Description
COMPOSITION DE CIMENT DENTAIRE A BASE DE CIMENT DE FORTLAND
La présente invention a pour objet une nouvelle composition de ciment dentaire à base principalement de ciment Portland pour le traitement des perforations et des obturations a rétro des racines dentaires et le coiffage de la pulpe dentaire.
Dans le cas où l'obturation étanche des sorties canalaires est difficile et qu'une chirurgie endodontique est nécessaire, ou dans les cas d'apex ouvert, immature ou de perforation, le dentiste a recours à différents produits conventionnels qui ne répondent pas à toutes les exigences requises afin d'assurer une obturation étanche des sorties canalaires.
En endodontie, une large variété de ciments et de produits est utilisée pour le nettoyage, la désinfection et l'obturation étanche des canaux radiculaires et l'isolation complète des sorties canalaires des tissus environnants.
Des ciments de ce type ont été décrits par exemple dans les documents US- A-5 415 547, 5 769638 et WO 2008/102214.
Les ciments de ce type sont utilisés dans l'obturation des racines dentaires, principalement pour fermer une barrière à l'infection par des microorganismes.
Ils doivent en outre offrir toutes les garanties exigibles de biocompatibilité pour des matériaux devant rester à demeure dans la bouche des patients. Les ciments de l'art antérieur donnent satisfaction sur ces points mais présentent néanmoins un certain nombre d'inconvénients tels que : une prise très retardée, après adjonction d'eau, ce qui oblige le praticien à ajourner la restauration coronaire jusqu'à prise définitive ; un état mécanique sableux qui rend la manipulation difficile. L'invention a pour objet de fournir un ciment présentant les mêmes qualités de biocompatibilité, de non cytotoxicité et d'herméticité, tout en permettant : un temps de prise court, un malaxage et une manipulation aisés du fait d'un état pâteux et non sableux.
Conformément à l'invention, ce résultat est obtenu avec une composition de ciment dentaire à base, principalement, de ciment de Portland, de type comportant également du carbonate de calcium et un matériau radio opaque, caractérisé en ce qu'il comporte en outre du gypse naturel et du chlorure de Baryum.
Avantageusement, le ciment selon l'invention aura la composition suivante : ciment (ou clinker) de Portland : 53 à 84.5 % gypse naturel : 0.5 à 3.5 % carbonate de calcium : 10 à 15 % - chlorure de Baryum : 5 à 18 % les pourcentages étant exprimés en poids.
Ce nouveau ciment se caractérise par un malaxage et une manipulation clinique faciles et un temps de prise court réduisant ainsi le temps de travail pour le dentiste avant son intervention sur la zone à traiter. Ces caractéristiques sont dues aux composants principaux, à savoir : du clinker de Portland blanc ou noir, du gypse naturel ou autres matériaux comme le gypse parisien ou le carbonate de calcium, le carbonate de calcium, - le chlorure de Baryum en tant que matériau radio opaque.
Les caractéristiques du nouveau ciment dépendent non seulement de la composition finale mais en grande partie de la technique de préparation qui est décrite ci-après.
Les différents matériaux sont broyés séparément à l'aide d'un broyeur spécial permettant d'homogénéiser les particules et de les rendre très fines. Ils sont tamisés par la suite avec un tamis de moins de 90 microns puis mélangés à des proportions bien spécifiques. La poudre obtenue est stérilisée avant d'être utilisée.
Le tamisage est effectué à une valeur de 4000-5500 cm2/g (indice Blaine). De manière préférentielle, le ciment selon l'invention aura la composition suivante : clinker de Portland : 64 % gypse naturel : 3 % carbonate de calcium : 15 % chlorure de Baryum : 18 % Le nouveau ciment élaboré est un ciment utile en endodontie possédant les caractéristiques optimisées. Il est indiqué dans le traitement d'apexogenèse et d'apexification, le traitement des perforations et l'obturation a rétro.
II répond aux propriétés requises d'un matériau dentaire : scellement hermétique du système canalaire biocompatibilité stimulation de la régénération des tissus périapicaux - durcissement sans être affecté par l'humidité stabilité volumétrique pas de corrosion, ni d'activité électrochimique pas de coloration de la dent, ni des tissus adjacents manipulation clinique aisée - opacité radiologique différente de celle de la dentine
II est conditionné en sachet prédosé de poudre, avec une unité d'eau stérile dont la quantité est calculée afin d'obtenir la bonne consistance pour la manipulation. Cette poudre à base de ciment de Portland est constituée de fines particules hydrophiles d'oxydes minéraux auquel on a ajouté du carbonate de calcium et du chlorure de baryum radio opaque. Ses principales composantes sont des silicates tricalciques, des aluminates tricalciques, des oxydes tricalciques et des oxydes silicates. L'hydratation de la poudre avec de l'eau stérile forme un gel de consistance pâteuse qui se solidifie en milieu humide en une structure dure.
Claims
1. Composition de ciment dentaire à base, principalement, de ciment de Portland, de type comportant également du carbonate de calcium et un matériau radio opaque, caractérisé en ce qu'il comporte en outre du gypse naturel et du chlorure de Baryum.
2. Composition de ciment dentaire selon la revendication 1, caractérisée en ce qu'il a la composition suivante :
- ciment (ou clinker) de Portland : 53 à 84.5 % - gypse naturel : 0.5 à 3.5 %
" carbonate de calcium : 10 à 13 %
• chlorure de Baryum : 5 à 18 % les pourcentages étant exprimés en poids.
3. Ciment dentaire selon l'une quelconque des revendications 1 et 2 caractérisé en ce qu'il a la composition suivante :
- clinker de Portland : 64% " gypse naturel : 3 %
" carbonate de calcium : 15 %
• chlorure de Baryum : 18%
