  • This invention relates to a fixing device for endotracheal tubes and surgical drainage tubes.
  • Endotracheal tubes are frequently used in operating theatres and intensive care units to facilitate mechanical ventilation of patients who require respiratory support. Drainage tubes are used in surgical, emergency and trauma procedures, normally to gain access to surgical, diagnostic or wound sites within a patient's body. These tubes maintain such access, to drain or feed fluids or gas from or to the patient. Abstract

According to this invention a fixing device (10) for an endotracheal tube (22) (or any other surgical drainage tube) comprises, in essence, a purpose designed ratchet strap fastener. The fixing device (10) comprises a ratchet strap fastener, made up of a flexible ratchet strap (12), a slotted keeper (14) and an anchor plate (16). The components of the fixing device (10) are integrally moulded in injection moulded plastics. In use the fixing device 10 is secured to the tube 22 of an endotracheal tube with the strap (12) encircling the tube (22). The anchor plate 16 angles away from the endotracheal tube (22) and since the anchor plate (16) is parallel to the plane of encirclement of the strap (12) secured around the endotracheal tube (22), the anchor plate (16) extends perpendicularly to the endotracheal tube (22). This means that the anchor plate (16) appropriately positioned to lie flat against the skin of the patient's upper lip area (24.1), where the anchor plate (16) can be secured in place, thereby to secure the endotracheal tube (22) in place. The free end of the fixing device strap (12) may be cut off (at 28) and discarded.

Description

Fixing device for surgical drainage tubes
Background to the invention
This invention relates to a fixing device for endotracheal tubes and surgical drainage tubes.
Endotracheal tubes are frequently used in operating theatres and intensive care units to facilitate mechanical ventilation of patients who require respiratory support. Drainage tubes are used in surgical, emergency and trauma procedures, normally to gain access to surgical, diagnostic or wound sites within a patient's body. These tubes maintain such access, to drain or feed fluids or gas from or to the patient. Unlike cannula-tipped tubes, such as fluid drip feeds, endotracheal tubes and wound drainage tubes suffer from the disadvantage that they are difficult to anchor in place in the intubation sites normally used for these tubes - the mouth, oral cavity and trachea of a patient, in the case of endotracheal tubes or a wound or purpose-made incision, in case of non-cannula drainage and feed tubes. As a result, endotracheal tubes and wound drainage tubes are frequently subject to unplanned extubation, sometimes with disastrous results for the patient.
It is an object of this invention to address these difficulties. Summary of the invention
According to this invention a fixing device for an endotracheal tube or surgical drainage tube comprises a ratchet strap fastener adapted to encircle the tube in use and, integral therewith, an anchor plate adapted for securement to a patient adjacent the intubation site.
In essence, this invention relates to a purpose designed ratchet strap fastener.
These types of fastener normally comprise a slotted keeper and a relatively flat strap that is adapted to encircle the item or items to be fastened. The strap includes a ratchet formation and a pawl arrangement is located within the slot in the keeper. In use, the free end of the strap is fed through the slot and, as the strap is tightened around the item or items to be fastened, the pawl engages and locks into successive ratchet formations on the strap to secure the strap progressively tighter and tighter around the items to be fastened. A conventional cable tie is a typical example of such a ratchet strap fastener.
When, in use, a ratchet strap fastener is encircled around an item to be fastened, the strap describes a curved loop in one dimension. With flat straps in particular, the looped strap tends to curve in one dimension only on a substantially flat, imaginary plane normal to the principal plane or principal axis of the strap.
In the preferred form of the invention, the anchor plate is inclined relatively to the principal plane or axis of the strap with the angle of inclination, which can be anything between parallel and perpendicular, being determined by the application of the fixing device.
In this way the invention provides a fixing device with an anchor surface that can be anchored, in use, against a body surface of the patient adjacent the intubation site, with the strap curving away from the anchor plate at an angle to provide an encircling strap for an intubation tube.
For most applications, a suitable angle of inclination of the strap relatively to the anchor plate is normal to the anticipated axis of intubation of the intubation tube, since most intubation applications require that the intubation tube extend substantially at right angles to the patient's body surface at the intubation site. For these applications, the anchor plate may therefore conveniently be angled perpendicularly to the principal plane or principal axis of the strap.
It is unlikely to expect that the patient body surfaces will be flat in the area of any intubation site, but since the anchor plate will normally be relatively small compared to the patient body surface concerned, the anchor plate can simply be flat.
The anchor plate can be made in a variety of sizes and shapes to accommodate different patient sizes, such as adult and pediatric patients.
The anchor plate may be formed with anchor formations, such as slots through which fastening loops or straps may be threaded to secure the anchor plate to the patient.
Suture apertures through which the anchor plate may be sutured to the patient or a dressing secured to the patient, constitute another example of anchor formations.
It is not entirely necessary to form the anchor plate with anchor formations, since a suitably shaped and dimensioned anchor plate can be adapted simply to be taped in place with surgical tape or the like.
Brief description of the drawings

Figure 1 is a diagrammatic side elevation of a fixing device for an endotracheal tube or surgical drainage tube according to the invention;

Figure 3 is a diagrammatic section through a keeper arrangement for the fixing device of Figures 1 and 2; Figure 4 is a diagrammatic side view of the fixing device for an endotracheal tube or surgical drainage tube of the invention in use to secure an endotracheal tube to a patient.
Description of embodiments of the invention
The drawings illustrate examples of the fixing device of the invention as well as the securement thereof to an endotracheal tube.
Referring to Figures 1 to 4, the intubation tube fixing device 10 comprises a ratchet strap fastener, made up of a flexible strap 12, a slotted keeper 14 and an anchor plate 16. The components of the fixing device 10 are integrally moulded in injection moulded plastics.
Referring to Figure 3, it can be seen that the slotted keeper 14 is formed with a slot
14.1 within which a ratchet pawl 14.2 is located. The ratchet pawl 14.2 is formed with ratchet teeth that are adapted to engage complementally shaped teeth formed on the ratchet strap 12. When, in use, the free end 12.1 of the ratchet strap 12 is fed through the slot 14.1 (in the direction of the arrow 18 in Figure 4) the ratchet teeth,
12.2 on the ratchet strap 12 ratchet past the ratchet pawl 14.2 which engages and locks into successive ratchet teeth 12.2 to secure the strap 12 in progressively tighter engagement around an intubement tube for instance.
To secure the strap 12 in this manner, the free end 12.1 of the strap 12 is first curved around in the direction of the arrow 20 (Figure 2) to the point where the strap 12 defines a complete circle and the free end 12.1 of the strap 12 can be fed through the slot 14.1 in the keeper 14.
When this is done, the curved strap describes a curve that is essentially co-extensive with the plane of the paper on which Figure 2 is drawn. This constitutes the plane of encirclement of the strap.
As can be seen from the drawings, the principal plane of the anchor plate 16 is normal to the principal plane (the flat plane) or the longitudinal axis of the strap 12, which ensures that the principal plane of the anchor plate 16 is parallel to the intended plane of encirclement of the strap 12.
The reasons for this can be seen in Figure 4 which illustrates the fixing device 10 of the invention secured to an endotracheal tube 22 which is entubed within the oral cavity and trachea of a patient (partly illustrated in dotted outline 24).
In this drawing, it can be seen how the fixing device 10 is secured to the tube 22 of an endotracheal tube with the device strap 12 encircling the tube 22. The anchor plate 16 angles away from the endotracheal tube 22.
Since the anchor plate 16 is parallel to the plane of encirclement of the strap 12 secured around the endotracheal tube 22, the anchor plate 16 extends perpendicularly to the endotracheal tube 22. This means that the anchor plate 16 appropriately positioned to lie flat against the skin of the patient's upper lip area 24.1 , where the anchor plate 16 can be secured in place, thereby to secure the endotracheal tube 22 in place.
The free end of the fixing device strap 12 may be cut off (at 28) and discarded.
Various alternatives are available to secure the anchor plate 16 in place. In the example shown, the anchor plate 16 is most conveniently secured in place by means of a Velcro-fastened strap 26 that passes around the patient's head and neck, the Velcro strap 26 being threaded through strap threading apertures 26 formed in the anchor plate 16.
Alternatively, the anchor plate 16 can possibly simply be taped in place with surgical tape.
This method of securement results in a substantially more reliable intubation of the endotracheal tube 22, with consequently minimised risk of unplanned extubation.

Claims

Claims
1. A fixing device for an endotracheal tube or surgical drainage tube comprises a ratchet strap fastener adapted to encircle the tube in use and, integral therewith, an anchor plate adapted for securement to a patient adjacent the intubation site.
2. A fixing device according to claim 1 in which the anchor plate is inclined relatively to the principal plane or axis of the strap with the angle of inclination, which can be anything between parallel and perpendicular, being determined by the application of the fixing device.
3. A fixing device according to claim 2 in which the anchor plate is adapted to be anchored, in use, against a body surface of the patient adjacent the intubation site, with the strap curving away from the anchor plate at an angle to provide an encircling strap for an intubation tube.
4. A fixing device according to claim 3 in which the angle of inclination of the strap relatively to the anchor plate is normal to the anticipated axis of intubation of the intubation tube, the anchor plate being angled perpendicularly to the principal plane or principal axis of the strap.
5. A fixing device according to any one of the preceding claims in which the anchor plate is flat and relatively small compared to the patient body surface concerned.
6. A fixing device according to any one of the preceding claims in which the anchor plate is made in any one of a number of sizes and shapes to accommodate different patient sizes, such as adult and pediatric patients.
7. A fixing device according to any one of the preceding claims in which the anchor plate is formed with anchor formations in the form of slots through which fastening loops or straps may be threaded to secure the anchor plate to the patient.
8. A fixing device according to any one of the preceding claims in which the anchor plate is formed with anchor formations in the form of suture apertures through which the anchor plate may be sutured to the patient or a dressing secured to the patient.
9. A fixing device according to any one of the preceding claims in which the anchor plate is formed with anchor formations in that the anchor plate is shaped and dimensioned to be taped in place with surgical tape or the like.
10. A fixing device substantially as described in this specification with reference to the accompanying drawings.
