Abstract
Laminectomy Retractor permits insertion of retraction blades without damaging the muscles and other tissues. This instrument also provides a clear field of view for an operation site, and with adequate and satisfying muscle retraction. Muscle retraction time and so overall operative time is shortened, reducing the time-related muscle ischemia and anesthesia exposure with their drawbacks. The instrument also minimizes the muscle injury because of the specially designed prongs. Laminectomy Retractor also reduces the drawbacks of post-operative pain and instability related to muscle injury and ischemia. While in use, Laminectomy Retractor avoids upward positioning of retractor shaft or hinges that obstruct the space used for manipulating other instruments. Change of position of the retractor's shaft and arms from one operative wound end to the other without removal and reapplication is facilitated with this improved instrument.
Description
LAMINECTOMY RETRACTOR Technical Field :
Currently many self retaining retractors are used in posterior spinal surgeries for muscle retraction and separation. The retractors have many different shapes including the hinged arms types.
Background Art :
Currently, the retractor that is utilized in Laminectomy surgical procedures have several drawbacks and shortcomings. The right arm- prongs of the instrument angle, keeping the retractor's prongs running oblique to the muscle fibers' direction which causes direct injury to muscle fibers. Also, this obliquity affects the adequacy of retraction escaping some muscle bulk, which may indicate excision of the field, obstructing portions and adding more injury to paraspinal muscles. Depending on the depth of the surgical bed, the retractor shafts or arms at hinge may protrude upward partially obstructing the space used during manipulating other instruments. During surgical procedures change of the retractor position from one wound end to the opposite end is needed sometimes which indicates release and reapply of the retractor causing new injury to the muscles as described.
Disclosure of Invention :
• Idea:
Separation of prongs from arms in the currently used hinged arms laminectomy retratctors.
Creation of distal articulation (second hinge) connecting arms to prongs.
• End Result:
Double hinged arms laminectomy retractor with distal articulation after creation of distal arms - prongs articulation (second hinge) with moving range of 180 degrees.

Best Mode for Carrying Out the Invention:
Yasho double hinged arms laminectomy retractor could be used in paraspinal muscle separation step during posterior spinal surgeries for disc surgeries tumors resection and spinal fixation surgeries.

Claims
1-Yasho double hinged arms laminectomy retractor with distal articulation can give us the following advantages:
2- The addition of distal articulation (arm-prongs hinge), will make it easier to adjust prongs to be Perpendicular to the ms fiber direction which facilitate adequate retraction process with no adding injury to ms fibers, which permits insertion of retracting blades without damage to the tissue.
3- On the same way, the satisfactory ms retraction improves surgical field exposure with no protruding ms which may be excised eliminating another way of adding ms injury, which provides a clear field of view of an operation site.
4- Both above mentioned advantage, beside eliminating adding ms injury, also improving surgical field exposure and indirectly shortening operative time avoiding long ms retraction with its ischaemic drawbacks.
5- Distal articulation (arm-prongs hinge), will enable us to appropriately adjust the position of the retractors shafts and arms, eliminating the undesired upward positioning influenced by variations of surgical field depth avoiding partially obstruction of the field space using for manipulating other instruments as described above, and also reducing duration of ms retraction and related ischaemic effect with its drawbacks.
6-Creating distal articulation (arm-prongs hinge) moving angle of 180 range enabled surgeon to change the position of the shaft from one end of the incision to the opposite end without removal and re-apply.
