- Limb lengthening using Ilizarov technique and external fixation is a well known technique and used for treatment of many complicated conditions as congenital and post traumatic bone deformity , short stature treatment, bone infection and nonunion, after removal of tumors to reconstruct or lengthen the bone and soft tissues.
- the technique is by creating a surgical fracture (corticotomy) in the bone followed after 5-10 days from surgery by gradual lengthening with a rate from half to one millimeter per day till the short bone reach the required length.
- the lengthening is done by the use of external fixation which is bulky with pins attached to the bone and coming out of the skin. Then the fixation need to stay till full consolidation of the new soft bone that was formed and full calcification which usually take about double the duration of lengthening. So for example for 7 centimeters lengthening the external fixation need to stay for about 7 months or more.

Abstract
Bone lengthening using gradual distraction proved to be effective technique in treatment of many orthopaedic problems like congenital and pediatric bone problems and old maleunited fractures and after tumor surgery and many other bone diseases. The present application refers to a device for lengthening bone. The device composed of a motor that controlled from out side the body by control unit, and a telescopic plate (4) that fixed to the bone (1 ) through intermediate plates (2) by four-six screws (5). Control unit which consist of a remote control and an electronic chip attached with the telescopic plate (4).

Limb lengthening using Ilizarov technique and external fixation is a well known technique and used for treatment of many complicated conditions as congenital and post traumatic bone deformity , short stature treatment, bone infection and nonunion, after removal of tumors to reconstruct or lengthen the bone and soft tissues. The technique is by creating a surgical fracture (corticotomy) in the bone followed after 5-10 days from surgery by gradual lengthening with a rate from half to one millimeter per day till the short bone reach the required length. The lengthening is done by the use of external fixation which is bulky with pins attached to the bone and coming out of the skin. Then the fixation need to stay till full consolidation of the new soft bone that was formed and full calcification which usually take about double the duration of lengthening. So for example for 7 centimeters lengthening the external fixation need to stay for about 7 months or more.
Complications of Ilizarov external fixation is well know including the pin infection which could extend to the bone with secondary bone infection, bulky device with marked discomfort with every movement & this lead to muscle wasting and joint stiffness, also the patient cant go with normal daily life as clothes, sleeping, personal relation ships, and many other psychological stress.
So despite this effective technique the side effects make it difficult for many patients to seek this type of treatment due to the heavy price they need to pay for it from there lives.
To improve these defects many trials were to make bone lengthening easier for the patient. This is now available in the form of intramedullary lengthening nails. The nails are either motorized with external remote control (eg. Fitbone), or mechanical nails (eg ISKD). The lengthening nails can be more comfortable to the patient.
Disclosure of Invention:
Complications of internal lengthening nails: But there are many prerequisites need to be available in the patient to fit the criteria needed to put these nails. The criteria include: wide bone medulla to put these large nails inside, no bone deformity or curve, skeletally mature bone as in adults because the nail will destroy the growth plate in children bone. Also the insertion technique is very difficult with many risks because the nails are strait and with large diameter while any bone has some normal curve or even a pathological curve. Another common problem is the mechanical failure of the nail or nail jamming. Also these nails are very expensive (Fitbone nail is about 15 000 US Dollars, just for the nail, the hospitalization and surgery and other treatment is not included)
Our design of an internal lengthening plate can solve these problems with a device that is put internally inside the limb on the surface of the bone and remote controlled from out side. This can help a wide variety of patients including kids and small bone size, who are the most common type of patients who need this treatment type. Description of our design; It is composed of
1- internal lengthening device with a motor,
2- This device is put in a telescopic frame.
3- The frame is secured & fixed to a plate on the bone surface.
4- The plate is secured to the bone by plates screws. This gives maximum stability and versatile to be applied to any type of bone.
Components:
It consists of four main parts and fixation bolts. The main four parts is:
• E-plate frame: (It is a telescopic frame which included the thrust motor. And it fixed in the bone with intermediate plate by small screws.)
• Bone fixation plate (It is the intermediate part which make the surgery easier because the doctor will fix the two small bone fixation plate first and then he will fix the E-plate)
• Thrust motor (It is a special small motors connected to telescopic power crew which is responsible for the elongation.)
• Control unit (It consists of two parts first one is the remote control which will control the internal system and it used by the patient, second part is a small electronic ship and it will be attached with the E-Plate. This part will be the method to transfer data from the remote control and the motor.)
Fixation method:
Inside the leg and out side the bone as shown in figures below.
Assembly method:
1- Make three holes in the bone.
2- Fix the two small fixation plates in the bone using bone fixation bolts.
3- Fix the E-plate in the two fixation plates by using small screws.
4- Adjust the bone gab by using remote control.
