Abstract
The invention relates to an implant for the osteosynthesis of an osteotomy close to the base of a metatarsal, in particular the first metatarsal. The implant is longitudinal and long enough to insert the ends thereof into the marrow of the bone fragments (4, 6) to be joined, and for the ends of the implant to have means for fixing to the bone fragments (4, 6). Die heute gängigen Osteosyntheseverfahren der basisnahen Osteotomie umfassen Versorgungen ohne Implantate mit Schrauben oder mit von außen an den Knochen anzubringenden winkelstabilen oder nicht winkelstabilen Platten. Die Versorgung ohne Implantat hat eine aufwendige Nachbehandlung zur Folge, während die Schrauben- bzw. Mittels der Erfindung kann die basisnahe Korrek- turosteotomie des ersten Mittelfußknochens über kleine Hautschnitte durchgeführt werden. Über einen ca. 2 cm langen Hautschnitt am Mittelfußköpfchen erfolgt zuerst die Abtragung der Pseudoexostose . Hierdurch wird der Markraum eröffnet. Über einen zusätzlichen, ca. 1 cm langen, Hautschnitt an der Basis des ersten Mittelfußknochens wird dann die basisnahe Osteotomie angelegt. Das erfindungsgemäße Implantat wird in den Markraum eingebracht und über ein Zielgerät durch die bestehenden Zugänge oder zusätzlichen Stichinzisionen mit Schrauben verriegelt. Dabei wird (werden) zuerst die proximale (n) Schraube (n) eingebracht. Als nächstes er- folgt das Einbringen der distalen Schraube. Das oval geformte distale Loch erlaubt das Anbringen einer Kompression auf den Osteotomiespalt , indem eine Madenschraube von distal in das Implantat geschraubt wird. Die Kompression wird dann durch Einbringen der mitt- leren Schraube gehalten. Fig. 1 zeigt ein Implantat 2 in Verbindung mit zwei geschnitten dargestellten Knochenfragmenten 4 und 6, in deren Innenraum das Implantat 2 durch eine Öffnung 3 in dem Knochenfragment 4 eingebracht sind. Das eine Ende des Implantats 2 ist mit dem Knochenfragment 4 durch Schrauben 8 und 10 verbunden, wobei die Schraube 10 durch eine langgestreckte Bohrung 12 führt, in die in der Zeichnung von rechts das Ende einer Madenschraube 14 ragt, die einen Kreuzschlitz 16 oder einen Innensechskant aufweist, mit deren Hilfe die Madenschraube 14 gegen die Schraube 10 spannbar ist, so daß das in der Zeichnung rechte Ende des Implantats 2 in dem Knochenfragment 4 in Längsrichtung spannbar ist. Das in der Zeichnung linke Ende des Implantats 2 ist mittels Schrauben 18 und 20 mit dem Knochenfragment 6 fest verschraubt, wobei die Schrauben 18 und 20 in einer Ebene senkrecht zu der Längsausdehnung des Implantats 2 in einem Winkel zueinander angeordnet sind. Das Einbringen der Schrauben 8 und 10 sowie 18 und 20 kann mit einem nicht dargestellten Zielgerät erfolgen. In der Praxis werden zunächst die Schrauben 18 und 20 eingebracht, um das in der Zeichnung linke Ende fest mit dem Knochenfragment 6 zu verbinden. Danach wird das in der Zeichnung rechte Ende in das Knochenfragment 4 eingebracht, die Schraube 10 eingeschraubt und die Madenschraube 14 gespannt, wodurch die Knochenfragmente 4 und 6 in Längsrichtung des Implantats 2 miteinander und gegeneinander verspannt werden.
