Abstract

A method for deciding the necessity of myocardial revascularization in the patients with ischemic heart disease and angina recurrence following the coronary by-passing comprises the selective coronagraphy and bypass angiography for assessing the hemodynamic significance of the stenosis of distal anastomosis of coronary bypasses. The bypass is catheterized selectively, and the coronary guide is passed through the guide catheter with the pressure probe at the end distal to stenosis area. The systolic pressure downstream stenosis in coronary artery and systolic pressure in aorta is measured in the setting of the maximal arterial hyperemia. The fraction reserve ratio is calculated by the formula: FFR=P/P. When FFR equals 0.75 or exceeds this value, the repeated revascularization is inexpedient since stenosis is not significant hemodynamically. Contrary, when FFR is lower than 0.75, the repeated revascularization is indicated since stenosis is significant hemodynamically.
