Die Erfindung betrifft den in den Patentansprüchen gekennzeichneten Gegenstand, daß heißt die Verwendung von paramagnetischen Metallkomplex-Konjugaten mit polymerem Grundgerüst und einem Molekulargewicht von 10.000 bis 40.000 Dalton in modernen bildgebenden Verfahren zur Differenzierung von malignen und benignen Tumoren.The invention relates to the subject characterized in the claims, that is, the use of paramagnetic metal complex conjugates with a polymeric backbone and a molecular weight of 10,000 to 40,000 daltons in modern imaging methods for differentiating between malignant and benign tumors.
In den entwickelten Industriestaaten nehmen bösartige Geschwulsterkrankungen den zweiten Rang in der Todesartstatistik ein (Statistisches Jahrbuch 1991 für das vereinte Deutschland, Hrsg. : Statistisches Bundesamt, Verlag Metzler-Poeschel (1992). In der Bundesrepublik Deutschland sind 1989 mehr als 200.000 Menschen an bösartigen Tumoren neuerkrankt (Ebenda). Sie stellen damit eines der wichtigsten medizinischen Probleme dar. Wegen der Weiterentwicklung der Methoden zur Behandlung maligner Tumoren (z.B. perkutane, interventioneile, und nicht-invasive Therapien) wird es für diagnostische Verfahren immer wichtiger, solche bösartigen (malignen) von gutartigen (benignen) Tumoren rechtzeitig, zuverlässig und vor allem ohne den (sonst notwendigen) Einsatz von Biopsien und Probeexzisionen differenzieren zu können. Die modernen bildgebenden Methoden (Ultraschall, Röntgen-CT, Kernspintomographie, Szintigraphie) versagen bei einer solchen Differenzierung von malignen und benignen Tumoren. Deshalb hat man u.a. Kontrastmittel bei allen solchen bildgebenden Verfahren eingesetzt, um durch die Kontrastmittel-Kinetik im Tumorgewebe Hinweise auf die Dignität (Artigkeit) des Tumors zu erhalten.In developed industrial countries, malignant tumors occupy second place in the type of death statistics (Statistical Yearbook 1991 for United Germany, ed .: Statistisches Bundesamt, Verlag Metzler-Poeschel (1992). Bei krankhaften Prozessen mit Störung der Blut-Hirn-Schranke (z.B. in malignen Tumoren) entstehen innerhalb des Hirns (Rückenmarks) dann Regionen mit erhöhter Blutgefäß-Durchlässigkeit (Permeabilität) für diese extrazellulären Kontrastmittel (Schmiedl et al., MRI of blood-brain barrier permeability in astrocytic gliomas: application of small and large molecular weight contrast media, Magn. Reson. Med. 22:288, 1991), wodurch erkranktes Gewebe erkannt werden kann. In diesen Bereichen ist die Anreicherung eines extrazellulären Kontrastmittels in erster Linie von der Vaskularisierung und der Größe des extrazellulären Raumes im entsprechenden Gewebe abhängig. Bekanntermaßen unterscheiden sich aber sowohl die Vaskularisierung als auch die Größe des extrazellulären Raumes bei malignen und benignen Tumoren nicht, folglich erlauben extrazelluläre Kontrastmittel in der klinischen Anwendung keine Differenzierung von malignen und benignen Tumoren (S.H. Heywang-Köbrunner, Contrast-enhanced magnetic resonance imaging of the breast, Invest. Radiol. 1994, 29: 94-104; S.H. Heywang, MR imaging of the breast with Gd-DTPA: use and limitations, Radiology 1989, 171: 95-103). Derartige sogenannte Blutpool-Kontrastmittel sollten somit im Ganzkörperbereich geeignet sein, erkrankte Gewebe mit erhöhter Gefaßpermeabilität - ähnlich den extrazellulären Kontrastmitteln im Gehirn - selektiv darzustellen (Brasch et al., Pulmonary oxygen toxicity: demonstration of abnormal capillary permeability using contrast-enhanced MRI, Pediatric Radiology 23:495, 1993).
Als Prototyp solcher Blutpool-Kontrastmittel wurden z.B. für die Kernspintomographie Gd-DTPA- Albumin mit einem Molekulargewicht von 85.000 Dalton (U. Schmiedl et al., Albumin labeled with Gd-DTPA as an intravascular, blood pool-enhancing agent for MR imaging: biodistribution and imaging studies. Radiology, 1987, 162:205-210) und Gd-DTPA-Polylysin mit 50.000 Dalton (Schuhmann-Giampieri et al., In vivo and in vitro evaluation of Gd-DTPA- polylysine as a macromolecular contrast agent for magnetic resonance imaging, Invest. Dabei war es allgemeine Auffassung, die auch in Einklang mit den publizierten experimentellen Resultaten steht, daß solche Blutpool-Kontrastmittel ein Molekulargewicht von mindestens 50.000 Dalton besitzen müssen, damit die gewünschten Blutpool-Eigenschaften erreicht werden können (V.S. Dabei macht man sich die Tumorphysiologie zunutze. Die Blutgefäße maligner Tumoren unterscheiden sich deutlich von denen in benignen Tumoren. In malignen Tumoren wachsen die Blutgefäße während des frühen Wachstum des Tumors aus dem umliegenden Gewebe ein (sogenannte Vasoneogenese). Dabei wird in diesen Tumorgefäßen keine Basalmembran ausgebildet, weshalb die Gefäßauskleidung wegen des fehlenden Stützgerüstes größere Lücken aufweist (G.E. Palade et al., Structural aspects of the permeability of the microvascular endothelium, Acta
Physiol. Scand. Suppl. 1979, 463: 11). Durch diese Lücken (endotheliale Poren) können makromolekulare Stoffe (z.B. Blutpool-Kontrastmittel) in den interstitiellen Raum des Tumors eindringen und sich dort anreichern (L.W. Seymour, Passive tumor targeting of soluble macromolecules and drug conjugates. Crit-Rev-Ther- Drug-Carrier-Syst 1992, 9: 135-187). Diese Eigenschaft wird als Hyperpermeabilität oder Leakiness von Gefäßen in malignen Tumoren bezeichnet. Im Gegensatz dazu entstehen Blutgefäße benigner Tumoren nicht durch Vasoneogenese, sondern sind normale Gefäße des entsprechenden Wirtsgewebes. Eine Hyperpermeabilität tritt nicht auf. Makromolekulare Stoffe (wie z.B. Blutpool-Kontrastmittel) können sich deshalb auch in solchen Tumoren nicht anreichern.Thereby one takes advantage of the tumor physiology. Eine Differenzierung von malignen und benignen Tumoren kann prinzipiell also durch das unterschiedliche Anreicherungsmuster (Enhancement-Kinetik) von polymeren Kontrastmitteln erreicht werden. Bisher war allerdings immer angenommen worden, daß eine zuverlässige Unterscheidung der Enhancement- Kinetik nur bei konstantem Blutspiegel, d.h. nur bei Blutpool-Kontrastmitteln mit einem Molekulargewicht größer als 50.000 Dalton möglich ist (R.C. Brasch, Rationale and applications for macromolecular Gd-based contrast agents, Magn. Reson. Med, 1991, 22: 282-287). Hierbei ist für maligne Tumoren ein gradueller Anstieg über einen längeren Zeitraum pathognomonisch. Im Gegensatz dazu werden bei benignen Tumoren konstante Kontrastmittel-Konzentrationen beobachtet. Blutpool-Kontrastmittel mit einem Molekulargewicht über 50.000 Dalton sind deshalb geeignet, sich in Gebieten mit erhöhter Gefäßpermeabilität wie z.B. in malignen Tumoren anzureichern. Der dazu notwendige nahezu konstante Blutspiegel des Kontrastmittels ist vor allem auf die Größe des Blutpool-Kontrastmittels oberhalb der Nierenfiltrationsschwelle (ca. 40.000-50.000) zurückzuführen.A differentiation of malignant and benign tumors can in principle be achieved by the different enhancement patterns (enhancement kinetics) of polymeric contrast agents. Aufgabe der vorliegenden Erfindung war es daher, Kontrastmittel zu finden, die für die Differenzierung von malignen und benignen Tumoren in Bereichen ohne Blut- Hirn-Schranke geeignet sind und die darüber hinaus die Nachteile des Standes der

Bezüglich ihres Ausscheidungsverhaltens und ihrer Blutkinetik unterscheiden sich derartige polymere Kontrastmittel nur unwesentlich von niedermolekularen extrazellulären Kontrastmitteln. So ist auch für diese Kontrastmittel der hauptsächliche Weg der Elimination aus dem Körper die glomeruläre Filtration, wie an einem polymeren Kontrastmittel (24-Gd-DTPA-Kaskadenpolymer) mit einem
Molekulargewicht von 17.650 Dalton bei Ratten gezeigt werden konnte. So ergab sich bei Untersuchungen des Blutspiegels bei der Ratte und deren Auswertung mittels eines Zweikompartment-Modells, daß die totale Blutclearance des Polymers (11.1 ml/min kg) nur wenig verändert im Vergleich zu der von Gd-DTPA (13.4 ml/min kg) war. Der Blutkonzentrationsabfall beider Substanzen war auch dementsprechend parallel, d.h. es wurde eine sehr schnelle Abnahme der Blukonzentration auch für das Polymer gesehen.With regard to their excretion behavior and their blood kinetics, such polymeric contrast media differ only insignificantly from low-molecular extracellular contrast media. Im Vergleich dazu können unter sonst identischen Bedingungen für das genannte extrazelluläre Kontrastmittel Magnevist® nach 3 Minuten nur noch 30% Kontrastmittel im Gefäßraum nachgewiesen werden. Wie leicht zu erkennen unterscheiden sich die Clearance-HWZ von bemgnen und malignen Tumoren signifikant. Die Clearance- HWZ der malignen Tumoren waren deutlich verlängert im Vergleich zu denen von benignen Tumoren. Diese Größe ist somit am besten geeignet, zwischen malignen und benignen Tumoren nach Injektion des polymeren Kontrastmittels zu unterscheiden, da insbesondere bei dieser Größe keine Überlappungen zwischen den Werten der beiden Gruppen auftraten.
