Description of the invention
The claimed invention relates to medicine, namely to neurology, traumatology and orthopedics, and can be used to determine the syndrome of intracranial hypertension.
Effective prevention and treatment of early forms of cerebrovascular diseases today is an extremely urgent task of medicine, since these diseases rank first among the causes of disability and mortality in economically developed countries and countries with economies in transition. Not the last place in the list of these diseases is occupied by impaired cerebral blood circulation. But until now, there are not enough 70 studied links of the etiology and pathogenesis of the disease, in particular the undefined vertebrogenic component, there are no methods of early diagnosis, which reduce the effectiveness of real prevention.
The closest to the proposed method, chosen as a prototype, is the method of transcranial duplex scanning, which includes the study of the brain substance in the B-mode (transcranial sonography) and the study of blood flow in large intracranial arteries, veins and sinuses with the use of the Doppler effect. . Transcranial duplex scanning is performed by a vector (sector) sensor that generates pulsed oscillations with a frequency of 1-2.5 MHz. The main access is transtemporal.
The sensor is installed on the temporal bone scale behind the auricle (posterior temporal window) with the initial orientation of the scanning plane parallel to the base of the skull. This makes it possible to obtain images of brain structures of hemispheric and central localization, as well as bone structures (pyramid of the temporal bone), which serve as a reference point for visualization of vessels. In this projection, the vein of Rosenthal is visualized
Galena and straight sinus (1). At the same time, in the Doppler mode, the blood flow has a monophasic pseudo-pulsating character. The degree of its pseudopulsation in the absence of violations is minimal and is caused by the transmission pulsation from the brain substance. The Doppler spectrum is characterized by an almost complete absence of a spectral window and a uniform spectral distribution. When quantifying venous blood flow indicators, the following can be analyzed: the maximum blood flow rate corresponding to the diastole phase (with parallel ECG monitoring) and time-averaged maximum and average blood flow rates. The normal maximum speed of blood flow in the vein of Rosenthal is no more than 15 cm/s, in the vein of Galen - no more than 20 cm/s, in the straight sinus - no more than 25 cm/s.
Другими венозна кров відтікає в двох напрямках: з одного боку - системою базальних вен (вени Ге»)Venous outflow from the cranial cavity is possible only if the pressure in the cerebral veins is higher than the intracranial pressure (IZ). There are several ways out. They carry blood from certain parts of the brain and have a certain direction. It is possible to distinguish the ways of outflow of venous blood from the hemispheres of the brain and the ways of outflow from the brain stem and subcortical formations. First, the blood drains into the upper sagittal and partially transverse -- sinuses (2). Second, venous blood flows in two directions: on the one hand - the system of basal veins (veins of Ge»)
Rosenthal) the blood goes to the great vein of the brain, and from the other - through many veins that have a relatively large diameter, venous blood from the region of the bridge of the medulla oblongata, and partially from the cerebellum drains into the venous sinuses of the base o (lower and upper stony, cavernous , transverse), as well as in the venous plexus (external and internal) of the spine |ZI.
Існує два основні типи внутрішньочерепної гіпертензії: « - внутрішньочерепна гіпертензія, зумовлена головним чином порушенням резорбції ліквору (41; З 70 - внутрішньочерепна гіпертензія, зумовлена головним чином порушенням венозного відтоку з порожнини с черепа |4).There are two main types of intracranial hypertension: "- intracranial hypertension, caused mainly by impaired resorption of cerebrospinal fluid (41; C 70 - intracranial hypertension, caused mainly by impaired venous outflow from the cavity of the skull |4).
From" Orthostatic and anti-orthostatic loads are used to assess the state of the venous department of the cerebral circulation system. Reflex changes in blood pressure and the response of the arterial tree lead to a collective reaction of the cerebral veins. In response to orthostatic load, a decrease in the maximum speed of blood flow in intracranial veins is noted 42 . In response to antiorthostatic - its increase. Russian researchers I5| in the investigation of blood flow in the direct sinus by the method of transcranial (se) dopplerography, during orthostatic load, a decrease in the maximum velocity of blood flow was observed by 2-10 cm/s (7-33906), during anti-orthostatic - its increase by 3-12 cm/s (11-3396). - The disadvantage of the prototype is the insufficient accuracy of the diagnosis, which can explain the pathophysiological aspects of the formation of the syndrome of intracranial hypertension, in particular, caused by impaired resorption of the cerebrospinal fluid, which is related to functional and biomechanical disorders at the level of the cervical spine |b1. c" The claimed invention solves the problem of increasing the accuracy of diagnosis of intracranial hypertension syndrome, clarifying the pathophysiological aspects of the occurrence of this syndrome.
The technical result of the task will be to increase the efficiency of diagnostic search 29 and the selection of treatment measures for patients with intracranial hypertension syndrome, increase the efficiency of application of therapeutic measures and prognosis of treatment.
The task is achieved by the fact that in the known method of diagnosing intracranial hypertension syndrome, which includes transcranial duplex scanning, measurement of quantitative indicators of blood flow in the vein of Rosenthal, the vein of Galen and direct sinus, orthostatic and 60 antiorthostatic tests, traction and rotation tests are additionally performed according to the invention of the cervical spine and determine the cervical rotation index according to the formula:

The method is performed as follows.
Patients are preliminarily examined clinically, including orthopedic and neurological examinations. All patients undergo x-ray spondylography in standard projections, echoencephaloscopy, with width determination
12).M-echo, its displacement, severity of pulsation (ZZS-12 echoencephaloscope).
29 Приклади конкретного дослідження «Venous outflow from the cranial cavity is examined using a computer sonography system ASOBOM 128ХР10, USA, sensors: linear with a penetration frequency of 7 MHz for the extracranial department and vector with a penetration frequency of 2.5 MHz for intracranial research. Two-dimensional, Doppler 19 modes (color, energy and spectral) are used, which made it possible to judge the morphological changes in blood vessels and evaluate the functional parameters of blood circulation. First, the study is carried out in a standard position, as in the prototype, while measuring the maximum blood flow rate, time-averaged maximum and average blood flow rates in the veins of Rosenthal, the vein of Galen, and the direct sinus. Then a rotational test is performed in the following way: fix the head with the right hand by the lower jaw, and with the first and third fingers of the left hand from below by the nipple processes with control palpation with the index finger of the left hand of the transverse process of the first cervical vertebra. At the same time, the edge of the left palm rests on the occipital hump. After that, under the influence of the head's own weight (gravitational force), traction is performed with unilateral rotation, and then the maximum blood flow velocity is measured again and the cervical rotation index is calculated. When it increases to 2195-3790, a syndrome of intracranial hypertension is diagnosed. 29 Examples of concrete research «

Patient K., born in 1978 with the diagnosis: "Instability of the C/ vertebra, insufficiency of blood circulation in the basin of the 60th right vertebral artery. Vegeto-vascular dysfunction of the hypotonic type with cephalgia" complains of periodic headache mainly in the frontal lobe, decreased vision. The symptoms were considered as a syndrome of increased intracranial pressure. The results of a duplex scan of the brachiocephalic vessels showed: the maximum speed of blood flow through the vein of Galen - 2bcm/s, after the test - 21cm/s, the cervical rotation index - 1595. Due to the fact that the index was within the normal range, vertebrogenic factors of the syndrome were excluded because of intracranial hypertension and the patient was offered a different treatment scheme.
345 patients who visited a neurologist in the Department of Vertebroneurology of the Central Medical Center of Kyiv were examined using the proposed method. Cervical spine pathology was detected in 8,295 patients. All patients were later diagnosed with intracranial hypertension syndrome.
The developed method, in comparison with the prototype, allows to improve the diagnosis of intracranial hypertension syndrome and obtain objective criteria for the effectiveness of the treatment.
