I accomplish this and other objects of my invention by means of a card and rulings therefor which is fully described in thefollowing specification and claims and illustratively exemplified in the accompanying drawings, in which Figure 1 is a perspective View of an arrangement of cards in my improved index system showing several of the main history cards placed therein; Figure 2 is an elevational view of the main card in open position and a visiting card in place; and Figure 3 is a transverse sectional View taken through the folded main card and visiting card. i
Referring to the drawings, 10 denotes a main or history card which is folded longitudinally thereof along one of two adjacent scored lines 11. The upper left portion has two ruled lines 12 for the name and address of the patient, while the upper right-handedge is provided with a. tab 13 ruled vertically into spaces numbered from 1 to 7, for example. The two scorings 11 on the history card permit the front portion to fold over the rear portion so that only the name and address of the patient is visible, one of the scorings 11 being for the purpose of forming a rest for visiting'cards 1 1, which are attached to.

The numbers in the spaces of the tabs 13 represent a diagnosis index and separate tabs 17 with corresponding numbers are attached one to each of the tabs 13.
The index system of which the history: cards are part, comprises a diagnosis index filecard 18, see Figure 1, on which is entered the type casesfound to bev of special interest. An adjustable indicator tab 17 is placed over the number on tab 13 which corresponds to the number 19 found on the diagnosis index card 18. This permits thesearcher to readily locate all cases of the same type under one number 19. For example, #1 on the index card 18 representing .Pneumonia would indicate that all history cardslO hav-' ing a tab 17 with #1 on it would relate to specific cases of pneumonia. 1 The back of the history card provides space for the date and. treatments given. When the back of the history card 10 is completed, another card called the subsequent visiting card 14: is used. Each card 1 1 is of a size to fit perfectly inside the folded history card and its upper edge extends just below the spaces for the'patients name and address indicated on the history card. The visiting cards 14 are provided with spaces for the patients name and address,-dates of treatments and'findings and each card is further provided with two holes 20 punched in the right side adjacent the corners, both holes being intended for the same purpose. The first hole receives the fastener 16, and the second hole permits the visiting card to be reversed when the first side is completed, the reverse side being the same as the first side. The arrangement described'herein enables the doctor at all times to have a complete view of his records by simply swinging the card 14 about the fastener 16.
Having now described my invention what I claim and desire to secure by Letters Patent is: c i
1. In a card system of the character described, a history card comprising a member folded on itself to provide space at the back portion to accommodate certain data visible above the forward portion, the back portion'having a tab subdivided by lines on its upper edge and designated by different characters, and a separate tab to be attached to the first tab and having a character corresponding with one of the characters of the first tab.
2. A history card as claimed in claim 1 in which the back portion of the history 5 card is provided with a fastening means and in combination therewith a subsequent visiting card having two holes adjacent the corners' on one end adapted to connect with the history card through the fastening means.
