Abstract

Disclosed is a form comprising a folding card that can be produced therefrom and a planar substrate (10) with a multilayer material (20, 30) which is supplied on said substrate (10). A punching process (11, 12, 13, 14, 15) that is carried out from the front side of the form is used for creating a folding line (15), core parts (A, B) of the folding card in a first area (10.1) covered by multilayer material (20), transparent lamination parts (E, F) of the folding card beyond the folding line (15) in a second area (10.2) covered by multilayer material (30), and congruent cover parts (C, D) for the lamination parts (E, F). The external surfaces of the core parts in the form when the folding card is completed are exposed on the front side of the form while the internal surfaces of the core parts in the form when the folding card is completed are provided with a first adhesive layer. The internal surfaces of the lamination parts in the form when the folding card is completed are provided with a second adhesive layer. The cover parts in the form are placed on top of the second adhesive layer on the front side of the form and can be removed from the form by detaching the same from the second adhesive layer. The lamination parts can cover the core parts in the form and be glued to said core parts by means of the second adhesive layer by folding the form about the folding line once the cover parts have been removed. The core parts can be removed from the form by detaching the first adhesive layer thereof from a layer adjoining the first adhesive layer, and the core part surfaces that are provided with the first adhesive layer can be glued together by folding said surfaces together. According to the invention, a first (20) and a second multilayer material (30) are provided on the substrate (10), the folding line (15) is punched only within the substrate (10), the lamination parts (E, F) are retained on the form exclusively by means of perforated webs, and the external surfaces of the lamination parts (E, F) in the form when the folding card is completed are exposed on the rear side of the form.
