Abstract

Cette invention concerne un système d'évaluation et de gestion de médicaments qui est constitué de divers outils de gestion de cas auxquels un fournisseur professionnel de soins médicaux peut accéder par réseau, de préférence par un réseau Intranet sécurisé. Ces outils comprennent pour chaque patient un dossier de santé et de mieux-être maintenu et soumis électroniquement, des services de pharmacie pouvant être consultés par des professionnels, tels que des services de consultation de régimes posologiques et de gestion de médicaments, ainsi qu'un outil d'évaluation des risques permettant de déterminer la fréquence de ré-examen de régimes posologiques. Selon un mode de réalisation préféré, Ce système permet également d'accéder à des services d'information sur des aspects tels que des médicaments ou des maladies, ainsi qu'à des informations d'ordre communautaire sur le lieu de vie d'un patient en particulier. Le dossier santé et mieux-être est accessible sur le réseau aux coordinateurs des services de mieux-être et aux installations de soins ainsi qu'aux patients eux-mêmes et aux fournisseurs de soin santé non professionnels tels que des membres de la famille, sous réserve de l'accord du médecin résident. Ce système assure également une fonction de programmation grâce à laquelle un patient ou un fournisseur de soins de santé reçoit un calendrier des rendez-vous chez un médecin, auquel le dossier santé et mieux-être est transmis automatiquement avant consultation et subséquemment actualisé en fonction des résultats de cette consultation. Enfin, et c'est particulièrement important, le système permet à un pharmacien de revoir périodiquement le dossier santé et mieux-être pour détecter tout problème en rapport avec des médicaments et de conseiller un régime posologique bien adapté au patient. Ces recommandations sont communiquées au coordinateur de mieux-être, au patient et/ou aux membres de la famille.
