment such as plastic sheath could be inserted to the artery (9) by sliding it on the wire (5) and the procedure
continues. If required the plastic tube (2) that is on the wire could easily be seperated from the other two and could be sent forward into the artery (9) (Figure 8) and this tube could also be used for monitoring arterial pressure and/or for frequent arterial blood gas sampling. The proximal part of the plastic tube (2) is compatable with a standart luerlock used in medical devices.
Claims
1- This new medical device is intended for cannulating arteries (9) and advancing a guidewire (5) to allow inserting arterial sheaths, plastic tubes or devices that need to be inserted to the arteries, and the system is consisted of multiple hollow needles (1) and multiple plastic tubing (2) system which are on the same plane and parallel to each other and the plastic tubes exactly fit on the needles.
2- The system is as stated in claim 1 , and wherein the multiple hollow needle (1) and plastic tubing (2) system are preferred to have three hollow needles (1) and three plastic tubing (2) that exactly fits the needles (1).
3- The system is as stated in claim 1 , and wherein the multiple hollow needles are attached to each other from the most proximal part (8) which is the plastic hub part far most from the tip of the needles.
4- The system is as stated in claim 1 , and wherein the multiple plastic tubing (2) that fits on the hollow needles (1), is preffered to be manufactured from a transparent material.
5- The system is as stated in the abovementioned previous claims, and wherein the three plastic tubes (2) that surround and fit the three needles (1) are attached to each other at the very proximal edge (7) and can easily be seperated by pulling slightly as they are bounded to each other at a very small point (7) also these three plastic tubes (2) over the three hollow needles (1) could be manufactured unattached to each other, if desired.
6- The system is as stated in the abovementioned previous claims, and wherein the distance- between the needles (6) is the same as the thickness of the hollow needle and the plastic tube (2).
7- The system is as stated in claim 6, and wherein the distance between the hollow needles (6) can be of different measures; less or more than the thickness of the hollow needle (1) and the plastic tube (2) if desired.
