Abstract

L'invention porte sur une forme (1) destinée à supporter de manière sécurisée des informations sensibles telles qu'un numéro d'identification personnel (NIP), cette forme comportant un plastifié supérieur (2) et un plastifié inférieur (3) collés l'un à l'autre avec un adhésif (4) disposé pour remplir des perforations (8) du plastifié supérieur (2). L'adhésif (4) placé dans les perforations (8) forme des têtes d'épingle sur le plastifié inférieur (2) lorsqu'il est séparé du plastifié supérieur (3) de façon à révéler des informations imprimées sur la forme (1) dans la zone des perforations (8). Les informations révélées par les têtes d'épingle peuvent être un numéro d'identification personnel (NIP) appliqué sur une région prédéterminée de la forme (1) de sorte que ce NIP soit dissimulé jusqu'à la séparation des plastifiés (2, 3).
