- the object of my invention is to provide a new and useful surgical apparatus adapted to facilitate examinations and operations atl'ecting the genito-urinary and rectal organs, and to this end comprises a portable table-like structure adapted to be secured to a bed or similar support in such relation as to present the patient, while reclining comfortably upon the bed or similar support, in the correct lithotomy position,with the buttocks and legs supported on the forward part of the tablelike structure well out from the edge or side of the bed, so that the surgeon may have ready access to the organs aforesaid from all points.

SURGICAL APPARATUSn SPECIFICATION forming part of Letters Patent No. 784,425, dated March '7, 1905.

Be it known that I, HENRY IVooDFIN PHIL- LIrs, a citizen of the United States, residing at Norfolk. in the county of Norfolk and State of Virginia, have invented certain new and useful Improvements in Surgical Apparatus; and I do hereby declare the following to be a full, clear, and exact description of the invention, such as will enable others skilled in the art to which it appertains to make and use the same.
The object of my invention is to provide a new and useful surgical apparatus adapted to facilitate examinations and operations atl'ecting the genito-urinary and rectal organs, and to this end comprises a portable table-like structure adapted to be secured to a bed or similar support in such relation as to present the patient, while reclining comfortably upon the bed or similar support, in the correct lithotomy position,with the buttocks and legs supported on the forward part of the tablelike structure well out from the edge or side of the bed, so that the surgeon may have ready access to the organs aforesaid from all points.
(treat difficulty and inconvenience have been experienced heretofore by gynecological and rectal practitioners in disposing the patients in properposition for effective examination of and operation upon the parts, particularly under the usual and normal conditions, when the surgical work has to be performed at the home of the patient. Furthermore, the patient is frequently subjected to inconvenience and sometimes to considerable danger through theinability of the surgeon to properly manipulate his instruments,owing to the position which the patient is forced to assume upon the bed or other supporting apparatus. My invention is designed to overcome these objections by providing a table-like support for the buttocks of the patient, which support rests upon the top of the bed with its flat surface disposed in the same plane as that of the bed-top and its front projecting laterally from the bed, so that when the feet of the patient engage suitable stirrups or footrests secured to the forward edge of the table-like structure the various organs located in the pelvic region will be exposed to view and may be readily reached by the surgeons instruments.
The invention is illustrated in the accompanying drawings, in which- Figure 1 is a perspective view of the apparatus applied to the side of a bed. Fig. 2 is a longitudinal section through the same, showing the mode of attaching the apparatus to the side rail of the bed. Fig. 3 is a perspective view of the strengthening-brace, showing the attaching-brackcts secured thereto. Fig. 4: is a perspective view of one of the brackets. Fig. 5 is a transverse section on line 5 5 of Fig. I. Fig. 6 is a perspective view of one of the adjustable stirrups or foot-rests.
Referring to the drawings, (1' indicates a table-like structure, preferably formed of a flat section of sheet metal having a reinforc ing-bead 7) turned about the under peripheral edge thereof. The forward edge of said tablelike section is provided with two forwardlyprojecting swells a, between which is a recess or depression (Z.
The table-like structure is adapted to be. placed on the upper surface of a bed or other supporting device with the forward portion thereof projecting over the side of the bed, so that the patient may be turned across the bed without being removed therefrom and the buttocks will rest upon and be supported by the table-like structure and all of the organs in the pelvic region will be presented to the operating surgeon. In order to firmly support the patient in this position, it is desirable that the table-like structure be rigidly secured to the bed, and for this purpose there is provided a clamping mechanism attached to the table-like structure and adapted to secure the same to the bed. This clamping mechanism may be of any desirable or convenient form, but is preferably composed of two depending brackets g g, which are hinged to a U-shaped brace e, riveted to the under side of the flat section a, so that said brackets normally de- 95 pend from the under side of the section (1. intermediate of the front and rear edges thereof. Each of said brackets is provided with a i longitudinal slot /i, which is provided throughout its length with a series of countersinks 2 which latter are adapted to receive a tapered shoulder on the under side of the bolt '1', which passes through the slot 71 aforesaid and turns freely in said slot. Each of said bolts is provided with a clamping-arm S, Which is adapted to engage the inner surface of the side rail of the bed when the bolt is turned up to securely clamp the rail against the bracket g, so that the apparatus is held rigidly in position on the bed.
The forward ends Z Z of the brace c, which are preferably bent outwardly toward the sides of the flat section a, are perforated, as at m m, to receive the inwardly-disposed ends 0 0 of the shanks n n, of stirrups or foot-rests p, so that said stirrups may be readily removed from or adjusted laterally with respect to the table-section a.
In the application of the above-described apparatus to this intended purpose the tablesection a, is laid upon the top of the mattress, the brackets g g dropped down against the side rail, and the clamping-bolts r are moved in the slots 72, until the clamp-arms s .s' are in position to engage the inner face of the side rail, the tapered inner faces of the bolt-heads are engaged in the proper countersink, and the bolts screwed up to clamp the apparatus in its adjusted position. It is to be noted that the longitudinal slot h and the series of countersinks in the respective brackets g permit the apparatus to be adjusted and securely clamped to the side rail no matter what the height of the mattress-top be above the same. After the apparatus has thus been adjusted in the desired position the patient is disposed crosswise of the bed, with the buttocks resting upon the table-section a, as aforesaid, with the heels engaging the stirrups of foot-rests p, so that all of the genito-urinary and rectal organs are presented in the most favorable position for examination or operation.
It is to be particularly noted that this apparatus comprises a practical and convenient device capable of being folded up and readily transported by the surgeon on his round of visits and also capable of being quickly applied to a bed or similar support without removing or disturbing the patient. The whole apparatus being preferably constructed of metal may be readily cleaned and rendered thoroughly aseptic.
\Vhat I claim as my invention is l. A surgical apparatus for use in examinations and operations affecting the pelvic region, comprising a table-like structure wholly supported by and projecting from a bed or similar structure to present the patient inthe lithotomy position.
2. A surgical apparatus for use in examinations and operations affecting the pelvic region, comprising a table-like structure resting upon and projecting from a bed or simi-' lar device, and means for adjustably securing and supporting the same in the plane of the beds surface.
3. A surgical apparatus for use in examinations and operations affecting the pelvic region, comprising a flat table like section, brackets depending therefrom, and means to secure said brackets to a bed or similar device to cause the flat section to rest upon the surface of the bed and to project from the bed in the plane of said surface.
4:. A surgical apparatus for use in examinations affecting the pelvic region, comprising a flat table-like section having two forward swells and an intermediate recess or depression in its front edge, brackets hinged to the under side thereof and adjustable clamps on said brackets adapted to secure the apparatus to a bed or similar structure with the flat section resting upon the surface at one end and projecting from the side of the bed at the other end.

A surgical apparatus for use in examinations and operations affecting the pelvic region, com prising a flat table-like section,brackets hinged to the under side thereof, adjustable clamps adapted to secure the apparatus to the side rail of a bed with the flat section resting upon and projecting laterally from the bed, and stirrups or foot-rests projecting from the forward edge of the flat section.

A surgical apparatus, comprising a flat section of metal or the like, a U-shaped brace secured to the under side thereof, brackets hinged to said brace, clamps adjustably engaging said brackets and adapted to secure the apparatus to the side rail of a bed, or similar structure, with the flat section resting upon and projecting laterally from the bed,
and removable and adjustable stirru ps or footrests mounted in the ends of said brace and projecting forwardly of said flat section.
7. A surgical apparatus for use in examinations and operations affecting the pelvic region, comprising a table-like structure supported by and projecting from a bed or similar structure and serving to support and project the hips and buttocks of a recumbent patient out from the side of the bed, and stirrups carried by said structure to support the feet of the patient.
