Abstract
A description is given of an acetabular cup (1) for a hip prosthesis, provided with a stem (1) for fixing it in the iliac bone (3), this stem (2) having a free end portion (2p) that fits by complementarity of shape into a cavity (4) formed in the acetabular cup (1) . In the acetabular cup (1) of the invention the longitudinal axis (y-y) of the said portion (2p) is inclined at an angle (α) with respect to the longitudinal axis (z-z) of the remaining part (2r) of the said stem (2) .
Description
Acetabular cup with stem having an inclined portion
The present invention relates to orthopaedic surgery, and more particularly to the technological sector dedicated to the design and manufacture of the various component parts of a hip prosthesis.
More specifically, the invention relates to an acetabular cup (that part of a prosthesis which is fitted into the acetabulum of the patient's pelvis) of the type that is provided with a stem which is inserted into the iliac bone in order to fix the acetabular cup and prevent it from moving relative to the acetabulum.
This stem is connected to the acetabular cup by the complementary insertion of its free end portion into a cavity formed in the cup and, in the current prior art, the longitudinal axis of this portion is in line with the longitudinal axis of the remaining part of the stem.
As is well known to specialists in orthopaedic surgery, the type of fixing of the acetabular cup described above is used especially in cases in which the inner surface of the acetabulum, being degraded and/or worn, may not provide a reliable bond between the bone and the outer surface of the cup.
However, the above fixing also has problems, mainly due to the difficulty of inserting the stem in the iliac bone: in fact it is virtually impossible to successfully insert the stem in the iliac bone with its longitudinal axis positioned in such a way as to provide an ideal seat for the cup in the acetabulum. In most cases, unfortunately, the resulting inclination of the axis of the cavity in which the stem has to be inserted, and in effect therefore the inclination of the axis of the stem, is not the desired inclination
that had been decided upon during the pre-operative preparation.
The result is the application of a cup that is not positioned correctly relative to the part of the prosthesis fixed to the femur, which leads to deflection or twisting of the anatomical parts of the patient which may cause serious problems.
The inventor of the acetabular cup of the invention has devised a solution that eliminates or at least greatly reduces the problems discussed above. The inventor has devised an acetabular cup of the type described above in which, however, the above-described free end portion of the stem, which is designed to fit into the cavity of the cup, has its longitudinal axis inclined with respect to the longitudinal axis of the remaining part of the stem, i.e. with respect to the longitudinal axis of that part which is inserted deeply into the iliac bone. With acetabular cup according to the invention, or preferably a series of stem ends having varying inclinations, it is possible, after measuring the position of the hole prepared in the iliac bone for the stem, to select the stem that is most appropriate for re-establishing a correct positioning of the cup with respect to the rest of the prosthesis.
This can be done in the first place, as already stated, by selecting a stem whose end portion has a suitable inclination and rotating this stem about its longitudinal axis until the desired final position of the abovementioned end portion is obtained.
Using a stem according to the invention it is therefore possible to compensate for potential aberrations in the direction of the axis of the hole in which the stem is placed, and to obtain the desired relative position between the various component parts of a hip prosthesis.
The subject of the present invention is therefore an acetabular cup for a hip prosthesis as described in the accompanying Claim 1.
A more detailed description will now be given of a preferred embodiment of an acetabular cup according to the invention, reference also being made to the accompanying figures in which: - Figure 1 is a side view of just the stem of an acetabular cup of the invention;
Figure 2 is a longitudinal section through the cup fitted to the acetabulum of a patient's pelvis.
As can be seen in Figures 1 and 2, a stem 2 forming part of the complete acetabular cup 1 according to the invention has, as usual, a free end portion 2p, which in this case is a frustum of a cone (it could also be cylindrical, although this is not shown in the drawings) , designed to fit by complementarity of shape into a cavity 4 (see Figure 2) formed in the cup 1.
The axis y-y of this end portion 2p is not aligned with the axis z-z of the remaining part 2r of the stem 2 which inserts into the iliac bone 3, but is inclined to it by an angle α.
To begin with Figure 2, this illustrates a case in which the longitudinal axis of the hole 5 prepared in the iliac bone 3 is not lying on a median plane of symmetry δ of the iliac bone, which had been the intention, but instead is inclined to it by an angle α.
If a stem of the current prior art (not shown) were to be fitted coaxially into the said hole 5, the cup 1 would be in the wrong position, which might prevent it from being positioned correctly in the acetabulum 6. If however a cup 1 provided with a stem according to the invention, selected from among various models having
different angles of inclination α of their end portion 2p with respect to the remaining part 2r of the stem 2, is used, the incorrect geometrical positioning of the said hole 5 can be compensated for. Thus, as Figure 1 shows, the cup 1, fitted as already explained to the end portion 2p of the stem 2, is positioned correctly, as if this stem had a single longitudinal axis y-y (as in the prior art) lying on the said plane δ of symmetry of the iliac bone 3.
In other words, it is possible to obtain a correct insertion of the cup 1 in the acetabulum 6 regardless of: an angular error α in the positioning of the hole 5 into which the stem is inserted; any peculiarities of shape of the acetabulum.
The surgeon has simply to use a stem 2 belonging to the cup 1 assembly according to the invention, in which the longitudinal axis y-y of the free end portion 2p is inclined by this angle α with respect to the longitudinal axis z-z of the remaining portion 2r of the said stem 2. The said stem 2 can even be rotated about the longitudinal axis z-z of its portion 2r inserted into the iliac bone 3 (arrow K in Figure 1) in order to obtain further adjustments of the relative position of the cup 1 relative to the acetabulum 6, thereby also varying the inclination of the end portion 2p with respect to a plane γ (Figure 2) on which the abovementioned longitudinal axis z-z lies.
In the course of an operation it is advisable to use templates having the same shape as the stems from which it is intended to select the most suitable, and then to proceed to the actual insertion of the stem by forcing it into the above-described hole 5 in the iliac bone 3.
Other, different techniques may however be used.
Claims
1. Acetabular cup (1) for a hip prosthesis, provided with a stem (1) for fixing it in the iliac bone (3), this stem (2) having a free end portion (2p) that fits by complementarity of shape into a cavity (4) formed in the acetabular cup (1), which cup is characterized in that the longitudinal axis (y-y) of the said portion
(2p) is inclined at an angle (α) with respect to the longitudinal axis (z-z) of the remaining part (2r) of the said stem (2) .
2. Acetabular cup according to Claim 1, in which the said portion (2p) of the stem (2) that fits into the said cavity (4) of the cup (1) is in the shape of a frustum of a cone.
3. Acetabular cup according to Claim 1, in which the said portion (2p) of the stem (2) that fits into the said cavity (4) of the cup (1) is of an essentially cylindrical shape.
