' This invention relates to an improved combination form and sheet for a hook-keeping machine, and is especially adapted to the Burroughs adding and book-keeping ina- Chine, and the principal object of my invention is the `constraction of a sheet with four boxes or compartments formed contiguous to or adjoining lines of penforationsiormed on the sheet, so as to readily post or advise a f' customer oil a bank as to the standing of his account with the institution, when toe eentral bodypf the sheet is handed to the customer `as a notice to him oi the condition' of' his accountJ with the bank. y
