Specification of Letters Patent,
To all whom it may concern:
Be it known that I, JOEL WITMER, a citizen of the United States, residing at Kansas City, in the county of Jackson and State of Missouri, have invented certain new and useful Improvements in Register Systems; and I do declare the following to be a full, clear, and exact description of the invention, such as will enable others skilled in theart to which it appertains to make and use the same, reference being had to the accompanying drawings, and to the letters and fig ures of reference marked thereon, which form a part of this specification.
My invention relates to a register for the keeping of accounts and records, particularly by physicians and surgeons, and has for its principal object to provide a system comprising record slips, envelops and files whereby information may be tabulated and filed so that part of the information is readily accessible, and full information regarding any particular matter or case vmay be secured. In accomplishing this object, I provide specially printed and ruled case slips and envelops adapted for disclosing the amount of the account on an exposed part of the slip when the latter is in the envelop, and a file for containing the envelop and exposing an index portion thereof so that the particular account may be readily found in the file, and by pulling the envelop from the file the amount of the patients balance may be seen without remov ing the slip from the envelop. In accomplishing this object, I have provided the details of structure hereinafter described and illustrated in the accompanying drawings, wherein Figure I is a perspective view of a cabinet or filing drawer constructed according to mv invention, showing a number of. en-

At the bottom of the slip, and at the right- I record of his name and address on the slip to be shown while the mass of the information contained on the. slip is hidden by the front of the envelop. The name and address of the patient is then Written on the frontexposed face of the back portion 7 and the envelop filed in the cabinet drawer 2, which latter is constructed for the purpose of containing suchenvelops and is of a depth to expose the name and address of the patient above the partitions 4, without exposing the charge or any other information contained on the record slips. An envelop for each of the physicians patients, having an unsettled account, may be kept in the drawer so that accountv may be found readily in case the patient wishes to pay his bill, or if the physician'wishes to look up the record of any particular case. Should the physician have another call from the same patient, a new slip is made out for that particular call, and when he' returns to the oflice he files same in the same envelop in front of the preceding slip, carrying the balance of the account forward above the last charge and bringing the new balance down to the proper column.

Having thus described my invention, what I claim as new therein and desire to secure by Letters-Patent is A'register comprising an envelop having a back leaf of greater width than the front ply thereof, the edges ofthe back leaf and the front portion being uninterrupted and parallel with each other throughout their .length, the portion of the. back leaf above the front'ply having spaces in which to in-' dicate the name, disease, address, and to whom the account is to be charged, and
an insert record slip constructed to fit in said envelop and project throughout its length above the said front ply to a point on the back leaf below the designations on the back leaf, whereby. said designations are always in unobstructed view, said insert having a space above the said front ply ap propriately designated to show the balance due in the account without removing the insert from the envelop.
