Brief Description of the Drawings
FIG. 1 is a view showing a configuration of a clinical pathway information service system according to a preferred embodiment of the invention.
FIG. 2 is a patient graph according to a preferred embodiment of the invention. FIG. 3 is a view showing an example of a general patient information extracting method according to a preferred embodiment of the invention.
FIG. 4 is a view showing an example of classification of prescription data according to a preferred embodiment of the invention.
FIG. 5 is a view showing a classification table for effects of medicine according to a preferred embodiment of the invention.
Mode for Carrying Out the Invention Hereinafter, preferred embodiments of the invention will be described in detail with reference to the accompanying drawings. First, a configuration of a clinical pathway information service system according to a preferred embodiment of the invention will be described with reference to FIG. 1. The clinical pathway information service system includes a control module 100, a database 112 and a user interface 114.
In accordance with the preferred embodiment, the control module 100 produces clinical pathway information from medicine information registered with the database 112. To this end, the control module 100 includes software modules of a patient information extracting unit 102, a prescription information extracting unit 104, a prescription information statistic processing unit 106, a medical treatment pattern selecting unit 108 and a clinical pathway information servicing unit 110.
<Patient information extracting unit> The patient information extracting unit 102 extracts only general patient information except special patient information, such as side effects and medical accidents, for generalization of patient information.
In more detail for the general patient information extracting process, the patient information extracting unit 102 produces a relative hospital residence reference date according to the following equation 1 based on a specified date, such as a hospitalization date and an operation date, among medical treatment dates of patient information. [Equation 1]
Hospital residence reference date = hospital leaving date - hospitalization date or operation date
The hospital residence reference date of a variety of patient information obtained in the above manner is expressed by a graph having an inverted "s" shape as shown in FIG. 2. The following equation 2 is a numerical expression of the graph. [Equation 2]
Figure imgf000006_0001
In the above equation 2, "a" represents the number of patients which is smaller than 0 and larger than the maximum number of patients, "b" is a variable representing a period of time during which the number of patients who leave a hospital decreases suddenly. A larger value of "b" means decrease of the number of patients who leave the hospital in s shorter time, "c" represents a hospital residence period. Increase of a value of "c" means increase of the hospital residence period and delayed trend of decrease of the number of patients who leave the hospital, "d" represents a gradient showing a degree of slow decrease of the number of patients who leave the hospital, "d" has a negative value. The patient information extracting unit 102 selects only the general patient information using the graph as shown in FIG. 2.
A method of selecting the general patient information may include a regression curve method, a box whisker method, a percentage method, an expert direct input method, etc. First, a patient generalization method by a regression curve is to differentiate the graph to extract the general patient information on the basis of a point at which a differential value is minimized. That is, a portion indicated in yellow in FIG. 2 represents a region where general patient information is distributed. The box whisker method uses the third quartile of a convergence diagram as a statistical graph to select only the general patient information. In more detail, with an order of appearing data, a value appearing in the order of "entire number of data *l/4" is referred to as "first quartile", a middlemost value is referred to as "middle value", and a value appearing in the order of "entire number of data *3/4" is referred to as "third quartile." For example, if five data are ordered in size, the second value next to the smallest value becomes the first quartile, the third value in the middle becomes a middle value, and the fourth value becomes the third quartile. Accordingly, when the third quartile of the convergence diagram is taken, the general patient information can be selected.
The percentage method uses a distribution of all patients to select patient information located in a certain distribution such as 60%, 80%, etc. of the accumulated number of patients from a starting date as the general patient information.
The expert direct input method is to directly receive range information of hospital residence period for determination of the general patients of all the patients and select patient information corresponding to the received range information of hospital residence period as the general patient information. Prescription information extracting unit>
The prescription information extracting unit 104 extracts only general prescription information from prescription information included in the patient information extracted by the patient information extracting unit 102. Here, the prescription information is grouped into types such as medicine, injection, pathological examination, diagnosis radiation examination, special examination, physical therapy, dietary prescription, doctor's clinical opinions, hospital leaving prescription, etc. The prescription information for each type is divided into items.
In particular, the prescription information on the medicine and injection is added with a classification code indicating an ingredient and effects of medicines according to a classification standard shown in FIG. 5.
In addition, the prescription information on the medicine and injection is added with information on usage of the medicine and injection and pharmaceutical company. The information added to the prescription information on the medicine and injection is used to guide a use trend of medicine having the same ingredient and the injection.
In more detail for the process of extracting the general prescription information, the prescription information extracting unit 104 rearranges the medical treatment information of the patient information extracted by the patient information extracting unit 102 on the basis of a certain date such as an operation date or a hospitalization date according to a prescription date before and after operation or a prescription data during residence in the hospital, which is produced according to the following Equation 3. [Equation 3]
Prescription date before and after operation = medical treatment date - operation date
Hospital residence date = medical treatment date - hospitalization date
That is, when the prescription date before and after operation or the prescription data during residence in the hospital is produced according to the above Equation 3, the medical treatment information of the extracted patient information is rearranged according to the prescription date before and after operation or the prescription data during residence in the hospital. For example, prescription information of a first patient who underwent an operation for appendicitis on January 1, 2007 and a second patient who underwent an operation for appendicitis on May 7, 2006 is rearranged into prescription information one day before operation, prescription information of the very day of operation, prescription information one day after operation, etc. on the basis of the operation date
As described above, when the prescription information of the patient information is rearranged on the basis of the prescription date before and after operation or the prescription date during residence in hospital, the prescription information extracting unit 104 graphs the prescription information of the patient information for each type or item and extracts only the general prescription information using the graph according to the regression curve method, the box whisker method, the percentage method or the expert direct input method. Prescription information statistic processing unit>
The prescription information statistic processing unit 106 produces statistical information by calculating the prescription information extracted by the prescription information extracting unit 104 as a percentage of the number of patients for the prescription date before and after operation or the hospital residence date. In particular, the statistical information is processed for each type or item. <Medical treatment pattern selecting unit>
The medical treatment pattern selecting unit 108 selects medical treatment pattern information for the statistical information produced in the prescription information statistic processing unit 106 and stores the medical treatment pattern information in correspondence to the statistical information. The medical treatment pattern information is selected according to discussion and agreement of experts, and requirement information on prescription, such as essentiality, need, selection, needlessness, etc. is input through the user interface 114.
<Clinical pathway information servicing unit>
The clinical pathway information servicing unit 110 processes the above- produced patient information, prescription information and statistical information on general patients for provision for later medical treatment.
That is, when a terminal requests service of information through the user interface 114 or a network (not shown), the clinical pathway information is produced by transforming the requested information into an XML format and is sent to the terminal.
The clinical pathway information may include information on whether the clinical pathway information is produced by disease or operation, collection period of the selected information, a medical institution that provides medical treatment information producing the selected information, etc.
In addition, the clinical pathway information may include information on frequency, percentage and prescription requirement for each prescription category based on the reference date such as the hospital residence date or the operation date. The clinical pathway information servicing unit 110 transforms the clinical pathway information of the XML format into data structure (schema) information such as XSD and data representation information such as XSL for share of information between medical institutions.
Industrial Applicability
As described above, the present invention produces medical treatment information-customized clinical pathway information by generating the model of the clinical pathway from the medical treatment information produced in the medical treatment information system. Accordingly, the objective and definite ground for the produced clinical pathway information can be presented such that the clinical pathway information can be widely used in research of disease and the field of medicine manufacture. While the invention has been shown and described with reference to certain embodiments and drawings, it will be understood by those skilled in the art that various changes in form and details may be made.
Thus, it is intended that the present invention covers the modifications and variations of this invention provided they come within the scope of the appended claims and their equivalents.
