Description of the invention
The claimed useful model relates to medicine, namely dentistry, and can be used 2 in the complex treatment of glossodynia.
Глосодинія - системне захворювання, яке проявляється головним чином печією в порожнині рота та психопатичним синдромом внаслідок дезінтеграції соматичних та вегетативних утворень, що інервують порожнину рота на системному та надсистемному рівнях |11.Glossodynia is a systemic disease that is manifested mainly by heartburn in the oral cavity and psychopathic syndrome due to the disintegration of somatic and vegetative formations that innervate the oral cavity at the systemic and suprasystemic levels |11.
(21.General and local factors contribute to the development of glossodynia. Of the general ones, there are chronic digestive diseases, 70 endocrine organs (most often the thyroid gland, ovaries), chronic insufficiency of blood supply to the brain, an anxious-lethargic personality type of the patient, stress (21.
At the same time, changes in the condition of the oral cavity and maxillo-dental apparatus are of particular importance, namely, poor-quality fillings and dental prostheses, worn teeth, irrational prosthetics, absence or violation of occlusal relationships, masticatory muscle dysfunction, galvanosis |Z).
(4).Clinical manifestations of glossodynia, in particular, heartburn, disorder of salivation and taste sensations, fatigue, discomfort of the tongue and lips are difficult for patients to tolerate and lead to changes in the psycho-emotional sphere (asthenia, neurosis, hypochondria, depression, cancerophobia), which negatively affects work capacity and generally reduces quality life (4).
In recent years, there has been a steady increase in the incidence of glossodynia among the population of both sexes of working age.
According to domestic and foreign authors, the prevalence of glossodynia reaches from 5 to 18 people per general population.
(5).For the treatment of patients with glossodynia, many methods have been proposed, both general and local, in particular, medicinal, physiotherapeutic, reflex and extended dental rehabilitation (treatment of caries and its complications, periodontal diseases, mucous membranes, elimination of dental defects). But in the absence of uniform standards for the treatment of patients with glossodynia, which is due to the complex mechanisms of its occurrence, the polymorphism of clinical manifestations, the choice of treatment for each patient is extremely difficult, and therefore the further improvement of treatment measures is an urgent problem (5).
A well-known method of diagnosis and treatment of glossodynia (6), which we chose as a prototype. According to this method, complex pathogenetic treatment of patients with glossodynia is carried out according to an individual scheme, taking into account the vegetative status, the nature of visceral pathology, the general condition and age of the patient.
Treatment includes drug therapy, psychotherapeutic methods. Treatment is carried out according to methods of general and local level, supplemented by general strengthening procedures, diet, rehabilitation of the oral cavity and rational prosthetics. 3. Treatment according to this scheme is long-term and takes a lot of time. Prescribing a large number of antipsychotics, tranquilizers, and sedatives affects the patient's work capacity and general condition.
8At the same time, observations are described in the literature that orthopedic measures, which include correction of prostheses, normalization of occlusal height and removal of dissimilar metals, often lead to recovery. 8
The basis of a useful model is the task of improving the method of treatment of glossodynia due to optimization of the orthopedic stage of treatment, shortening the duration of treatment, and reducing the pharmacological load on the body.
From" The technical result achieved as a result of solving the given task consists in the disappearance of the symptoms of glossodynia, improvement of the functioning of the chewing apparatus, and an increase in the quality of life.
The task is solved by the fact that in a known method, which includes general and local drug treatment and rational prosthetics according to a useful model against the background of basic therapy, a diagnostic and therapeutic intermaxillary splint is additionally fixed on one of the jaws, made of orthodontic plastic, taking into account the central ratio of the jaws ; at the same time, the patient is recommended to use the splint 8-10 hours a day for 2-3 weeks with occlusal correction of the splint in accordance with the patient's individual feelings. se" 20 The clinical assessment of the patient's well-being for 2-3 weeks confirms the effectiveness of the specified position and the disappearance of the symptoms of the pathological condition. Fixation of the defined ratio of the jaws by rational prosthetics or restoration of occlusal surfaces taking into account the requirements of static and dynamic occlusion completes the orthopedic stage of treatment.
The method is performed as follows:
So" Against the background of prescribed general and local drug therapy, a clinical analysis of static and dynamic occlusion, muscle condition, and temporomandibular joints of a patient with glossodynia is performed. With the help of muscle deprogrammers, the central ratio of the jaws and the interocclusal height are fixed (registered with silicone), which is confirmed by X-ray examination. Jaw models are fixed in the articulator in a gnathostatic position thanks to the use of a facial arch. The ratio of the models of 60 jaws is set in a fixed position on the basis of a silicone register. On the basis of the obtained ratios of the jaw models, a diagnostic and treatment splint is made from orthodontic plastic, which is fixed on one of the jaws. Another jaw when closing is fixed in a certain position, which is formed on the surface of the tire. The patient is recommended to use such a splint for 8-10 hours a day. During 2-3 weeks, patients appear for correction, during which, based on the individual feelings of the patient, they carry out occlusal correction of the tire.
Almost all patients noted a reduction or disappearance of symptoms of heartburn, numbness, dryness within 1-3 days after applying the splint. At the same time, 8 out of 12 patients noted that they are forced to use a splint almost all the time, because after removing the splint, about half an hour later, the symptoms of glossodynia begin
Recover.
After achieving a stable state of comfort, the occlusal relations of the jaws are analyzed in the articulator, after which the restoration of the shape of the occlusal surfaces of the teeth is planned. Prosthetics of patients with glossodynia are performed simultaneously using high-quality alloys. This way of prosthetics is prevention of galvanosis.
After carrying out prosthetics, taking into account the individual requirements of the patient, correction of prostheses is not required, 7/0, which ensures full functioning of the chewing apparatus.
The main differences of the claimed method are: the use of gnathological principles of jaw positioning, which in turn ensures a much higher level of functioning of the components of the chewing apparatus, and affects the state of the vascular and nervous system. Due to taking into account the achieved comfortable ratio of the jaws, it is possible to make more functional structures of prostheses. Objectively evaluated and functionally adapted /5 ratio of the jaws create conditions for quick getting used to the created comfortable restorations of the occlusal surface, which in turn affect the activity of the gnathological complex as a whole.
In comparison with the prototype, the claimed method makes it possible to: - significantly shorten the terms of glossodynia treatment; - reduce the pharmacological load on the body during treatment; - in a short period of time, achieve improvement in the functioning of the chewing apparatus; - ensure restoration of occlusion through prosthetics and restorations taking into account individual functional requirements of the patient; - perform prosthetics at once using a high-quality alloy; - the need for individual correction of prostheses disappears.

Ge) occlusion and central ratio. As a result of the examination, a decrease in the height of the bite, hypermobility of the articular heads was revealed. When closing the dental rows in a normal bite, overloading of the frontal group of teeth was observed, as a result of which the teeth changed their position. When analyzing the models in 2) central ratio, a distal displacement of the position of the lower jaw in relation to the 5th upper jaw with a vertical interocclusal distance of 3-4 mm was revealed. c" on 07/09/04, the patient was made an intermaxillary stabilizing splint made of orthodontic plastic in the position of the central relationship. The tire is removable and fixed on the lower jaw. Recommendations are given to use the tire at night.
After three days, the patient noted that the pain and heartburn disappeared. A feeling of discomfort remained from the (7 55) distal position of the lower jaw. The patient underwent correction of the surface of the tire with the formation of a forward slip of 1 mm.
Four days later, the patient complained of feeling more contact with the tire on the left side. Tire correction was carried out until the feeling of uniformity of contact with the surface.
A week later, taking into account the patient's complaints about the desire to clench her teeth all the time, the bite height was increased by 1-1.5 mm with the help of fast-hardening plastic.
During the planned correction, the patient no longer complained of heartburn. But the patient noted that, despite the recommendation, she used the tire all the time, since after an hour or two a feeling of discomfort appears.
The patient underwent a clinical examination of her teeth and dental rows. Recommended therapeutic preparation of 65 teeth for the purpose of further rational prosthetics of the dentition taking into account the achieved comfortable ratio of the jaws and the creation of functional occlusal contacts of the tooth surfaces.
Example 2. Poziy Ihor Valeriyovych, born in 1965 (medical card No. 6704) applied to the dental clinic of NMU on September 21, 2004 with complaints of burning tongue and gums, a feeling of thick sticky saliva.
Heartburn occurs during the day, is not felt during meals. Considers himself sick for two years. Two years ago, the patient received metal-ceramic bridge prostheses (20 units) in the lateral areas of the upper and lower jaws. Two weeks after dental prosthetics, heartburn appeared in the oral cavity. A month after the removal of dentures, the heartburn did not disappear. It is known from the anamnesis that the patient has been suffering from chronic duodenitis and chronic cholecystitis for 5 years. Despite constant treatment for 2 years with drugs that normalize the secretory and motor functions of the organs of the digestive system and drugs of local effect on the 7/0 mucous membrane of the oral cavity, this did not lead to the disappearance of heartburn.
In the stomatological clinic of NMU, the patient underwent a clinical and laboratory and radiological (orthopantomography) examination. A clinical diagnosis of glossodynia, malocclusion was established. The patient is scheduled to consult an orthopedist at the Department of Orthopedic Dentistry for the purpose of orthopedic examination and treatment.
A clinical examination, computer tomography of the temporomandibular joints, analysis of models in the articulator in the usual occlusion and in the position of the central ratio of the jaws were performed. A decrease in bite height was detected. When analyzing the models in the central ratio, the distal position of the lower jaw with a vertical interocclusal distance of 2 mm was obtained. On September 29, 2004, the patient had a therapeutic and preventive intermaxillary stabilizing splint made of orthodontic 2o plastic in the position of the central ratio. The tire is removable and fixed on the lower jaw. Recommendations are given to use the tire at night. On October 6, 2004, the patient appeared for correction. There were no complaints. The patient uses the splint all the time, as after half an hour a feeling of discomfort arises.
For control, observation was carried out for another three weeks. The patient did not complain of heartburn and discomfort in the oral cavity when using the splint.
