This invention relates to order forms and more particularly to special order forms such as used by merchandising establishments, etc.
The principle object of the invention is the provision of a special order form incorporating a plurality of detachable sections which comprise the complete record of the special order.
A further object of the invention is the provision of a special order form, the formation of which provides means for placing a special merchandise order, establishing multiple records of the special order and provides a copy of the order for transmittal to the manufacturer and further provides the manufacturer with an address stub or reply card.
A still further object of the invention is the provision of a special order form incorporating a follow-up card to enable the establishment using the form to contact the manufacturer and request advice as to shipment of the goods in the event that shipment has been delayed beyond the usual time. r
The special order form shown and described herein has been designed to expedite customers special orders as handled by a retail merchandising establishment such as a department store,
and provides on one form a copy and correlated record of the customers special order, the handling of the order in the merchandising establishment and provides the manufacturer or shipper of the special order with an address stub or reply card if the shipping date is other than normal. At the same time the special order form provides aninquiry card which may be used by merchandising establishments placing the special order to inquire about the order and all of the various parts of which correspondingly numberedso that a uniform and accurate record may be kept.
With the foregoing and other objects in View which will appear as the description proceeds,
the invention resides in the combination and arrangement of parts of the special order form hereinafter described and claimed, it being understood that changes in the precise embodiment of the invention herein disclosed can be made within the scope of what is claimed Without departingfrom the spirit of the invention.

The binding may be conventional gluing or stitching or stapling and is shown in the drawing as wire stitching and indicatedby the numeral [2. Positioned between thefrontcover l0 and the back cover H of the special order form'leaflet by means of the binding 12 there aretwo sheets, generally indicated by thenumerals I3 and [4, which with the back cover H comprise the usable portions of the special order form, the front cover It] carrying directions for use-of the same on the inside cover thereof. The sheet l3 which is uppermost in the drawing is perforated on four lines, indicated by the numerals l5, I6, I! and I8, respectively, so that it may be conveniently detached in appro- The uppermost portion of the sheet 13, indicated by the numeral I3A, comprises the portion of the form filled in by the sales person and carries such data as the customers name, the address as-well as the department and section in which the special order originates, and the name and number of the sales person, the kind of sale; .the advanced payment, if any, and the receipt number, if any, as well as providing for the. quantity, style, color, description, etc., concerning the specially ordered merchandise. Immediately beneath this portion 13A, on the back side thereof and indicated in the drawing by the numeralv 13B, there is a provision made for a follow-up record and alphabetical customer file listing. These spaces can be filled in with various data concerning the special order if and When necessary. A one time carbon [9 separates this top section ISA of the top sheet l3 from the second sheet l4 and a correspondingly marked section-MA which in use comprises the department manager's .copy. "Another one time carbon 20 separates thissecond sheet. M from the back cover forproviding for a second copy of the portion I3A on the first sheet and comprises a special order date tickler which is filed in a datetickler file to be used by the merchandising establishment in checking on the special order in View of the delivery date promised the customer.
By again referring to the uppermost or first sheet I3 it will be noted that the central section thereof comprises a detachable portion which is filled in by the department manager thereby completing the special order form. The middle portion is indicated by the numeral I3D and is detached. The bottom portion of the form is separated therefrom by the line of perforation l1, and-isgenerally indicated by the numeral l3F,
and is folded upwardly and the resulting post card shape is mailed to the manufacturer or shipper of the merchandise desired, it being observed that the opposite side of the portion [3D comprises a post card form with space for filling in the manufacturers or shippers name as indicated by the numeral 13E. These two portions 13D and 13F, when received by the manufacturer or shipper of the merchandise, may be employed for expediting the delivery of the special order to the retail establishment originating the same. This is possible by the formation of the lower portion I3F of the sheet l3 in the form of a combination address stub and business reply card. If the merchandise is to be shipped immediately, the bottom portion |3F is detached on the perforated line I! and the same again separated on the perforated line 18, the upper portion of this portion I3F being discarded and the lower portion thereof which carries the merchandising establishments name on the lines 22 is used for attachment to the goods, and as such provides an address label. If for some reason the merchandise cannot be sent at once, the back side of this portion BB is indicated by the numerals l3G, is sent with the anticipated shipping date of the merchandise and the whole portion 13F which comprises a complete business reply card properly addressed is mailed back to the merchandising establishment originating the special order. It will be observed that the one time carbons l9 and 2!] do not extend in under the portion I 3F of the form as they terminate slightly above the perforation line IT. This enables the. bottom portion of the back cover H, as indicated by the numeral NC to be used by the merchandising establishment as a follow-up card on the special order concerned. If the merchandise is not received in the anticipated time this portion of the form is used to make inquiry. of the manufacturer or shipper. The back side thereof is indicated by the numeral 1 ID comprising a standard post card form on which the shippers name and manufacturers name will be filled in.
By referring again to the middle section [3D 45 of the form it will be observed that the carbon 2| separates it from the lower portion of the center sheet M which is indicated in the drawings by the numerals MB and this section comprises a duplicate of the form I 3D'and is retained by the department manager for his record of the special order. The center section of the, back portion 1 I, which is illustrated as being separated somewhat along the perforation lines l6. and I1 and indicated by the numeral B which is separated from the portion MB by a, one time carbon 23, is a third copy of the center section I3D of the sheet [3 and as such is detached by the department manager at the time the special order form is filled in and is sent to the order department for their record. The uppermost portion of this back cover, which is also illustrated as being partially separated from the remainder on the perforation lines I5 and I6 and generally indicated by the numeral HA, is a third copy of the special order originally filled in by the sales person taking the same and is detached by the department manager and placed in the date tickler as heretofore described.
It will thus be seen that a composite special order, form has been disclosed which anticipates every requirement of a merchandising establishment with respect to the various forms and orders necessary in making, recording and forwardin the special order to the manufacturer'or shipper,
and further that the special order form includes portions which are sent along to the manufacturer or shipper of the goods with the actual order and are detachable by the manufacturer or shipper to become either an address stub for attachment to the merchandise or a business reply card advising of a delayed delivery. The special order form enables two writings on the part of the employees of the merchandising establishment on a single form to establish a complete special order and record of the same, the first person being the sales person taking the order who completes only the top portion of the form such as is indicated by the numeral I3A, the carbons I9 and 20' providing duplicate copies on the portions HA and 14A of the back cover H and center sheet [4, respectively. The middle section is filled in by the department manager and comprises the second and last writing on the part of the merchandising establishment originating the special order. The carbons 2| and 23 provide duplicate copies of this actual order on the portions MB and I IB of the center sheet l4 and back cover II, respectively, while the lowermost portion of the special order form provides business reply cards or address stubs for use by the manufacturer or shipper and a business inquiry card for use on the part of the merchandising establishment originating the order.
It will thus be seen that a special order form has been designed which will, in use, expedite the placement of the special merchandise orders on the part of the retail establishment and provides all of the several records necessaryfor the establishments complete memorandum of the order, etc.
HaVing thus described my invention, what I claim is:
1. A special order form comprising a front and back cover having two printed sheets bound therein, the first of the said printed sheets divided into three sections and the second of the said sheets divided by a perforation into two sections, the said two sections matching and underlying two of the sections on the first sheet, perforations on the back cover dividing it into three sections, two of which match and underlie the said two section sheets, carbons interposed between the said sheets, the said first sheet having one of the said sections including a form for entering the customers order, another section including a form for the department managers order, and another section including a combined business reply card and address stub, the said second sheet comprising a form for duplicates of the customers order and the department managers order, the said back cover having two forms of which correspond with the two sections of the said second sheet and the third section of which comprises a follow-up post card.
2. A special order form comprising a cover having at least two printed sheets positioned therein, the first of the said printed sheets divided into several sections, and the second of the said sheets divided into several sections, at least two of the sections of the said second sheet matching and underlying two of the sections of the said first sheet, the portion of the cover underlying the said printed sheets being divided into several sections at least two of which match and underlie the said printed sheets, carbons interposed between the said printed sheets, the said first sheet having one of the said sections including a form for entering the customers order, another section including a form for the department managers order, and another section including a combined business reply card and address stub, the said second sheet including a, form for duplicates of the customers order and the department managers order, the said cover underlying the said printed sheets having forms of which correspond with the sections of the said second sheet and an additional section which comprises a follow-up post card.
