Abstract
An apparatus for preventing or treating vitamin D deficiency of an infant comprising an irradiation unit with at least one phototherapy lamp and a support unit suitable to support the infant above the irradiation unit and operable to allow radiation of the at least one phototherapy lamp to pass therethrough, wherein at least one of said at least one lamp is a lamp suitable for preventing or treating vitamin D deficiency.
Description
Apparatus and method for preventing and treating vitamin D deficiency of an infant
Field of the invention
This invention relates to an apparatus and a method for preventing and treating vitamin D deficiency of an infant.
Background of the invention
All infants need vitamin D for their optimal development. Vitamin D is essential for the absorption of Calcium in the intestines. Calcium is important for bone and tooth development as well as for nervous and muscle action. Deficiency of vitamin D can therefore cause rickets.
Vitamin D in the body is derived primarily from the skin and secondarily from the diet. Vitamin D is synthesized in the skin by the action of UV-B. The precursor molecule is the provitamin 7-DHC or 7-dehydrocholesterol which occurs in the strata granulosum and basale of the epidermis. The UV-B exposure causes a photochemical conversion of 7-DHC into previtamin D which is then further transformed to vitamin D and, assisted by liver and kidney, into an active form of vitamin D. The conversion of 7DHC into vitamin D requires ultraviolet (UV) irradiation between 275 - 310 nm. Vitamin D is susceptible to degradation from UV irradiation between 260 - 290 nm. Therefore the optimal wavelengths for vitamin D production are between 290 and 300 nm. However sunlight exposure is difficult to measure.
Usually infants are given vitamin D supplements and care is taken that they are exposed sufficiently to the sun. However, the following babies are still especially at risk for vitamin D deficiency:
- babies who have little exposure to sunlight, because they live in a far northern latitude or because they live in an area with tall buildings or high pollution
- both the mother and the baby have a dark skin and thus require more sun exposure to generate an adequate amount of vitamin D
- the mother is deficient in vitamin D and is breastfeeding the infant.
The general treatment with these infants is to increase the doses of vitamin D supplements.
For another disease, namely the neonatal hyperbilirubinemia, another treatment is known. This hyperbilirubinemia is treated by irradiating the affected infant with therapeutic levels of light within a given wavelength range, namely between 420 run and 480 nm.
US 5 792 214 and US 6253 380 describe an apparatus for treating neonatal hyperbilirubinemia, this apparatus being known on the market since several years as BiliBed. This apparatus includes a frame for supporting the infant and a source of radiation positioned below the frame and the infant. The frame includes a fully transparent section for allowing the radiation to pass therethrough to the infant. The irradiation unit can be placed into an infant's cot instead of a mattress. The infant may be treated in his cot and in close proximity to its mother which makes the infant feel comfortable and aids in mother/infant bonding. This apparatus can also be used in incubators and other equipment to be used for infants with other pressing medical conditions. No light sources above the affected infants crib is needed. The infant does not need eye patches. In addition the treatment apparatus provides light that is precisely directed at a minimum distance to the affected infant, which results in superb therapeutic performance.
Summary of the invention
It is therefore an object of the invention to provide an apparatus and a method for preventing and treating vitamin D deficiency of an infant.
This object is achieved by an apparatus for preventing and treating vitamin D deficiency of an infant comprising an irradiation unit with at least one phototherapy lamp and a support unit suitable to support the infant above the irradiation unit and operable to allow radiation of the at least one phototherapy lamp to pass therethrough, wherein at least one of said at least one lamp is a lamp suitable for preventing or treating vitamin D deficiency.
Preferably the at least one lamp suitable for preventing or treating vitamin D deficiency emits a main light spectrum in the range of 275 to 310 nm, preferably in the range of 290 to 300 nm.
In a preferred embodiment the irradiation unit comprises a recess, wherein the at least one lamp is arranged in said recess. Preferably the support unit comprises a support frame and a blanket and/or a foil attachable to the frame.
In a preferred embodiment one single lamp is arranged in the recess, preferably approximately in the middle of the recess.
In another preferred embodiment, said lamp suitable for preventing or treating vitamin D deficiency is arranged perpendicular to a second lamp, wherein this second lamp is suitable for treating bilirubinemia.
In a preferred embodiment, no phototherapy lamps are positioned above the infant. Preferably, no phototherapy lamps are positioned sideways or in any other position except underneath the plane or surface where the infant is lying or resting on.
The inventive method for preventing or treating vitamin D deficiency comprises the
steps of : providing an irradiation unit with at least one phototherapy lamp and providing a support unit suitable to support the infant above the irradiation unit and operable to allow radiation of the at least one phototherapy lamp to pass therethrough, placing the infant on this support unit activating at least one of said phototherapy lamps with a main light sprectrum in the range of 275 to 310 nm and exposing the infant to the light of this at least one activated phototherapy lamp.
In a preferred embodiment the apparatus is identical with the known BiliBed but using at least one lamp being different from the ones used in BiliBed. The inventive apparatus has the same advantages as the ones described above for the BiliBed. The infant can stay in the same room as the mother. The apparatus can be used in a cot or crib. The infant does not need to wear eye patches. The apparatus can also be used in an incubator or in other medical equipments. However it is also possible to use an inventive apparatus which can be taken home so that the infant can be treated at home. Furthermore the apparatus can also be used for treatment or for preventing other diseases by using the same and/or other lamps than the ones described above, i.e. other wavelengths or other light intensities..

Description of the preferred embodiments
The vitamin D deficiency treatment apparatus of the present invention is intended to be inserted in, and thereby used with conventional infant hospital cribs or cots. Specifically it is contemplated that the treatment apparatus shown and described herein can be placed on a hospital crib or cot that has had its mattress removed.

The lamp 5 preferably emits a main light spectrum in the range of 275 to 310 nm, preferably with a main peak in the region of 290 to 300 nm. Different kind of lamps can be used. In the following, just a few examples are given but other solutions are possible as well: The lamp can have a main light emittance in this range of wavelength, the lamp or the environment of the lamp can include a filter which only transmits light in the above mentioned range of wavelength or the lamp comprises a coating which works as a filter transmitting only in the preferred range.
Preferably the apparatus includes a sensor to quantify the exposure of the infant to this light and/or a timer to avoid over-exposure, wherein the timer switches off the lamp when a preset exposure period has passed. This quantity of exposure and/or the time the infant was exposed is preferably displayed and/or stored in a memory. The data can also be directly transmitted to a control system which records the data into the infants personal record.
Preferably the apparatus comprises a control unit to control the light intensity level of the lamp in order to avoid an irradiation above a preset maximum level and in order to inform the user that the intensity has dropped under a minimum level, so that the lamp can be exchanged with a new one.
Furthermore the electronics and the apparatus as such can have the same features as incorporated in the BiliBed and/or described in the prior art patents mentioned above.
In figure 4 another embodiment of the inventive apparatus is shown. Instead of the above mentioned lamp 5 with the main peak in the region of 290 to 300 nm, a lamp 4 for treating bilirubinemia is arranged in the recess 11. This light of this second lamp 4 is within another range, namely between 420 nm and 480 nm. According to this embodiment the first lamp 5 for treating or preventing the vitamin D deficiency is arranged also in the recess 11 but perpendicular to the second lamp 4. The electronics used for both lamps can be the same or they can have different electronics. In a preferred embodiment there is a switch so that the user can choose if the first, the second or both lamps shall be switched on. This embodiment has the advantage that infants who have to lay in this BiliBed for bilirubinemia treatment also can be treated with this first lamp in order to avoid vitamin D deficiency.
In this description two preferred embodiments were given. However the invention can be modified in different ways. Changes to the embodiments mentioned above can be made without departing from the spirit of invention. For example: the arrangement of the lamps can be different. There can be used only one single lamp having two main peaks in the region of 420 nm to 480 nm and 290 nm to 300 nm. More than one phototherapy lamp of each kind can be used as well. The shape and the contents of the irradiation unit can be different as well. There can be used an irradiation unit with a support unit suitable to support an infant wherein the unit can not be used in a crib or a cot. Furthermore the reflector can be omitted in case only the first lamp but not the second lamp is used.

Claims
1. An apparatus for preventing and treating vitamin D deficiency of an infant comprising an irradiation unit with at least one phototherapy lamp and a support unit suitable to support the infant above the irradiation unit and operable to allow radiation of the at least one phototherapy lamp to pass therethrough, wherein at least one of said at least one lamp is a lamp suitable for preventing or treating vitamin D deficiency.
2. The apparatus of claim 1, wherein the at least one lamp suitable for preventing or treating vitamin D deficiency emits a main light spectrum in the range of 275 to 310 nm.
3. The apparatus of claim 1, wherein the at least one lamp suitable for preventing or treating vitamin D deficiency emits a main light spectrum in the range of 290 to 300 nm.
4. The apparatus of claim 1, wherein the irradiation unit comprises a recess and wherein the at least one lamp is arranged in said recess.
5. The apparatus of claim 1, wherein no phototherapy lamp is positioned above the infant.
6. The apparatus of claim 1 , wherein no phototherapy lamp is positioned in any other position except underneath a plane or surface where the infant is lying on.
7. The apparatus of claim 1, wherein the support unit comprises a support frame and a blanket and/or a foil attachable to the frame.
8. The apparatus of claim 1 , wherein one single lamp is arranged in the recess.
9. The apparatus of claim 8, wherein this lamp is arranged approximately in the middle of the recess.
10. The apparatus of claim 1 wherein said lamp suitable for preventing or treating vitamin D deficiency is arranged perpendicular to a second lamp, wherein this second lamp is suitable to treat bilirubinemia.
11. An apparatus for preventing and treating a deficiency of an infant comprising an irradiation unit with at least one phototherapy lamp and a support unit suitable to support the infant above the irradiation unit and operable to allow radiation of the at least one phototherapy lamp to pass therethrough, wherein no phototherapy lamp is positioned above the infant.
12. A method for preventing or treating vitamin D deficiency wherein the method comprises: providing an irradiation unit with at least one phototherapy lamp and providing a support unit suitable to support the infant above the irradiation unit and operable to allow radiation of the at least one phototherapy lamp to pass therethrough, placing the infant on this support unit activating at least one of said phototherapy lamps with a main light spectrum in the range of 275 to 310 ran and exposing the infant to the light of this at least one activated phototherapy lamp.
