Abstract
Ein Medizinsystem (2) umfasst ein elektromagnetisch oder optisch arbeitendes Navigationssystem (4), ein Röntgensystem (6), und ein medizinisches Instrument (8), an dem ein Röntgenmarkersystem (14) und ein Navigationsmarker (12) des Navigationssystems (4) in bekannter Relativposition (R) zueinander fest angebracht sind.

Beschreibung
Medizinsystem
Die Erfindung betrifft ein Medizinsystem mit einem elektromagnetisch oder optisch arbeitenden Navigationssystem, einem Rontgensystem und einem medizinischen Instrument.
Derartige medizinische Systeme werden verwendet, um mit Hilfe des Instruments, unterstutzt von Patientenbilddaten navigierte medizinische Maßnahmen an Patienten durchzufuhren. Das Instrument ist hierbei z.B. ein medizinisches Werkzeug, ein Implantat o.a. In einer sogenannten Registrierungsprozedur müssen die zu verwendenden Instrumente und Patientenbilddaten ortsrichtig einander zugeordnet werden. Heute bekannte Registrierungsverfahren sind entweder zeitaufwandig, fehleran- fallig oder lassen sich nicht schnell genug vor Ort, z. B. in einem Operationssaal, in welchem das Medizinsystem aufgebaut wurde, durchfuhren. Heute bekannte Ansätze sind die punktba- sierte Registrierung anhand praoperativer Daten, die oberfla- chenbasierte Registrierung oder die Registrierung anhand praoperativer Daten. Aus dem „NaviLmk"-System der Firma Siemens ist die Verbindung einer intraoperativen 3D-C-Bogenbildgebung und eines Navigationssystems bekannt. Bekannt sind auch bild- basierte Ansätze, z. B. die Erkennung von rontgenundurchlas- sigen Rontenmarkersystemen in Röntgenbildern und die daraus folgende Berechnung von Geometrien. In einem Rontgenmarker- system sind in der Regel die Marker in einer 3D-Struktur angeordnet. Ein derartiges System ist z.B. von der Firma Surgix Ltd, Tel Aviv bekannt.
Aufgabe der Erfindung ist es, ein verbessertes Medizinsystem anzugeben .
Ziel der Erfindung ist eine Echtzeitregistrierung von Instrumenten, Werkzeugen und Patientenbilddaten, die schneller als bei den bekannten Systemen erfolgt.
Die Aufgabe wird gelost durch ein Medizinsystem gemäß Patent¬ anspruch 1. Das Medizinsystem umfasst ein elektromagnetisch oder optisch arbeitendes Navigationssystem, ein Rontgensystem und ein medizinisches Instrument. Am medizinischen Instrument sind ein Rontgenmarkersystem und ein Navigationsmarker des
Navigationssystems fest angebracht. Alle Marker befinden sich hierbei in bekannter Relativposition zueinander.
Erfindungsgemaß wird also vorgeschlagen, ein einziges Instru- ment sowohl mit Röntgen- als auch Navigationsmarkern auszustatten. Die Rontgenmarker sind rontgenundurchlassig, z.B. kleine Kugeln, die in einem bestimmten räumlichen Muster zu einem Rontgenmarkersystem angeordnet sind, so dass aus Ihnen die Position und Orientierung des Instruments anhand eines vom Instrument mit dem Rontgensystem aufgenommenen 2D-
Rontgenbildes bestimmt werden kann. Zum Anderen wird im Instrument als Navigationsmarker des Navigationssystems z. B. eine Sensorspule mit 5 oder 6 Freiheitsgraden (DoF, Degrees of freedom) integriert, deren Position und Orientierung wie- derum mittels des elektromagnetischen Navigationssystems ermittelt werden kann. Das Instrument stellt somit bezuglich der räumlichen Zuordnung die direkte Verbindung zwischen dem Navigationssystem und den vom Rontgensystem erzeugten Bildda- ten dar. Die Registrierung kann sofort ohne jeglichen sonsti- gen Zwischenschritt anhand des parallel vom Rontgensystem und vom Navigationssystem erfassten Instruments geschehen. Die Rontgenmarker und die Navigationsmarker werden bei der Her¬ stellung des Instruments kalibriert, so dass ihre raumliche Beziehung zueinander auf dem Instrument bezüglich Ihrer Posi- tionen und Orientierungen m allen Freiheitsgraden bekannt ist.
Sollten die Rontgenmarker das Feld des elektromagnetischen Navigationssystems stören, kann eine Störung bei der System- kalibπerung auch sofort berücksichtigt werden. Der Navigati¬ onsmarker, z. B. eine Sensorspule, kann hierbei kabelgebunden oder kabellos, z. B. mittels eines aktiven oder passiven Schwingkreises, ausgebildet sein.
Das erfindungsgemaße Medizinsystem erlaubt eine sehr schnel¬ le, auch „online" genannte Registrierung, zwischen Patientendaten, z. B. den mit dem Rontgensystem aufgenommenen Rontgen- projektionsaufnahmen in 2D oder 3D, und dem Navigationssystem. Jegliches zusätzliche chirurgische Werkzeug, z. B. ein Pomter oder Implantat, welches ebenfalls mit einem Navigati- onsmarker des Navigationssystems oder mit Rontgenmarkern ausgestattet ist, kann direkt ohne zusätzliche Registπerverfah- ren in vom Rontgensystem aufgenommene 2D-Rontgenbilder eingeblendet werden. Eine 3D Bildgebung im Rontgensystem ist nicht notwendig, da der Ort der Rontgenmarker im Bezugsystem des Navigationssystems auch aus 2D-Rontgenbildern ermittelt werden kann. Das Verfahren bzw. das Medizinsystem erlaubt daher auch eine bildgefuhrte bzw. navigierte Chirurgie bei ausschließlicher Verwendung von 2D-Rontgenprojektionsbildern .
In einer vorteilhaften Ausfuhrungsform des Verfahrens ist das Instrument eine an einem Patienten fixierbare Klemme. Die Klemme wird z. B. an der Wirbelsaule des Patienten als Referenzklemme fixiert. In dieser Ausfuhrungsform sowie in allen Anderen, bei welchen das Instrument in bekannter Relativposi- tion zum Patienten liegt, können auch Bewegungen des Patien¬ ten durch die Verschiebung des Instruments mit dem Patienten sofort direkt in der Navigation und Ortsberechnung sämtlicher Komponenten berücksichtigt werden, sind also nicht mehr so störend.
Im Medizinsystem kann das Instrument auch dazu verwendet wer- den, um eine Beziehung zwischen einem intraoperativ gemessenen 3D-Bilddatensatz und 2D-Proj ektionsbildern herzustellen. Bei dieser sogenannten 2D/3D-Registπerung wird der SD- Datensatz als digital rekonstruiertes Röntgenbild (DRR) in der Kamerageometrie des 2D-Rontgenbildes berechnet und ge- meinsam mit diesem dargestellt. Das erfindungsgemaße Instru¬ ment kann dazu verwendet werden, die genannte Kamerageometrie zu bestimmen und daraus das DRR zu berechnen.
Umgekehrt kann aus dem 2D-Rontgenbild auch direkt die Kamera¬ geometrie bestimmt werden, wenn sowohl das Rontgenmarkersys- tem als auch die Navigationsmarker vorhanden sind. Dann kann aus dem 3D-Datensatz entsprechend ein DRR berechnet werden.
In einer weiteren vorteilhaften Ausfuhrungsform ist das In¬ strument ein Interventionswerkzeug, z. B. ein Endoskop oder ein sonstiges interventionell genutztes Instrument.
Das Instrument kann hierbei auch als Hand-held-System konzipiert sein. Bei der Verwendung eines Hand-held-Systems in Röntgenaufnahmen kann wegen der Realtime-Fahigkeit die Position des Systems direkt in 3D-Daten eingeblendet werden. Die Registrierung geschieht dabei einfach durch die Kombination der Rontgenmarker mit den Navigationsmarkern und einer 2D/3D- Registπerung .
In einer weiteren vorteilhaften Ausfuhrungsform ist das Instrument ein in einen Patienten einbringbares Implantat. Ein Implantat kann hierbei z. B. auch eine Prothese sein. Eine entsprechende Prothese oder ein Implantat wird in der Regel fest in einem Patienten platziert. Das Instrument hat dann eine definierte geometrische Beziehung zu Strukturelementen des Patienten, z. B. Gewebeteilen oder Knochen. Die Patien- tenposition steht damit stets in fester Relativposition zum Implantat und mit dessen Position stets im Navigationssystem bekannt .
Steht die Geometrie des Instruments, insbesondere auch wenn dieses ein Implantat ist, als Datensatz zur Verfugung, so kann ein virtuelles Abbild des Instruments oder Implantats ebenfalls in Echtzeit ortsgetreu in die verfugbaren, registrierten Bilddatensatze integriert werden.
Für das Medizinsystem sind eine Vielzahl klinischer Anwendungen denkbar, unter Anderem z. B. die muskuloskelettale Chirurgie, Knie- und Huftendoprothetik, Frakturversorgung mittels Osteosynthese mit Hilfe von Platten, Markennagelung,
Verschraubung sowie Wirbelsauleneingriffe, wie Pedikel- verschraubung, Bandscheibenersatz etc.
Denkbar sind auch Anwendungen in der Mund, - Kiefer- oder Ge- Sichtschirurgie, Hals-Nasen-Ohrenchirurgie oder Gefaßchirurgie, in Form eines Stents oder einer Endoprothesenplatzierung bzw. einer Katheternavigation. Das Medizinsystem ist nützlich bei der Navigation von Endoskopen in der flexiblen Endoskopie, wie in der Bronchoskopie, Gastroskopie sowie bei der starren Endoskopie, z. B. der Arthroskopie. Mit dem Medizin¬ system ist außerdem eine Navigation von interventionellen Kathetern und Nadeln, z. B. bei der Vertebroplastie, Kyphoplastie oder der HF-Ablation denkbar.
Für eine weitere Beschreibung der Erfindung wird auf das Aus- fuhrungsbeispiel der Zeichnung verwiesen. Es zeigt in einer schematischen Prinzipskizze:
Fig. 1 ein Medizinsystem gemäß der Erfindung.
Fig. 1 zeigt ein Medizinsystem 2, welches im Wesentlichen ein elektromagnetisch arbeitendes Navigationssystem 4 und ein Rontgensystem 6 in Form eines Röntgen C-Bogens sowie ein me¬ dizinisches Instrument 8 umfasst. Mit Hilfe des Instruments 8 wird an einem Patienten 10 eine medizinische Maßnahme, hier ein interventioneller Eingriff, durchgeführt. Erfindungsgemaß ist am Instrument 8 sowohl ein vom Navigationssystem 4 ortba¬ rer Navigationsmarker 12 als auch ein Rontgenmarkersystem 14 fest angebracht. Deren Relativposition zueinander wurde in einer Kalibrierprozedur bei der Herstellung des Instruments einmalig aufgenommen bzw. festgelegt. Die Relativposition ist durch einen Ortsvektor R symbolisiert.
Das Navigationssystem 4 spannt ein an diesem ortsfestes Koor- dmatensystem N auf. Wahrend des medizinischen Eingriffs wird nun vom Patienten 10 mit Hilfe des Rontgensystems 6 ein Röntgenbild 16 angefertigt, welches einen Teil des Patienten 10 und das Instrument 8 zeigt. Die Position der Navigationsmar-
ker 12 im Koordinatensystem N ist durch die Erfassung durch das Navigationssystem 4 bekannt. Über den Ortsvektor R ist daher auch die Lage des Rontgenmarkersystems 14 im Koordinatensystem N im Moment der Aufnahme des Röntgenbildes 16 be- kannt. Da gleichzeitig das Rontgenmarkersystem 14 im Röntgenbild 16 sichtbar sind, ist somit auch die ortsrichtige Zuord¬ nung des Röntgenbildes 16 im Koordinatensystem N möglich. Somit sind sämtliche Komponenten des Medizinsystems 2 zueinander ortsrichtig registriert und der navigationsgestutzte Em- griff am Patienten 10 kann sofort erfolgen.

Claims
1. Medizinsystem (2), mit einem elektromagnetisch oder optisch arbeitenden Navigationssystem (4), mit einem Rontgen- System (6), und mit einem medizinischen Instrument (8), an dem ein Rontgenmarkersystem (14) und ein Navigationsmar- ker (12) des Navigationssystems (4) in bekannter Relativposi- tion (R) zueinander fest angebracht sind.
2. Medizinsystem (2) nach Anspruch 1, bei dem das Instrument (8) eine an einem Patienten (10) fixierbare Klemme ist.
3. Medizinsystem (2) nach einem der vorhergehenden Ansprüche, bei dem das Instrument (8) ein Interventionswerkzeug ist.
4. Medizinsystem (2) nach einem der vorhergehenden Ansprüche, bei dem Instrument (8) ein in einen Patienten (10) einbringbares Implantat ist.
