Description

방사선 치료용 표적 표시자
본 발명은 대체로 표적 표시자에 관한 것으로, 보다 구체적으로는 방사선 치료용 표적 표시자에 관한 것이다.
오늘날, X선 컴퓨터 단층 촬영(Computed Tomography, 이하 CT) 기기나 자기공명영상(MRI) 기기, 양전자 단층촬영기기들로부터 얻어지는 인체 내부의 영상들은 임상의가 수검자의 이상(abnormality) 유무를 판단하기 위한 중요한 수단으로 이용되고 있다. 특히, 암과 같은 중대한 이상이 진단된 경우에, 그 이상을 제거 또는 사멸시키기 위해 환자에게 화학요법, 수술 또는 방사선 요법 등과 같은 적절한 치료를 받을 수 있도록 하고 있다.
화학요법은 약물을 사용하여 종양세포를 사멸하는 치료법이다. 수술은 외과적인 절개와 절단을 통해 종양 및 주변의 의심부위를 분리, 제거하는 치료방법이다.
방사선 요법은 종양에 대한 진단 영상을 바탕으로 치료방사선을 종양에 집중적으로 조사하여 종양세포를 사멸하는 치료법이다. 이때 종양 주위의 정상 조직은 치료방사선으로부터 보호하여 치료에 의한 부작용을 최소화하는 것이 필수적이다. 따라서 방사선 요법에서 종양의 위치를 시각화하는 기술은 치료의 효과를 높이는데 매우 중요한 역할을 차지한다. 현재, 일반적으로 종양의 위치를 정확히 알기 위해 작은 크기의 금이나 티타늄, 탄탈륨과 같은 금속을 종양에 여러 개 심어두고 그 위치를 표시하며, 일반적인 2차원 진단용 X선 영상에서 그 위치를 쉽게 확인 가능하다.
본 발명이 해결하고자 하는 기술적 과제는 첫째, 진단용 X선 영역에서의 식별성이 우수하여야 한다. 둘째, 치료용 방사선에 의해 종양에 전달되는 선량의 왜곡을 최소화하여야 한다. 셋째, CT 기반 치료계획 수립시 CT영상의 왜곡을 최소화 할 수 있어야 한다. 넷째, 인체 내에서의 표적 표시자의 이동을 최소화 하여야 한다.
상기 기술적 과제를 해결하기 위하여 본 발명은 방사선 치료용 표적 표시자(fiducial marker)를 제공한다. 상기 방사선 치료용 표적 표시자는 금속입자와 방사선 비투과성 폴리머를 포함하고 방사선 치료 시 체내에서 환부의 위치를 표시한다.
본 발명의 표적 표시자는 진단용 X선(MV 영역 포함) 영상에서도 충분히 식별 가능하다. 치료용 방사선에 의해 종양에 전달되는 선량의 왜곡을 최소화할 수 있다. CT 영상을 기반으로 한 치료계획 수립 시 CT 영상에서의 왜곡을 최소화할 수 있다. 인체 내에서 표적 표시자의 이동을 최소화하는 형태로 쉽게 제작할 수 있다.
도 1은 일 비교예에 따른 표적 표시자에 의해 발생하는 양성자 빔의 선량 감소 현상을 나타내는 도면이다.

도 1의 (b)는 일 비교 예에 따른 각각 지름 1.25 mm, 길이 3 mm의 기존 표적 표시자에 의해 발생하는 선량 변동(dose perturbation)에 대해 인체골반모형 내의 표적 표시자 삽입 깊이와 실험된 상대적 흡수선량과의 함수관계를 나타내고 있다. 각각의 표적 표시자는 금, 스테인레스 스틸 그리고 티타늄으로 각각 만들었으며, 비교를 위해 선량변동이 발생하지 않을 때의 깊이 선량 그래프(control)도 표시하였다. 도 1의 (a) 및 (b)를 참조하면 기존의 표적 표시자 후방에 위치한 종양부위에는 치료방사선의 선량이 충분히 전달되지 않는 것을 확인할 수 있다. 연구보고에 따르면 X선 치료의 경우 20%의 선량감소를 나타내고 양성자 치료에서는 선량감소 폭이 더욱 커져서 80%에 달한다.
도 2의 (a)는 일 비교 예에 따른 순도 99.9 % 금 표적 표시자의 사진이다. 도 2의 (b)는 순도 99.9 % 금 표적 표시자를 환자의 환부에 삽입하였을 때 환자의 CT영상에 나타나는 왜곡을 나타내는 그림이다. 도 2의 (b)를 참조하면, 환자의 CT 영상에서는 점선원 내부와 같이 표적 표시자 주변에 강한 CT 영상왜곡이 발생하고 있다. 이처럼 왜곡된 CT영상을 기반으로 치료계획을 수립하는 경우 계획된 치료와 실제 치료 사이에는 상당한 차이가 있을 수 있다.
본 발명의 일 실시 예에 따른 방사선 치료용 표적 표시자는 상술한 비교 예와 대비하여, 인체에 무해한 방사선 비투과성 고분자 및 소정의 중량비를 갖는 무독성 금속입자들을 포함한다.
상기 방사선 비투과성 고분자는, 일례로서, PMMA (polymethyl methacrylate), 실리콘 (silicone), 폴리우레탄 (polyurethane), 테플론 (Teflon), 하이드로젤 (hydrogel), 하이드록시 아파타이트 (hydroxyapatite) 로 구성되는 그룹에서 선택된 어느 하나에 BaSO4, ZrO2 또는 요오드(iodine)로 구성되는 그룹에서 선택된 어느 하나를 결합시킨 것일 수 있다.
상기 금속입자는 일례로서, 인체에 무해한 금, 백금, 티타늄, 탄탈륨, 스테인레스 스틸 등 일 수 있다. 상기 금속입자의 크기는 1 나노미터 이상 50 마이크로미터 이하일 수 있다. 상기 금속입자들은 본 발명의 방사선 치료용 표적 표시자 내에서 중심부에 모여있거나 균일하게 분산되어 존재할 수 있다. 상기 금속입자들은 1.0 wt% 이상 70 wt% 이하의 중량비(wt%)를 가질 수 있다.
일 실시 예에 있어서, 상기 방사선 치료용 표적 표시자의 상기 방사선 비투과성 고분자와 상기 금속입자들은 방사선의 종류 및 에너지에 따라 차등적으로 상호작용을 할 수 있다. 즉, 진단용 에너지 영역의 X선과는 광전자 상호작용을 통해 방사선 치료용 표적 표시자의 위치를 명확히 표시해 주면서, 치료방사선인 양성자빔이나 탄소빔, MV(megavoltage)에너지 영역의 X선과는 상대적으로 상호작용을 적게 하여 치료 선량에는 영향을 미치지 않을 수 있다. 특히, 방사선 비투과성 고분자 물질에 첨가하는 금속입자의 종류, 크기 또는 함량비를 조정하여 진단방사선 및 치료방사선과의 상호작용 정도를 조절할 수 있다. 이를 통해 비교 예의 표적 표시자가 가지는 문제점인 치료방사선의 선량 감소와 방사선의 빛번짐(artifact)에 의한 CT 영상의 왜곡 현상을 극복 할 수 있다. 그리고, 방사선 비투과성 물질과 금속입자를 혼합하여 제조함으로써, 자유로운 성형성으로 인해 다양한 형태로의 제작이 용이하여 각종 종양에 대한 접근성이 우수할 뿐 아니라, 움직이는 종양 내에서 표적 표시자의 위치이동을 억제할 수 있다.
이하, 구체적인 실시 예 및 비교 예를 통해 본 발명의 구성 및 효과를 보다 상세히 설명한다. 다만, 이들은 본 발명의 사상을 보다 명확하게 이해시키기 위한 것일 뿐 본 발명의 범위를 한정하고자 하는 것은 아니다.
금속입자를 포함한 방사선 비투과성 고분자 물질이 종양 표시물질로 사용되기 위해서는 기본적으로 진단용 X선을 조사(照射)하였을 때 종양 주변의 뼈들과 구별이 될 정도의 충분한 명암대비를 가져야 한다.
도 3의 (a)는 일 비교 예로서의 순도 99.9 % 금 표적 표시자(300) 와 본 개시의 실시 예들로서 방사선 비투과성 고분자와 금 입자를 포함하는 표적 표시자들(310, 320, 330)에 대한 사진이다. 상기 표적 표시자들(310, 320, 330)은 PMMA(poly methyl methacrylate)에 BaSO4 를 함유하는 방사선 비투과성 고분자 및 소정 중량비를 가진 금 입자를 포함하고 있다. 각각 1.7 wt%, 3.4 wt%, 5.0 wt%의 금(Au) 중량비를 가진 표적 표시자들(310, 320, 330)을 준비하였다. 도 3의 (b)는 일 실시 예에 있어서, 순도 99.9 % 금 표적 표시자(302)와 상기 표적 표시자들(312, 322, 332)을 모형뼈(305)에 적층하여 촬영한 2차원 X선 영상이다. 도 3의 (b)를 참조하면, 순도 99.9 %금 표적 표시자(302)에 비해 매우 낮은 금 중량비를 가지고 있음에도 불구하고 상기 표적 표시자(312, 322, 332)들은 모형뼈(305)와 확연히 구분되는 명암대비를 가지고 있음을 확인할 수 있다. 또한 방사선 비투과성 고분자를 포함하고 있는 본 개시의 표적 표시자에 MV 영역의 X선을 조사하는 경우 방사선 투과성 고분자를 포함하고 있는 표적 표시자에 비하여 명암대비가 보다 선명한 영상을 얻을 수 있다. 따라서, 일례로 6MV의 X선을 사용하여 영상을 획득하는 EPID (Electronic Portal Imaging Device)나 토모테라피의 MVCT(Megavoltage Computed Tomography)에 사용하는 것이 가능하다.
도4는 CT 스캐너를 이용하여 촬영한, 각각 순도 99.9 % 금 표적 표시자(도 4(a)), 1.7 wt% 금 표적 표시자 (도 4(b)), 3.4 wt% 금 표적 표시자 (도 4(c)) 및 5.0 wt% 금 표적 표시자 (도 4(d))의 CT영상이다. 도 4를 참조하면, 발생된 CT 영상왜곡은 기존의 금 표적 표시자에서 그 범위가 가장 넓고, 표적 표시자의 금 입자 중량비가 줄어들수록 CT 영상왜곡의 범위가 줄어드는 것을 알 수 있다. 따라서 금 입자의 중량비를 적절히 조절함으로써 CT 영상왜곡을 충분히 줄일 수 있음을 확인하였다.
도 5와 도 6은 표적 표시자를 양성자빔의 SOBP (Spread-out Bragg Peak)영역의 중간에 놓아두고 그 후면에서의 선량감소를 각각 선량평가용 필름을 이용하여 얻은 결과(도 5)와 공간 분해능이 우수한 다이오드 디텍터 (Scanditronix 사)를 사용하여 측정한 결과(도 6)를 나타낸다. 도 5은 도 3에서 보여진 순도 99.9 % 금 표적 표시자(300) 및 각각 1.7 wt%, 3.4 wt%, 5.0 wt%의 금을 함유하는 표적 표시자(310, 320, 330)를 수조에 담그고 양성자빔의 진행방향과 나란히 GafChromic EBT (ISP Inc.) 필름을 설치하여 깊이별 선량분포를 측정한 결과이다. 도 5의 (a)를 참조하면 원기둥 모양 표적 표시자(510)의 대칭축에 수직한 방향으로 양성자빔(500)을 조사하는 모습이다. 도5의 (b)를 참조하면 원기둥 모양 표적 표시자(510)의 대칭축과 평행한 방향으로 양성자빔(500)을 조사하는 모습이다. 도 5(c)를 참조하면 상기 원통형 표적 표시자의 대칭축과 수직하게 양성자빔(500)을 조사하였을 때 표적 표시자의 후방에 저선량 영역이 발생하는 모습이다. 도 5(d)를 참조하면 상기 원통형 표적 표시자의 대칭축과 평행하게 양성자빔(500)을 조사하였을 때 표적 표시자의 후방에 저선량 영역이 발생하는 모습이다.
도 6은 필름대신 다이오드 디텍터(Scanditronix 사)를 사용하여 깊이별 선량분포를 정량적으로 측정한 결과를 나타낸다. 도 6(a)는 본 개시의 원통형 표적 표시자의 대칭축과 수직하게 양성자빔을 조사하였을 때 깊이별 선량분포를 정량적으로 측정한 결과이다. 도 6(b)는 본 발명의 원통형 표적 표시자의 대칭축과 평행하게 양성자빔을 조사하였을 때 깊이별 선량분포를 정량적으로 측정한 결과이다. 실험에 사용한 양성자빔 에너지는 213 MeV 이고 SOBP는 8 cm이다. 도5및 도 6을 참조하면, 기존의 순도 99.9 % 금 표적 표시자 뒤쪽으로 수 cm 정도에서는 치료방사선의 선량이 충분히 전달되지 못해서 저선량 영역이 생기는 반면, 각각 1.7 wt%, 3.4 wt%, 5.0 wt%의 금(Au) 중량비를 가진 표적 표시자 뒤쪽에는 저선량 영역이 거의 생기지 않는다. 이것은 상기 표적 표시자 후방으로도 치료방사선에 의해 선량이 충분히 전달되고 있음을 의미한다.
도7은 본 발명의 체내 삽입형 표적 표시자의 외형에 대한 실시 예들을 보여준다. 도 7을 참조하면, 상기 표적 표시자는 고분자 물질을 바탕으로 하기 때문에 원기둥(도 7(a)), 타원체(도 7(b)), 표면에 돌기가 형성된 구형(도 7(c)), 비대칭 입체모양(도 7(d)) 등 다양한 형태로 제작이 용이하다. 특별히 체내에서의 이동이 억제되는 형태로도 제작이 간편하다. 기본적으로 표적 표시자는 표면적을 최대화하여 종양 또는 주변 조직과의 접촉면을 넓히는 형태를 가진다. 상기 표적 표시자들은 대칭형과 비대칭형으로 나눌 수 있다. 비대칭 형태의 표적 표시자들은 종양의 변위에 대해 대칭 형태보다 더 많은 정보를 제공할 수 있다.
도 8은 본 발명의 일 실시 예로서 표적 표시자를 제조하는 방법을 표현한 도면이다. 도 8을 참조하면, 금 입자, 방사선 비투과성 고분자 분말 및 액체 단량체(liquid monomer)를 혼합한다. 상기 혼합물을 성형틀 내부에서 원기둥 형태로 성형하고 경화시킨다. 경화된 혼합물을 일정한 길이로 절단하여 표적 표시자를 제조한다. 혼합하는 금 입자의 중량비를 달리하여 X선 촬영시 사진의 명암비를 조절할 수 있다.
표적 표시자를 인체 내에 넣는 방법은 위에서 언급한 삽입형 표적 표시자를 체내에 삽입하는 방법과 액체나 젤(Gel) 형태의 표적 표시자를 주입하는 방법이 있다. 삽입형 표적 표시자들은 체외에서 이미 경화된 상태로 인체에 넣어지지만, 주입형 표적 표시자들은 액체나 젤 상태에서 주사기 등과 같은 기구를 통해 체내에 일정량만큼 주입된 후 체내에서 경화되는 특징을 갖는다. 주입형 표적 표시자들은 고분자물질이 공기 중에서 경화되는 속도를 완화함으로써 사용가능하다.
상기 주입형 표적 표시자는 방사선 비투과성 고분자 및 액체 단량체(monomer)를 포함할 수 있다. 상기 방사선 비투과성 고분자는 분말 형태일 수 있다. 각각의 방사선 비투과성 고분자 분말은 금속입자를 포함할 수 있다. 상기 액체 단량체는 첨가중합반응을 통하여 방사선 비투과성 고분자 분말끼리 결합시킬 수 있다. 액체 단량체는 PMMA(poly methyl methacrylate), 실리콘 (silicone), 폴리우레탄 (polyurethane), 테플론(Teflon), 하이드로젤 (hydrogel) 또는 하이드록시 아파타이트(hydroxyapatite)일 수 있다.
이상에서 살펴본 바와 같이 본 발명을 다양한 구현 예를 들어 상세히 기술하였지만, 본 발명이 속하는 기술분야에 있어서 통상의 지식을 가진 사람이라면, 첨부된 청구 범위에 정의된 본 발명의 정신 및 범위를 벗어나지 않으면서 본 발명을 여러 가지로 변형하여 실시할 수 있을 것이다. 따라서 본 발명의 앞으로의 구현 예들의 변경은 본 발명의 기술을 벗어날 수 없을 것이다.

Claims (16)

  1. 금속입자; 및
    방사선 비투과성 고분자를 포함하고, 방사선 치료 시 체내에서 환부의 위치를 표시하는 방사선 치료용 표적 표시자.
  2. 제 1 항에 있어서,
    상기 금속입자는 금(Au), 백금(Pt), 탄탈륨, 티타늄(Ti) 및 스테인레스 스틸 로 구성되는 그룹에서 선택된 어느 하나인 방사선 치료용 표적 표시자.
  3. 제1항에 있어서,
    상기 금속입자는 크기가 1 nm이상 50 μm 이하인 방사선 치료용 표적 표시자.
  4. 제1항에 있어서,
    상기 금속입자의 중량비는 1 wt% 이상 70 wt% 이하인 방사선 치료용 표적 표시자.
  5. 제 1 항에 있어서,
    상기 방사선 비투과성 고분자는 PMMA(poly methyl methacrylate), 실리콘 (silicone), 폴리우레탄 (polyurethane), 테플론(Teflon), 하이드로젤 (hydrogel) 및 하이드록시 아파타이트(hydroxyapatite)로 구성되는 그룹에서 선택된 어느 하나에 BaSO4, ZrO2 및 요오드(I)로 구성되는 그룹에서 선택된 어느 하나를 결합시킨 방사선 치료용 표적 표시자.
  6. 제 1 항에 있어서,
    상기 금속입자가 상기 방사선 비투과성 고분자 내에 분산되어 존재하는 방사선 치료용 표적 표시자.
  7. 제 1 항에 있어서,
    상기 방사선 치료용 표적 표시자는 고체 상태로 체내에 삽입하는 삽입형인 방사선 치료용 표적 표시자.
  8. 제 7 항에 있어서,
    상기 방사선 치료용 표적 표시자는 대칭 형태를 가지는 방사선 치료용 표적 표시자.
  9. 제 7 항에 있어서,
    상기 방사선 치료용 표적 표시자는 비대칭 형태를 가지는 방사선 치료용 표적 표시자.
  10. 제7항에 있어서,
    상기 방사선 치료용 표적 표시자의 표면에 주름이나 돌기가 형성되는 방사선 치료용 표적 표시자.
  11. 제 9 항에 있어서,
    상기 방사선 치료용 표적 표시자는 환부의 변위에 대한 정보를 제공해 주는 방사선 치료용 표적 표시자.
  12. 제 1 항에 있어서,
    상기 방사선 치료용 표적 표시자는 액체나 젤(Gel) 상태로 체내에 주입하는 주입형인 방사선 치료용 표적 표시자.
  13. 제 12 항에 있어서,
    상기 방사선 비투과성 고분자는 분말형태이고 각각의 고분자 분말은 금속입자를 포함하고 있는 방사선 치료용 표적 표시자.
  14. 제 12 항에 있어서,
    상기 방사선 치료용 표적 표시자는 체내에서 경화되는 방사선 치료용 표적 표시자.
  15. 제 12 항에 있어서,
    상기 방사선 비투과성 고분자를 결합하고 경화시키는 액체 단량체(monomer)를 더 포함하는 방사선 치료용 표적 표시자.
  16. 제 15 항에 있어서,
    상기 액체 단량체는 PMMA(poly methyl methacrylate), 실리콘 (silicone), 폴리우레탄 (polyurethane), 테플론(Teflon), 하이드로젤 (hydrogel) 및 하이드록시 아파타이트(hydroxyapatite)로 구성되는 그룹에서 선택된 어느 하나인 방사선 치료용 표적 표시자.
